Aim: to study the process of proteins transamination in syncytiotrophoblast of placenta villi of women who suffered the acute form of cytomegalovirus (CMV) infection during pregnancy. Methods: 30 pregnant women with CMV infection recurrence at the 25–28th week of pregnancy were examined. The activity of γ-glutamyltransferase in the peripheral blood of pregnant women was determined by spectrophotometry at the device «Stat-Fax-2100» (The USA). Hsp-70 and caspase-3 in placenta homogenate were found out with serological methods. The activity of glutamatdehydrogenase and pyridoxal-5-phosphatase was studied with histochemical method of Z. Loyd at the placenta slice of parturient women. The apoptotic changes in syncytiotrophoblast nuclei were defined by ISEL-method.Results. The peripheral blood of CMV-seropositive parturient women showed a reduction of γ-glutamyltransferase in 1.30 times. Histochemically we identified the reduction of reaction products' concentration in response to pyridoxal-5-phosphate by 2.14 times, to glutamatdehydrogenase by 1,57 times. At the same time there was an increase of caspase-3 in 2,8 times and reduction of Hsp70 in 2.6 times in placenta homogenate. The number of apoptotic changes in syncytiotrophoblast nuclei increased by 4 times.Conclusion. Worsening of CMV infection in the period 25–28 weeks of pregnancy leads to disruption of amino acid metabolism in the placenta, causing structural and functional and metabolic adjustment, and is one of the reasons for slow growth and lack of development of the fetus.

The work was carried out to study the processes of lipid peroxidation, antioxidant defense, the content of phospholipase A2, the main structural phospholipids (phosphatidylethanolamine, phosphatidylcholine and its lizoform) in the peripheral blood of women with reactivation of chronic cytomegalovirus (CMV) infection in the III trimester of gestation depending on the titer of IgG antibodies to CMV. The method of random sample was used to select 90 pregnant women. The study group consisted of 60 pregnant women with exacerbation of chronic CMV infection in the third trimester. It was divided into two subgroups: subgroup A (30 women with active CMV infection, the titer of IgG antibodies to CMV was 1:1600); subgroup B (30 women with latent CMV infection, the titer of IgG antibodies to CMV was 1:800). The control group consisted of 30 healthy pregnant women who matched the study group by the period of gestation and age. It was found out that lipid metabolism imbalance in plasma and erythrocytes of the peripheral blood was one of the reasons for progression of the pathological process of CMV infection in pregnant women. Changes in the composition of erythrocyte membrane phospholipids, i.e the reduction of phosphatidylethanolamine by 22% (p<0.001), of phosphatidylcholine by 23% (p<0.001), an increase in the content of lysophosphatidylcholine by 34% (p<0.001), are regarded as membrane destabilizing processes. Their development is caused by uncontrolled free radical reactions leading to the initiation of peroxidation of cell membrane lipid oxidation (the increase in the concentration of TBA-active products by 78% (p<0.001), and the activation of lipolytic enzymes (increased concentration of phospholipase A2 by 60% (p<0.001). These changes occur against the oppression of antioxidant protection (reducing the concentration of α-tocopherol by 26% (p<0.001). The rate of change of lipid metabolism in these structures as manifestations of membrane destructive phenomena depends on the titer of IgG antibodies to CMV and is most pronounced when there is the active form of CMV infection (titer IgG antibodies to CMV is 1:1600). The mentioned pathological processes taken together aggravate the condition of pregnant women with CMV infection and create a favorable background for further progression of the pathological process, which greatly burdens the prenatal period and childbirth. The results obtained allow us to establish the important role of lipid metabolism disorders in the pathogenesis of CMV infection, which can be the basis for the expansion of the diagnostic and therapeutic base for this pathology of pregnant women.

In the review we have integrated all up-to-date knowledge concerning clinical course and treatment of glaucoma among pregnant women to help specialists choose a proper policy of treatment for such a complicated group of patients. Glaucoma is a chronic progressive disease. It rarely occurs among childbearing aged women. Nevertheless the probability to manage pregnant patients having glaucoma has been recently increasing. The situation is complicated by the fact that there are no recommendations on how to treat glaucoma among pregnant women. As we know, eye pressure is progressively going down from the first to the third trimester, so we often have to correct hypotensive therapy. Besides, it is necessary to take into account the effect of applied medicines on mother health and evaluate possible teratogenic complications for a fetus. The only medicine against glaucoma which belongs to category B according to FDA classification is brimonidine. Medicines of the other groups should be prescribed with care. Laser treatment or surgery may also be a relevant decision when monitoring patients who are planning pregnancy or just bearing a child. Such treatment should be also accompanied by medicines.

Abstract Background Sleep disorders, which are among the foremost important medical care issues, are prevalent in pregnancy. The present study is a meta-analysis of the prevalence of insomnia in the third trimester of pregnancy. This study aims to systematically review the overall prevalence of insomnia in the third trimester of pregnancy through conducting a meta-analysis. Method The literature used in this meta-analysis for the topic discussed above were obtained through searching several databases, including SID, MagIran, IranDoc, Scopus, Embase, Web of Science (WoS), PubMed Science Direct and Google Scholar databases without time limitation until December 2020. Articles developed based on cross-sectional studies were included in the study. The heterogeneity of studies was investigated using the I2 index. Also, the possible effects of heterogeneity in the studied studies are investigated using meta-regression analysis. Result In 10 articles and 8798 participants aged between11–40, the overall prevalence of insomnia in the third trimester of pregnancy based on meta-analysis was 42.4% (95% CI: 32.9–52.5%). It was reported that as the sample size increases, the prevalence of insomnia in the third trimester of pregnancy increases. Conversely, as the year of research increases, the prevalence of insomnia in the third trimester of pregnancy decreases. Both of these differences were statistically significant (P < 0.05). Conclusion Insomnia was highly prevalent in the last trimester of pregnancy. Sleep disorders are neglected among pregnant women, and they are considered natural. While sleep disturbances can cause mental and physical problems in pregnant women, they can consequently cause problems for the fetus. As a result, maintaining the physical and mental health of pregnant mothers is very important. It is thus recommended that in addition to having regular visits during pregnancy, pregnant women should also be continuously monitored for sleep-related disorders.

Abstract Objectives An elevated sFlt-1/PlGF-ratio has been validated as a significant predictor of preeclampsia, but has not been established in women with rheumatoid arthritis (RA). We explored whether the sFlt-1/PlGF-ratio could be altered due to disease activity in RA, and could be applied in this population to predict preeclampsia. Since sulfasalazine has been suggested to improve the angiogenic imbalance in preeclampsia, we also aimed to examine whether sulfasalazine could affect sFlt-1 or PlGF levels. Methods Making use of a nationwide, observational, prospective cohort study on pregnant women with RA, sFlt-1 and PlGF were measured in the third trimester. A total of 221 women, aged 21–42 years, were included, with a median gestational age of 30 + 3 weeks. Results No differences in sFlt-1 or PlGF were observed between women with high, intermediate or low disease activity (p= 0.07 and p= 0.41), whereas sFlt-1 and PlGF did not correlate with DAS28-CRP score (r=-0.01 and r=-0.05, respectively). Four (2%) women with a sFlt-1/PlGF-ratio ≤38 developed preeclampsia in comparison to three (43%) women with a ratio > 38, corresponding to a negative predictive value of 98.1%. Sulfasalazine users (n = 57) did not show altered levels of sFlt-1 or PlGF in comparison to non-sulfasalazine users (n = 164, p= 0.91 and p= 0.11). Conclusion Our study shows that in pregnant women with RA, the sFlt-1/PlGF-ratio is not altered due to disease activity and a cut-off ≤38 can be used to exclude preeclampsia. Additionally, sulfasalazine use did not affect sFlt-1 or PlGF levels in this population.

Ouabain was recently isolated from human plasma, bovine hypothalamus and bovine adrenal in attempts to identify endogenous substances inhibiting the cell membrane sodium pump. A number of radioimmunoassays have been developed in order to study the clinical significance of ouabain. The results have been controversial with regard to the presence and chemical nature of plasma ouabain-like immunoreactivity. We have now measured ouabain in healthy and pregnant individuals using solid-phase extraction of plasma samples followed by a new radioimmunoassay with the extraordinary sensitivity of at least 2 fmol/tube (5 pmol/l). Plasma extracts, a previously isolated human plasma ouabain-like compound and bovine hypothalamic inhibitory factor displaced the tracer in parallel and eluted identically with ouabain in high-performance liquid chromatography. Plasma ouabain immunoreactivity was found to be much lower than reported previously: 12.6+/-1.3 pmol/l in healthy men (mean+/-s.e., n=20) and 9.4+/-0.7 pmol/l in women (n=14). In pregnant women (n=28) plasma ouabain concentration was 16.3+/-4.0 pmol/l during the first trimester, 18.8+/-4.3 pmol/l during the second trimester and 24.3+/-4.0 pmol/l during the third trimester (all P<0.01 compared with non-pregnant women). Plasma ouabain 3-5 days after the delivery was 13.6+/-1.1 pmol/l (n=10, P<0.05-0.01 compared with second and third trimesters). The pregnancy-related changes in the plasma concentrations of ouabain resembled those of cortisol. Therefore cortisol was measured from the same plasma samples and a significant positive correlation was found (r=0.512, P=0.006). The similar profiles of plasma ouabain and cortisol during pregnancy and their rapid decreases postpartum are consistent with the adrenal cortical origin of ouabain and also show that the secretions of these hormones are possibly under the control of same factors.
