ABSTRACT Objective: to identify with the nursing team strategies to promote the safety of the patient with heart disease using warfarin. Method: an exploratory and descriptive research with a qualitative approach, developed in clinical/surgical inpatient units at a public referral hospital in cardiovascular care. The participants were 20 professionals from the nursing team who work in these sectors. Data collection took place in May and June 2017, through discussion groups and covered the five stages of the Maguerez Arch, which were held in a single meeting that was repeated four times in order to involve the largest number of professionals. The obtained data were submitted to thematic analysis. Results: two categories emerged: Recognizing the problem and identifying its causes; and Building hypotheses of solution to the problem. The results showed that the group recognizes the use of oral anticoagulant as a risk factor for serious adverse events. Strategies such as knowing the target international normative relationship of each patient, providing information on the use of anticoagulant to patients and their families, among others, can contribute to make this care safer. Conclusion: the moments of discussion favored by this study served as learning to continue the future implementation of the strategies pointed out by the group and thus increasingly improve the care provided to patients with heart disease and their families.

Introduction: This article discusses the experience of mothers during the process of falling ill of their children with congenital heart disease. Method: This is a descriptive research with qualitative approach conducted with nine mothers who had their babies admitted to a pediatric cardiology unit in a reference center for treatment of congenital heart disease in the North and Northeast regions in Brazil located in Fortaleza, Ceará. Information and reports on the experience of these mothers emerging from the diagnosis process, surgery, treatment and hospitalization were collected in semi-structured individual interviews. After interviews, the testimonies of mothers were transcribed in full-length, identifying the speeches with the codes M1, M2,...M9, thereby ensuring anonymity. We used the thematic analysis technique of Minayo (2012) for treatment of information and design of the content of this article. This article is part of the research project: Breastfeeding babies with heart disease, approved by the Research Ethics Committee of the Messejana Hospital, under Opinion No 1,285,784. Results: The study demonstrated that infants with congenital heart disease require specific and specialized care and that mothers undertake a long and sometimes painful journey in the search for the healing of their child. Conclusion: Based on the collected speeches, we learned that mothers also need to be assisted by professionals of the health team. They must support the mother's empowerment in the child care and strengthen the relationship of the mother-infant binomial, and help in coping and adapting to the reality in the process of falling ill and healing of their child.   Key words: Congenital heart disease. Experience of mothers

Objective: The study’s purpose has been to analyze the activities performed by long-stay institution caregivers. Methods: It is a descriptive research with a qualitative approach, which was performed at a long-stay philanthropic institution for the elderly located in the East of Teresina city, Piauí State. A total of 12 randomly selected elderly caregivers have participated of the study. Data collection took place from February to March 2017 through semi-structured interviews. Results: Based on the thematic analysis, it was possible to underline two categories, as follows: Activities concerning basic human needs; and, The caregivers’ overload due to elderly-related activities. Conclusion: The actions taken do not go beyond the caring activities. Although some caregivers mention doing some plays, dances, and games, those actions are considered unusual, because not everyone does it. There is a setback, because the activities lead such professionals to work overload, mood swings, adaptation difficulties, total dependency, then becoming a barrier to its own execution.

Women’s access to and control over productive resources, including land, has increasingly been recognized in global discussions as a key factor in reducing poverty, ensuring food security and promoting gender equality. Indeed, this argument has been widely accepted by both feminists and development theorists since the 1980s. Based on qualitative research with 50 purposively selected men and women, this study explored the complexity of women’s access to and control over land within a specific relationship of contestations, negotiations, and manipulations with men. Data were analyzed using thematic analysis. While theoretically, participants showed that women’s [secure] access to and control over land has beneficial consequences to women themselves, households and the community at large, in principle, women's access and control status was premised in the traditional framework which largely deprives women, equal access and/or control over the land. The study indicates that even though land is the most revered resource and indeed, the dominant source of income for the rural poor, especially women, gender-erected discrimination and exclusion lie at the heart of many rural women in gaining access to land. This study argues that women's weak access rights and control over land continue to perpetuate the feminization of gender inequality–while men were reported to possess primary access and control over land as the heads of households, women were argued to have secondary rights due to their ‘stranger statuses’ in their husbands’ families. Overall, the degree of access to land among women was reported to be situated within two broad contexts–marriage and inheritance.

Abstract Funding Acknowledgements Type of funding sources: None. Background Conflicting reports exist on the prevalence and clinical impact of interstitial fibrosis in right ventricular (RV) congenital heart disease (CHD). This study evaluates the longitudinal evolution of native myocardial T1 relaxation time (T1) in RV CHD. Methods On a 1.5T scanner, an ECG-triggered modified Look-Locker inversion recovery sequence (scheme 3(3)3(3)5) was acquired on a short-axis basal slice covering the RV and left ventricle (LV) on two consecutive CMR exams. Global and segmental (LV = 6, RV = 4) RV and LV T1 values  were calculated (Figure). Results Mean time between CMR exams for 36 included patients (age 34 ± 2y) was 22 ± 2 months. All LV segments and 81/88% of RV segments of first and second CMR could be analyzed, respectively.  T1 increased mildly but not significantly (table). There was no relationship of T1 to pulmonary regurgitation fraction, pulmonary stenosis or RV enddiastolic volume (p > 0.05). Global RV T1 of the second CMR was related to RV ejection fraction (RVEF): r = 0.353, 3.0 ± 1.4, p = 0.038. T1 of the infero-septal LV segment of first and second CMR, global LV T1 of second CMR and increase of T1 of global LV, anterior, antero-lateral and –septal LV segments, were related to age at CMR: r = 0.333 - 0.463, p < 0.05, respectively. Conclusions Native T1 values increased mildly in patients with stable RV CHD, which was not statistically significant probably due to the short to median follow-up. Global RV T1 appears to be related to RVEF which could be sign of increasing interstitial fibrosis whereas the relationship of LV T1 to age might be a physiological finding.
