The author prefers a fresh infusion of 15.0 to 180.0 to a liquid extract of gossypii herbacei, after 1 / 2-1 hour a tablespoon; indications - uterine bleeding, atonic state of the uterus after childbirth. The author did not have to observe any side effects from this remedy.

Background: Dysfunctional Uterine Bleeding (DUB), is the commonest cause of Abnormal Uterine Bleeding (AUB). It causes morbidity, anaemia, and unnecessary hysterectomies in women of fertile age group. This study attempts to study efficacy of medical management especially Selective Estrogen Receptor Modulator (SERM) namely Ormeloxefine (ORM) (Sevista®) in Perimenopausal women. Ormeloxifene was marketed in India for contraception under brand names Centron, Saheli, Choice-7, Novex and Novex-DS. It’s a benzopyran derivative also known as Centchroman which causes asynchronousity between ovulation and menstrual cycles possibly because of both estrogenic and anti-estrogenic actions. It has been known to cause delay in ovulation in clinical trials; however, majority have been unaffected. It causes delay in proliferation of endometrium thereby causing asynchronous cycles. It also improves motility of ciliary lining of Fallopian tubes thereby reducing the chances of implantation of fertilized egg.  Methods: This study is aimed at evaluation of subjective and objective stastical benefits and side effects in treatment of DUB in perimenopausal age group with ORM and commonly used 19 nortestosterone compound (progesterone); Norethisterone (NET).Results: Primary outcome were analyzed at the end of every 3 months and at the end of one year finally. Secondary outcomes of the study in each arm were also assessed. There was stastically significant increase in Hb and stastically significant decrease in ET. Data analysis was done for variables in each arm by t-test to estimate the mean, median, range P and t value for a conclusion. Differences were taken as significant when P<0.05.Conclusions: ORM is a safe, cost effective, non-steroidal, non-hormonal drug with convenient dosage and better compliance for medical management of perimenopausal DUB with minimum focal pathology. Side effects observed need more evaluation with larger sample size to be statistically significant.

Dysfunctional uterine bleeding is a type of abnormal uterine bleeding where vaginal bleeding occurs outside of the menstrual cycle in the absence of any known pelvic pathology. Dysfunctional uterine bleeding can be treated safely with hormone therapy. Combined oral contraceptives help in increased menstrual cycle regularity and decreased blood loss. In this study, a reliable drug for the dysfunctional uterine bleeding with maximum effectiveness and minimal side effects were assessed. This study was conducted on 120 cases of dysfunctional uterine bleeding. Patients who were diagnosed with dysfunctional uterine bleeding were randomly assigned into two groups. Group D and group L included patients who were given Ethinyl estradiol 0.02mg + desogestrel 0.15mg and Ethinyl estradiol 0.03mg + levonorgestrel 0.15mg respectively for the four consecutive 28- day cycles. Menstrual blood loss was assessed using the pictorial blood assessment chart (PBAC) score on 2nd and 4th months of recruitment. Side effects such as weight gain, acne and headache were assessed in both groups. This study shows 56.68% reduction in mean PBAC score in 2 months in desogestrel group whereas only 44.96% reduction in levonorgestrel group and 79.87% reduction in mean PBAC score in desogestrel group in 4 months whereas only 74.46% reduction in levonorgestrel group. Side effects like weight gain, acne and headache were more prominent in the levonorgestrel group than desogestrel group. Desogestrel containing combined oral contraceptive can be a useful and safe treatment for dysfunctional uterine bleeding.

Background: Dysfunctional uterine bleeding (DUB) is a common gynaecological disorder that usually ends up in hysterectomy and causes psychological and physiological stress. Medical management with hormones and NSAIDS has inherited side effects. Ormeloxifene, a selective estrogen receptor modulator, is emerging as a safe and effective agent for dysfunctional uterine bleeding. The objective of the study was to evaluate the role of ormeloxifene in medical management of dysfunctional uterine bleeding.Methods: 30 patients, on whom diagnosis of dysfunctional uterine bleeding was made, were included in the study. Patients were given ormeloxifene 60mg twice a week for 12 weeks and then once a week for 12 weeks. The primary outcome measures were menstrual blood loss (assessed by pictorial blood assessment chart score), hemoglobin concentration and endometrial thickness. The secondary outcome measures were acceptability and side effects of ormeloxifene.Results: There was a significant reduction in mean PBAC score from 316 to 52 after six months of treatment. The mean hemoglobin concentration increased significantly from 8.4 to 9.8 gms/dl with a rise of 1.4gm/dl (p <0.05). The mean pretreatment endometrial thickness was 10.8mm and it decreased significantly to 8.1mm after 6 months of treatment with ormeloxifene (p < 0.05). 76.7% of the women showed marked subjective improvement in symptoms. The most common side effect reported was amenorrhea (13.3%).Conclusions: Ormeloxifene can be considered as an effective and safe therapeutic option for the medical management of dysfunctional uterine bleeding.

Objective: To determine the effectiveness and safety of elective termination of pregnancies from 56 days gestational age 2ndtrimester in which ingestion of 20 mg tamoxifen for 4 days was followed by oral administration of 1 or 2 days of 800 μg misoprostol. Design: Aclinical trial. Setting: Department of Obstetrics and Gynaecology, Nishtar Hospital, and Private setup, Multan. Period: January 2009 toJanuary 2010. Material and methods: A total of 50 patients were included in the trial on outdoor basis. Results: Of the 17 subjects with agestational age between 50 and 56 days, 15 (88.2%) aborted their pregnancies. The mean duration of uterine bleeding in the group of womenwho aborted their pregnancies was 8.1 days with a range of 1-34 days. Conclusions: In conclusion, the regimen of oral tamoxifen followed byoral administration of misoprostol was found to be an effective and rapid method to electively terminate early gestation of 8 weeks duration orless with no serious side effects.

Analyzing the pharmaceutical treatment of uterine bleeding, the author quite rightly stops at ergot preparations and hydrastis canadensis, as the most effective means. Ergot acts mainly on the very muscle of the uterus and, therefore, is shown in atonic bleeding, depending for example. from insufficient postpartum involution of the uterus, while hydrastis is most useful for so-called secondary bleeding that occurs under the influence of diseases of the uterine appendages, with chronic endometritis, in the menopausal period, in girls, etc. The disadvantage of this remedy, in the form of a liquid extract, is that it does not work well when used when bleeding has already occurred, and that to get the proper effect, it must be taken long (two weeks) before the expected Meno-or metrorrhagia.

<strong>Background:</strong>Menorrhagia accounts for 12% of all gynaecology consultations and is one of the most common causes of iron deficiency anaemia in females after nutritional anaemia.<p><strong>Objectives:</strong> To evaluate the efficacy and side effects of ormeloxifene in cases of dysfunctional uterine bleeding.</p><p><strong>Material and Methods:</strong> 30 women aged 28-46 years who attended the outpatient gynaecology department in a tertiary care hospital with complaint of heavy menstrual flow were recruited for the study. Two pretreatment baseline cycles were compared to the treatment cycles of ormeloxifene. The main outcomes measured were menstrual blood, blood haemoglobin levels and endometrial thickness in proliferative phase as studied by TVS. Data thus collected was compared and analysed statistically.</p><p><strong>Results:</strong> Total number of bleeding days per year decreased by 76%. Total no. of pads soiled per cycle decreased by 76.3%. All the patients reported disappearance of clots, 66.66% within 1 month of the treatment only. Dysmenorrhoea was relieved in 62.5% patients. Mean increase in Hb was 0.42g%.</p><p><strong>Conclusion:</strong> Ormeloxifene is very effective in improving all the parameters of blood loss in DUB including the no. of days of bleeding, no. of pads soiled and the passage of clots. Ormeloxifene has a good patient acceptability and compliance due to its minimal side effects, low cost and simple dosage schedule. In the peri-menopausal age group, drug is protective against breast malignancy and osteoporosis. Ormeloxifene has the potential to be an effective treatment for DUB and should always be considered amongst the treatment options.</p>

Dysfunctional uterine bleeding (DUB) is a state of abnormal uterine bleeding. It is a diagnosis of excluding any clinically detectable organic, systemic and iatrogenic cause. Current treatments available have many side effects, including hormonal imbalance. So there exists a lacuna for a safe and effective treatment without side effects for this condition. The present study is an effort to evaluate the combined effect of fruit of Badara [Ziziphus jujuba(L). Lam] with Palm jaggery in Dysfunctional uterine bleeding. The study design is interventional with a sample size of 15 subjects within the age group of 16-50 years. The study drug was administered in 6gm in the morning and the evening before food for 90 days. The following 30 days was follow up period without medication. The effect was assessed on subjective and objective parameters before treatment and on the 31st, 61st, 91st and 121st days. The results were statistically analyzed using Paired t-test and Wilcoxon Signed Rank test. The study drug showed the statistically significant result in DUB as reducing the amount and duration of bleeding, an interval of menstruation, number of clots, pain, fatigue, and increasing the Haemoglobin percentage.

Introduction. Uterine bleeding is an important social and economic problem. Herba drugs are used both in folk and in evidence medicine for atony of the uterus and uterine bleeding. Therefore, the development and research of herbal preparations with a hemostatic effect for use in uterine bleeding is an important issue.The aim of the study – to conduct a phytochemical study of the liquid extract of corn silk, as well as study its hemostatic effect.Research Methods. Phytochemical study of qualitative composition and quantitative content of biologically active substances of liquid extract of cron silk was carried out by qualitative reactions, spectrophotometric and titrimetric methods. A study of the hemostatic effect of a liquid extract of corn silk was carried out on outbred, mature white female rats on a model of capillary bleeding from a cut wound of the uterus. The preparation of the comparison was an extract of water pepper.Results and Discussion. Sugars, glycosides, tannins, flavonoids, coumarins, saponins were identified in the liquid extract of corn silk. It was determined that the sum of polyphenols in the liquid extract of corn silk is (8.0±0.51) %, tannins – (1.4±0.03) %, flavonoids – (2.35±0.07) %, hydroxycinnamic acids – (3.3±0.23) %, organic acids – (3.23±0.46) % by the quantitative methods of investigation. As a result of prophylactic administration of a liquid extract of corn silk to female rats, the duration of capillary bleeding from the cut cervical wound was significantly reduced. The haemostatic activity of the reference preparation was somewhat lower.Conclusions. The conducted researches give grounds to assert about the prospects and the need for further research of the hemostatic effect of the liquid extract of corn silk.
