Aim. To evaluate the clinical effectiveness of impulse electrotherapy in the complex treatment of patients with different variants of chronic endometritis based on the evaluation of adaptive reactions and immune status. Methods. Conducted was a clinical and laboratory examination of 550 women with early reproductive losses, who were divided into three groups. Studied was the efficacy of electro-impulse therapy with high-amplitude short-wavelength low-frequency electrical signals using the apparatus «SKENAR» (SKENAR-therapy) in the complex treatment of 269 women with chronic endometritis (first group). The second group included 67 women with similar variants of chronic endometritis, who were treated without the physiotherapy component. 54 patients refused the proposed rehabilitation activities (third group). Results. New data has been obtained on the adaptive reactions of the female body before and after treatment during different types of chronic endometritis. The frequency of disadaptation was highest during the hypoplastic form of chronic endometritis. Shown was the high efficiency of electro-impulse therapy, using the apparatus «SKENAR» in the complex treatment of chronic endometritis due to the normalization of parameters of immunoreactivity. It was established that SKENAR-therapy of chronic endometritis promotes early normalization of adaptive reactions in comparison to conventional methods of treatment. Conclusion. Rehabilitation treatment of chronic endometritis in combination with electro-impulse therapy with high-amplitude short-wavelength low-frequency electrical signals contributes to the recovery of immunoreactivity and to the formation of favorable adaptation reactions.

A complex treatment of a patient with acute amitriptyline and cyclodol poisoning using enterosorption, intestinal lavage and hemosorption on a new column with a synthetic sorbent is presented. A two-layer synthetic polymer column developed for the selective sorption of cytokines by direct hemoperfusion was used for hemosorption. Quantitative determinations of amitriptyline and cyclodol levels before and after the column, as well as before and after hemosorption have showed high efficiency of the sorbent to remove the toxicant from the blood. The use of 6-hour hemosorption allowed to reduce the level of amitriptyline from the initial level by more than 4 times and the level of cyclodol - by more than 3 times to therapeutic levels and to obtain a pronounced positive clinical effect in the complex treatment of a patient with severe poisoning.

Intermittent theta-burst stimulation (iTBS) is a high-efficiency transcranial magnetic stimulation (TMS) paradigm that has been applied to post-stroke aphasia (PSA). However, its efficacy mechanisms have not been clarified. This study aimed to explore the immediate effects of iTBS of the primary motor cortex (M1) of the affected hemisphere, on the functional activities and connectivity of the brains of PSA patients. A total of 16 patients with aphasia after stroke received iTBS with 800 pulses for 300 s. All patients underwent motor, language, and cognitive assessments and resting-state functional MRI scans immediately before and after the iTBS intervention. Regional, seed-based connectivity, and graph-based measures were used to test the immediate functional effects of the iTBS intervention, including the fractional amplitude of low-frequency fluctuation (fALFF), degree centrality (DC), and functional connectivity (FC) of the left M1 area throughout the whole brain. The results showed that after one session of iTBS intervention, the fALFF, DC, and FC values changed significantly in the patients’ brains. Specifically, the DC values were significantly higher in the right middle frontal gyrus and parts of the left parietal lobe (p < 0.05), while fALFF values were significantly lower in the right medial frontal lobe and parts of the left intracalcarine cortex (p < 0.05), and the strength of the functional connectivity between the left M1 area and the left superior frontal gyrus was reduced (p < 0.05). Our findings provided preliminary evidences that the iTBS on the ipsilesional M1 could induce neural activity and functional connectivity changes in the motor, language, and other brain regions in patients with PSA, which may promote neuroplasticity and functional recovery.

Introduction. Degenerative-dystrophic changes in the spine are the most common chronic human diseases. Pronounced clinical manifestations of vertebrogenic diseases are observed during active labor activity and represent one of the most frequent causes of temporary disability. It is also known that any pain syndromes are accompanied by the development of psychovegetative disorders, which reduces the effectiveness of treatment. In recent years, there has been a reasonable increase in the interest of the medical community to the non-drug treatment methods. It is caused by the polypharmacotherapy side effects, an increased numbers of allergic reactions, problems with polypragmasia, and a low level of compliance. At the same time, the question of the possibility of various non-drug treatment methods combining is quite acute.The goal of research is to substantiate the clinical effectiveness of the combined application of osteopathic correction and reflexotherapy in the complex treatment of patients with dorsopathy at the cervical-thoracic level.Materials and methods. A prospective controlled randomized study was conducted on the basis of the medical clinics of LLC «Family Health» (Kazan) and ANO «Center for SEB assistance» (Kazan) from April 2018 to May 2019. The study involved 52 people with dorsopathy at the cervical-thoracic level, aged from 25 to 45 years. In accordance with the exclusion criteria, 7 people dropped out of the study. The remaining patients (45 people), depending on the used treatment method, were divided by a simple randomization method using a random number generator into three groups of 15 people. The first group (main group) received osteopathic correction and reflexotherapyon the background of standard drug therapy; the second and the third groups (control groups) received only osteopathic correction orreflexotherapy, respectively, on the background of standard drug therapy. The osteopathic examination was performed for all patients regardless of the group, before and after the course of therapy, with the formation of an osteopathic conclusion.Also there was the estimation of the degree of pain syndrome severity,the asthenia, and the degree of accumulated emotional and energy charge that does not get out in a person.Results. It was found in the course of the study that the inclusion of osteopathic correction and reflexotherapy in the complex therapy of patients with dorsopathy at the cervical-thoracic level is clinically more effective than the isolated use of these methods. Such complex therapy allows to achieve a more significant reduction in the severity of the pain syndrome by VAS (p<0,05), relief of internal emotional tension (p<0,05), and increase the effectiveness of correction of dominant somatic dysfunction. Based on the obtained data, it could be assumed that reflexotherapy potentiates the effects of osteopathic correction.Conclusion. Based on the study, it can be concluded that the combination of osteopathic correction and reflexotherapy in the treatment of patients with dorsopathy at the cervical-thoracic level increases the clinical effectiveness of the standard drug therapy. At the same time, it is worth noting the potentiating effect of the combined use of two non-medicinal methods. The question of combining of various non-drug treatment methods is quite acute today, so this study will be continued.

The article presents results of a study on the effectiveness of using lowfrequency ultrasound and photochromotherapy in the complex treatment of patients with chronic bacterial and viral rhinosinusitis. The study involved 69 people aged 18 to 55 years: 39 (56.83 %) women and 30 (45.11 %) men with chronic rhinosinusitis during the period of clinical exacerbation and concomitant chronic tonsillitis without exacerbation. All patients included in the study underwent X-ray examination or computed tomography and magnetic resonance imaging of the paranasal sinuses. 100 % of the patients were examined for the presence of antibodies to Human gammaherpesvirus 4 (ELISA), had study of blood serum for IgM and IgG and test to determine the avidity of Ig G antibodies to γ-herpesviruses. The obtained data indicate the effectiveness of including low-frequency ultrasound and photochromotherapy in the complex treatment of patients with chronic rhinosinusitis and concomitant chronic tonsillitis of bacterial and viral nature without exacerbation.

ABSTRACT. Seismic processing aims to provide an adequate image of the subsurface geology. During seismic processing, the filtering of signals considered noise is of utmost importance. Among these signals is the surface rolling noise, better known as ground-roll. Ground-roll occurs mainly in land seismic data, masking reflections, and this roll has the following main features: high amplitude, low frequency and low speed. The attenuation of this noise is generally performed through so-called conventional methods using 1-D or 2-D frequency filters in the fk domain. This study uses the empirical mode decomposition (EMD) method for ground-roll attenuation. The EMD method was implemented in the programming language FORTRAN 90 and applied in the time and frequency domains. The application of this method to the processing of land seismic line 204-RL-247 in Tacutu Basin resulted in stacked seismic sections that were of similar or sometimes better quality compared with those obtained using the fk and high-pass filtering methods.Keywords: seismic processing, empirical mode decomposition, seismic data filtering, ground-roll. RESUMO. O processamento sísmico tem como principal objetivo fornecer uma imagem adequada da geologia da subsuperfície. Nas etapas do processamento sísmico a filtragem de sinais considerados como ruídos é de fundamental importância. Dentre esses ruídos encontramos o ruído de rolamento superficial, mais conhecido como ground-roll . O ground-roll ocorre principalmente em dados sísmicos terrestres, mascarando as reflexões e possui como principais características: alta amplitude, baixa frequência e baixa velocidade. A atenuação desse ruído é geralmente realizada através de métodos de filtragem ditos convencionais, que utilizam filtros de frequência 1D ou filtro 2D no domínio fk. Este trabalho utiliza o método de Decomposição em Modos Empíricos (DME) para a atenuação do ground-roll. O método DME foi implementado em linguagem de programação FORTRAN 90, e foi aplicado no domínio do tempo e da frequência. Sua aplicação no processamento da linha sísmica terrestre 204-RL-247 da Bacia do Tacutu gerou como resultados, seções sísmicas empilhadas de qualidade semelhante e por vezes melhor, quando comparadas as obtidas com os métodos de filtragem fk e passa-alta.Palavras-chave: processamento sísmico, decomposição em modos empíricos, filtragem dados sísmicos, atenuação do ground-roll.

AbstractBiological aerated filters (BAFs) have high filtration efficiency due to their tolerance of hydraulic and organic shocks are suitable for the treatment of complex and sanitary wastewater. In this study, for the first time, natural media of date kernel from Bam city was used as the BAF reactor media, with a meshing sand filter separated by a standard metal grid from the natural filter section used at the end of the reactor. This can be considered an innovation in the media and filtration. Aeration in the related reactor with 160 cm height was performed bilaterally as up-flow and continuously by nozzles throughout the reactor media. In this work, the actual effluent of the hospital wastewater treatment plant was employed as the inflow wastewater to the reactor, and its organic and inorganic parameters were measured before and after the treatment by the BAF reactor. The backwashing process was also studied in three ways: bottom backwashing (TB), top backwashing (BB), and top and bottom backwashing (TBBS), to determine the amount of water consumed and to achieve the desired result. According to the results obtained in this study, the removal efficiencies of inorganic and microbial contaminants, amoxicillin and azithromycin were obtained as follows: BOD5: 98.48%, COD: 92.42%, $${\text{NO}}_{3}^{ - }$$ NO 3 - : 99.4%, P: 93.3%, Coliforms: 97%, Color: 42.8%, Turbidity: 95%, Sulphate: 30%, TSS: 98.9%, Amoxicillin: 20% and azithromycin: 13%. In the backwashing process, the amount of water consumed in these three TB, BB, and TBBS methods were obtained 300, 164, and 118 L, respectively, So, TBBS method was selected as the optimal method. Based on the results obtained in this study, it is concluded that the BAF process with natural date kernel has a high efficiency in removing organic and inorganic contaminants from hospital wastewater, also the concentration of most of the effluent parameters was less or in accordance with EPA standard.

No comparison of the amount of low-frequency fatigue (LFF) produced by different activation frequencies exists, although frequencies ranging from 10 to 100 Hz have been used to induce LFF. The quadriceps femoris of 11 healthy subjects were tested in 5 separate sessions. In each session, the force-generating ability of the muscle was tested before and after fatigue and at 2, ∼13, and ∼38 min of recovery. Brief (6-pulse), constant-frequency trains of 9.1, 14.3, 33.3, and 100 Hz and a 6-pulse, variable-frequency train with a mean frequency of 14.3 Hz were delivered at 1 train/s to induce fatigue. Immediately postfatigue, there was a significant effect of fatiguing protocol frequency. Muscles exhibited greater LFF after stimulation with the 9.1-, 14.3-, and variable-frequency trains. These three trains also produced the greatest mean force-time integrals during the fatigue test. At 2, ∼13, and ∼38 min of recovery, however, the LFF produced was independent of the fatiguing protocol frequency. The findings are consistent with theories suggesting two independent mechanisms behind LFF and may help identify the optimal activation pattern when functional electrical stimulation is used.

Non-smooth piecewise functional isolators are smart passive vibration isolators that can provide effective isolation for high frequency/low amplitude excitation by introducing a soft primary suspension, and by preventing a high relative displacement in low frequency/high amplitude excitation by introducing a relatively damped secondary suspension. In this investigation a linear secondary suspension is attached to a nonlinear primary suspension. The primary is assumed to be nonlinear to model the inherent nonlinearities involved in real suspensions. However, the secondary suspension comes into action only during a short period of time, and in mall domain around resonance. Therefore, a linear assumption for the secondary suspension is reasonable. The dynamic behavior of the system subject to a harmonic base excitation has been analyzed utilizing the analytic results derived by applying the averaging method. The analytic results match very well in the transition between the two suspensions. A sensitivity analysis has shown the effect of varying dynamic parameters in the steady state behavior of the system.
