This article discusses relevant aspects in the treatment of patients with COVID-19. Up-to-date information about principles for administration of statins, antithrombotics, and antiarrhythmics is presented. The authors addressed in detail specific features of reversing heart rhythm disorders in patients with coronavirus infection and the interaction of antiarrhythmic and antiviral drugs. Recommendations are provided for outpatient and inpatient antithrombotic therapy for patients with COVID-19. Issues of antithrombotic and antiviral drug interaction are discussed.

Background: An outbreak of SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) infection or COVID 19, causing serious threats to all around the world. Until an effective and safe vaccine for novel coronavirus is developed by scientists, current drug therapy should by optimize for the control and treatment of COVID 19. Objective: In this manuscript, we are presenting a perspective on possible benefits of reformulating antiviral drug dosage form with nanoemulsion system against novel coronavirus infection. Methods: Literature review has been done on COVID 19, treatment strategies, novel drug delivery systems and role of pulmonary surfactant on lungs protection. Results: Nanoemulsion system and its components have certain biophysical properties which could increase the efficacy of drug therapy. Antiviral drugs, delivered through a nanoemulsion system containing P-gp inhibitor (surfactant and cosolvent), can inhibit the cellular resistance to drugs and would potentiate the antiviral action of drugs. Pulmonary surfactant (PS) assisted antiviral drug delivery by nanoemulsion system could be another effective approach for the treatment of COVID 19. Use of functional excipients like pulmonary surfactant (PS) and surfactant proteins (SPs), in the formulation of the antiviral drug-loaded nanoemulsion system can improve the treatment of coronavirus infection. Conclusion: In our opinion for synergizing antiviral action, lipid and protein portion of PS and their commercial analogs should be explored by pharmaceutical scientists to use them as a functional excipient in the formulation of antiviral drugloaded nanoemulsion system.

According to statistics, 20-40% of hospitalized patients have chest pain, heart rhythm disorders, heart failure, and sudden cardiac death syndrome. This review focuses on recent studies and clinical cases related with this issue.

In the current epidemic of a new coronavirus infection, COVID-19 treatment of patients with chronic non-communicable diseases (NCDS) and cardiovascular diseases (CVD), in particular, requires close attention of doctors. The main objectives of such patients’ management tactics are to ensure that patients comply with self-isolation measures that prevent infection, motivate patients to follow all medical recommendations. Special attention should be paid to comply with recommended treatment of CVD. In the case of an infectious disease, it is necessary to assess the prescribed therapy for interaction with medications already taken by the patient. However, the tactics for COVID-19 therapy, including in patients with NCDS, may change over time, since there are currently no drugs with proven effectiveness for the treatment of a new coronavirus infection.

Aim.To evaluate the frequency and identify predictors of long­term fatal cardiovascular events (CVE) after planned percutaneous coronary interventions (PCI).Material and methods. We conducted a retrospective study that included 150 patients who underwent planned endovascular intervention on the coronary arteries. Outcomes of interventions were assessed 6 years after the PCI, by analyzing medical records and telephone interviews. The primary endpoint of the study was cardiovascular­related death.Results.Fatal CVE were recorded in 10,6% of patients. A statistically significant relationship was found between the development of fatal CVE in the long­term period after planned PCI and the presence of initial chronic obstructive pulmonary disease in patients (OR=12,3; CI (3,6­41,5); р<0,001), atrial fibrillation (OR=5,1; CI (1,6­16,3); p=0,003), diabetes mellitus (OR=3,2; CI (1,1­9,8); p=0,032), acute cerebrovascular accident (OR=8,6; CI (2,0­36,4); p=0,001); any clinical complications of interventions (OR=3,1; CI (1,1­9,0); p=0,028), taking of antiarrhythmic drugs (OR=5,9; CI (1,3­27,4); p=0,012), statins taking at the time of PCI (OR=0,3; CI (0,1­0,8); p=0,013). According to the results of the ROC­analysis, the most significant predictor of fatal CVE in the long­term period was erythrocyte sedimentation rate more than 14,5 mm/h (AUC=0,677; CI (0,507­0,835); p=0,027). According to 6­year observation, Kaplan­Meier curves showed a significant effect of multifocal atherosclerosis, acute periprocedural kidney injury and heart rhythm disorders recorded during PCI, on the incidence of long­term fatal CVE.Conclusion.A statistically significant relationship between the development of fatal CVE in the long­term period after planned PCI and the presence of initial comorbid pathology in patients (chronic obstructive pulmonary disease, atrial fibrillation, diabetes mellitus, multifocal atherosclerosis, acute cerebrovascular accident), erythrocyte sedimentation rate more than 14,5 mm/h, concomitant drug therapy at the time of the intervention (taking of antiarrhythmic drugs, statins). Significant predictors of adverse long­term outcome after planned endovascular myocardial revascularization were clinical complications of PCI, recorded during the hospital period, especially acute periprocedural kidney injury and heart rhythm disorders.

Methodical recommendations are devoted to one of the most pressing problems of cardiology – paroxysmal heart rhythm disorders in clinical practice. The recommendations consider the basic principles of determining supraventricular tachycardias (SVT) and their electrophysiological mechanisms. The clinic and features of ECG diagnostics of separate forms of tachycardias are stated. Emergency care and long-term therapy for SVT with narrow and wide QRS complexes are presented. Methods of treatment and algorithms for the management of patients with SVT in accordance with the updated in 2019 recommendations of the European Association of Cardiologists for the management of patients with SVT, antiarrhythmic drugs and their use, the principles of catheter treatment of tachycardia. Recommendations are addressed to cardiologists, therapists, doctors of functional diagnostics, general practitioners and family medicine, interns.

In a review article based on my own clinical experience of managing patients with acute myocardial injury and fulminant myocarditis, taking into account expert recommendations on the clinical treatment of myocardial damage associated with novel coronavirus infection a National clinical geriatric medical research center, division of cardiovascular diseases, the Chinese geriatrics society, Department of cardiology, Beijing Medical Association and European clinics discusses the pathogenesis, diagnosis and treatment of myocardial damage and FM patients, infected with SARS-CoV-2 in the context of the COVID-19 pandemic. Clinical features and diagnostic criteria are presented, including screening tests of markers of myocardial damage in the form of a highly sensitive troponin test, a natriuretic peptide. The article discusses in detail the pathogenesis and mechanisms of myocardial damage, including immune mechanisms, cytokine storm, systemic inflammation with macro- and microvascular dysfunction and the development of myocardial dysfunction with acute heart failure, hypotension, cardiogenic shock and/or life-threatening heart rhythm disorders caused by hypoxia and metabolic disorders at the cellular level. Features of the clinical course of fulminant myocarditis in infected patients (SARS-CoV-2) in the conditions of the COVID-19 pandemic are presented. For the first time, a detailed histo-morphological analysis of pathological myocardial injuries and complications is presented on the basis of unique autopsy material on post-mortem diagnostics of various pathoanatomic autopsies of those who died from COVID-19 in Moscow. Based on the clinical, functional and morphological material, the Protocol of etiopathogenetic treatment is presented. The basis of standard therapy is considered antiviral drugs, immunoglobulin G, the use of monoclonal antibodies to interleukin-6, anticoagulants, glucocorticoids, depending on the clinical situation, cardioprotectors and symptomatic treatment are recommended to maintain the heart, which in combination can achieve a certain clinical effectiveness. As adjuvant cardioprotective targeted therapy, the sodium salt of phosphocreatine is considered in order to preserve the myocardium, maintain its contractility and vital activity.

AbstractA novel coronavirus infection coronavirus disease 2019 (COVID-19) emerged from Wuhan, Hubei Province of China, in December 2019 caused by SARS-CoV-2 is believed to be originated from bats in the local wet markets. Later, animal to human and human-to-human transmission of the virus began and resulting in widespread respiratory illness worldwide to around more than 180 countries. The World Health Organization declared this disease as a pandemic in March 2020. There is no clinically approved antiviral drug or vaccine available to be used against COVID-19. Nevertheless, few broad-spectrum antiviral drugs have been studied against COVID-19 in clinical trials with clinical recovery. In the current review, we summarize the morphology and pathogenesis of COVID-19 infection. A strong rational groundwork was made keeping the focus on current development of therapeutic agents and vaccines for SARS-CoV-2. Among the proposed therapeutic regimen, hydroxychloroquine, chloroquine, remdisevir, azithromycin, toclizumab and cromostat mesylate have shown promising results, and limited benefit was seen with lopinavir–ritonavir treatment in hospitalized adult patients with severe COVID-19. Early development of SARS-CoV-2 vaccine started based on the full-length genome analysis of severe acute respiratory syndrome coronavirus. Several subunit vaccines, peptides, nucleic acids, plant-derived, recombinant vaccines are under pipeline. This article concludes and highlights ongoing advances in drug repurposing, therapeutics and vaccines to counter COVID-19, which collectively could enable efforts to halt the pandemic virus infection.

The paper is devoted to the examination of issues related to the increasing importance of budgetary regulation for the proper functioning of a modern innovative society. The key role of the budgetary regulation in the financial process of the State is particularly acute in the context of systemic crises that include socio-economic consequences caused by the spread of a new coronavirus infection (COVID-19) in Russia. In the course of the study, the features of changes in the state financial policy caused by the above-mentioned crisis phenomena are highlighted. The paper describes various approaches to the interpretation of the budgetary regulation as a category of financial law, explores various types and legal forms of methods of the budgetary regulation, analyses mechanisms and the impact of the State on the budget system through the existing legal structure of the budgetary regulation. It is determined that the rules of financial law governing the whole complex of public relations concerning the distribution and redistribution of the national product between the levels of the budget system of the Russian Federation constitute the institution of financial law, namely: the budgetary regulation. The author concludes that the approach based on the concentration of basic powers in the financial field at the federal level significantly slows down the dynamics of development of economic activity in the majority of regions of Russia, while the need for breakthrough innovative development of Russian society determinates the expansion of long-term tax sources of income for regional budgets. In this regard, it is proposed to consolidate additional regulation for revenues gained by regional and local budgets in the form of targeted deductions from federal taxes on a long-term basis.

Endothelial dysfunction is considered one of the main mechanisms for the further development of arterial hypertension (AH) and its complications, and endothelin-1 (ЕТ-1) plays one of the key roles in this process. Endothelium is drawn into the pathological process at the earliest stages of AH development. ЕТ-1 is currently considered as a marker and predictor of the stage and consequences of AH, chronic heart failure (CHF), coronary heart disease (CHD), in particularly, acute myocardial infarction (MI), heart rhythm disorders, pulmonary hypertension, atherosclerotic vascular injury, target vascular disorders. One factor that may influence the level of ЕТ-1 is the endothelin-1 gene Lys198Asn polymorphism, which is considered by many researchers as a possible genetic marker of AH. However, the effect of this polymorphism on the level of plasma ЕТ-1 level in patients with AH of varying severity in Ukraine is understudied. The basis of this work were the materials of a complete examination of 160 patients with a verified diagnosis of AH I, stage II of 1, 2, 3 degrees (main group) and 110 apparently healthy persons (control group). Patients of the main group were divided into three subgroups: 1st group ­– 75 patients with normal body weight; 2nd group – 48 overweight patients; 3rd group – 37 patients with alimentary obesity. In obese patients, the waist circumference in women was around 103 (96–115) cm, and in men – 108 (105–116) cm, which indicates an abdominal type of obesity in these patients. As a result of the study, it was found that the level of ЕТ-1 is higher in patients with AH who suffer with overweight and obesity as compared with patients with AH and normal body weight. Also, the peptide level depends on the genotype of patients and is higher in AH patients with the genotype Asn198Asn and Lys198Asn as compared to carriers of the genotype Lys198Lys.

Abstract   The COVID-19 pandemics is a global challenge with a huge impact on medicine, politics, economy, education, travel and many other aspects of human life. The treatment of heart rhythm disorders has also been affected by the disease itself and by restrictions in order to constrain the spread of the virus. Catheter ablations of cardiac arrhythmias are nowadays frequently guided by electro-anatomic mapping systems. Technical staff with medical training, or medical staff with technical training, is needed to assist the operator. Travel restrictions due to current COVID-19 pandemics have limited the in person availability for technical support staff. To overcome these limitations we explored the feasibility of remote support with an internet based communication platform. A total of 9 patients (87,5% male, mean age 66,6 years) with different arrhythmias (atrial fibrillation, left atrial flutter, typical right atrial flutter, left ventricular tachycardia), having undergone ablation procedures between October 2020 and February 2021, were included. Acute procedural success was obtained in 9 out of 9 procedures. No complications occurred. Our experience with remote support for electro-anatomic mapping for complex electrophysiological ablation procedures, show the feasibility and safety of this approach. It increases the availability of technical support at reduced costs and a reduced CO2 footprint. Remote support for electro-anatomic mapping may therefore facilitate continuous care for patients with arrhythmias during the COVID-19 pandemics. Due to its advantages beyond COVID-19 pandemics related problems, it will likely play a greater role in the future. FUNDunding Acknowledgement Type of funding sources: None.
