<p class="abstract"><strong>Background:</strong> Vertigo<strong> </strong>or giddiness is a common symptom seen in patients attending ENT OPD. Vestibular and neurological pathologies being the known common causes. Cervical spondylosis is being established as a common condition leading to vertigo. Our aim is to evaluate the significance of cervical spondylosis as a cause in patients with vertigo.</p><p class="abstract"><strong>Methods:</strong> Hundred patients of either sex, between the age group of 23-64 years, with vertigo were evaluated with complete history and clinical examination followed by PTA and impedance audiometry. All common causes of vertigo were ruled out and these patients were screened with digital x-ray cervical spine in lateral view to rule out cervical spondylosis. The results were analysed statistically using Chi square test and inference was drawn.  </p><p class="abstract"><strong>Results:</strong> Out of 100 (100%) patients evaluated, 58 (58%) were female patients and 42 (42%) male patients. Cervical spondylosis was found to be present in 47 (47%) patients.</p><p class="abstract"><strong>Conclusions:</strong> Cervical spondylosis was found as a common finding in patients with vertigo. It can be ruled out easily using a simple investigation of cervical spine x-ray, in patients where other commoner causes cannot be established.</p>

Background: Tuberculosis (TB) and diabetes mellitus (DM) prevalence rates are increasing rapidly, especially in developing countries like Indonesia. There is a relationship between TB and DM that are very prominent, which is the prevalence of pulmonary TB with DM increased by 20 times compared with pulmonary TB without diabetes. Chest X-ray picture of TB patients with DM is atypical lesion. However, there are contradictories of pulmonary TB lesion on chest radiograph of DM patients. Nutritional status has a close relationship with the morbidity of DM, as well as TB.Objectives: The purpose of this study was to determine the relationship between the lesions of TB on the chest radiograph of patients who su?er from DM with their Body Mass Index (BMI) in Hasan Sadikin Hospital Bandung.Material and Methods: The study was conducted in Department of Radiology RSHS Bandung between October 2014 - February 2015. We did a consecutive sampling of chest radiograph and IMT of DM patients with clinical diagnosis of TB, then the data was analysed by Chi Square test to determine the relationship between degree of lesions on chest radiograph of pulmonary TB on patients who have DM with their BMI.Results: The results showed that adult patients with active pulmonary TB with DM mostly in the range of age 51-70 years old, equal to 62.22%, with the highest gender in men, equal to 60%. Chest radiograph of TB in patients with DM are mostly seen in people who are obese, which is 40% and the vast majority of lesions are minimal lesions that is equal to 40%.Conclusions: There is a signifcant association between pulmonary TB lesion degree with BMI, with p = 0.03

Background: Schizophrenia is a clinical syndrome of variable, but profoundly disruptive, psychopathology that involves cognition, emotion, perception and behaviour. Caregiver burden refers to negative response that occurs when the caregiver assumes an unpaid and unanticipated responsibility for the person with disabling mental health problems. The burden varies with socio-demographic parameters of patients and caregivers. Aims & Objectives: To study socio demographic details of schizophrenia patients and caregivers. To assess perceived level of burden in caregivers and it’s association with socio demographic profile of patients and caregivers. Materials & Methods: Ethics committee approval and informed consent was taken. 100 consecutive patients with their primary caregiver, presenting to psychiatry OPD of general hospital, satisfying inclusion criteria, were included in the study. Socio-demographic details of caregiver were collected using semi-structured proforma. Caregiver’s perceived burden was assessed with Burden Assessment Schedule. Data thus obtained, was pooled & analyzed by using SPSS software, descriptive statistics, mean, percentage, Chi- Square test. Results: The sample consisted of 52 female & 48 male patients. Prevalence of burden in caregivers was found to be 52%. The perceived burden was found to be high in caregivers of female patients, patients of older age, less educated, single, and unemployed patients. Majority of caregivers were female and of older age. The burden perceived was found to be significantly high in female caregivers, especially mother.

<p class="abstract">Dysphagia may occur in various pathological, inflammatory diseases of esophagus. It may also occur due to motility disorders of esophagus, benign and malignant diseases of mediastinum, cervical spine diseases. Dysphagia secondary to compression of esophagus by a cervical osteophyte is rare. The most common causes of osteophyte (bony outgrowth) in the cervical spine are diffuse idiopathic skeletal hyperostosis (DISH), ankylosing spondylitis (AS), and cervical spondylosis. Patients with cervical osteophytes are mostly asymptomatic. Hence, when considering cervical osteophytes as a cause of dysphagia other pathologic entities in the esophagus (e.g. tumors, webs, rings, strictures) should be excluded. We present a 68 year female patient who presented with complaints of dysphagia and neck stiffness since 3 months. She has been evaluated and found that dysphagia is due to large anterior cervical osteophytes compressing pharynx at C2/C3 and esophagus at C5/C6 and C6/C7 vertebral levels respectively. The objective of this case report is to emphasize the importance of anterior cervical osteophyte as a cause of dysphagia in elderly.</p>

Abstract Background: Pediatric brain tumors (PBT’s) from previous studies are associated with poor outcomes in our subregion. Methods. An 8 -year single center prospective study. All cases investigated with neuroimaging and treated were enrolled. Data was analyzed with SPSS (Inc) Chicago IL, USA version 23. Chi Square test, One-way Anova and confidence limits were used to evaluate associations using the 95% level of significance. Patients were followed up for a range of 1 to 7.5 years with a mean of 4.9 ±1.3years. Ethical approval was obtained for our study. Results: 95 patients were enrolled, 84 satisfied the study criteria. There were 45 males and 39 females, M: F=1.1. The mean age was 9.9±2.7 years 95%CI with a range of 9 months to 16 years. The most common symptom was headache for supratentorial lesions (73%) and gait disturbance (80.2%) for infratentorial lesions. More tumors were supratentorial in location (45(54.2%), while 33(37.1%) were infratentorial. Craniopharyngiomas (n=19), medulloblastomas(n=17) and astrocytomas (n=11) were the most common tumors. Hemoglobin genotype(AA and AS) had some influence on tumor phenotype, Odds ratio 8.9 and 3.3 for medulloblastoma and craniopharyngioma. 69 cases were microsurgically resected while 14 patients were treated with radiotherapy alone. The 30-day mortality for operated cases is 7.9±1.3%. Overall 1-year and 5-year survival was 67.9% and 53.6 % respectively. Survival rates varied among treatment groups (X2=8.9, P=0.017). Conclusion: Survival profile in this series suggests some improvement in comparison to previous studies from our region.

Hemoglobin genotype (AA and AS) had some influence on tumor phenotype FT, P=0.033. 76 cases were microsurgically resected while 16 patients were treated with radiotherapy alone. The 30-day mortality for operated cases is 7.2±0.7%. Overall 1-year and 5-year survival was 66.7% and 52.3 % respectively. Survival School performance varied among treatment subgroups Conclusion: Survival profile in this series suggests some improvement in comparison to previous studies from our region. ABSTRACT Objective: The purpose of this clinical study was to evaluate the effect of implant body form (cylindrical and conical implants) on crestal bone levels during 6 months' follow-up after loading. Materials and Methods: A total of 32 SPI implants (19 conical implants/13 cylindrical implants) were randomly placed in 12 male patients using a submerged approach. None of the patients had compromising medical conditions or parafunctional habits. Periapical radiographs using the parallel technique were taken after clinical loading and 6 months later. Clinical indices including pocket depth and bleeding on probing (BOP) were recorded on 6-month follow-up. Data were analyzed by independent samples t-test and Chi-square test with a significance level of 0.05. Results: Six months after loading, crestal bone loss was 0.84 (±0.29) mm around the cylindrical implants and 0.73 (±0.62) mm around the conical types, which was not significantly different (P = 0.54). Pocket depth around the cylindrical and conical implants was 2.61 (±0.45) mm and 2.36 (±0.44) mm, respectively (P = 0.13). BOP was observed among 53.8% and 47.4% of the cylindrical implants and conical (P = 0.13). Bone loss and pocket depth in the maxilla and mandible had no significant difference (P = 0.46 and P = 0.09, respectively). Conclusion: In this study, although bone loss and clinical parameters were slightly higher in the cylindrical implants, there was no significant difference between the conical- and cylindrical-shaped implants.

Background: Entanglement of the umbilical cord around the foetal neck (nuchal cord) is a common finding at delivery, but its clinical relevance is not entirely clear. Nuchal cords have generally been considered to be rather benign. However, the insufficiency in data regarding the role of nuchal cord in foetal morbidity and mortality is a source of anxiety and frustration to both parturient and healthcare professionals. Considering the above facts the present study was taken up to establish the incidence of nuchal cord at delivery in the existing setup.Methods: This was a cross sectional comparative study conducted over a period of one year from May 2013 to April 2014. A total of 362 women were selected for the study. These women were briefed about the nature of the study and a written informed consent was obtained. The demographic data and obstetrical history were documented on predesigned proforma. The data obtained was analysed using percentages and comparison done using chi-square test for discrete variables.Results: Incidence of Nuchal Cord was 17.7% among study cases. Based on this study gestational age may have an effect on the presence of nuchal cord (p<0.05). Out of 64 cases with nuchal cord’, 11.4% of cases had meconium stained liquor present. 9.8% of cases with nuchal cord had FHR irregularity, which was significantly less as compared to 20.0% of cases who did not show FHR irregularity. This was statistically significant. (p=0.032). 18.7% of mother who showed presence of nuchal cord had delivery by caesarean section. 22.7% of cases with nuchal cord had NICU admissions, which was insignificantly more as compared to 17.0% of cases who did not have NICU admissions.Conclusions: Intrapartum events such as meconium staining of liquor and foetal heart rate irregularities were more commonly associated with nuchal cord but it did not affect the perinatal outcome. However, the mode of delivery was independent of the presence of nuchal cord, thereby reducing the morbidity to the mother. Hence nuchal cord is not associated with adverse perinatal outcomes and the presence of a nuchal cord per se is not found to be an indication of operative delivery.

Abstrak Sindrom Ovarium Polikistik (SOPK) merupakan kelainan endokrin dan metabolik pada wanita usia reproduksi. SOPK merupakan kumpulan gejala dari amenore, oligomenore, infertilitas, obesitas, hirsutisme, acne, alopesia, dan akantosis nigrikan. Resistensi insulin diyakini sebagai salah satu penyebab tersering dari SOPK melalui berbagai mekanisme. Tujuan penelitian ini adalah untuk mengetahui hubungan resistensi insulindengan gambaran klinis SOPK. Penelitian ini dilakukan pada pasien SOPK dengan menggunakan studi cross sectional dengan pendekatan retrospektif, yaitu mengumpulkan kejadian masa lalu dari tahun 2009 - 2011, jumlah sampel 105 orang. Analisis statistik yang digunakan adalah uji chi-square. Hasil penelitian didapatkan 33,3% penderita SOPK mengalami resistensi insulin. Berdasarkan gambaran klinis 35,23% amenore, 64,77% oligomenore, 72,04% infertilitas, 50,5% obesitas, 0,95% hirsutisme, acne 20%, alopesia dan akantosis nigrikan0%. Dari 33,3% SOPK dengan resistensi insulin 40% amenore, 60% oligomenore, 71,9% infertilitas, 77,14% obesitas, dan 0% hirsutisme. Berdasarkan hasil uji statistik ditemukan hubungan bermakna antara resistensi insulin dengan obesitas (p<0,05) dan tidak ditemukan hubungan bermakna antara resistensi insulin dengan infertilitas, hirsutisme, dan acne (p>0,05).Kata Kunci: SOPK, resistensi insulin, gambaran klinisAbstract Polycystic Ovary Syndrome (PCOS) is an endocrine and metabolic disorders that is common in reproductive-aged women. PCOS is a group of symptoms, such as amenorrhea, oligomenorrhea, infertility, obesity, hirsutism, acne, alopecia, and achanthosis nigricans. Insulin resistance is believed to be one of the most common causes of PCOS through a various mechanisms. The objective of this study was to find out the relationship between insulin resistance and clinical manifestation of PCOS. This research was done in patients with PCOS using cross sectional study with retrospective approach. Data was collected from 2009-2011, with the sample of 105 patients. This research used statistical analysis, that was chi square test. This research found that  33.3% patients of PCOS have insulin resistance. Based on clinical manifestation found that 35.23% amenorrhea, 64.77% oligomenorrhea, 72.04% infertility, 50.5% obesity, 0.95% hirsutism, 20% acne, 0% alopecia and achanthosis nigricans. From 33.3% PCOS with insulin resistance, 40% amenorrhea, 60%  oligomenorrhea, 71.9% infertility, 77.14% obesity, and 0% hirsutism. The results showed that there is a significant relationship between insulin resistance and obesity (p<0.05) and no significant reslationship between insulin resistance and infertility, hirsutism, and acne (p>0.05).Keyword: PCOS, insulin resistance, clinical manifestation

Background: Patients with neck pain are frequently encountered in cervical spine (C-spine) magnetic resonance imaging (MRI) practice. However, the exact distribution and prevalence of cervical abnormalities are not known. Aim: The aim of this study is to evaluate the association between age, gender, and prevalence of abnormal cervical MRI findings. Methods: Records of 111 cervical MRIs were collected in 12 months from January to December 2019 from adults aged 20–89 years who were referred from neurosurgery, neurology, and orthopedic clinics. Findings were classified and analyzed using the Statistical Package for Social Science (SPSS), version 24.0 (IBM, Armonk, NY, USA). The chi-square test was used to determine the association between demographics and abnormalities using a significance of p = 0.05. Results: The majority of patients were female (72.1%). The number of abnormal incidences increased with age until it reached a peak at ages 50–59. Spondylodegenerative changes were the most frequent finding, which was present in 52.2% of the total sample, and was followed by disc bulge (25.2%). Incidences increased in lower discs, with C5–C6 being the most frequent in 65% of the total sample. Younger males in their 20s had more injuries than females of the same age. However, this rate was reversed in patients over 40, as women were the dominant gender among patients in their 40s with cervical injuries, with a rate of 81.5%. Conclusion: In our study, we found that older patients developed more C-spine injuries. Gender may play a role in the rate of incidents. However, we did not find any significant differences between men and women or between different types of abnormalities.

This study examined the effects of X-rays on bovine oocytes used for in vitro embryo production. In recent years, airport security has become more stringent and the use of X-ray screening may be required for all items, which could be problematic when transporting oocytes. Ionizing radiation such as X-rays are high-energy particles that can penetrate into the nucleus of a cell and cause single and double-strand breaks in the DNA chain. Oocytes exposed to X-ray radiation may have DNA damage affecting maturation, fertilization, and embryonic development. Bovine ovaries obtained from a local abattoir were used to collect immature oocytes by follicular aspiration. The oocytes were divided into a control group and an experimental group then placed in maturation medium. The oocytes in the experimental group were placed in a hospital X-ray machine with a single exposure of 100 mA, 120 kVp for 0.3 s. This exposure challenged the oocytes with a comparable, calculated amount of mrems equivalent to a single screening from an airport X-ray machine (30 mrems). After X-ray exposure, the experimental group was cultured with the control at 37°C and 5% CO2. After 18 h, cryopreserved bovine sperm were thawed and processed using BoviPure (Nidacon International, Mölndal, Sweden) density gradient centrifugation and the oocytes were inseminated with 1 × 106 sperm in fertilization medium at 37°C and 5% CO2. After 24 h, both groups were transferred to 50-μL droplets of G1 medium (Vitrolife, Göteborg, Sweden) at 37°C and 6% CO2 and both were transferred into G2 medium after 72 h. Development of the oocytes was scored after an additional 72 h. Results were based on 2 criteria: the percent cleaved (presumptive fertilization) and the percent cleaved that developed to morula and blastocyst stages. The results indicated no significant difference in the percentages of oocytes cleaving after insemination for the control and experimental groups (P = 0.403, chi-square test). However, there was a significant increase (P = 0.037, chi-square test) in the percentages of morulae and blastocysts developing in the control (non-irradiated) v. experimental (X-rayed) groups. The outcome of this study cautions on the potential damage caused to bovine oocytes used for embryo production in vitro when exposed to X-ray electromagnetic radiation during routine air transport screening operations.
