Subject. In recent systematic reviews, the question of the clinical value of cytokines of the interleukin-1 (IL-1) family in chronic periodontitis (CP) has been discussed. Some authors consider them to be reliable biomarkers, while others believe that although they allow differentiating gingivitis and severe CP, the quantity and quality of published works require further research in this area. The aim of the study was to evaluate the clinical value of the level of cytokines of the IL-1 family in chronic mild periodontitis. Methodology. All study participants were divided into two groups: main and control. The main group included 45 patients with mild chronic periodontitis, the control group consisted of 32 practically healthy volunteers without periodontal pathology. The concentration of IL-1α and IL-1β, IL-18, IL-1RA (receptor antagonist) in the oral fluid was determined by the method of multiparameter fluorescence analysis with magnetic microspheres (хMAP-technology, Luminex). Results. In the group of patients with periodontitis, there was an increase in the oral fluid levels of IL-1RA by 3.14 times (p = 0.01), IL-1α - by 2.3 (p = 0.02) in comparison with the control group. The integral characteristic of the clinical value of the parameters (AUC) is quite high for both indicators: IL-1RA ― 0.91 and IL-1α ― 0.94. The content of IL-1β and IL-18 did not change significantly. Conclusions. The study found an increase in the concentration of salivary cytokines of the IL-1 family in CP. Clinical and diagnostic characteristics of such tests as the content of IL-1α and IL-1RA in the oral fluid make it possible to consider them as biomarkers of mild chronic periodontitis.

   This study is planned to find relationship between interleukin-33 (IL-33) with its receptor interleukin-1 receptor 4 (IL-1R4), and assurance IL-33/IL-1R4 proportion as biomarker to atherosclerosisin rheumatoid arthritis (RA) Iraqi females patients with and without dyslipidemia. This study was attempted at Baghdad Teaching Hospital included 60 females patients with RA that were isolated into: 30 patients with dyslipidemia(G2), 30 patients without dyslipidemia(G3) and 30 individual as control group (G1) .Patients were experiencing treatment by methortexiene medication, analyzed by rheumatoid factor(RF) and erythrocyte sedimentation rate( ESR) tests. All patients and control groups age ranged from (30-55) years. The results show an increase in ESR, RF, IL-33, and IL-1R4 levels. In addition to decrease in IL-33/IL-1R4 ratio in the two patients groups when contrasted and control group. The momentum examine inferred that the level of ESR, and  IL-33 in RA Iraqi females  patients with dyslipidemia were higher than that in RA Iraqi females patients without dyslipidemia, while the level of IL-33/IL-1R4 ratio in RA Iraqi females patients with dyslipidemia was lower than that in RA Iraqi females patients without dyslipidemia patients; in this manner the IL-33/IL-1R4 ratio may be used as a biomarker in diagnostic early porn to atherosclerosis in RA females patients with dyslipidemia

Objective: To explore the effectiveness of pain nursing interventions for patients in ENT. Methods: The 200 ENT patients admitted from March 2018 to March 2020 were divided into observation group (n=100) and control group (n=100). The control group was used conventional clinical nursing methods, and the observation group received pain nursing intervention as a comparison with the control group. Results: The observation group had 83 cases, 14 cases and 3 cases of mild, moderate and severe pain, respectively, while the control group are of 43 cases, 43cases and 14 cases. Nursing intervention can significantly reduce the pain of patients after otolaryngology surgery, improve the quality of life of patients, and has high clinical value for application.

Introduction: Oral fluid cytokine levels can vary considerably during the onset of Inflammatory Periodontitis (IP) especially in people with hepatitis C virus (HCV), hepatitis B virus (HBV) and human immunodeficiency virus (HIV). Aim of our study was to evaluate levels of oral cytokines during the onset of IP among HCV, HBV and HIV negative and positive individuals in order to evaluate local immunity state during these infections. Methodology: This was a case control study with 3 groups of virally infected individuals and control group. All had IP including control group. Results: 45 patients (51.7%) had HCV, 18 (20.7%) HBV and 24 (27.6%) HIV. For IL-2 we received significant difference for all groups compared with control -2.83; HBV-31.1 (p < 0.001), HCV-25.99 (p < 0.001) and HIV-24.57 (p < 0.001). For IL-10 significant difference was observed between control -0.94 and HCV-3.63 (p = 0.027), HBV-8.38 (15.51) groups (p < 0.001). IL-4 was significantly higher in control group 14.29 compared to HCV 0.2 (p < 0.001) and HIV 0.21 (p = 0.037) group. The adjusted analysis where we consider age as possible confounder revealed that only IL-2 significantly differs for all groups compared with control group: control vs HCV (p = 0.001); control vs HBV (p = 0.024); control vs HIV (p = 0.004). Conclusions: Evidence for significant differences when comparing oral fluid cytokines of individuals with HCV, HBV and HIV with non-viral individuals was more obvious for IL-2. IL-2 levels were significantly higher in all 3 groups vs non-viral group even when age is confounder.

Objective:Periodontitis is one of the main diseases in the oral cavity that causes tooth loss. The host immune response and inflammatory factors have important role in periodontal tissue. The current study was done with the objective to determine the effect of scaling and root planning on the salivary concentrations of tumor necrosis factor-alpha (TNF-α) and interleukin-1-alpha (IL-1α).Methods:In this quasi-experimental clinical trial, 29 patients with chronic periodontitis and 29 healthy subjects without periodontitis were studied. Clinical examination findings and salivary TNF-α and IL-1α (using ELISA method) were compared before and after scaling, root planning.Results:Before starting treatment, salivary TNF-α and IL-1α concentrations were higher in healthy control group than in periodontitis group (P< 0.05). Non-surgical treatment increased the concentration of these two biomarkers in the saliva. However, increase in IL-1α concentration was not statistically significant (P= 0.056). There was a negative relationship between TNF-α and IL-1α levels with pocket depth and attachment loss (P< 0.05).Conclusion:Scaling and root planning improved periodontal disease indices and salivary TNF-α and IL-1α levels.

Studies on the link between periodontal disease and atherosclerosis have generated conflicting results and the mechanisms underlying this relationship are incompletely understood. Therefore, this study aimed to assess the levels in serum and in gingival crevicular fluid (GCF) of TNF-a, IL-1b and IL-6, to clarify the possible link between periodontitis and hyperlipidemia, as well as the effects of conventional periodontal treatment through scaling and root surfacing on these pro-inflammatory molecules. The study was carried out on a total of 40 subjects divided into two main groups: the study group (n=26) and control group (n=14). The cases included patients with atherosclerosis with prescribed diet (D) or antilipemic therapy with a drug from the statin group (S). Controls (C) were selected from systemically healthy subjects with chronic periodontitis. Samples were performed from crevicular fluid and serum, by determining the initial and post-treatment of TNF-a, IL-1b and IL-6. For all groups there were significant serum decreases in TNF-a, IL-1b and IL-6 from baseline, and the decreases were more significant IL-1b for statin group. Significant decreases were found in the crevicular fluid for all cytokines. The decrease was most evident for IL-6 in the statin group. Combining antilipemic periodontal therapy and treatment can provide beneficial effects on metabolism and control of inflammatory atherosclerosis by lowering serum pro-inflammatory cytokines.

OBJECTIVE: Periodontitis is the second most prevalent microbiome associated inflammatory disease posing a threat to oral health. Nigella sativa (Kalonji) has been used since ancient times as a remedy for oral inflammatory conditions. Interleukin1 (IL-1 ) is critical for periodontal inflammation, collagen degradation and bone turn over. The motive of our study was to determine the change in the levels of salivary IL-1 after the use of Nigella sativa oil to determine if it has any correlation with salivary IL-1 . METHODOLOGY: A parallel-arm triple-blind placebo-based randomized control trial was conducted on a total of ninety three patients. Out of these, fifty individuals with chronic periodontitis were included in the study as per the eligibility criteria. Baseline screening of the participants was done via clinical periodontal parameters such as periodontal pocket depth (PPD), clinical attachment loss (CAL), plaque index (PI) and bleeding on probing (BoP). These individuals were categorized into two groups; 1. Control Group (n=25), which was given normal saline as placebo; 2. Treatment Group, which was given Nigella sativa oil (n=25). All participants underwent scaling and root planing before the start of the trial. The intervention was given for two weeks. Salivary samples were collected on day 0 and day 15 and were evaluated for interleukin-1 levels using ELISA. The statistical interpretation was done using IBM SPSS (version 25.0, SPSS Inc.) on forty participants due to loss to follow up. RESULTS: Levels of salivary interleukin-1 came out to be statistically insignificant after two-week use of Nigella sativa oil. CONCLUSION: No correlation was found between the salivary IL-1 and the use of Nigella sativa oil in patients with chronic periodontitis in contrast to the patients using normal saline. Introduction: Mechanical and chemical debridement had been practiced as one of the management for chronic periodontitis. Recent interest and advances in the field of alternative medicine has promoted the use of various herbal and natural products for multiple uses in the field of medicine. As for it, coconut water is one of the potential natural sources that have various health benefits.  Materials and method:  Randomized control study was chosen as study design in this research project. 12 patients who have moderate to severe chronic periodontitis were selected from patients whom attended Polyclinic Kulliyyah of Dentistry, IIUM Kuantan Campus from November 2016 to January 2018. After stratified random sampling done, the subjects divided into 2 groups; Intervention Group and Control Group. All patients will go for ultrasonic scaling and root planning by using hand instruments. 6 patients in Intervention Group were given fresh coconut water to drink during daytime. The first re-assessment was done after 6 weeks. The control group of another 6 patients didn’t receive any medicament. Paired sample t-test was used to analyse the pre and post intervention data. The study had obtained ethical approval from IREC (IREC 849).  Results:  Probing depth (PD) at mid-buccal site and bleeding on probing (BoP) shown significant result (p<0.05 ) in intervention group. There is no significant difference noted in the control groups, which are for PD, BoP and Plaque Score (PS) variables. Conclusion: Coconut water has potential benefit in reducing the periodontal pockets with chronic periodontitis as an adjunct after mechanical debridement.

ABSTRACT Introduction Micronutrients have been extensively studied in recent years; of these magnesium and iron are essential for a variety of physiologic functions. Activation of neutrophils is an early effect of hypomagnesemia, which, in turn, causes periodontal tissue destruction. Since magnesium also influences blood pressure regulation by vascular tone and reactivity, its altered levels could contribute to the pathophysiology of hypertension. Iron plays a key role in the formation of reactive oxygen species that causes peroxidative damage to tissues. Aims and objectives The present study is designed to estimate the levels of serum iron and magnesium in healthy, chronic periodontitis patients with and without hypertension. Materials and methods This study includes 90 subjects, age ranging from 18 to 65 years. They were categorized into three groups as group I: 30 periodontally healthy, group II: 30 chronic periodontitis, and group III: 30 chronic periodontitis with hypertension. Serum magnesium levels were determined using the modified xylidyl blue reaction method and serum iron levels using ferrozine method without deproteinization via photometry. Results Biochemical parameters showed mean serum iron levels in chronic periodontitis (59.17) to be significantly reduced as compared with the control group (76.90) and slightly increased in the chronic periodontitis with hypertension group (69.63). Serum magnesium levels were found to be significantly lower in the hypertensive group as compared with the chronic periodontitis and control groups. Conclusion Iron and magnesium have a link in the pathophysiology of periodontitis. Serum iron and serum magnesium levels were closely linked to periodontitis and hypertension. Total reflection X-ray fluorescence analysis (TXRF) was used to determine chromium, selenium and bromine concentrations in blood serum samples of 50 patients with parenteral nutrition treatment. The concentrations were measured two times, namely in the first day (I measurement) of the treatment and the seventh day (II measurement) after the chromium and selenium supplementation. For comparison purposes also serum samples of 50 patients without nutritional disorders, admitted to a planned surgical procedure to remove the gall bladder (cholecystectomy), were analyzed and treated as the control group. Descriptive statistics of measured concentrations of Cr, Se and Br both for the studied and control groups was determined. In order to check the effectiveness of Cr and Se supplementation, the results of the first and seventh day measurements for studied group were statistically compared with each other, with literature reference values and with the results of the control group (two-group comparison). These comparisons indicate the effectiveness of selenium supplementation in the applied treatment procedure. In the case of Cr and Br concentrations no statistically significant differences were observed. We conclude that monitoring of the concentration of the important trace elements in human serum should be standard procedure in parenteral nutrition treatment. In this monitoring the TXRF technique can be successfully used.

Abstract Diagnosis of invasive candidiasis (IC) is still challenging due to absence of specific clinical signs and symptoms. In this study we investigate the clinical value of (1,3)-β-D-glucan (BDG), mannan (MN), antimannan immunoglobulin G (AM-IgG), and antimannan immunoglobulin M (AM-IgM) assay in diagnosis of IC. During 2016 to 2018 serum samples from 71 patients with IC and 185 patients without IC were collected. Serum samples from 41 patients with bacteremia were also enrolled as additional control. Significant differences in mean serum biomarkers levels between IC and control group were observed. At low cutoff threshold the sensitivity and specificity of BDG (70 pg/ml), MN (50 pg/ml), AM-IgG (80 AU/ml), and AM-IgM (80 AU/ml) assay were 64.8% and 90.8%, 64.8 and 89.2%,74.6% and 87.0%, 57.7% and 60.0%, respectively. Combined use of BDG/MN, BDG/AM-IgG and MN/AM-IgG improved the sensitivity and specificity to 85.9% and 81.1%, 85.9% and 80.0%, 81.7% and 81.6%, respectively. The combination of BDG/MN, BDG/AM-IgG, or MN/AM-IgG may provide an encouraging approach for diagnosis of IC.
