Introduction: Preeclampsia is described as a systemic disorder and leads to a potentially life threatening disease. The evidences show that stress oxidative of lipid profile is effective in the pathogenesis of Preeclampsia. In this study, specificity and sensitivity of lipid profile as a predictive factor in the incidence of preeclampsia was studied. Methods: This study was a cohort study that conducted on pregnant women referring to hospitals from 2016 to 2017. In this study, 244 prim gravid women with normal blood pressure in the weeks of 16-18 were included and a lipid profile test wasrequested. Finally, it was followed the cases for the incidence of preeclampsia till the end of pregnancy. In this study, the SPSS 20 software was utilized. Moreover, the Chi-Square, Covariance and independent t tests was run. Results: During this study, 16 cases developed preeclampsia. In preeclamptic cases it was found considerable changes in lipid profile such as significant increase in LDL and TC, and significant decrease in HDL (P-value < 0.001). Likewise, it was not found considerable changes in TG (P-value=0.21). Furthermore, TC with sensitivity of 68.8 % and specificity of 69.6 %, LDL with sensitivity 75 % and specificity of 75.8 % and HDL with sensitivity of 76.2 % and specificity of 75 % were reported. Conclusion: The findings in our study suggested that cases that developed preeclampsia had disturbed lipid profile and the best predictive factors are HDL, LDL and TC, respectively.

Abstract Delirium is a serious and potentially life-threatening problem, but it remains clinically under-recognized. Various factors contribute to this under-recognition, including limited understanding of delirium, insufficient training and application of delirium assessments, potential stigma for the patient and increased workload for the clinician. As a part of an NIH funded study testing a rapid two-step delirium identification protocol at two hospitals in the U.S. (one urban and one rural), clinicians completed a 12-item survey to assess their knowledge and attitudes about delirium and their confidence in preventing and managing delirium. Survey response options followed a 5-point rating scale (strongly disagree, disagree, undecided, agree, strongly agree). The sample for this analysis included 399 clinicians (MDs=53; RNs=235; CNAs=111). Chi-square was used to test for group differences between clinician types. Less than half of the clinicians reported agreeing with the statement, “delirium is largely preventable” (MDs: 47%; RN: 44%; CNA: 41%, p-value=0.021). MDs and RNs indicated a high level of confidence in recognizing delirium while CNAs endorsed lower levels of confidence (MDs: 87%; RN: 81%; CNA: 65%, p-value=0.001). All types of clinicians reported lower confidence in managing delirium (MDs: 29%; RN: 36%; CNA: 44%, p-value=0.117). 47% of CNAs and 37% of RNs agreed there is a need for additional training in caring for persons with delirium while only 21% of MDs agreed (p = 0.031). Understanding how different types of clinicians think and feel about delirium will inform training and communication initiatives, clinical implementation, and research on best practices for delirium identification and management.

High blood pressure is defined as systolic blood pressure that is equal to or above 140 mm Hg or diastolic blood pressure equal to or above 90 mm Hg (JNC VIII, 2013). In 2018, the prevalence of hypertension in Indonesia is based on the characteristics of the age 45-75 years and above with an average of 58.33% (Riskesdas, 2018). The aimed of this study was to determine of fiber intake, stress levels, and physical activity with blood pressure in pre elderly and elderly at RW 03 Lubang Buaya and RW 09 Kampung Tengah, East Jakarta. This research method is observational with cross sectional approach followed by 80 respondents with simple random sampling technique Data collection was taken, namely blood pressure measurement using a Sphygmomanometer, fiber intake using the Food Recall form 2 x 24 hours (Weekend and Weekday), stress levels with the DASS-14 questionnaire, and physical activity with the Baecke questionnaire. Data processing was analyzed by univariate and bivariate using Chi-Square test. The results of bivariate analysis with chi-square test showed a significant relationship between fiber intake (p value = 0.007), stress level (p value = 0,000), and physical activity (p value = 0.022) with blood pressure. There is a relationship between fiber intake, stress level, and physical activity with blood pressure in the elderly and elderly in Lubang Buaya and Kampung Tengah.

Background: Aromatase inhibitors (AIs) represent an effective endocrine treatment for hormone receptor-positive postmenopausal breast cancer patients with early stage or metastatic disease.Objective: Assessment of Cardiotoxicity in Hormone positive Postmenopausal Breast Cancer Patients receiving AIs (upfront orswitch therapy).Methods: This cross sectional study included 123 postmenopausal breast cancer patients presented to the Clinical Oncology Department, Ain Shams University (Cairo, Egypt) in the interval from August 2016 to June 2017 with hormone receptor positive receiving Aromatase Inhibitors, To assess cardiotoxicity in these patients, they were subjected to blood pressure and lipid profile measurement, electrocardiography (ECG), and electrocardiography (ECHO) and classified into patients had Nolvadex then A.I (arm 1) and others had upfront A.I (arm 2).Results: The age of patients ranged from 41 years to 85 years with mean age of 61 years. Seventy one patients (57.7%) showed cardiotoxicity as assessed by ECHO. They showed significant correlation with rising age above 62 years, IHD, history of HTN and DM (p value: .001, .001, .017 and 0.035 respectively). However, correlation between cardiotoxity and blood pressure changes, lipid profile changes and ECG findings and ECHO changes in switch therapy and upfront A.I were not statistically significant (p value = .275, .116, .081 and .761 respectively).Conclusion: Assessment of cardiotoxicity in hormone positive postmenopausal breast cancer patients receiving Aromatase Inhibitors showed evidence of cardiotoxicity in half the patients (57.7%) as detected by ECHO only. They showed statistically non significant correlations either recievied switch therapy or upfront A.I.

Background: Abnormalities of the heart and blood vessels marked by an increase in blood pressure are known as hypertension. Worldwide, high blood pressure is estimated to affect more than one in three adults aged 25 years and over, or about one billion people. Overall, high-income countries have a lower prevalence of hypertension (35% of adults) than low and middle-income groups (40% of adults). The prevalence of hypertension in Indonesia based on the Basic Health Research (Riskesdas) data in 2018 was 34.1%. Objective: This study determines the relationship of family support, social support, and health worker support with compliance of hypertensive patients. Methods: This research was conducted at the Tegal Gundil Community Health Center, North Bogor District, Bogor City, Indonesia, which was conducted from May to December 2017. The design of this study was cross-sectional. Sampling was carried out with a purposive sampling method with inclusion and exclusion criteria. The former consisted of all hypertensive patients aged 25-65 years who routinely control during the last six months who are present and willing to be interviewed during the study. The later includes hypertensive patients who have memory disorders with the number of samples of 110 respondents. Results: The results showed the respondents were dominated by the age of under 59 (75.5%), female (86.4%), junior high school as the highest education (68.2%), have no job (81.8%), and the prevalence of their sufferers’ compliance was 47.3%. The results of the Chi-square test showed that family support, social environment, and health workers were associated with compliance in hypertensive patients (OR = 2.461; CI 95% 1.140 to 5.310; P Value = 0.034). Conclusion: In order to improve compliance of patients with hypertension, it is necessary to pursue a program of activities focused on health promotion activities not only for patients but also involving family and social members.

Introduction Data obtained from the working area of Puskesmas Tumpaan found that hypertension in the elderly was the first prominent disease in the range of January-December 2017, as many as 465 patients and in 2018 the number of patients was 510. Based on preliminary studies of elderly people with high blood pressure, said their blood pressure often increases because they do not adhere to a diet. Lack of support from family members can influence the compliance behavior of implementing a hypertension diet. The purpose of this study is to find out the relationship between family support and adherence to the diet of elderly people with hypertension in the work area of the Puskesmas Tumpaan in the coastal areas and and mountain areas. This type of research is Correlation Analytic with cross sectional design. The population in this study is the elderly with hypertension. The sampling technique uses proportional random sampling with a total of 84 people divided into 42 people in coastal areas and 42 people in mountain areas and data analysis techniques using the Chi Square test. The results showed that in the coastal areas 31% of respondents with good family support, 40% of respondents had adequate family support and 29% of respondents had less family support while 52% showed adherent diet adherence and 48% adherence to non-adherent diet. The results of the chi-square statistical test (Pearson chi-square) obtained a P-value of 0.008, meaning that the P-value <0.05. The results of the study in the mountain areas of 40% of respondents with good family support, 31% of respondents with adequate family support and 29% of respondents with less family support, 57% showed adherent diet adherence and 43% of adherent diet adherence. The results of the chi-square statistical test (Pearson chi-square) obtained a P-value of 0.021, meaning that the P-value <0.05. The conclusion of this study is that there is a relationship of family support with adherence to the diet of elderly patients with hypertension in both coastal and mountain areas.

Preeclampsia is a multisystem complication that occurs after 20 weeks of gestation. In recent years there has been no significant decrease in the incidence of preeclampsia, even in developed countries it is still the main cause of maternal and fetal morbidity and mortality. This study aims to analyze differences in individual characteristics, lifestyle (physical activity), nutritional status and diet in pregnant women with a history of preeclampsia. This research design is descriptive analytic with cross sectional approach. Samples were obtained from 55 pregnant women with consecutive sampling who met the inclusion criteria. Data analysis used chi-square test and spearman correlation test. The results showed a relationship between physical activity with systolic blood pressure (p value 0.001, r = 0.449) and diastolic (p value 0.43, r = 0.273), there was also a relationship between diet and risk factors for preeclampsia through blood pressure measurements as evidenced by increase in blood pressure (p value 0.000) in pregnant women in the working area of Puskesmas Kedungkandang Malang. The results of this study are expected to be used as a reference in intervening pregnant women at risk of preeclampsia, so that the disease does not develop more severely so that it does not endanger the lives of the mother and the fetus she is carrying

Abstract: According to Riskesdas in 2010, the prevalences of nutritional status based on BMI at age group 13-15 in North Sulawesi were 0.7% very skinny; 5.3% thin; 90, 5% normal; and 3.4% obese. In Indonesia, the incidences of hypertension in adolescents varies from 3.11% to 4.6%. BMI has a strong relationship with blood pressure; BMI >95th percentile was strongly associated with increased blood pressure >90th percentile. This study aimed to determine the relationship of the nutritional status and blood pressure. The study was conducted in SMPN 1 Bitung. This was an analytic obsevational study with a cross-sectional design. Samples were 105 students aged 11-14 years, obtained by using simple random sampling. The nutritional status was defined as independent variables and the blood pressure as the dependent variable. Data were analyzed by using a chi-square test. The results showed that the nutritional status of students were underweight 10.5%, normal 71.4%, overweight 13.3%, and obese 4.8%. The normal blood pressure were found in 89.5%; high normal blood pressure 8.6%; and hypertension 1.9%. The chi-square test results showed a significant relationship between the nutritional status and the blood pressure (P = 0.001). Conclusion: There was a significant relationship between the nutritional status and the  blood pressure  among the junior high school students in Bitung. Keywords: blood pressure; nutritional status.     Abstrak: Berdasarkan laporan Riskesdas tahun 2010, prevalensi status gizi berdasarkan IMT pada kelompok usia 13-15 tahun di Provinsi Sulawesi Utara menunjukkan angka 0,7% sangat kurus, 5,3% kurus, 90,5% normal, dan 3,4% gemuk. Di Indonesia, angka kejadian hipertensi pada remaja bervariasi dari 3,11% sampai 4,6%. IMT mempunyai hubungan yang kuat dengan tekanan darah, yaitu IMT >persentil 95 berhubungan kuat dengan peningkatan tekanan darah >persentil 90. Penelitian ini bertujuan untuk mengetahui hubungan status gizi dan tekanan darah. Penelitian dilakukan di SMP N 1 Bitung. Penelitian ini menggunakan metode survei analitik observasional dengan desain potong lintang, dengan jumlah sampel 105 siswa berusia 11-14 tahun, yang diambil dengan simple random sampling. Status gizi ditetapkan sebagai variabel independen dan tekanan darah sebagai variabel dependen. Uji statistik yang digunakan ialah chi-square. Untuk status gizi di dapat underweight 10,5%, normal 71,4%, overweight 13,3%, dan obes 4,8%. Untuk tekanan  darah didapatkan tekanan darah normal 89,5%, normal tinggi 8,6%, dan hipertensi 1,9%. Hasil uji chi-square menyatakan adanya hubungan bermakna antara status gizi dan tekanan darah (P = 0,001). Simpulan: Terdapat hubungan bermakna antara status gizi dan tekanan darah. Kata kunci: status gizi, tekanan darah.

Stroke is a cerebrovascular disease that is often found in developed countries and currently also presents in developing countries, and one of them is in Indonesia. The high prevalence of stroke in Indonesia is affected by several risk factors such as obesity, lack of physical activity, unhealthy diet, smoking, high blood pressure, elevated blood sugar, and elevated blood lipid profile. The purpose of this study was to analyze the relationship between predictors of hemorrhagic stroke such as obesity, blood pressure, blood sugar and blood lipid profile with mortality in hemorrhagic stroke patients in RSD dr. Soebandi Jember. This research was a quantitative research with a retrospective approach. The number of samples was 101 medical record data. Univariate analysis was used to identify the description of each variable, a chi-square test was applied to analyze the correlation between variables, and the logistic regression analysis was applied to identify the most dominant factor influencing mortality of hemorrhagic stroke. The result of bivariate test showed p-value of obesity= 0,039, p-value of blood pressure= 0,478, p-value of blood sugar= 0,04, p-value of blood lipid profile= 0,026. Logistic regression obtained p-value for obesity= 0.043, OR = 2.689; p-value of blood sugar= 0,042, OR = 2,656; p-value of blood lipid profile= 0.069, OR = 3,749. There was a significant association between obesity, blood sugar, and blood lipid profile with mortality in hemorrhagic stroke patients. Obesity and blood sugar are predictors that can be used to predict mortality in hemorrhagic stroke patients, where obesity is the most dominant factor affecting mortality in hemorrhagic stroke patients.

Abstract Introduction: It is imperative that efforts are to be made to recognize individuals with metabolic syndrome from different sets of population so that they may be identified for intensive risk factor management to reduce cardiovascular risk. We evaluated the incidence and associated factors of metabolic syndrome in inmates of old age homes. Methods: This study involved 189 subjects aged between 55-75 years of either sex housed in old age homes. In every subject waist circumference, blood pressure, fasting blood sugar and plasma lipids were measured. Ongoing treatments were taken into account. Patients with metabolic syndrome were identified using IDF-2005 definition. Data analysis was done by Chi -square test. p value less than 0.05 was taken as significant. Result: Metabolic syndrome was found in 57.67% of study subjects. Prevalence of metabolic syndrome was higher in 55 - 60 years age group (x2 =17.06, p < 0.01) and in females (x2 = 4.33, p < 0.05). Disease load of dyslipidemia, diabetes mellitus and hypertension was higher in metabolic syndrome group compared to non-metabolic syndrome group (x2 = 11.40, p<0.001; x2 = 47.90, p<0.0001; x2 = 4.72, p<0.05 respectively). In patients receiving treatment for diabetes and hypertension, frequency of subjects with hyperglycemia and high blood pressure was higher in metabolic syndrome group compared to non-metabolic syndrome group (x2 = 10.69, p<0.01; x2 = 4.46, p<0.05 respectively). Conclusion: In this population it is found that, 55-60 years age group and females are at greater risk for metabolic syndrome. Subjects with metabolic syndrome are resistant to anti-diabetic and antihypertensive treatment.

Hypertension is a disorder of the circulatory system, when the systolic blood pressure is 140 mm Hg and the diastolic blood pressure is 90 mm Hg. The causes of hypertension are heredity, age, gender, obesity (overeating), lack of exercise, stress, excessive salt consumption, other influences: caffeine, smoking, alcohol consumption, taking drugs. The purpose of this study was to determine the relationship between the level of knowledge and lifestyle of hypertensive patients with stroke prevention efforts at the Binjai Village Health Center Medan in 2020. The type of research conducted was descriptive analytic with a cross sectional design. The population in this study were all hypertensive patients at the Binjai Village Health Center Medan with a sample of 87 people using purposive sampling technique. The results showed that the majority of respondents had less knowledge as many as 42 people (48.3%) and an unhealthy lifestyle as many as 67 people (77.0%). Based on the chi-square test, it was found that there was no relationship between the level of knowledge and the prevention of stroke in hypertension patients at the Binjai Village Health Center in 2020 with a p-value = 0.395 > = 0.05 and there was no relationship between lifestyle and stroke prevention in hypertension patients in Binjai Village Health Center in 2020 with a p-value = 0.516 > =0.05. The conclusion in this study is that there is no relationship between the level of knowledge and lifestyle of hypertensive patients with stroke prevention efforts. It is recommended for people with hypertension to further increase their knowledge about prevention of stroke due to uncontrolled blood pressure and pay more attention to their health, especially in early examinations and it is hoped that the puskesmas will be more active in inviting hypertension sufferers to do exercise every week and make early stroke introduction counseling.   Abstrak Hipertensi adalah suatu gangguan pada sistem peredaran darah, yaitu apabila tekanan darah sistol sebesar 140 mmHg dan diastol sebesar 90 mm Hg. Penyebab hipertensi adalah keturunan, umur, jenis kelamin, kegemukan (makan berlebih), kurang olahraga, stress, konsumsi garam berlebih, pengaruh lain : kafein, merokok, konsumsi alkohol, minum obat-obatan. Tujuan penelitian ini adalah untuk mengetahui hubungan tingkat pengetahuan dan gaya hidup penderita hipertensi dengan upaya pencegahan stroke di Puskesmas Desa Binjai Medan Tahun 2020. Jenis penelitian yang dilakukan adalah diskriftif analitik dengan desain cross sectional. Populasi dalam penelitian ini adalah seluruh pasien hipertensi di Puskesmas Desa Binjai Medan dengan jumlah sampel 87 orang dengan menggunakan teknik Purposive sampling. Hasil penelitian menunjukkan mayoritas responden memiliki pengetahuan kurang sebanyak 42 orang (48,3%) dan gaya hidup yang kurang sehat sebanyak 67 orang (77,0%). Berdasarkan uji chi-square didapatkan tidak ada hubungan hubungan tingkat pengetahuan dengan pencegahan stroke pada penderita hipertensi di Puskesmas Desa Binjai Tahun 2020 dengan nilai p-value = 0,395 > α=0,05 dan tidak ada hubungan gaya hidup dengan pencegahan stroke pada penderita hipertensi di Puskesmas Desa Binjai Tahun 2020 dengan nilai p-value = 0,516 > α=0,05. Kesimpulan dalam penelitian ini yaitu tidak ada hubungan tingkat pengetahuan dan gaya hidup penderita hipertensi dengan upaya pencegahan stroke. Disarankan bagi penderita hipertensi untuk lebih meningkatkan pengetahuannya tentang pencegahan stroke akibat tekanan darah yang tidak terkontrol dan lebih memperhatikan kesehatannya khususnya dalam pemeriksaan dini dan diharapkan bagi pihak puskesmas untuk lebih aktif mengajak para penderita hipertensi untuk melakukan senam setiap minggunya dan membuat penyuluhan pengenalan stroke secara dini.
