Background: The decreased level of estrogen during menopause may be one of the risk factors of periodontal disease. The influence of estrogen to periodontal tissue disturbance is mediated by the presence of estrogen receptor on tissue. The precise mechanism how the estrogens mediate this effect is still unclear. Purpose: The aim of this study was to determine the correlation between estrogen receptor α and ß on the periodontal pocket of women who had severe chronic periodontitis measured based on the periodontal pocket depth. Methods: Twenty four periodontitis patients from menopausal and productive women according to the criteria were examined upon her periodontal status and immunoexpression of estrogen receptor α and ß on their periodontal pocket wall. Results: The result showed that in the menopausal and productive women, immunoexpression of estrogen receptor α and ß was not correlated with the periodontal pocket depth (p>0.05). However, the pocket depth seemed to show higher correlation with immunoexpression of estrogen receptor α than that with estrogen receptor ß, r=0.37 vs. r=0.12 for menopausal women, and r=41 vs. r=0.11 for productive women. Conclusion: It was concluded that no significant correlation was found between the estrogen receptor and periodontal pocket depth both on menopausal and productive women, presumed that estrogen has little role in the severity of periodontitis based on periodontal pocket depth. However, the estrogen receptor α has valuable effect on the severity of periodontal disease more than the estrogen receptor ß.Latar belakang: Berkurangnya kadar estrogen pada masa menopause merupakan salah satu faktor resiko penyakit periodontal. Peran estrogen dalam kerusakan jaringan periodontal dimediatori oleh reseptor estrogen α dan ß yang terdapat dalam jaringan. Akan tetapi, mekanisme estrogen mempengaruhi efek ini sampai saat ini masih belum jelas. Tujuan: Penelitian ini bertujuan untuk menentukan korelasi antara reseptor estrogen pada poket periodontal wanita menopause penderita periodontitis kronis dengan keparahan periodontitis yang ditentukan berdasarkan kedalaman poket. Metode: Dilakukan pemeriksaan status periodontal dan immunoekspresi reseptor estrogen α dan ß pada dinding poket periodontal dari 24 wanita menopause dan belum menopause penderita periodontitis yang sesuai dengan kriteria yang telah ditetapkan. Hasil: Hasil mendapatkan bahwa immunoekspresi reseptor estrogen α dan ß tidak berkorelasi dengan kedalaman poket periodontal wanita menopause dan belum menopause (p>0,05). Meskipun demikian, kedalaman poket periodontal tampak lebih berkorelasi dengan reseptor estrogen α daripada dengan reseptor estrogen ß, dengan nilai r=0,37 vs r=0,12 pada wanita menopause, dan r=0,41 vs r=0,11 pada wanita belum menopause. Kesimpulan: Dapat disimpulkan bahwa tidak terdapat korelasi yang signifikan antara reseptor estrogen dan kedalaman poket periodontal wanita menopause dan belum menopause sehingga diduga bahwa estrogen mempunyai sedikit pengaruh pada keparahan periodontitis. Meskipun demikian, reseptor estrogen α tampak lebih berperan terhadap keparahan penyakit periodontal dibandingkan reseptor estrogen ß.

Background. The aim of our study was to assess the subgingival profile of 9 periodontal pathogens, by means of real-time PCR, in a group of predialysis chronic kidney disease patients with and without periodontal disease and to identify the risk factors associated with periodontal disease in these patients.Material and Methods. This is a single centre cross-sectional cohort study performed on 70 CKD patients. Patients received a full-mouth periodontal examination and the following parameters were assessed: periodontal pocket depth (PPD), clinical attachment level, bleeding on probing, and plaque index; subgingival biofilm samples were collected from the deepest periodontal pocket of each quadrant and were pooled in one transporting unit. Clinical data were drawn from the medical file of the patients.Results.T. denticola(P=0.001),T. forsythia(P<0.001), andP. micros(P=0.003) are significantly associated with periodontal disease in CKD subjects but in a multivariate model only age andT. forsythiaremain independent risk factors for periodontal disease in patients with CKD.Conclusions. In our cohort, age andT. forsythiaare independently associated with periodontitis in CKD patients. Within the limits of this study, CKD was not significantly associated with a particular subgingival periodontal pathogens profile in periodontitis patients.

Abstract: Periodontal disease has still a high prevalence in the community. It occurs due to the accumulation of plaque or calculus on the tooth surface. The plaque, a thin layer of biofilm, contains a collection of pathogenic microorganisms that cause periodontal tissue and bone disease. The initial treatment of periodontal disease is to eliminate the etiological factors, namely scaling, root planning, and curettage. Curettage is a mechanical cleaning action in periodontitis therapy to remove inflammatory tissue, calculus, and bacterial colonies. This study was aimed to discuss the management of gingival curettage treatment in a 26-year-old female patient who had gingivitis with a 4 mm deep periodontal pocket in her maxillar left premolar, treated with scaling and gingival curettage. Clinical further examination after treatment revealed that the gingiva looked normal without any swelling, and the pocket depth had decreased by about 2 mm.Keywords: periodontal disease, periodontal pocket, gingival curettage Abstrak: Prevalensi penyakit periodontal masih cukup tinggi di masyarakat. Penyakit periodontal diawali ketika plak atau kalkulus terakumulasi pada permukaan gigi. Plak merupakan lapisan tipis biofilm berisi kumpulan mikroorganisme patogen yang menyebabkan kerusakan jaringan periodontal dan tulang. Perawatan awal pada penyakit periodontal ialah dengan menghilangkan faktor etiologi yaitu dengan scalling, root planning, dan kuretase. Kuretase merupakan tindakan pembersihan secara mekanis dalam terapi periodontitis untuk menghilangkan jaringan inflamasi, kalkulus, dan koloni bakteri. Studi ini bertujuan untuk membahas tentang penatalaksanaan perawatan kuretase gingiva pada seorang pasien perempuan berusia 26 tahun yang mengalami gingivitis dengan poket periodontal sedalam 4 mm pada gigi premolar sebelah kiri rahang atas, yang kemudian dilakukan perawatan scalling dan kuretase gingiva. Hasil pemeriksaan klinis perawatan mendapatkan gingiva sudah tampak normal, tidak ada pembengkakan seperti kondisi sebelum kuretase dan kedalaman poket telah berkurang sekitar 2 mm.Kata kunci: penyakit periodontal, poket periodontal, kuretase gingiva

The prevalence of periodontal disease in Indonesia for all age groups has reached 96.58%. Periodontal disease has been identified as a consequence of diabetes mellitus (DM). The purpose of this study is to determine the differences of periodontal tissues in type 2 diabetes and non-diabetic patients based on CPITN index (Community periodontal index of Treatment index). This study employed case control design. The population of the study consisted of 3.544 visits in the Internal Disease Polyclinic of RSUD Cimahi Cibabat in January 2015. The samples for case group were 50 people (diabetes mellitus type 2) and control group were 50 people (non-DM). The sampling technique used accidental sampling. The study was conducted in 2015 by conducting measurement of periodontal pocket depth using CPITN index. The analysis of data was carried out with independent T test. The results show that the average pocket depth based on CPITN index in the case group of type 2 DM (4.26) is greater than control group of patients without DM (3.14). There is different condition on the periodontal tissue in the group of type 2 diabetes (value-p = 0.002 <0.05). The awareness in increasing the oral health protection is done by providing consultation related to blood glucose.

The link between immune mediated rheumatic disorders and oral health, particularly periodontal disease, is widely accepted, based on shared immune and inflammatory processes as well as local (articular, gingival) damage mediated by similar pro-inflammatory cytokine and destructive mediators. We aimed to evaluate periodontal status in psoriatic arthritis (PsA) before and after 24-weeks treatment with TNF inhibitors and to identify potential relation between disease activity, inflammatory parameters, therapeutic response and chronic periodontitis. Patients were prospectively assessed according to a standard protocol comprising a complex rheumatologic (PsA activity, inflammatory prolife) and dental evaluation (plaque and gingival index, bleeding on probing, periodontal pocket depth, clinical attachment level). Up to one third PsA presented with moderate to severe periodontitis at baseline, with high prevalence of sites with dental plaque, abnormal bleeding, increased periodontal pocket depth and clinical attachment loss. Higher levels of inflammatory parameters were described in the subset of PsA presenting with aggressive periodontal diseases, while significant correlation between dental pathology and CRP (p[0.05). A significant improvement in both PsA-related parameters and periodontal status was demonstrated after 24 weeks of anti-TNF therapy (p[0.05). Periodontal disease may develop in PsA and should be commonly evaluated, particularly patients with active disease. Benefits of TNF inhibitors, with significant response in articular and periodontal parameters, suggest common inflammatory pathways in both entities.

Introduction: Various chemical agents such as nonsteroidal, anti-inflammatory drugs and antimicrobial agents has gained popularity in treatment of periodontal disease but simultaneously lead to condition such as drug resistance and drug allergy. Hence , the topical application of herbal agents such as propolis, aloevera, green tea extracts, Neem reduces the potency and effectiveness to prevent progression of periodontal disease. NanoBioFusion(NBF)gel contains the natural antioxidant power of propolis,vit C,vit E which allows the ultrafine antioxidant to surpase the moist intraoral environment to enter the cells and rejuvenate,revitalize,support,protect and optimize gum and soft oral tissue.Hence the present study is aimed to evaluate the clinical effect of locally delivered NBF gel as an adjunctive therapy to scaling and polishing in the treatment of Periodontitis. Materials and Methods: Chronic Periodontitis patients with 40 sites and probing pocket depth (PD) between 5 and 7 mm were selected in a randomized controlled clinical trial. SRP was performed in both control and test group followed by NBF gel application in 40 sites. The plaque index, gingival index and probing Pocket depth,were recorded at baseline, 6 weeks, and 3 months.The statistical analysis with paired t‑test was used to compare the test and control sites. Results: From baseline to a period of 3 months, a statistically significant difference was seen between both groups for Pocket probing depth and from baseline to 6 weeks the mean GI and PI score have a statistically significant result was obtained(P=0.01& 0.00). Conclusions: Locally delivered NBF gel exhibited a significant improvement compared with SRP alone in chronic periodontitis.

Introduction: Bronchial asthma patients use inhalation drugs as therapy to achieve a controlled asthma state. Inhaled corticosteroids and β2 agonists are the medication that is being used in this case. Prolonged use of inhalation drugs will affect the health of periodontal tissue. The effect of these drugs can reduce the periodontal defense mechanism and increase the risk of periodontal disease. This study aims to determine the periodontal clinical features of bronchial asthma patients using inhalation drugs. Method: This research was a descriptive study. The sample of the study were 22 females and 8 males bronchial asthma patients at the Community Lung Health Center of Bandung (Balai Besar Kesehatan Paru Masyarakat Bandung/BBKPM Bandung) who had been using inhalation drugs for at least one year. The periodontal clinical feature was assessed by employing a Community Periodontal Index (CPI) modified with probing the gingival sulcus to assess the presence of gingival bleeding, periodontal pocket, and loss of clinical attachment where then a periodontal status was obtained. The periodontal status data were then processed using the periodontitis case definitions from the Centers for Disease Control and Prevention/American Academy of Periodontology (CDC/AAP) to determine periodontal disease in asthmatics patients. Results: Clinical features of gingival bleeding were found in 26 asthmatic patients (86.7%), periodontal pockets in 18 asthmatic patients (60%), and loss of clinical attachment in 27 asthmatic patients (90%). Based on the CDC/AAP case definition, 23 asthmatic patients (76.7%) had periodontitis. Bronchial asthma patients who used inhalation drugs had a risk of periodontitis.Conclusion: Bronchial asthma patients who used inhalation drugs for at least one year had gingival bleeding and loss of clinical attachment and periodontal pocket.

This study examined the prevalence of highly and minimally leukotoxic Actinobacillus actinomycetemcomitans in patients with periodontal disease. Pooled subgingival plaque samples from 136 patients with some form of periodontal disease were examined. Subjects were between 14 and 76 years of age. Clinical examinations included periodontal pocket depth (PD), plaque index (PI) and bleeding index (BI). The obtained plaque samples were examined for the presence of highly or minimally leukotoxic A. actinomycetemcomitans strains by the polymerase chain reaction (PCR). Chi-square and logistic regression were performed to evaluate the results. Forty-seven subjects were diagnosed with gingivitis, 70 with chronic periodontitis and 19 with aggressive periodontitis. According to chi-square there was no significant correlation detected between PD (chi2 = 0.73), PI (chi2 = 0.35), BI (chi2 = 0.09) and the presence of the highly leukotoxic A. actinomycetemcomitans. The highly leukotoxic A. actinomycetemcomitans strains were correlated with subjects that were 28 years of age and younger (chi2 = 7.41). There was a significant correlation between highly leukotoxic A. actinomycetemcomitans and aggressive periodontitis (chi2 = 22.06). This study of a Brazilian cohort confirms the strong association between highly leukotoxic A. actinomycetemcomitans strains and the presence of aggressive periodontitis.

Background: Plasma glutathione peroxidase (eGPx) is an important selenium containing antioxidant in human defense against oxidative stress. While crevicular fluid (GCF) eGPx levels and its association with periodontal disease is well documented, there is no data on correlation of GCF and serum eGPx levels in chronic periodontitis. Hence this study was undertaken to further probe into the role of oxidative stress in periodontal diseases and effect of nonsurgical periodontal therapy (NSPT) by correlating GCF and serum levels of eGPx.Materials and methods: Thirty subjects (16-Males and 14-Females; age: 30–38 years) participated in the study. The subjects were divided, based on gingival index, probing pocket depth and clinical attachment level into: Healthy (group-1,n=10), Gingivitis (group-2,n=10) and Periodontitis (group-3,n=10). Chronic periodontitis patients after NSPT constituted group 4. GCF and serum samples collected from each subject were quantified for eGPx levels using Enzyme linked Immunosorbent Assay.Results: The mean eGPx concentrations increased from health (14.01 ng/μl and 78.26 ng/ml) to gingivitis (22.86 ng/μl and 90.44 ng/ml) and then to periodontitis (29.89 ng/μl and 103.43 ng/ml), in GCF and serum respectively. After NSPT, there was statistically significant reduction in eGPx concentration in GCF and serum (19.41 ng/μl and 85.21 ng/ml). Further, all the GCF eGPx values showed a positive correlation to that of serum eGPx level.Conclusion: Thus, increased eGPx concentration in GCF can be considered as an indicator of local increase in oxidative stress. While, increase in serum eGPx levels indicates that periodontal disease can also lead to increased oxidative stress at the systemic level.

This paper reports estimates of the periodontal status of US population derived from data from Phase 1 of the Third National Health and Nutrition Examination Survey conducted by the National Center for Health Statistics in collaboration with the National Institute of Dental Research from 1988 to 1991. A total of 7,447 dentate individuals 13 years of age and older, representing approximately 160.3 million civilian non-institutionalized Americans, received a periodontal assessment. Measurements of gingival bleeding, gingival recession level, periodontal pocket depth, and calculus were made by dental examiners. Assessments were made at the mesiobuccal and mid-buccal sites of all fully erupted permanent teeth present in two randomly selected quadrants, one maxillary and one mandibular. All data were weighted and standard errors calculated by special software to adjust for the effect of sample design. Although over 90% of persons 13 years of age or older had experienced some clinical loss of attachment (LA), only 15% exhibited more severe destruction (LA ≥ 5 mm). Prevalence of moderate and severe LA and gingival recession increased with age, while prevalence of pockets ≥ 4 mm or ≥ 6 mm did not. These data suggest that the increasing prevalence of LA with age is more associated with increasing prevalence of recession than with changes in the prevalence of pockets or age. The extent or number of affected sites with advanced conditions for loss of attachment, pocket depth, or recession was not large for any age group. Differences in prevalence of moderate and severe loss of attachment, moderate and deep pockets, and recession were found among gender and race-ethnicity groups. Females exhibited better periodontal health than males, and non-Hispanic whites exhibited better periodontal health than either non-Hispanic blacks or Mexican-Americans.

Background. lncRNA and microRNA affect the occurrence and development of many diseases, so they are expected to become diagnostic or predictive indicators. But the relationship between lncRNA FGD5-AS1 and miR-130a and the prognosis of chronic periodontitis is still unclear. The purpose of this study is to explore the prognostic value of the two in chronic periodontitis. Objective. This study set out to investigate the prognostic value of lncRNA FGD5-AS1 and miR-130a in chronic periodontitis. Methods. Eighty-seven patients with chronic periodontitis who visited our hospital from March 2016 to August 2017 were collected as an observation group (OG), and 72 subjects with periodontal health who underwent physical examination at the same time were collected as a control group (CG). The FGD5-AS1 and miR-130a expression levels of subjects in the two groups were compared, and prognosis of 87 patients who were reviewed one year later was counted. The expression levels of patients with different prognoses were compared when they were admitted to our hospital. We drew the ROC curve and explored the prognostic value of FGD5-AS1 and miR-130a. The risk factors for adverse prognosis were analyzed through logistic regression. Results. FGD5-AS1 was lowly expressed in patients, while miR-130a was highly expressed. FGD5-AS1 and miR-130a had certain diagnostic and predictive value in chronic periodontitis and patient prognosis. The higher the periodontal pocket, the higher the attachment loss. Lower FGD5-AS1 and higher miR-130a levels were independent prognostic risk factors. Conclusion. lncRNA FGD5-AS1 is lowly expressed in patients with chronic periodontitis, while miR-130a is highly expressed. Both of them have certain diagnostic and prognostic value in chronic periodontitis and may be potential diagnostic and prognostic indicators.
