Bone is a dynamic tissue, which undergoes continuous remodelling throughout life. This process requires deposition of new bone, a process which is controlled by the bone-forming osteoblasts, as well as the degradation of bone, exerted by the bone-resorbing osteoclasts. Osteoblast differentiate from mesenchymal precursors and are metabolically active cells, which produce bone matrix proteins such as collagen and osteocalcin, as well as alkaline phosphatase. These proteins can be used to measure osteoblast function in humans. Osteoclasts are haematopoietic cells of the monocyte linage degrading the bone by enzymes cleaving collagen such as metalloproteinases and cathepsins. Collagen cleavage products are indicators for bone resorption in the body. Osteoblast-osteoclast actions are controlled by a fine network of bone cells, osteocytes, which are responsible for sensing bone damage and directing bone remodelling to the right anatomical locations. Finally, mineralization of the newly formed bone is governed by vitamin D and parathyroid hormones, the key regulators of calcium homeostasis in our body.

Objective: To investigate the factors influencing the levels of vitamin D (vitD) in the umbilical cord blood of neonates born in Naqu, Tibet (4,500 m above sea level), and Shenyang, Liaoning Province (500 m above sea level). Methods: This prospective study was conducted from June 2017 to October 2018 in Naqu (the plateau group) and Shenyang, (the non-plateau group). Healthy mothers that gave birth to healthy neonates of >2,000g after 38 weeks' gestation were enrolled in the study, as were their neonates. After separation of serum from the umbilical cord and mothers for routine biochemical tests, discarded samples were remained for analyses of vitD, calcium, phosphorus, alkaline phosphatase (ALP) and parathyroid hormone (PTH). Questionnaires were developed covering the demographic characteristics and possible risk factors for neonatal vitD deficiency of mothers. Statistical analysis was performed to identify associations between the calcium, phosphorus, ALP, PTH, maternal factors and neonatal vitD levels. Results: In total, 295 neonates and 225 mothers were enrolled in the study. VitD deficiency was common in neonates and mothers. The risk of vitD deficiency was higher in the plateau group than in the non-plateau group. The mean levels of 25-hydroxy vitD (25(OH)D) in mothers and neonates from the plateau group were 8.49 ± 4.12 ng/mL and 10.17 ± 5.07 ng/mL, respectively. Such levels were significantly lower than those in the non-plateau group (19.77 ± 9.57 ng/mL and 23.93 ± 11.01 ng/mL, respectively). The vitD levels of neonates and mothers were highest in the summer and lowest in the winter. Cord blood vitD was positively correlated with the vitD levels in mothers' serum (r = 0.75, P < 0.05). Increased PTH levels in mothers and decreased cord blood calcium levels were risk factors for neonatal vitD deficiency. A lack of vitD supplementation during pregnancy was associated with an 8.91-fold higher probability of neonatal vitD deficiency (OR = 8.91, 95% CI = 1.521–9.429, P < 0.001).Conclusions: The levels of neonatal and maternal vitD in the plateau group were generally lower than those in the non-plateau group. VitD supplementation during pregnancy could effectively reduce the risk of vitD deficiency in neonates.

The paper presents the case of a 55-year-old man whose body was found at home at the foot of the stairs. The inspection of the body at the scene showed that the probable cause of the man’s death was positional asphyxia after falling down the stairs. The forensic autopsy revealed a 10-cm-long tip of a nephrostomy catheter. To determine the time and circumstances of the foreign body’s entry into the heart and its possible impact on the man’s death, the medical history was analyzed. It indicated that the piece of the catheter entered the circulatory system during surgery performed few years before man’s death. The results of the postmortem examination, taking into consideration the obtained medical information, excluded the foreign body in the right ventricle of the heart as a contributory cause of the man’s death.

AbstractConventional pharmaceuticals generally display the inability to transport active ingredients directly to specific regions of the body, amongst some of their main limitations. The distribution of the drugs in the circulatory system may lead to undesired toxicity, and therefore, adverse reactions. To address this situation, a selective transport of drugs is required, that is, releasing drugs specifically to the site of action in appropriate concentrations and in the right time. To achieve this goal, it is necessary to develop delivery systems that respond to several features, such as low toxicity, optimum properties for the transport and release of the drug, as well as a long half-life in the body. This feature paper critically provides an overview of different strategies of controlled drug release for two model antineoplasic drugs, i.e. doxorubicin (DOX) and 5-fluorouracil (5-FU). Any of the presented strategies for drug release possess advantages and disadvantages, and the selection of the strategy used will depend on the targeted tissue and nature of the drug.

Neonatal late onset hypocalcemia is the one which occurs after 72 hours of life. 1,25 (OH)2- vitamin D and Parathormone (PTH) play crucial role in regulation of calcium and phosphorus homeostasis in the body. Vitamin D helps in the absorption of calcium and phosphorus from the gut. PTH promotes absorption of calcium from thick ascending loop of henle and distal tubule whereas it increases excretion of phosphorus. During hypocalcemia, parathormone level shoot up in the body to maintain normal calcium levels. In a patient with poor resources of vitamin D, there is disturbance of Calcium, phosphorus homeostasis leading on to clinical manifestations. Exclusively breast-fed infants without vitamin D supplementation and infants born to mothers with Vitamin D deficiency are at risk to develop Hypovitaminosis D and manifest symptoms of hypocalcemia.

Purpose This paper aims to discuss the possibility that Hyperloop using air levitation technology would create a revolution in the future industry of transportation. It also analyses the financing dimension in bringing Hyperloop technology towards full-scale commercialisation. Design/methodology/approach The study uses a case study of SpaceX Hyperloop. The analyses if Hyperloop would bring about a paradigmatic shift in the transportation industry are based on the classical economic theories of technology S-curves (Utterback and Abernathy, 1975; Fisher and Pry, 1971) and Schumpeter’s model of economic development (Schumpeter, 1939, 1967). The three factors influencing a paradigmatic change (a shift in technoeconomic paradigm) according to Freeman and Perez (1988) are also explored. Findings The analyses of findings have shown that Hyperloop has not yet met three factors influencing a paradigmatic change. However, the Hyperloop technology has the potential to create a new market of mass transportation. In terms of technology financing, SpaceX needs the right financing infrastructure – corporate venture arm, the crowdfunding platform, initial coin offering, as well as debt and equity financing to accelerate the process of commercialisation. As the project moves forward, Hyperloop technology would create a market for itself (innovation to create the demand and not demand to create innovation).v Originality/value Although the body of literature on technology management is already voluminous, there has been a scarcity of past research devoted to elucidating the technology disruption and technology financing. This research study has applied the concepts of technology S-curves to better understand if SpaceX Hyperloop would bring about a paradigm shift or revolution in the future of the transportation industry. The research findings would help fill an identified knowledge gap in the body of research in technology disruption and financing.

The history of the discovery of vitamin D (VD) has crossed 100 years. Initially, VD deficiency was limitedly associated with the development of rickets in children. Later, on the basis of a number of studies, it was found that VD deficiency is a risk factor for osteoporosis in men and women. However, the traditional concept of VD as the main regulator of calcium-phosphorus metabolism has undergone dramatic changes over the past two decades. A prerequisite for revising the role of VD and the spectrum of its biological properties was the identification of the distribution of its specific receptors in the body. VD receptors are expressed in the intestine, thyroid and parathyroid glands, kidneys, ovaries, uterus, placenta, hypothalamus, pituitary gland and play a vital function not only in calcium homeostasis. Accordingly, the list of target organs for VD action has been modified. VD deficiency, calculated on a pandemic scale, is a factor that increases the risk of developing not only osteoporosis, but also a number of other diseases. Observational and epidemiological studies have shown that reproductive system disorders are very often combined with VD insufficiency / deficiency. Subsequent scientific research aimed at establishing the role of VD in the formation of a particular pathology of the reproductive system has shown very ambiguous results. VD is recognized as an essential component of maintaining a woman’s reproductive health. Of course, the presence of an established association is not evidence of a causal relationship. There is a need for additional testing. This review focuses on the effect of VD on ovarian function and the formation of various dysfunctional conditions.

Aim. To investigate the vitamin D status in highly productive cows during winter housing period and effect of cholecalciferol by various ways of vitamin D 3 injection to cows in last days of gestation and after calving. Methods. Enzyme-linked immunoassay, spectrophotometry. Results. It has been stated that intramuscular injection of cholecalciferol into cows caused increase of the vitamin D 3 active metabolite – 25-OHD 3 , calcium, phosphorus and magnesium levels together with decrease of alkaline phosphatase level in pre- and post-natal periods. Oral supplementation makes little infl uence on the studied blood parameters of cows. Conclusions. Extrabuccal administration and oral supplementation of cholecalciferol in winter housing period to high-yield cows in the last days of gestation and after calving is accompanied by increased levels of its metabolites and their effect on mineral metabolism in the postnatal period. The nature of these changes depends on the mode of vitamin D administration and the physiological state of the cows.
