DYNAMICS IN INCIDENCE OF DISEASES OF THE CIRCULATORY SYSTEM AMONG ADULTS IN THE RUSSIAN FEDERATION IN 2007-2019

Significance. Diseases of the circulatory system remain one of the leading causes of death and morbidity of the adult population. Over the years, the efforts of the State aimed at controlling diseases of the circulatory system have made the contribution of this disease class to the total mortality in Russia gradually reduce. Nevertheless, diseases of the circulatory system remain the leading ones among diseases, characterized by a steady growth and rapidly increasing prevalence among the population. The purpose of the study is to analyze dynamics in incidence and prevalence of the diseases of the circulatory system among the Russian adult population in 2007-2019. Material and methods. The study used data of the statistical collections of the Ministry of Health of the Russian Federation and the Federal Research Institute for Health Organization and Informatics of the Ministry of Health of Russia: "Morbidity of the adult population of the Russian Federation" for 2007-2019. The study, which is a continuous statistical observation, used the method of direct ranking, calculation of indicators of time series. Results. Diseases of the circulatory system in 2019 ranked fourth in the incidence structure of the adult population (8%), while its prevalence ranked second to none (21%). Over 13 years, the incidence of diseases of the circulatory system in adults increased by 42% (2007-2019) while its prevalence increased by 24%. The highest levels of incidence of diseases of the circulatory system in 2019 are registered in the Krasnodar Territory (9,024.3 per 100 000 population, + 324%), the Kurgan Region (7,404.2, + 103%) and the Karachayevo-Circassian Republic (7 286.2, + 268%), and prevalence - in the Altai Territory (52 557.6, + 2%), the Chuvash Republic (51 814.6, + 38%) and the Voronezh Region (45 339.4, + 59%). The maximum rates of increase in morbidity with diseases of the circulatory system from 2007 to 2019 are registered in the South Federal district (incidence - by 119%, prevalence - by 56%), among the constituent entities of the Russian Federation: incidence - in the Krasnodar Territory (by 324%), prevalence – the Karachayevo-Circassian Republic (by 136%). According to the state statistical reporting, on average, every 7th acute myocardial infarction is complicated by recurrent heart attack in Russia. Conclusions. Throughout the study period of 13 years, the leading nosological forms include: Hypertensive diseases (35%; 45%, respectively), Ischemic heart diseases (22%; 21,6%) and Cerebrovascular diseases (23%; 20%). Over half of the diseases of the circulatory system are registered in the older working ages (55-60 and over). Scope of application. The study results can be useful to regional authorities in public health in planning and implementing regional programs aimed at controlling diseases of the circulatory system.

The article presents results of the study ofproblems of the repeated disability due to diseases of the musculoskeletal system and connective tissue in retirement age citizens of the Russian Federation in the period of from 2005 to 2014. There were analyzed the number of this category of invalids, their specific weight among re-recognized as disabled cases with account of classes of diseases and the age, structure of the repeated disability in groups, the level of repeated disability and ranking places of subjects of the Russian Federation on this index in 2014. The results of the executed analysis indicate that only in the Russian Federation for 10 years (2005-2014) there were re-recognized as disabled due to diseases of the musculoskeletal system and connective tissue 487.1 thousand people from citizens of retirement age with distinct dynamics of the increase as their absolute number (from 22.1 thousand persons in 2005 to 69.3 thousand people in 2014) and the index of disability level (respectively from 7.6 to 20.5 per 10 thousand of the corresponding adult population). This pathology was established to determine a high level of repeated disability in this age group (20.5 per 10 thousand population). Comparative analysis by age revealed that in the Russian Federation in the structure of the repeated disability due to diseases of the musculoskeletal system and connective tissue in the adult population invalids of the retirement age prevail (42.8% in 2014). Along with diseases of the circulatory system (of rank 1) and malignant neoplasms (rank 2), diseases of the musculoskeletal system and connective tissue form the contingent of cases repeatedly recognized as disabled of retirement age (3th rank place). A ranking ofall subjects of the Russian Federation by the level of repeated disability of citizens of retirement age due to diseases of the musculoskeletal system and connective tissue allowed to select 70 ranking places, that with bearing in mind the present demographic situation, has a practical orientation for planning the organization of medical and social care for the given large category of the population.

The purpose of current work was to conduct an economic and biological estimation of the main morphobiological traits and properties of the ultra-early-ripening winter barley variety ‘Foks 1’ in comparison with the standard variety. In connection with the increasing climate aridity in recent years, the relevance of the production uses of early-ripening varieties of grain crops, including winter barley, has increased. For several decades the Federal State Budgetary Scientific Institution “Agricultural Research Center “Donskoy” has been working in this direction, which resulted in the development of the new ultra early-ripening winter barley variety ‘Foks 1’. Since 2019 the variety ‘Foks 1’ has been included in the State List of Breeding Achievements of the Russian Federation in the North Caucasus region. The variety parentage has the highly winter-resistant, lodging-resistant and large-grain varieties. The variety ‘Foks 1’ ripens on average 8–10 days earlier than the middle-ripening standard variety ‘Timofey’. On average, over the years of study in the Competitive Variety Testing (2017–2019), the trait “1000 grains weight” of the variety was 42.5 g, that of the standard variety was 40.2 g, the trait “grain nature” was 672 and 656 g/l, respectively. The trait “number of grains per head” of the new variety was 51, that of the standard variety was 45. The new variety formed an average yield of 8.1 t/ha, the excess over the standard was 0.5 t/ha. The variety ‘Foks 1’ was of a high level of winter tolerance and was resistant to lodging, it was also tolerant to damage by the main leaf diseases widespread in the region. The study results at the state variety plots of the Russian Federation and ecological testing in various soil and climatic zones have shown that the new ultra early-ripening winter barley variety ‘Foks 1’ under increasing climate aridity is undoubtedly of practical interest for cultivation in the North Caucasian region of the Russian Federation.

The aim of the work is to determine the external, anthropogenic environmental factors that negatively affect the morbidity of the population and associated nosologies. The knowledge gained in this area will allow us to identify the adverse impacts of man-made nature and identify tools for environmental quality management to promote public health. Geoinformation data will help to identify the areas with the most intense environmental situation in the Murmansk region. It will also identify the dominant diseases in these areas. Mining and metallurgical enterprises mainly contribute to the pollution of the territory of the region: Kola MMC, Severonikel, Phosagro, Kovdor mining and processing plant, North-Western Phosphorus Company, Olkon and Rusal, as well as the nuclear fleet and fish processing enterprises, power supply facilities. The air basin is dominated by fluoride compounds, which are the emissions of ethylene into the atmosphere. Data on the demographic situation and medical statistics, including data on general morbidity and mortality, were obtained from the Territorial Body of the Federal State Statistics for Murmansk Region, Ministry of Health of the Russian Federation, Department of Monitoring, Analysis and Strategic Development of Health Care, Federal State Budgetary Institution “Central Research Institute for the Organization and Informatization of Health Care” of Ministry of Health of the Russian Federation and Ministry of Natural Resources and Ecology of Murmansk Region. The medical and environmental analysis of adverse factors in the regions of Murmansk region was carried out taking into account the environmentally dependent diseases of the adult population. The assessment of the morbidity of the population in the Murmansk region showed that several areas with a tense environmental situation are identified in the region. Diseases of the respiratory system, digestive system, genitourinary system, and musculoskeletal system are leading in the structure of overall morbidity.

The article presents the results of the activity of the Main Bureau of Medical and Social Expertise for the city of Sevastopol for state services provision to citizens on medical and social expertise. There are presented staffing data, the state of the primary disability in the adult population in it employable sector, indices of rehabilitation of the disabled persons over three years (2013-2015) in the city of Sevastopol in modern conditions after the annexation of Crimea to the Russian Federation. There are highlighted basic directions of the organization and peculiarities in the activities of experts of the Bureau of the Medical and Social Expertise.

To solve the problem of information security management the method was proposed that allows determining the degree of importance of confidential documents of the organization. The urgency of the proposed algorithm was substantiated taking into account the requirements of the legislation of the Russian Federation in the sphere of information security. The stages prior to the formation of the list of confidential documents of the organization were described. A review of the main documents of the legal and regulatory framework was carried out including documents relating to the state regulation of relations in the sphere of information security. The classes of protected information for the accessing categories were considered. The criteria changes of the value of information in the process of time were represented. The algorithm of formation of the list of confidential documents of the organization based on the properties of information was offered. The algorithm is based on an expert method of pair comparison of alternatives. The result of the use of this method is a number of confidential documents, ranked in descending order of importance. For each document the weighting factor of importance can be calculated. The verification stage of the degree of expert consistency was included in the methodology to eliminate the use of erroneous expert data. The application of the methodology is illustrated by a calculated example.

Subject. The article considers a procedure for calculating and distributing State support to depressed agricultural regions in the framework of a new type of budget financing, i.e. compensating and stimulating subsidies. Objectives. The purpose is to evaluate the efficiency of the system of State regulation of agribusiness in a depressed region, determine the priority of modern methods of budgetary support from the State, improve the methodological approach to the distribution of State aid among subjects of the Russian Federation with low level of socio-economic development. Methods. The study draws on the abstract and logical method, methods of statistical analysis, grouping, comparison, and generalization. Results. We realized the objectives of the study, using the agricultural producers of the Kurgan Oblast case. The paper analyzes the condition of agricultural sector in the depressed region, assesses the State support system efficiency for the agricultural sector of the Trans-Ural region, considers government measures related to the regulation of the economy of backward regions. It presents an updated method of budget financing within an incentive subsidy by clarifying the calculations and introducing an additional indicator that reflects the level of remuneration of regional agricultural producers. Conclusions. The updated calculation methodology for incentive subsidies to depressed regions enables to build a certain economic rating of ten depressed regions of the Russian Federation, taking into account not only the volume of production and employment, but also the level of wages of agricultural workers.

The characteristics of new varieties of potatoes of the selection of the Kamchatsky RIA are created in accordance with the priority for the region selection areas included in the State Register of Selection Achievements of the Russian Federation and protected by patents.
