Abstract Background: In the conduct of their daily duties, law enforcement officers (LEO) are often required to perform dynamic, physically demanding tasks with little or no notice, sometimes at maximal levels of exertion. Given these requirements, training for prospective LEOs must be rigorous enough to ensure that when trainees graduate, they are competent in their response to crisis and resilient enough to maintain this for the span of their career. Therefore, based on previously reported effectiveness of fitness testing in predicting injury risk in predominantly military settings, the aim of this study was to investigate relationships between a physical ability test (PAT) and risk of injury during police recruit training. Methods: Retrospective PAT results and trainee injury records were obtained from a national police department and Mann-Whitney U tests were performed to investigate fitness differences between trainees who were, or were not, injured. Results: Significant differences in mean performance between groups existed for the following PAT components: pushups (injured mean 32.94±8.66 reps, uninjured mean 35.67±9.04 reps, p=0.01) and (R) grip strength (injured mean 49.61±12.51kg, uninjured mean 52.12±11.17kg, p=0.042) for all injuries; vertical jump height (injured mean 51.75±7.54cm, uninjured mean 55.06±8.19cm, p=0.032) for lower limb injuries, and all measures of grip strength for trunk injury. Conclusions: Results of this study suggest that a clinically relevant relationship between some PAT fitness components and injury risk exists during police recruit training.

Specialist police tactical teams, like special operations military personnel, are tasked with dangerous, high risk missions which are beyond the scope of general police. Consequently, the selection courses for entry into these teams are physiologically and psychologically demanding. The purpose of this study was to examine the physiological effects of a five-day selection course to aid in candidate preparation and course planning. Measures included body mass, grip strength, sit-and-reach flexibility, and a vertical jump assessment. Eleven candidates finished the selection course with significant decreases in body mass (−2.05 kg, p = 0.006 (95% CI = 3.65–0.45)), grip strength in the right (−14.48 kg, p < 0.001 (95% CI = 21.32–7.64)) and left (−14.27 kg, p < 0.001 (95% CI = 21.89–6.66)) hands and in sit-and-reach flexibility (−6.64 cm, p < 0.001 (95% CI = 9.94–3.33)). No significant decreases in power output or peak jump velocity of 669.77 W (95% CI = 1942.92–603.39) and 0.28 m/s (95% CI = 0.69–0.14) were found and a non-significant, overall increase in vertical jump height of 6.09 cm (95% CI = −6.08 to 18.79) was seen. Decreases in body mass, grip strength and lower limb flexibility are evident in a grueling five-day selection course. Individuals planning on attending these courses should plan for these negative effects and build redundancy into their performance to minimize the effects of fatigue, decrease injury risk and maximize chances of completion.

Accident data shows that driver’s kinematics response in real accidents can be significantly different from that in dummy or cadaver tests because of driver’s muscle contraction. In this study, a finite element human-body model consisting of an upper body of a dummy model and a lower limb–pelvis biomechanical model with three-dimensional active muscles was developed to investigate in depth the lower-limb injuries. Driver’s emergency reaction during frontal impact was simulated by modelling muscle active contraction based on a series of volunteer experimental tests. Besides, a realistic impact environment with the response of the restraint system and the invasion of the driver’s compartment was established in this study. The results show that muscle contraction can cause extra loads on lower limbs during the impact, which can increase the injury risk of lower limbs. As for the femur injury, muscle contraction caused an additional 1 kN axial load on the femur, and the femur resultant bending moment of active models was also higher by about 10–40 N m. Besides, the tibial index of the model with muscle activation was about 0.1 higher. In addition, the results indicate that the femur injury is strongly related to the combined action of both axial force and bending moment. The variation of the injury tolerance along the tibia shaft should be considered when evaluating the tibia injury. Overall, the current lower-limb injury criteria can be still the lack of robustness.

BACKGROUND Cell phone use brought convenience to people, but using phones for a long period of time or in the wrong way and with a wrong posture might cause damage to the human body. OBJECTIVE To assess the impact of improper cell phone use on transport and chronic injuries. METHODS Studies were systematically searched in PubMed, EMBASE, Cochrane, and Web of Science up to April 4, 2019 and relevant reviews were searched to identify additional studies. A random-effects model was used to estimate the overall pooled estimates. RESULTS Cell phone users were at a higher risk for transport injuries (RR: 1.37, 95%CI: 1.221.55), long-term use of cell phones increased the transport injury risk to non-use or short-term use (RR: 2.10, 95% CI: 1.632.70). Neoplasm risk caused by cell phone use was 1.07 times that of non-use (95% CI: 1.011.14); Compared with non-use, cell phone use had a higher risk of eye disease, with a risk of 2.03 (95% CI: 1.273.23), the risk of mental disease was 1.26 (95% CI: 1.171.35), the risk of neurological disorder was 1.16 (95% CI: 1.021.32), and a pooled risk of other chronic injuries was 1.20 (95% CI: 0.981.59). CONCLUSIONS Cell phone use at inappropriate situations has a negative impact on the human body. Therefore, it is necessary to use cell phones correctly and reasonably.

Abstract Background The World Health Organisation declared a global pandemic on the 11 March 2020 resulting in implementation of methods to contain viral spread, including curtailment of all elective and non-emergent interventions. Many institutions have experienced changes in rostering practices and redeployment of trainees to non-surgical services. Examinations, study days, courses, and conferences have been cancelled. These changes have the potential to significantly impact the education and training of surgical trainees. Aim To investigate the impact of the COVID-19 pandemic on training, educational, and operative experiences of Irish surgical trainees. Methods Surgical trainees were surveyed anonymously regarding changes in working and educational practices since the declaration of the COVID-19 pandemic on 11 March 2020. The survey was circulated in May 2020 to both core and higher RCSI surgical trainees, when restrictions were at level five. Questions included previous and current access to operative sessions as well as operative cases, previous and current educational activities, access to senior-led training, and access to simulation-/practical-based training methods. A repeat survey was carried out in October 2020 when restrictions were at level two. Results Overall, primary and secondary survey response rates were 29% (n = 98/340) and 19.1% (n = 65/340), respectively. At the time of circulation of the second survey, the number of operative sessions attended and cases performed had significantly improved to numbers experienced pre-pandemic (p < 0.0001). Exposure to formal teaching and education sessions returned to pre-COVID levels (p < 0.0001). Initially, 23% of trainees had an examination cancelled; 53% of these trainees have subsequently sat these examinations. Of note 27.7% had courses cancelled, and 97% of these had not been rescheduled. Conclusion Surgical training and education have been significantly impacted in light of COVID-19. This is likely to continue to fluctuate in line with subsequent waves. Significant efforts have to be made to enable trainees to meet educational and operative targets.

Abstract Background Antegrade intramedullary nailing (AIMN) with Kirschner wire (K-wire) is a minimally invasive osteosynthesis technique. This procedure has been widely performed to treat the fifth metacarpal neck fracture (FMNF) in adults. This study was performed to determine whether using AIMN with a single K-wire to treat FMNF in adolescents would have good clinical and radiographic outcomes. Methods In this retrospective study, 21 children (aged 11–16 years) with FMNF were treated using AIMN with a single K-wire from May 2017 to January 2020 in our hospital. Indications for intervention were severe displacement with malrotation deformity, apex dorsal angulation of greater than 40°, or both. Collected data included apex dorsal angulation, range of motion (ROM) in the fifth metacarpophalangeal (MCP) joint, Visual Analog Scale (VAS) for pain, grip strength, and Disabilities of the Arm, Shoulder, and Hand (DASH) score. Results All patients were followed up for 12–24 months (average, 16.57 months), and all patients obtained anatomical reduction postoperatively. The healing time was 2.69 ± 0.83 months (range, 2–4 months). Average apex dorsal angulation was reduced significantly from 44.49°±2.64° to 15.74°±2.47° (P < 0.001). The average ROM in the MCP joint and apex dorsal angulation of the injured side were not significantly different from those of the uninjured side. The average DASH score was 1.76 ± 1.48 (range, 0–4), the mean VAS was 0.19 ± 0.60 (range, 0–2), and the mean grip strength was 91.55 %±4.52 % (range, 85–101 %). No secondary displacement, dysfunction, nonunion, infection, or osteonecrosis was observed during the follow-up. Although premature epiphyseal closure was found in one patient, no long-term clinical finding of angulation or shortening was identified. Conclusions Antegrade intramedullary fixation with single K-wire was an effective and reliable technique that successfully resulted in good functional and cosmetic outcomes for treating adolescents with FMNF. The impact on the growth plate was low in this population given that most patients were at or approaching skeletal maturity. Level of evidence Level IV.

Background: Anterior cruciate ligament (ACL) injuries predominantly occur via non-contact mechanisms, secondary to motor coordination errors resulting in aberrant frontal plane knee loads that exceed the thresholds of ligament integrity. However, central nervous system processing underlying high injury-risk motor coordination errors remain unknown, limiting the optimization of current injury reduction strategies. Purpose: To evaluate the relationships between brain activity during motor tasks with injury-risk loading during a drop vertical jump. Methods: Thirty female high school soccer players (16.10 ± 0.87 years, 165.10 ± 4.64 cm, 63.43 ± 8.80 kg) were evaluated with 3D biomechanics during a standardized drop vertical jump from a 30 cm box and peak knee abduction moment was extracted as the injury-risk variable of interest. A neuroimaging session to capture neural activity (via blood-oxygen-level-dependent signal) was then completed which consisted of 4 blocks of 30 seconds of repeated bilateral leg press action paced to a metronome beat of 1.2 Hz with 30 seconds rest between blocks. Knee abduction moment was evaluated relative to neural activity to identify potential neural contributors to injury-risk. Results: There was a direct relationship between increased landing knee abduction moment and increased neural activation within regions corresponding to the lingual gyrus, intracalcarine cortex, posterior cingulate cortex, and precuneus (r2= 0.68, p corrected < .05, z max > 3.1; Table 1 & Figure 1). Conclusion: Elevated activity in regions that integrate sensory, spatial, and attentional information may contribute to elevated frontal plane knee loads during landing. Interestingly, a similar activation pattern related to high-risk landing mechanics has been found in those following injury, indicating that predisposing factors to injury may be accentuated by injury or that modern rehabilitation does not recover prospective neural control deficits. These data uncover a potentially novel brain marker that could guide the discovery of neural-therapeutic targets that reduce injury risk beyond current prevention methods. This study aimed to examine the views of runners regarding their experiences with congestion during running events, including its prevalence, its impact on their safety and satisfaction, and their preferred controls to mitigate congestion. Runners (n = 222) with varied experience participating in running events (1-5+ years, 5-km races to Ultramarathons, and a mixture of road, trail, and cross-country events) completed an electronic survey. The survey was developed to assess the characteristics of respondents, whether they have experienced congestion during running events, the impact of congestion they have experienced during running events on their safety and satisfaction, and their preferred controls for congestion during running events. Survey data indicated runners had experienced some form of congestion prior to the race in the start corrals (93% of respondents), as the race started (97% of respondents), and during the race while running (88% of respondents). In turn, 73% of respondents indicated their experiences with congestion somewhat to extremely (i.e., rating of at least 3 on a 5-point Likert scale) negatively impacted their satisfaction with an event, while 43% of respondents indicated congestion somewhat to extremely negatively impacted their safety during an event. Regarding the impact of congestion on runner safety, 38% of respondents indicated they had slipped, while 27% of respondents indicated they had fallen during running events due to congestion. Further, congestion was attributed to injuries sustained (9%) and not finishing a race due to sustaining an injury (5%) during running events in some respondents. Respondents identified seeding runners based on previous run times (91%), use of wave starts (91%), and designing courses with limited pinch points, U-turns, and narrow paths (89%) as their most preferred controls to mitigate congestion during running events. Respondents resoundingly indicated self-seeding is not an effective method of managing congestion during running events. This study provides novel evidence that congestion is an issue for runners during running events, subsequently diminishing their satisfaction with events and posing safety concerns. In this way, race directors should involve runners in their decision-making processes when implementing appropriate controls to combat congestion for minimising injury risk to runners and ensuring a viable participant base remains attracted to their events in the future.

This exploratory case study focused on multicultural communication training within the community policing context. Little research has addressed what constitutes effective content and delivery of multicultural training for law enforcement officers. Brislin and Yoshida's four-component multicultural plan was combined with limited law enforcement-related multicultural training literature to design a training program for a small city's police department. Two 4-hour training sessions were conducted by one of the investigators using a culture-general content approach and selected training methods to determine their usefulness for improving officers' multi-cultural competencies. The case included the training sessions together with pre-and posttraining activities over a period of approximately 3 months. Data were collected with selected test instruments and also from the field notes taken during the case activities. The results suggest that the culture-general model and interactive training methods and trainer qualifications may be key to yielding positive training outcomes.

Abstract Introduction: The presented paper deals with the issues of the work of class teachers and their further education in the field of personal and social training. The main goal of the research was to find out about changes in personal and social development after the realization of social-pedagogical training. Methods: On the level of personal development, the authors were interested in the field of values and attitudes. On the level of social development, they focused on the changes in communication and opinion scales. The changes in the above fields were measured by means of a pre-test and a post-test which were administered before and after the realization of the training. Results: In the participants of the realized research, the research team, to a certain extent, succeeded in reducing prejudice and beliefs and the participants learnt about the necessity of considering students’ individual abilities and specific environmental influences on their behavior and manifestations at school. On the level of opinions, there was a shift towards a stronger belief in the significance of the impact of the environment and the family background on students’ behaviour and their personality traits. Discussion: The presented data are the results of a pilot probe and have brought initial insights related to the presented issues for the purposes of a longer and deeper research, which is in the phase of its realization. Limitations: As the project was realized with ten groups of teachers showing a deep interest in participating in it, it is not our ambition to generalize the obtained results; nevertheless, we find them interesting and inspiring. Conclusions: Along with knowledge from pedagogy and psychology, class teachers need a huge amount of creativity, ideas, techniques and methods, which can promote the development of students’ value orientation. The authors can see a clear perspective for teachers’ lifelong learning here
