Abstract Background: The high rate of second hand smoke (SHS) exposure puts pregnant women at risk of various harms and identify relevant influence factors are vital for primary prevention. The study aimed to explore the effect of individual socioeconomic status (SES) on exposure to SHS exposure among pregnant women.Methods: A total of 678 non-smoker pregnant women from 14 communities of Hengyang city, Hunan province of China were recruited in this survey. A self-designed structural questionnaire was used to collect variables. Exposure to SHS was defined as self-reported smoking habit of spouse/partner. The individual SES consisted of martial status, educational attainment, employment and per-capita monthly income.Results: There were 238 (35.1%) participants suffered from SHS exposure during pregnancy. After adjusted for confounding variables, compared to the pregnant women who were employed, others were unemployed were more likely to suffer from SHS exposure (OR= 1.697; 95%CI: 1.102-2.614). Similarly, those women who had high school or technical secondary school education level were associated with SHS exposure compared with college or above education attachment (OR: 1.577, 95%CI: 1.020-2.437). The interaction effects between age and junior middle school or below educational attainment (OR: 1.131, 95%CI: 1.015-1.261), unstable marriage (OR: 1.380, 95%CI: 1.075-1.772) on SHS exposure was detected.Conclusion: Exposure to SHS was very common among pregnant women. Those pregnant women of low level of SES should considered as key population to implement public health intervention. Pregnant women of unstable martial status with older age were more likely to SHS exposure.

Despite the advancement of the healthcare system, low birth weight (LBW) remains as one of the leading causes of under-five mortality. This cross-sectional study aimed to determine the prevalence of LBW and its associated factors among 483 third trimester pregnant women recruited from six selected public health clinics in the Federal Territory of Kuala Lumpur and the state of Selangor, Malaysia. Pregnant women were interviewed for information on socio-demographic characteristics, smoking behaviour, and second-hand smoke (SHS) exposure at home and in the workplace. Information on the obstetrical history and prenatal care visits history were retrieved from the maternal medical records, while infant’s birth outcomes were retrieved from infant medical records. The prevalence of LBW (<2.5 kg) in infants was 10.4%, with a mean birth weight of 3.0 [standard deviation (SD) 0.4] kg. Results from the multivariable logistic regression model showed that inadequate weight gained during pregnancy [odds ratio (OR) = 2.41, 95% confidence interval (CI) = 1.18–4.90] and exposure to SHS at home (OR = 1.92, 95% CI = 1.03–3.55) were significantly associated with LBW. In conclusion, pregnant women should monitor their rate of weight gain throughout pregnancy and avoid SHS exposure at home to reduce the risk of delivering LBW infants.

Exposure to second-hand smoke (SHS) among non-smoking pregnant women can lead to adverse maternal and fetal outcomes. A cross-sectional study was performed from July to August 2016 among 432 pregnant women at Bach Mai Hospital, Hanoi, Vietnam, to assess the prevalence and sources of SHS exposure among non-smoking pregnant women. Socio-economic characteristics and information regarding SHS exposure of participants were collected. Multivariable logistic regression was employed to identify associated factors. Overall, 92.6% and 64.5% of pregnant women were exposed to SHS in their lifetime and in the last 30 days, respectively. Cafeterias and restaurants had the highest proportion of pregnant women exposed to SHS. Those who reported that “smoking is allowed at home” (OR = 3.18; 95%CI = 1.97–5.13); going to working place (OR = 1.86; 95%CI = 1.08–3.19), going to state authority offices (OR = 1.98; 95%CI = 1.15–3.41), and cafeterias (OR = 1.96; 95%CI = 1.22–3.16) had the highest risk of SHS exposure in the last 30 days. We have found a high proportion of SHS exposure among non-smoking pregnant women in Vietnam. This comes from a multitude of sources, including homes, workplaces, cafeterias, and restaurants. The data emphasises the need for further intervention to address this health issue.

AbstractThe objective of this study was to analyze the influence of place of permanent residence (urban or rural) on the tourist activity of senior citizens (60+) of different socioeconomic statuses. The study involved 380 senior citizens (305 female and 75 male) aged 60 years and older who were permanent residents of the region of Warmia and Mazury, Poland. In this group, 244 subjects resided in urban areas and 136 participants were rural dwellers. The respondents were asked to complete a questionnaire regarding their socioeconomic status (place of permanent residence, age, gender, educational attainment, financial status, membership in senior organizations, marital status, and professional activity) and tourist activity. A significance test of two structure coefficients (α=0.05) was applied. Factors such as gender, professional activity, and marital status were not related with the travel propensity of seniors from different groups (urban and rural), but were significant when rural residents were compared with urban dwellers. Seniors residing in urban areas of Warmia and Mazury, Poland, were significantly more likely to travel for leisure than those residing in rural areas. The tourist activity of seniors decreased significantly (p<0.05) with the age (60-74 years) and financial status of rural residents. The travel propensity of elderly people increased significantly (p<0.05) with educational attainment and membership in senior organizations. The study revealed considerable differences in the socioeconomic status and social characteristics of seniors residing in rural and urban areas, and those variations significantly influenced their propensity for travel: urban residents traveled more frequently than rural residents. It can be concluded that place of residence was a crucial factor determining the tourist behavior of senior citizens, and urban dwellers were more likely to travel.

Background: Racial and economic disparities in low birth weight (LBW) deliveries is among the most well-established differences between Blacks and Whites. As LBW is an established risk factor for chronic diseases such as asthma and diabetes, it is particularly important to understand drivers of racial and economic disparities in LBW deliveries in urban areas. Aims: Built on the Minorities’ Diminished Returns framework, which argues that educational attainment generates fewer positive health outcomes for Black than White Americans, we conducted this study with three aims: 1) to test the association between mothers’ educational attainment and LBW of babies born in urban areas, 2) to compare Blacks and Whites for the effect of mothers’ educational attainment on LBW, and 3) to test whether LBW is predictive of future chronic diseases 15 years later. Methods: Data came from the Fragile Families and Child Well-being / included a random sample of births in cities larger than 200,000 population. For the aims 1 and 2, we analyzed data of 2,922 births to Black (n = 2,146) or White (n = 776) mothers. For aim 3, we analyzed data of a subsample of 1,604 Black or White newborns who were followed to age 15. The presence or absence of chronic diseases was determined at age 15. Logistic regression was used for data analysis. Results: Maternal educational attainment was inversely associated with LBW overall. We, however, found a significant interaction between maternal educational attainment and race, suggesting that the inverse association between maternal education and LBW is weaker for Black than White babies. At the same time, LBW increased the odds of chronic disease 15 years later. Conclusions: Diminished returns of maternal educational attainment contribute to racial disparities in LBW, which in turn contributes to future racial inequalities in chronic diseases in urban settings. That is, smaller protective effects of maternal education on reducing LBW for Black than White children contribute to the high prevalence of chronic diseases among adolescents in urban settings. Health disparities are not just due to racial differences in socioeconomic status but also diminishing returns of socioeconomic status indicators such as education for racial and ethnic minorities. Research should study contextual factors that reduce Blacks’ ability to translate their human capital to health outcomes in urban settings.

Background: Anaemia in pregnancy is now considered as a major public health problem, caused by iron deficiency. WHO has estimated that prevalence of anaemia in developed and developing countries in pregnant women is 14% in developed countries and 51 percent in developing countries and 65-75% in India. Various programmes regarding anaemia prophylaxis and maternal health have been implemented but still the prevalence of anaemia related complications among pregnant women continues to be high. Hence the present study was conducted to study the prevalence and pregnancy outcome as a result of anaemia in a tertiary care center in Visakhapatnam. Methods: A cross sectional descriptive study was carried out from June 2016 to August 2016 in the Department of Obstetrics and Gynecology among postnatal women in King George hospital, Visakhapatnam. A semi-structured questionnaire was prepared and information regarding age, socioeconomic status, parity, gestational age interval between pregnancies and outcome and complication of pregnancy were collected. Results: Mean age of study population is 22.4 years. Out of 100 women involved in the study 65 are from urban areas 29 from rural areas and 6 from tribal areas. Majority of the study population 66% were having HB% between 8-11% i.e. mild anaemia. When Outcome of pregnancy was seen in relation to anaemia majority of the women had live births (90%) followed by 7% IUDs and 2% still births. Conclusions: Majority of the women are in the age group of 20-24 years. Socioeconomic status is directly related to anaemic status of the women. Majority of the women had complication of puerperal sepsis.

Abstract Focus of Presentation Tobacco has always been a major public health issue. Being tobacco a major risk factor for so many diseases, it has also become a matter of concern in current situation of the pandemic. Tobacco smoking is not only harmful for the person who is actively smoking but also to ones who are in the vicinity of that smoke. There is plenty of evidence regarding harmful effects of both tobacco and inhaling tobacco smoke passively. This passive smoke is known as Second Hand Smoke. The aim of the study was to estimate percentage and sources of Second Hand Smoke exposure among pregnant women. It was a cross sectional study conducted among pregnant women registered at a Primary health centre. Findings Out of total participants, 62% were exposed to Second hand smoke. 33% participants were exposed to SHS at home only, 24% at public places only and 5 % were the one exposed both at home and public places. More than half (69%) of the participants were having inadequate knowledge about the harms caused by Second hand smoke Conclusions/Implications More than half of the participants were exposed to Second hand smoke. Inadequate knowledge among the participants about adverse effects of SHS exposure and insufficient Key messages Smoke-free homes should be promoted and health-care providers should ask pregnant women about their exposure to SHS and inform about the harms of SHS exposure during anti natal visits.

Smoking during pregnancy threatens the health of mothers and their fetus. There are limited data in Indonesia about risk factors associated with smoking during pregnancy. This study aimed to explore levels of smoking, exposure to second-hand smoke (SHS), and related risk factors among pregnant women. Pregnant women who received antenatal care at the Temanggung Primary Health Centre in Central Java, Indonesia were interviewed face-to-face. They were asked about their smoking status, socio-economic status, pregnancy characteristics, and knowledge about health risks associated with smoking. A total of 80 participants were interviewed, and none of whom reported smoking, but the majority of whom (91.3%) reported exposure to SHS. This is a significant proportion when compared to rates of exposure to SHS recorded in other countries. The age of the mother affected the likelihood of exposure to SHS, with older mothers having a reduced likelihood of SHS exposure (p = 0.013; OR = 0.8; 95% CI = 0.74-0.96). However, the number of pregnancies, ANC visits, level of education, income, and awareness of the health risks associated with smoking were not found to be statistically significant. Although women appeared to be relatively aware of the negative effects associated with smoking and SHS exposure, the high incidence of passive smoking illustrated the challenges encountered by women in avoiding SHS. ANC programme should place greater stress on the dangers of SHS exposure, and also involve the whole family. Additionally, laws should be enacted to restrict or prohibit smoking in public places.Keywords: pregnant women, second-hand smoke, smoking

Abstract Background this study aims to examine the different factors associated with exposure to second-hand smoke (SHS) between urban and rural areas and to facilitate a reduction in SHS exposure in Northeast China. Methods a multistage stratified random cluster sampling design was used in this 2012 cross-sectional survey in Jilin Province, Northeast China. A total of 13 056 non-smokers were included in this study. The Rao–Scott χ2 test, multiple regression analysis and discriminant function analysis were used. Results the SHS prevalence among adult non-smokers was 60.2% in urban areas and 61.8% in rural areas. In urban areas, males were more likely to be exposed to SHS, while in rural areas, females were more likely to be exposed to SHS (P < 0.05). Increasing age was a protective factor against SHS exposure both in urban and rural areas (P < 0.05). Tobacco-relevant knowledge was positively associated with SHS exposure. Among urban non-smokers, high education level and engagement in manual work were risk factors for SHS exposure, and retired subjects were less likely to be exposed to SHS (P < 0.01). Conclusions non-smokers from urban and rural areas differ in the factors associated with SHS exposure, and urban–rural differentials, especially with regard to gender, should be considered in tobacco control.

: Obesity is a worldwide public health problem, affecting at least one-third of pregnant women. One of the main problems of obesity during pregnancy is the resulting high rate of cesarean section. The leading cause of this higher frequency of cesarean sections in obese women, compared with that in nonobese women, is an altered myometrial function that leads to lower frequency and potency of contractions. In this article, the disruptions of myometrial myocytes were reviewed in obese women during pregnancy that may explain the dysfunctional labor. The myometrium of obese women exhibited lower expression of connexin43, a lower function of the oxytocin receptor, and higher activity of the potassium channels. Adipokines, such as leptin, visfatin, and apelin, whose concentrations are higher in obese women, decreased myometrial contractility, perhaps by inhibiting the myometrial RhoA/ROCK pathway. The characteristically higher cholesterol levels of obese women alter myometrial myocyte cell membranes, especially the caveolae, inhibiting oxytocin receptor function, and increasing the K+ channel activity. All these changes in the myometrial cells or their environment decrease myometrial contractility, at least partially explaining the higher rate of cesarean of sections in obese women.

Hepatitis E virus (HEV) is one of the causative agents for liver inflammation across the world. HEV is a positive-sense single-stranded RNA virus. Human HEV strains mainly belong to four major genotypes in the genus Orthohepevirus A, family Hepeviridae. Among the four genotypes, genotype 1 and 2 are obligate human pathogens, and genotype 3 and 4 cause zoonotic infections. HEV infection with genotype 1 and 2 mainly presents as acute and self-limiting hepatitis in young adults. However, HEV infection of pregnant women with genotype 1 strains can be exacerbated to fulminant hepatitis, resulting in a high rate of case fatality. As pregnant women maintain the balance of maternal-fetal tolerance and effective immunity against invading pathogens, HEV infection with genotype 1 might dysregulate the balance and cause the adverse outcome. Furthermore, HEV infection with genotype 3 can be chronic in immunocompromised patients, with rapid progression, which has been a challenge since it was reported years ago. The virus has a complex interaction with the host cells in downregulating antiviral factors and recruiting elements to generate a conducive environment of replication. The virus-cell interactions at an early stage might determine the consequence of the infection. In this review, advances in HEV virology, viral life cycle, viral interference with the immune response, and the pathogenesis in pregnant women are discussed, and perspectives on these aspects are presented.
