Abstract Objective To investigate the digestive disease spectrum of Chinese army servicemen during a field training on the midland and to analyze risk factors of diseases in order to discuss the relative prevention and treatment strategies. Methods Seven troops during a field training on the midland of China were randomly selected by using stratified cluster random sampling methods,all soldiers in that received a questionnaire survey,and then we collected all questionnaires and summarized the soldiers’ digestive system disease spectrum, and discussed risk factors for diseases. Results 3055 valid questionnaires were finally collected,896 soldiers had digestive symptoms, and of all symptoms diarrhea was the most common,whose ratio was 29.3%; 507 soldiers suffered from digestive system diseases, including gastritis,enteritis,digestive ulcer,functional bowel disease, which were more common.By the logistic regression analysis, length of military service time,identity of army officer,length of a field training time,training pressure,chilled abdomen,and defiled food were possible risk factors for digestive diseases.Conclusions During a field training,a healthy and regular diet, the thermal outfit,appropriate decompression of negative emotions and reasonable training time may effectively decrease the risk of digestive diseases, so that to reduce attrition.

Relevance. Information about the cardiovascular diseases risk factors (CVD RF) for in men with chronic lung inflam-matory pathology (CLID) is contradictory and requires clarification. Aim. To evaluate the peculiarities of CVD RF in men under 60 years of age with CLID in myocardial infarction (MI) to improve prevention. Material and methods. The study included men aged 19-60 years old with type I myocardial infarction. Patients are divided into two age-comparable groups: I - the study group, with CLID - 142 patients; II - control, without it - 424 patients. A comparative analysis of the frequency of observation of the main and additional cardiovascular risk fac-tors in groups was performed. Results. In patients of the study group, more often than in the control group we observed: hereditary burden of is-chemic heart disease (40.8 and 31.6%, respectively; p = 0.0461) and arterial hypertension (54.2 and 44.6%; p = 0.0461), frequent colds (24.6 and 12.0%; p = 0.0003), a history of extrasystoles (19.7 and 12.7%; p = 0.04); chronic foci of infections of internal organs (75.4 and 29.5%; p˂0.0001), non-ulcer lesions of the digestive system (26.1 and 14.6%; p = 0.007), smoking (95.1 and 66.3%; p˂0.0001), MI in winter (40.8 and 25.9%; p = 0.006). Less commonly were observed: oral cavity infections (9.2 and 23.6%; p˂0.0001); hypodynamia (74.5 and 82.5%; p = 0.0358), over-weight (44.4 and 55.2%; p = 0.0136), a subjective relationship between the worsening of the course of coronary heart disease and the season of the year (43.7 and 55.2%; p = 0.0173) and MI - in the autumn (14.1 and 21.9%; p = 0.006) period. Conclusions. The structure of CVD RF in men under 60 years of age with CLID with MI is characterized by the pre-dominance of smoking, non-ulcer pathology of the digestive system, frequent pro-student diseases, meteorological dependence, a history of cardiac arrhythmias and foci of internal organ infections. It is advisable to use the listed factors when planning preventive measures in such patients.

: As a medicinal fungus, Inonotus obliquus (IO) has been widely used in the treatment of cancer and digestive system diseases. Despite the progress that has been made in the studies of IO and its active compounds, their applications in other important clinical diseases, such as cardiovascular diseases, which are major global issues with limited treatment strategies, are seldom reported. This review summarizes the separation and purification methods of chemical components of IO, the advances in their applications, and research progress on the pharmacological effects and related mechanisms of IO in disease prevention. This review will help researchers and clinicians to further understand the pharmacological functions and mechanisms of IO and its active components, which may extend their medical applications in the prevention and treatment of other diseases in addition to tumors and digestive system diseases in the near future.

Abstract Background The exact rate and relevant risk factors of radiation pneumonitis (RP) for non-small-cell cancer (NSCLC) patients treated with the combination of epidermal growth factor receptor-tyrosine kinase inhibitors (EGFR-TKIs) and thoracic radiotherapy have not been reported. Thus, this study aimed to investigate the rate and risk factors of RP for EGFR-positive NSCLC patients simultaneously treated with first-generation EGFR-TKI and TRT. Patients and methods We retrospectively evaluated NSCLC patients simultaneously treated with first-generation EGFR-TKI and thoracic radiotherapy between January 2012 and December 2019 at Shandong Cancer Hospital and Institute, Shandong, China. RP was diagnosed via computed tomography and was classified according to the Common Terminology Criteria for Adverse Events v5.0. The risk factors of RP were identified using uni- and multivariate analyses. Results Of the 67 patients included, 44.78% (30/67) developed grade ≥ 2 RP. Grade ≥ 2 RP occurred within a median of 3.48 (range 1.07–13.6) months. The EGFR-TKI icotinib, ipsilateral lung V30 > 34%, and overlap time of > 20 days between EGFR-TKI and thoracic radiotherapy were identified to be independent predictive factors of grade ≥ 2 RP. Conclusions Grade ≥ 2 RP is highly frequent in NSCLC patients simultaneous treated with first-generation EGFR-TKI and thoracic radiotherapy. Icotinib, ipsilateral lung V30 ≤ 34%, and overlap time of ≤ 20 days for EGFR-TKI and thoracic radiotherapy will be helpful to lower the risk of RP in these patients. The addition of thoracic radiotherapy should be cautious, and the treatment strategies can be optimized to reduce the rate of RP for patients treat with simultaneous EGFR-TKI and thoracic radiotherapy.

In the context of the reform of the Armed Forces of the Russian Federation, the issues of recruiting troops with healthy, physically developed and mentally stable young conscripts are of paramount importance. Only citizens "A" - fit for military service and "B" fit for military service with minor restrictions are subject to conscription. When analyzing the results of medical examination of persons of military age, it was found that over the studied period of time, fitness for military service for health reasons decreased by 3.8% (from 78.3% in 2016 to 74.5% in 2020). However, despite the general decrease in fitness for military service, there is an increase in the fitness for military service "A" by 13.5% (from 23.4% in 2016 to 36.9% in 2020). Every year, during the period of work of the draft commissions, a significant number of citizens (more than 10%) were sent for additional examination to medical organizations. As a rule, the examination of conscripts was carried out on an outpatient basis (up to 85%) and much less often in an inpatient setting (up to 15%). The first place was occupied by diseases of the musculoskeletal system and connective tissue, which accounted for 24.9%. Mental and behavioral disorders ranked second with an indicator of 18.7%. The third place was taken by diseases of the circulatory system - 14.9%. Diseases of the eye and adnexa ranked fourth - 8.2%. Diseases of the digestive system were less common, accounting for only 4.5%. Thus, in total, the listed diseases accounted for 71.2% and were the main ones in determining the fitness of conscripts for military service for health reasons.

Avascular osteonecrosis (AVN) of the talus (AVNT) is a painful and challenging clinical diagnosis. AVNT has multiple known risk factors and etiologies and presents at different stages in severity. Given these unique factors, the optimal treatment solution has yet to be determined. Both joint-preserving and joint-sacrificing procedures are available, including core decompression and arthrodeses. Recently, new salvage and replacement techniques have been described including vascularized pedicle bone grafts and total talus replacement using patient-specific prosthesis; however, evidence remains limited. This review examines the current trends AVNT treatment and the emerging data behind these novel techniques.

Analysis of digestive function (DF) disorders in cosmonauts-participants in 83 main missions to the station Mir and ISS showed that episodic DF problems had been abated successfully with the help of onboard medicaments. Comparative assessment of the spaceflight adversities and digestive disease risk factors resulted in drawing up a list of predictable digestive diseases that includes the gastroesophageal reflux disease, irritable bowel syndrome and functional dyspesia. Following the ensuing recommendations, the onboard medical kits have been complemented with the proton pump inhibitors, H2-histamine receptor antagonists, prokinetics of new classes, antibacterials, ursodesocholic acid preparations, and probiotics to treat gut dysbiosis. A broad use of prefilled syringes is advisable.

BackgroundAnalyzing risk factors for hyperperfusion-induced intracranial hemorrhage (HICH) after carotid artery stenting (CAS) in patients with symptomatic severe carotid stenosis.MethodsThis study retrospectively analyzed clinical data of 210 patients, who had symptomatic severe carotid stenosis (70–99%) and received CAS treatment between June 2009 and June 2015, and evaluated the relationship of HICH with patients’ clinical baseline data, imaging features, and treatment strategies.ResultsSeven patients (3.3%) developed HICH after CAS. The incidence of HICH among patients with near total occlusion was significantly higher than among those without (10.1% vs 0%, P<0.001). Out of the seven, five had no development of either anterior or posterior circulations, and two had no development of anterior circulation and poor development of posterior circulation. Results showed that patients with poor compensation of Willis’ Circle were more likely to develop HICH compared with other patients (P<0.001). All patients received preoperative CT perfusion. TTP index was defined as the TTP ratio between the affected and contralateral side. The results showed that the TTP index was significantly different between the HICH group and non-HICH group (1.15±0.10 vs 1.30±0.15, P<0.001). An analysis of the ROC curve indicated that patients with TTP index >1.22 were more likely to develop HICH compared with other patients (sensitivity 100%, specificity 75.9%).ConclusionsPatients with severe unilateral carotid stenosis, the presence of near total occlusion, poor compensation of Willis’ Circle, and preoperative TTP index>1.22, have a higher risk of developing HICH after CAS.
