Herbal medicines are being used since ancient times and are an important part of the alternative and traditional medicinal system. In recent decades, scientists are embracing herbal medicines based on the fact that a number of drugs that are currently in use are derived directly or indirectly from plant sources. Moreover, herbal drugs have lesser side effects, albeit are potentially strong therapeutic agents. The herbal medicine market is estimated to be around US $62 billion globally. Herbal medicine has gained widespread acceptance due to its low toxicity, low cost, ease of accessibility and efficacy in treating difficult diseases. Safety and efficacy are another important factors in the commercialization process of herbal medicines. Nanotechnology has been shown to be potentially effective in improving the bioactivity and bioavailability of herbal medicines. Development of nano-phytomedicines (or by reducing the size of phytomedicine), attaching polymers with phytomedicines and modifying the surface properties of herbal drugs, have increased the solubility, permeability and eventually the bioavailability of herbal formulations. Novel formulations such as niosomes, liposomes, nanospheres, phytosomes etc., can be exploited in this area. This article reviews herbal medicines, which have prominent activity in the Central Nervous System (CNS) disorders and reported nano-phytomedicines based delivery systems.

Use of complementary and alternative medicines (CAM) for preventive and therapeutic purposes has increased tremendously in the last two decades internationally. The manufacturers of these products are not required to submit proof of safety or efficacy to the Food and Drug Administration. As a result, the adverse effects and drug interactions associated with them are largely unknown. In this chapter, the author presents interactions of herbal medicines with other medicines (herbal or non-herbal). A large number of herbal drugs, including from single drug to a variety of mixtures have been used to treat kidney disorders. Herb-herb or herb drug interaction has been reported intensively during last decade, therefore it becomes important to keep an eye on the use of combination herbal therapy in order to avoid serious results because of interactions with each other. Due to the growing awareness about the interactions and side effects of herbal drugs/supplements over the past few years, regulatory bodies are working on these issues and pharmacopoeias are being developed for reference.

In this review article, we aim to document the efficacy, adverse effects, mode of action, required doses, and availability of the major vaccines available in Pakistan till 20 May 2021. We reviewed all available literature on COVID-19 vaccines in PubMed and Google scholar. We also reviewed articles from grey literature. Currently, Pfizer–BioNTech and Moderna, Sinopharm (China), Sputnik V (Russia), CoronaVac (popularly known as Sinovac) (China), Cansino, and Vaxzevria vaccines have been authorized for emergency use in several countries. Pakistan has sanctioned the use of all the aforementioned vaccines except Pfizer and Moderna. As per their efficacy, Pfizer and Moderna have been found most effective among all the vaccines with 95% effectiveness, while the Vaxzevria, Sputnik V, Sinopharm, and Cansino have shown 70%, 91.6%, 79.34%, and 90% effectiveness, respectively. All the vaccines have shown milder side effects like headache, fever, and pain on injection sites. To curb the pandemic, more clinical trials are being conducted throughout the world. Importantly public awareness is warranted to achieve the target of vaccinating a large population.

INTRODUCTION: The concomitant intake of herbal medicine and conventional medicine amongst patients is steadily increasing worldwide. However, concerns have been raised regarding potential adverse effects and drug interactions when consuming both medications together. Therefore, this work aimed to comprehending the perception of Type 2 Diabetic Mellitus (T2DM) patients on conventional medicine and the manner of its use with herbal medicine in managing diabetes mellitus. METHODS: A qualitative study was conducted among 28 diabetic patients in four government clinics under the jurisdiction of the state of Negeri Sembilan, Malaysia. Purposive sampling was employed to recruit informants who consumed herbal medicine alongside their prescribed conventional medicine. This study employed semi-structured interviews, which were all digitally recorded, transcribed, and analysed thematically. RESULTS: This study found that most of the patients perceived the negative side effects of conventional medicine, which led them to consider incorporating herbal medicine in treating diabetes. Such perception was influenced by the personal experiences of their family members and friends. A majority of the informants utilised herbal medicine as a complementary consumption to conventional medicine as opposed to implementing it as an alternative. However, they would alter the dosage and intake time of conventional medicine for several reasons, such as to complement their herbal medicine intake, being fed-up with conventional medicine, and reduce the risk of its side effects. Lastly, this study revealed that some of the patients noted their intention to discontinue conventional medicine, while some even tried to quit. CONCLUSIONS: A complete understanding of patient’s perceptions regarding conventional medicine and its usage with herbal medicine will aid healthcare providers to deliver education on the importance of conventional medicine usage compliance. Additionally, people should be aware that its combination with herbal medicine when consumed may present adverse effects and subsequently cause serious health problems.

Seizures and Epilepsy are those major conditions that are common symptoms of many diseases that affect the human nervous system. It is one of the most chronic and commonest of the neurological disorders that is prevalent in almost 5crore people around the world. Even though the drugs are effective and potent, there are various side effects and adverse effects are associated with those drugs. General side effects include the nausea and vomitings, many other specific side effects include altered mental consciousness, confusion anorexia and excessive aggression are also noted in many cases of drugs. In this research, herbal formulations were designed to fight back the free radicals that are generated in the brain and those protective enzyme levels were analyzed to estimate the activity of the formulation in the brain tissue. In the research, the prepared formulation showed a dose dependent activity in restoring the brain protective enzymes and balancing them. The formulation contained herb powders that contain antioxidant chemical constituents which helped for the anti-epileptic formulation. The herbal capsules at dose 500mg/kg showed a better activity compared to the standard drug but without notable side effects and adverse effects.

Mental illness is one of the most fundamental emotional states of conscious being which becomes unbalanced and leads to neurological disorders. It is a significant contributor to the global burden of disease and there is a strong desire to devise a remedy. Ayurveda represents a traditional medicine system of India that endorses antiquity than western medicine and relies on formulations rather than their active components. It has categorised a group of herbal medicines to improve mental abilities. Conversely, the mechanistic details of the therapy are not available in ayurvedic literature and there is a need to fortify this system with modern scientific analysis. The design of nanosystems encompasses promising characteristics in the field of drug delivery with a limited dosage thereby decreasing adverse effects. This chapter confers stratagems of devising polymeric nano herbal formulations as smart nano brain drugs to espouse mental health.
