aim of this study was to evaluate the efficacy of periodontal scaling and oral hygiene instruction for patients with mild chronic periodontitis and rheumatoid arthritis through clinical periodontal parameters and laboratory tests for CRP (C- reactive protein) and ESR (erythrocyte sedimentation rate). Twelve individuals with rheumatoid arthritis and 12 healthy individuals were evaluated, with a mean age of 45.38 and 46.75 respectively, all female and with mild, chronic periodontitis. The participants were evaluated clinically and periapical radiographs were taken (T1), after which periodontal treatment was instituted. After ninety days (T2), new clinical and laboratory data were obtained. Probing depth, bleeding index, and plaque indexes were observed in both groups, and the results demonstrated reductions but no statistical differences. Laboratory tests for CRP and ESR produced higher values for the rheumatoid arthritis group with T1- T2 reductions on the average, but the values were still higher than in the health group. We conclude that periodontal therapy in patients with rheumatoid arthritis and mild chronic periodontitis showed a improvement in the periodontal clinical parameters and laboratory tests that were evaluated.

Carrot juice is a critical source of vitamins, selenium, and β-carotene, which is suggested to protect from Rheumatoid Arthritis (RA). The present study aimed to show the effect of carrot juice supplementation compared to selenium tablet supplementation, so our study includes (44) blood samples belonging to young men with RA. All patients aged (20-45) years, 44 blood samples were obtained before treatment at week (0), twenty-four men with RA were supplied with fresh carrot juice. The other group of 20 patients was given an artificial selenium supplement for 21 days as well, and the results were analyzed.The samples were collected from Kirkuk hospital, and external specialized clinical from October/2019 to September /2020; experimental groups were divided into three groups: Group 1 : (44) men Rheumatoid arthritis (RA) before treatment, Group 2: (24) men with RA+ Carrot juice, Group 3: (20) men with RA +Selenium Tab, We reach to following results: the patients who have RA consumption Carrot juice and patients take up Se tab. show significant decrease respectively in RBCs, WBCs, ERS, and RF compared with the Rheumatoid arthritis group, so as the results show a significant decrease in Leptin, IL-6, C-Reactive Protein, and TNF-α concentrations in comparison with the Rheumatoid arthritis group. In contrast, we found a significant increase in GSH, Selenium concentrations, and VD3 in men with RA administration carrot juice and patients' consumption Se tab. Respectively compared with the RA group and, finally, our finding shows no difference in Ceruloplasmin in experimental groups.

Background and Objectives: C?reactive protein (CRP) is a type I acute phase reactant. A number of studies have reported elevated gingival crevicular fluid (GCF) CRP levels in periodontitis subjects, which decrease following periodontal therapy. Effect of diode laser as an adjuct to Scaling & Root planing is also well stablished. The aim of the present study was to evaluate the effect of periodontal treatment SRP with diode laser, on CRP levels in GCF in patients with chronic periodontitis. Materials and Methods: A total of 40 subjects with moderate periodontitis based on community periodontal index scores, were included in the study. Periodontal therapy was performed dividing each side of jaw as a group (Split mouth); one side SRP alone & another Diode laser with SRP. GCF was collected from each subject at Baseline (prior to treatment) and 1 month after periodontal therapy. The collected sample was subjected to biochemical analysis to detect CRP levels by using a commercially available highly sensitive kit. Results: The present study demonstrated that the mean CRP values at baseline were found to be 0.11043mg/l in side-I (side treated with SRP alone); 0.11042mg/l in side-II (side treated with SRP & laser) of the patient, which reduced to 0.4148 mg/L in side I and 0.3985mg/L in side II after treatment, which are highly significant according to statistical analysis but the changes between two sides were non- significant statistically. Interpretation and Conclusion: Within the limitations of this study, it can be concluded that periodontal therapy is able to reduce the GCF C reactive protein level significantly, but there was no statistically significant result in CRP level, between sides treated with SRP & SRP with laser. Introduction: Rheumatoid arthritis (RA) is an autoimmune disease which mainly attacks synovial membrane and causes systemic manifestation. During treatment, controlling disease activity is needed to prevent further complication. On the other hand, medications used in the treatment of RA may bring various side effects. It is important to evaluate side effects from the given therapy.Methods: This study aimed to evaluate response and side effects of therapy in RA patients. The samples were collected from 59 RA patients at Rheumatology Division of Outpatient Clinic in Department of Internal Medicine Dr. Soetomo General Hospital Surabaya in 2017. This study method was descriptive observational with cross sectional design using medical records.Results: Pain was reduced in 83.1% patients, Erythrocyte Sedimentation Rate (ESR) increased in 61.4% patients, and C-Reactive Protein (CRP) decreased in 50% patients. Based on the statistic analysis, ESR decreased significantly (p = 0.012) while CRP decreased not significantly (p = 0.415). The side effects were observed from clinical and laboratory data. Based on clinical symptoms, there were alopecia in 1.7% patient, dyspepsia in 78% patients, infection in 27.1% patients, and other symptoms including itchy skin, neuropathy, hyperuricemia, hyperkalemia and Acute Kidney Injury (AKI). Meanwhile, abnormalities in laboratory data include increased aspartate transaminase (AST) in 3.8% patients, increased alanine transaminase (ALT) in 26.1% patients, increased Blood Urea Nitrogen (BUN) in 7.9% patients, increased creatinine serum in 7.9% patients, decreased hemoglobin in 15.5% patients, and decreased leukocytes in 3.4% patients.Conclusion: Most patients had a good therapeutic response based on decreased pain, while ESR had a significant decrease and CRP did not have significant decrease. Pendahuluan: gingival enlargement umumnya terjadi akibat inflamasi kronis yang disebabkan oleh faktor lokal, seperti induksi plak pada gingiva dan faktor sistemik seperti  pengaruh hormon dan obat-obatan sistemik tertentu. Hidrosefalus merupakan kondisi yang diakibatkan adanya volume cerebrospinal fluid (CSF) berlebih yang tidak terserap sempurna, dalam jumlah yang sangat tinggi pada ventrikel di otak dan terkadang juga terdapat di ruang subarachnoid, yang menyebabkan terjadinya dilatasi ventrikel secara progresif. Kondisi ini menghasilkan peningkatan tekanan intrakranial dan sering merusak jaringan di sekitarnya. Pasien hidrosefalus menunjukkan pembesaran kepala dan mengalami keterlambatan pertumbuhan. Tujuan penelitian laporan kasus ini adalah menjelaskan keberhasilan terapi fase inisial perawatan periodontal pada gingival enlargement pasien anak dengan hidrosefalus. Laporan kasus: Pasien hidrosefalus laki-laki berusia 12 tahun, mengalami pembesaran gingiva pada rahang atas, sering berdarah pada saat tersentuh sikat gigi ataupun terkena sentuhan lainnya, terjadi kurang lebih sejak dua bulan yang lalu, pasien tidak mengonsumsi obat-obatan secara sistemik. Hasil pemeriksaan klinis dan radiologis dapat ditegakkan diagnosis inflammatory gingival enlargement rahang atas disertai periodontitis kronis gigi 27, dengan diagnosis banding periodontitis kronis. Gingival enlargement merupakan faktor predisposing pembentukan plak. Gingival enlargement pada pasien ini dirawat dengan terapi inisial yaitu Oral hygiene Instruction (OHI), scaling, root planing, dan kontrol. Simpulan: Terapi fase inisial periodontal berupa OHI, scaling, root planing, dan kontrol, berhasil menghilangkan gingival enlargement pada pasien anak dengan hidrosefalus.Kata kunci: Gingival enlargement, hidrosefalus, perawatan inisial periodontal. ABSTRACTIntroduction: Gingival enlargement generally occurs due to chronic inflammation caused by local factors, such as plaque induction on the gingiva and systemic factors such as hormonal influences and certain systemic drugs. Hydrocephalus is a condition that results from the incompletely absorbed excess volume of cerebrospinal fluid (CSF), with very high amounts in the ventricles in the brain and sometimes also in the subarachnoid space, leading to progressive dilation of the ventricles. This condition results in increased intracranial pressure and often damages surrounding tissue. Hydrocephalus patients show head enlargement and growth delay. This case report was aimed to describe the success of the initial phase of periodontal therapy in gingival enlargement of paediatric patients with hydrocephalus. Case report: A 12-year-old male hydrocephalus patient, had enlarged maxillary gingiva, often bled when touched by a toothbrush or when exposed to other touches, occurred for about two months prior, and the patient did not take any medication systemically. The clinical and radiological examination results can confirm the diagnosis of inflammatory maxillary gingival enlargement with chronic periodontitis in tooth #27, with a differential diagnosis of chronic periodontitis. Gingival enlargement is a predisposing factor for plaque formation. Gingival enlargement in this patient was treated with initial therapy, namely oral hygiene instruction (OHI), scaling, root planing, and control. Conclusion: The initial periodontal therapy phase in the form of OHI, scaling, root planing, and control, succeeded in eliminating gingival enlargement in paediatric patients with hydrocephalus.Keywords: Gingival enlargement, hydrocephalus, initial periodontal therapy.

Purpose: The purpose of this study was to evaluate the relationship between the progression of arthritis and the erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) through the treatment-resistant mono joint rheumatoid arthritis treated with arthroscopic synovectomy.Methods: We retrospectively reviewed 65 patients who underwent arthroscopic synovectomy for rheumatoid wrist or elbow which had not responded to systemic treatment for at least 6 months. The main treatment response evaluation methods of rheumatologists were the American College of Rheumatology (ACR) response criteria, and the ACR/EULAR (European League Against Rheumatism) remission criteria. The medical records, results of laboratory tests and radiographs of wrist or elbow that had been performed by rheumatologists before arthroscopic synovectomy were investigated. We examined whether the results of laboratory tests and arthritic change of radiograph had statistically significant correlation. Results: The mean duration of joint symptom until referral to orthopedic department was 23 months (range, 1 month to 9.2 years). During this period, rheumatologists had took average one X-ray (range, 0–3). Forty-seven patients (72.3%) had CRP of 1 mg/dL or less as included in the ACR/EULAR remission criteria. Seventeen out of 65 patients who had two or more X-ray were able to evaluate the progression of arthritis. The change of arthritis and mean ESR (p=0.787) or CRP (p=0.303) the same period didn’t have statistically significant relationship (Spearman correlation analysis).Conclusion: Since the method of evaluating the treatment response of rheumatologists is to assess the systemic condition, arthritis of one joint that is resistant to treatment is considered to require periodic radiographs as method to assess itself.

Abstract Background Recent studies have shown that coagulations indicators hava a strong correlation with apoptosis ,flammatory indexes.In the present study, we measured coagulation indicators such PAF,PGI2 and TXB2 in patients with RA and investigated their association with apoptosis,flammatory indicators and Sharp scores of rheumatoid arthritis (RA). Methods A total of 90 patients diagnosed with rheumatoid arthritis were enrolled for comparisons. The clinical and laboratory data of patients were retrospectively reviewed. The X-ray findings of both hands were scored according to Sharp's criteria. In addition, the statistics of patients with apoptosis indicator,coagulation indicator and flammatory indicator. Explored the relationship between coagulation indiccators and the above indicators. Results In this study,there was a strong correlation between the abnormal increase of apoptosis parameters,immune inflammation indicators,Sharp Score and the increase of coagulation indicator in patients with rheumatoid arthritis, and the increase of Sharp Scores,Caspase-3,FasL indexes or the decrease of C-reactive protein (CRP)could be risk factors for the development of RA. The higher the coagulation indicator, the higher the Sharp score,and the high parameters of hypercoagulable state RA. Conclusions Higher coagulation indicators are associated with RA disease activity, as well as apoptosis ,flammatory indicators and decreased physical activity. Moreover, coagulation indicator are associated with Sharp scores.Coagulation indicator are important for the diagnosis of rheumatoid arthritis and are potential markers of activity and hypercoagulable state in RA patients.
