Vector control using insecticide is a major effort to control Dengue Haemorrhagic Fever. The excessive use of insecticides leads to insecticide resistance in Aedes aegypti. This study aimed to determine the resistance status of Aedes aegypti in North Sumatra and Jambi Province to malathion, deltametrin, cypermethrin, lambda cyhalothrin and alpha cypermethrin. A cross sectional study was conducted during 2015, the research location covers North Sumatera area (Tebing Tinggi, Deli Serdang, and Pematang Siantar) and Jambi (Muaro Jambi, Jambi, and Batang Hari). Susceptibility test using WHO kits was applied on F1 generation and interview with Local Public Health Officer was carried out to obtain information about the usage of insecticide in both provinces. Resistance to malathion, cypermethrin, and lambda cyhaltorhin was observed in Ae. aegypti populations from all study area. Tolerant status to deltamethrin only found in Aedes aegypti from Deli Serdang, whilst the other strains were resistant. Ae. aegypti population from all study area were tolerant to alpha cypermethrin. Malathion and cypermethrin were the only insecticide ever used in both provinces. We conclude that alpha cypermethrin was effective to combat Ae. aegypti and can be used as an alternative insecticide for dengue control program in Northern Sumatera and Jambi Province.

Abstract Background Glucose 6-phosphate dehydrogenase deficiency (G6PDd) plays a central role in readiness assessment for malaria elimination in Myanmar by 2030 that includes primaquine (PQ) use. The risk of hemolysis in G6PDd individuals hampers the widespread use of primaquine safely in malaria-infected patients. In the pre-elimination era, it is important to screen initially for asymptomatic malaria in combination with G6PD deficiency by applying more sensitive diagnostic tools. Therefore, this study examined the proportion of G6PDd and the distribution of G6PD genotypes among malaria-infected national groups in Myanmar before initiation of malaria elimination strategies. Methods A cross-sectional study in one township each with high malaria burden from two states in the western part of Myanmar, was conducted during 2016-2018, and 320 participants (164 Rakhine and 156 Chin National groups) were recruited. We used RDT and ultrasensitive polymerase chain reaction (us PCR) method to confirm malaria infection, and a G6PD RDT(CareStart) to detect G6PDd and PCR/restriction fragment length polymorphism (RFLP) method to confirm the variant of G6PDd for genotyping. G6PD enzyme activity was measured by G6PD Biosensor (CareStart). Results Malaria positivity rates detected by RDT were lower than those detected by us PCR in the combined samples [13% (42/320) vs. 21% (67/320)] as well as in the Rakhine samples [17% (28/164) vs. 25% (41/164)] and in Chin samples [9% (14/156) vs. 17% (26/156)]. G6PD deficiency rates were approximately 10% in both the combined samples and specific national groups. For G6PD enzyme activity in the combined samples, G6PDd (defined as < 30% of adjusted male median) was 10% (31/320) and severe G6PDd (< 10% of AMM) was 3% (9/320). Among malaria-infected patients with positive by both RDT and usPCR, G6PDd was less than 20% in each national group. G6PD genotyping showed that the G6PD Mahidol (G487A) was the major variant. Conclusions The varying degree of G6PDd detected among malaria-infected national groups by advanced diagnostic tools, strongly support the recommend G6PD testing by the National Malaria Control Program and the subsequent safe treatment of P. vivax by primaquine for radical cure. Establishing a field monitoring system to achieve timely malaria elimination is mandatory to observe the safety of patients after PQ treatment.

Intestinal parasite infections can have detrimental health consequences in children. In Mozambique, soil-transmitted helminth (STH) infections are controlled through mass drug administration since 2011, but no specific control program exists for enteric protozoa. This study evaluates STH and protozoan infections in children attending healthcare in Manhiça district, Southern Mozambique, and its association with water and sanitation conditions. We conducted a cross-sectional study in children between 2 and 10 years old in two health centers (n = 405). A stool sample and metadata were collected from each child. Samples were analyzed by multi-parallel real-time quantitative PCR (qPCR). We fitted logistic regression-adjusted models to assess the association between STH or protozoan infection with household water and sanitation use. Nineteen percent were infected with at least one STH and 77.5% with at least one enteric protozoon. qPCR detected 18.8% of participants with intestinal polyparasitism. Protected or unprotected water well use showed a higher risk for at least one protozoan infection in children (OR: 2.59, CI: 1.01–6.65, p-value = 0.010; OR: 5.21, CI: 1.56–17.46, p-value = 0.010, respectively) compared to household piped water. A high proportion of children had enteric protozoan infections. Well consumable water displayed high risk for that.

SUMMARY BACKGROUND The Medical Control Program for Occupational Health establishes the required supplementary exams, according to the activity exercised by the worker and its inherent risks. The Regulatory Norm No. 35, recently deployed, stipulates that at-height workers must undergo electrocardiogram exams as an additional routine examination. OBJECTIVE To evaluate the electrocardiographic standard in at-height. METHODOLOGY A cross-sectional study, developed from May 2014 to January 2015 with male at-height workers. Anthropometric and clinical data were collected after the electrocardiogram (ECG). The workers included in the program were evaluated by an occupational medicine service of Serra Gaúcha, responsible for medical assessment and occupational tests. All workers were assessed by the researcher. RESULTS A total of 561 at-height workers participated in the study. The average age was 35.9 ± 12.2 years. A total of 176 (31%) presented electrocardiographic changes in the analysis of the resting ECG. Regarding the amendments in the resting ECG, 15.7% were attributed to changes in ventricular repolarization, 8% as blocks conductions, and 5.8% as left ventricular overload. Demographic variables were not associated with changes in the electrocardiographic tracing CONCLUSION This study demonstrated the electrocardiographic alterations and the profile of at-height workers. These findings can help determine prevention strategies and provide warnings of possible future harms to the health of these workers.

Determinants of dengue mosquito eradication (PSN) in the households in Bogor in 2016PurposeThis study aimed to determine the factors that affect of PSN on housewives in Cibinong and the factors found that influence were: job, knowledge, attitude, behavior of housewife, counselling, active jumantik, giving abatement and the number of container.MethodsA cross-sectional study was conducted using 125 housewifes in Cibinong sub-district. The statistical analysis used chi-square and multiple logistic regression tests.ResultsThis study showed that counseling and the existence of jumantik cadre were the determinants of the application of mosquito nest eradication. Counseling was the most influential factor on the application of mosquito nest eradication.Conclusion Dengue control and prevention counseling should be given to all age groups in the community regularly and it is recommended to reactivate jumantik cadres.

RESUMOObjetivo: avaliar as competências dos ACS no desenvolvimento das atribuições esperadas e definidas pelo Ministério da Saúde nas ações de pré-natal de risco habitual, desenvolvidas no âmbito da ESF, em relação à prevenção da infecção pelo zika vírus na gestação. Método: estudo quantitativo, descritivo, transversal, com coleta de dados em 14 equipes da Estratégia de Saúde da Família, mediante um questionário com três partes: A (enfrentamento ao Aedes aegypti); B (pré-natal de baixo risco) e C (medidas de prevenção pessoal contra o Zika vírus), analisadas por agrupamentos e pelo Teste Exato de Fisher. Resultados: melhor desempenho foi observado em B, seguido de A. As maiores dificuldades estavam relacionadas com a atividade C. Conclusão: há um panorama favorável de atuação desses profissionais, embora com dificuldades quanto à alimentação dos sistemas de informação, à baixa participação comunitária e à pouca integração com o trabalho das equipes. Background: Tobacco is a major risk factor for a number of diseases affecting all age groups. One person dies every six seconds due to tobacco use and up to half of current users will eventually die of a tobacco-related disease. Government of India has enacted cigarette and other tobacco products act (COTPA) in the year 2003 to control tobacco use. The objective of study was to estimate the level of compliance to selected sections (4 to 9) of COTPA (cigarette and other tobacco products – prohibition of advertisement and regulation of trade and commerce, production, supply and distribution) act, 2003 in Koppal district, Karnataka.Methods: The present study was a cross sectional study conducted as an independent evaluation of COTPA act 2003 in Koppal district as per National Tobacco Control Program guidelines. Cluster sampling technique was used for sample selection and sample consisted of 158 public places, 102 educational institutional, 87 shops/ sell points and 60 tobacco products. Data entry and analysis was done using microsoft office excel 2013.Results: The prevalence of compliance to section 4, 5, 6A, 6B, 7, 8 and 9 of COTPA act 2003 in Koppal district were 80.3%, 96.5%, 98.9%, 64.8%, 53.3%, 63.3% and 50% respectively.Conclusions: Compliance level was poor especially in relation to display of health warnings on tobacco products which needs to be tackled through strict enforcement of the existing laws.

Background: Globally, due to low health care coverage, there have been continued efforts to ensure that there is increased accessibility to quality, affordable and equitable universal health care (UHC) services in most parts of developing countries, such as within Nakuru county in Kenya.Methods: The study focused on determining health workforce gaps in health care facilities that affect the implementation of UHC in Nakuru West Sub County. Cross-sectional study design was applied, data was collected using structured questionnaires, analyzed using statistical package for social sciences 23rd version and presented in charts for ease comparison.Results: The findings of this study indicated that there was shortage of HWCs. This implies that the staffing in the facilities located in Nakuru West Sub-County is inadequate for the implementation of the Kenyan government UHC and it was evident that the mostly affected sections were the nursing, public health officer (PHO), pharmacy, clinical officers and laboratory in that order. Most alarmingly, majority of the HCWs (58.2%) were not being appreciated for good work, despite the fact that they play a key role in the implementation of UHC.Conclusions: It is therefore imperative for the county government which is the arm of the Kenyan government to ensure that they recruit adequate health care workers in all cadres to serve the continuously rising population and HCWs should have adequate level of education, acceptable training skills and be well- motivated.

Purpose The purpose of this paper is to investigate the influence of breeding containers on the production of Aedes mosquitoes after a vector-control program in households that might support dengue transmission in tourist attraction areas of Bang Kachao Riverbend, Thailand. Design/methodology/approach A cross-sectional study was conducted in an ecotourism area of Bang Kachao Riverbend, Thailand, during the period October 2016 to September 2017. A total of 832 households from five villages participated in the study. Data collection employed an interview questionnaire, larval mosquito survey and measurements of the chemical properties of the water in each container. A binary logistic regression model was used to investigate the characteristics of water containers influencing the presence or absence of Aedes immatures. Findings The study found that water containers located inside households had a highly presence of Aedes immatures (2.22 times) compared with outdoor containers. Water containers without lids and water containers with ineffective had a high presence of Aedes immatures (3.69 and 2.54 times, respectively). In addition, the chemical properties of the water inside the containers, such as pH, influenced the presence of Aedes immatures (1.76 times) (OR=1.76, 95% CI=1.59–1.96). Originality/value The study results emphasized the characteristics of water containers in households located in a tourist attraction area. The findings may inform public health vector-control messages for households located in the tourist attraction area.

The study objective is to describe patients and professionals’ perspectives on the Tuberculosis Control Program (PCT) in Recife, Brazil, contributing to the program evaluation. A cross-sectional study was conducted in three purposively selected sites, representing the three levels of care in the public health system. All eligible PCT patients in sites A, B and C were invited to participate (n = 123). Physicians, nurses, pharmacists and community health agents providing care to PCT patients in these sites, plus their managers, were purposively selected. Data were collected by means of interviews with 44 patients and a questionnaire to 24 professionals. Instruments encompassed previously published items to capture stakeholders’ perspectives (16 and 12 closed-questions, respectively), grouped into categories. The overall evaluation by patients was unsatisfactory (median score 35%; third quartile below 50%; interquartile range 21.9%). Analysis of scores by categories showed that opinions about organizational accessibility were significantly worse than about economic and geographical accessibility, taken together. Overall the median score attributed by professionals was 52% (third quartile below 65%). Professionals had significantly worse opinions about diagnosis, clinical and laboratory assistance. Patients and professionals’ perspectives highlight potential opportunities for improvement. Our findings can be used by managers as a starting point for shared decision-making, potentially contributing to a better performance of the PCT in Recife and, consequently, reducing the risk posed by tuberculosis.

The spatial distribution of surveillance-reported dengue cases and severity are usually analyzed separately, assuming independence between the spatial distribution of non-severe and severe cases. Given the availability of data for the individual geo-location of surveillance-notified dengue cases, we conducted a cross-sectional study to model non-severe and severe dengue simultaneously, identifying the spatial patterns of dengue distribution, using individual and area level covariates within a hierarchical Bayesian model. Results showed that age and socioeconomic status were associated with dengue presence, and there was evidence of clustering for overall cases but not for severity. Our findings inform decision making to address the preparedness or implementation of dengue control strategies at the local level.
