Introduction: Both bone diaphysis fracture of forearm is common in children of hilly area in Nepal because the children climbs tree and cliff for playing and cutting grass. Close reduction and casting is the preferred method of treatment these fractures but the chances of re-displacement is very high. Intramedullary nailing with titanium elastic nails or rush pins is widely accepted these days with good outcome. Titanium nails are popular in western world but is costly whereas rush pins are cheap and are preferred in developing world. The aim of this study was to evaluate the functional outcome of intramedullary rush pin for pediatric both bone fracture of forearm. Methods: In this retrospective, observational study done from 1st of February 2017 to 31st of March 2017, a total of sixty patients with both bone fractures of forearm were treated with intramedullary rush pin and followed up for six months for evaluation of functional outcome. T-test and Chi-square tests were done. Results: Closed reduction and internal fixation was done in 48 (80%) patients. Mean age of the patients was 9.23 year (SD=2.77). Fifty-six (93.3%) patients were male with a significant difference (p<0.001). With price et al. grading system, 49 (81.7%) patients had excellent results, nine (15%) had good and two (3.3%) patients had fair results. There was no major complication. Mean time to implant removal was 24.16 weeks (SD=1.62) from the time of surgery with range of 20 to 28 weeks. Among transverse and oblique fractures, patients with transverse fracture were likely to have better outcome (p=0.04). Conclusion: Intramedullary nailing with rush pin gives excellent to good functional outcome in majority of the cases without major complications at low cost for management of both bone fractures of forearm in children.

BACKGROUND: Small bone fractures of hand and feet are very common in a developing country like India and consist of 15% to 20% among all patients presenting at trauma centre and orthopaedic opds. Small bone fractures such as fractures of phalanx or metacarpals are often tend to be overlooked and of deemed as less serious in nature. However even a small deformity or disability of hand and feet can hamper the functional and psychological wellbeing of a person in long term. The purpose of this study is to evaluate the functional outcome and complications associated with application principles of JESS (Joshi's External Stabilization System) xation in phalanx, metacarpal and metatarsals and their management as a day care procedure. MATERIAL AND METHODS: This prospective observational study was done over a period of 1 year on 21 patients for injuries of metacarpal, phalanx and metatarsals and followed up for a minimum period of 3 months post frame removal. RESULTS: Young patients constituted almost 60% of the study population. RTA, assault and industrial / factory setup injury is most prevalent mode of injury. Over all closed injury patients had better functional outcome than open fractures. CONCLUSION: JESS is a versatile, easy to use system that can be applied at a very average set up. Optimum management of comminuted, open and unstable small bone fractures of hand and feet is possible with JESS. We advocate more extensive use of such a low cost and versatile system in Indian setup where cost of treatment is a major factor for going forward with a treatment option.

Objective: In our study, it is aimed to remove the cast more easily and safely without using the cutting tools by leaving the cast ends marked by folding in the idiopathic clubfoot patients treated with Ponseti method. Material and Methods: Forty feet of 29 patients treated for Pes Equinovarus were included in the study. Patients were followed up in two groups. The group treated with Ponseti method by cast marking were named as “modified group” and cast wrapped group without marking were named as “classical group”. Neurological, teratologic and syndromic clubfoot patients were not included in the study. During the six series of casting, cast removal times for each extremity are recorded in minutes and it is noted that whether any additional cutting tool is used during cast removal or not. A summary of the data was presented as mean, standard deviation and percentage. Comparisons of the categorical characteristics were analysed by using the Chi-square test and the Mann-Whitney test. IBM-SPSS 20 program was used for analysis. In all tests, the level of significance was adjusted to 0.05. Results: Thirteen (44.8%) of the 29 patients were male and 16 (55.2%) were female. While the mean time to start treatment for the 15 patients in the modified group was 3.46 (2-7) days, mean time for the 14 patients in the classical group was 3.78 (2-10) days. While the mean cast removal time of the 20 extremities of 15 patients in the modifying group was 10.9 minutes (8-14.3 min);it was 22.2 minutes (17.1-29.5 min) for the 20 extremities of 14 patients in the classical group. While no additional cutting tool was used during cast removal in the modified group, additional cutting tools were used during removal of cast in 75% (15/20) of the patients in the classical group and statistically significant difference was found between two groups in terms of the use of cutting tools (p<0.001). Conclusion: We found that the cast ends’ being marked by folding during plastering in idiopathic clubfoot patients treated with Ponseti technique is costless, easy to apply, significantly shortens cast removal time, does not require the use of cutting tools, and thus is a notably safe method for these patients.

<p class="abstract"><strong>Background:</strong> The aim of this study was to evaluate the results of intramedullary nailing in diaphyseal fractures of radius and ulna in age group of 10 to 49 years and to understand its clinicoradiological and functional results.</p><p class="abstract"><strong>Methods:</strong> This is a retrospective case series study of forearm bone fractures and the selected management for the same over a period of 3 years. We chose the cases in which intramedullary nailing was the treatment modality which were followed up over a period of minimum 6 months. Patients with galeazzi variety, monteggia variety, pathological fracture or non-union after previous surgery were excluded. The outcomes were then evaluated with disabilities of the arm, shoulder and hand (DASH) score, Green and O’Brien score, and Grace and Eversmann functional outcome score.<strong></strong></p><p class="abstract"><strong>Results:</strong> Of the 22 patients, 10 patients had excellent functional outcome according to Grace and Eversmann score, 7 patients had good outcome, 4 patients had acceptable while 1 was unacceptable. Green and O’Brien also had similar results, except that patients among fair category were 3 and poor category were 3. The mean DASH score was 16.2.</p><p><strong>Conclusions:</strong> This study shows that closed method for fixation by intramedullary nailing of both bone forearm fractures leads to excellent to good functional outcomes (according to DASH score, Green and O Brien, and Grace and Eversmann score) with less complications. In 6 months follow up x ray there is radiological union in all cases with no angulation, malunion or non-union. </p>

Introduction: Metabolic disorders in childhood and adolescence have been increasing considerably. Thus, the importance of performing an early diagnosis is emphasized. Objective: To analyse the occurrence of metabolic risk using a non-invasive marker in schoolchildren. Methods: This is a descriptive study with a quantitative approach, with random sample of 174 schoolchildren (70 boys and 104 girls) from 10 state high schools in the city of Passo Fundo, Rio Grande do Sul, Brazil. The height (cm) was verified according to the procedures of the Brazilian Sport Project and the waist circumference (cm) was measured with a flexible and inelastic tape measure. From this the waist-to-height ratio was calculated, which takes into account the proportion of abdominal fat by the individual’s height, considering the cut-off point of Ashwell & Hsieh. For data analysis we used descriptive and chi-square statistics. Results: The metabolic risk of schoolchildren was 13.8%, when stratified by sex, the occurrences were 11.4% for males and 15.4% for females, but there was no significant difference between the sexes (X2= 0.54; p= 0.45). Conclusion: The use of non-invasive markers for the diagnosis of metabolic risk indicated a high occurrence of it in schoolchildren, with the girls presenting a higher risk. The use of this method is important because it allows the evaluation of a greater number of schoolchildren and the early identification of health risk. In addition to being a low-cost, easy-to-apply method.

<p><strong>INTRODUCTION: </strong>Chronic renal failure exposes patients to the risk of several complications, which will affect every aspect of patient's life, and eventually his hope. This study aims to determine the effect of stress management group training on hope in hemodialysis patients.</p><p><strong>METHOD:</strong> In this quasi-experimental single-blind study, 50 patients with renal failure undergoing hemodialysis at Motahari Hospital in Jahrom were randomly divided into stress management training and control groups. Sampling was purposive, and patients in stress management training group received 60-minute in-person training by the researcher (in groups of 5 to 8 patients) before dialysis, over 5 sessions, lasting 8 weeks, and a researcher-made training booklet was made available to them in the first session. Patients in the control group received routine training given to all patients in hemodialysis department. Patients' hope was recorded before and after intervention. Data collection tools included demographic details form, checklist of problems of hemodialysis patients and Miller hope scale (MHS). Data were analyzed in SPSS-18, using Chi-square, one-way analysis of variance, and paired t-test.</p><p><strong>RESULTS:</strong> Fifty patients were studied in two groups of 25 each. No significant difference was observed between the two groups in terms of age, gender, or hope before intervention. After 8 weeks of training, hope reduced from 95.92±12.63 to 91.16±11.06 (P=0.404) in the control group, and increased from 97.24±11.16 to 170.96±7.99 (P=0.001) in the stress management training group. Significant differences were observed between the two groups in hope scores after the intervention.</p><p><strong>CONCLUSION:</strong> Stress management training by nurses significantly increased hope in hemodialysis patients. This low cost intervention can be used to improve hope in hemodialysis patients.</p>

<p class="abstract"><strong>Background:</strong> Pediatric long bone fractures can have a high morbidity and long term disability among the survivors. The present study was aimed to study the clinical outcomes of children with long bone fractures managed by elastic intramedullary nail.</p><p class="abstract"><strong>Methods:</strong> We retrospectively reviewed the medical records of children (aged 2 to 15 years) who were admitted and treated for unilateral femoral, tibial or forearm fractures with elastic intramedullary nail from January 2018 till February 2020. All were managed with elastic intramedullary nailing. The union times and complications were noted from the medical records.<strong></strong></p><p class="abstract"><strong>Results:</strong> Femoral, tibial and forearm bones were involved in 15 patients each (33.3%). Road traffic accident was reported by 31%) and physical abuse by one case (2%). The mean time of union was 10 weeks, ranging from 6 to 12 weeks. The mean time for union of the femur and tibia was 11.54 and 11.34 weeks respectively. It was significantly lower for forearm (6.67 weeks, p value &lt;0.05). Two patients reported of bursitis and impingement, which were resolved after nail removal. One case had limb length discrepancy of about 0.5 cm in femur fracture, which was not impairing functional activity. Functional status was assessed based on Flynn criteria was found to be excellent in 36%, good in 44% and fair in 20%.</p><p class="abstract"><strong>Conclusions:</strong> The results of the present study show that titanium elastic nail (TEN) is an effective treatment modality for managing pediatric long bone fractures. We recommend the use of TEN in managing long bone fractures in children.</p>

Purpose The aims of this study were to characterize hearing symptoms, habits, and sound pressure levels (SPLs) of personal audio system (PAS) used by young adults; estimate the risk of developing hearing loss and assess whether instructions given to users led to behavioral changes; and propose recommendations for PAS users. Method A cross-sectional study was performed in 50 subjects with normal hearing. Procedures included questionnaire and measurement of PAS SPLs (real ear and manikin) through the users' own headphones and devices while they listened to four songs. After 1 year, 30 subjects answered questions about their usage habits. For the statistical analysis, one-way analysis of variance, Tukey's post hoc test, Lin and Spearman coefficients, the chi-square test, and logistic regression were used. Results Most subjects listened to music every day, usually in noisy environments. Sixty percent of the subjects reported hearing symptoms after using a PAS. Substantial variability in the equivalent music listening level (Leq) was noted ( M = 84.7 dBA; min = 65.1 dBA, max = 97.5 dBA). A significant difference was found only in the 4-kHz band when comparing the real-ear and manikin techniques. Based on the Leq, 38% of the individuals exceeded the maximum daily time allowance. Comparison of the subjects according to the maximum allowed daily exposure time revealed a higher number of hearing complaints from people with greater exposure. After 1 year, 43% of the subjects reduced their usage time, and 70% reduced the volume. A volume not exceeding 80% was recommended, and at this volume, the maximum usage time should be 160 min. Conclusions The habit of listening to music at high intensities on a daily basis seems to cause hearing symptoms, even in individuals with normal hearing. The real-ear and manikin techniques produced similar results. Providing instructions on this topic combined with measuring PAS SPLs may be an appropriate strategy for raising the awareness of people who are at risk. Radiographic Mandibular Indices serve as easy and relatively cheap tools for evaluating bone mineralization. Objectives: To examine the effect of age and gender on three mandibular indices: the panoramic mandibular index (PMI), the mandibular ratio (MR) and the mandibular cortical index (MCI), among Libyan population. Methods: The three indices were measured on 317 digital (OPGs) of adult humans (155 males, 162 females). The sample was divided into six age groups (from 18-25 years through 56-65 years). The measurements were analyzed for interactions with age and sex, using SPSS (Statistical Package for Social Studies) software version no. 22. The tests employed were two way ANOVA, the unpaired T-test and chi-square test. Results: The mean PMI fluctuated between 0.37 s.d. 0.012 and 0.38 s.d. 0.012. among the sixth age groups. One-way ANOVA statistical test revealed no significant of age on PMI. On the other hand gender variation has effect on PMI, since independent sample t-test disclosed that the difference between the male and female PMI means statistically significant. ANOVA test showed that the means of MR among age groups showed a negative correlation i.e. MR mean declined from 3.01 in 18-25 age groups to 2.7 in 55-65 age groups. In contrary, the gender showed no effect on MR according two sample t-test at p> 0.05. In regards with MCI, statistical analysis showed that it affected by age that is C1 was decreasing by age while C2 and C3 were increased by age. Using chi square test the result indicated that there is a significant difference among the different age group and the two genders in MCI readings. Conclusion: PMI was influenced significantly by age but minimally by the gender. MR is not affected by gender but has a negative correlation with age. MCI is affected by both age and gender

Smartphone has some constructive as well as some cynical aspects too. Nomophobia is a negative face of the smartphone. Nomophobia is the irrational dread of being without cell phones or being not able to utilize phones (situational phobia) for some reason such as some signal or battery issues. Theprimary point of the current research is to find out the proportion of Nomophobia among understudies with regardof gender and age in Agra city. For this purpose, a total number of 300 students were selected by randomization (150 males and 150 females) from the age scope of 15-20 years. The data was taken by using a self-administered questionnaire NMP-Q developed by Yilidirim and Correia. The obtained data were analyzed using descriptive statistics, Mean and Chi-square. Results revealed that there is a significant difference between male and female score on nomophobia. The other result indicates that early adolescents positively related to Nomophobia as compare to late adolescents. In India, one person can use multiple cell phones. There is no minimum age to use mobile phones. It is adding fuel to the pre-existing problem of Nomophobia. Telecom Administrative Authority of India (TRAI) should assume a functioning job by making an arrangement/law that "setting based methodology" on cell phone limitations among individuals ought to be followed. A demonstration will be detailed, under this demonstration. There ought to choose the least age for portable use. It will assist a great deal with curbing the circumstance.

Gonado-somatic index and fecundity are tools for measuring the sexual maturity and ability of animals to reproduce.  This study investigates the reproduction of Trichiurus lepturus. Specifically, this aimed to determine the sex ratio, the GSI, the relationship between fecundity and total length, fecundity and total weight, fecundity and ovary weight. The Descriptive Method of research was used.  Percentage and chi-square was utilized in determining the percentage of occurrence and sex ratio respectively.   Pearson r Product Moment Coefficient of Correlation was used to determine the relationships of the parameters. The study revealed that females outnumbered males and the sex ratio for different month showed significant difference.  Spawning season was observed to occur in November and December as revealed in its GSI values and it synchronized with the full and new moon phases.  Fecundity is positively correlated with body weight, body size, and ovary weight where ovary weight is observed to be the best index for fecundity.  The results of this study could be used further for formal stock assessment of cutlassfish fishery.
