While it is important to ensure that all the necessary indications are met 1 , it is also important not to apply unnecessary contraindications. Some published lists of supposed contraindications have achieved wide acceptance without having much evidence to support them. We have recently argued with evidence that many of the suggested contraindications are unnecessary.

Freeze-etched membranes usually appear as relatively smooth surfaces covered with numerous small particles and a few small holes (Fig. 1). In 1966 Branton (1“) suggested that these surfaces represent split inner mem¬brane faces and not true external membrane surfaces. His theory has now gained wide acceptance partly due to new information obtained from double replicas of freeze-cleaved specimens (2,3) and from freeze-etch experi¬ments with surface labeled membranes (4). While theses studies have fur¬ther substantiated the basic idea of membrane splitting and have shown clearly which membrane faces are complementary to each other, they have left the question open, why the replicated membrane faces usually exhibit con¬siderably fewer holes than particles. According to Branton's theory the number of holes should on the average equal the number of particles. The absence of these holes can be explained in either of two ways: a) it is possible that no holes are formed during the cleaving process e.g. due to plastic deformation (5); b) holes may arise during the cleaving process but remain undetected because of inadequate replication and microscope techniques.

In the 1970s, Japanese cooks began to appear in the kitchens of nouvelle cuisine chefs in France for further training, with scores more arriving in the next decades. Paul Bocuse, Alain Chapel, Joël Robuchon, and other leading French chefs started visiting Japan to teach, cook, and sample Japanese cuisine, and ten of them eventually opened restaurants there. In the 1980s and 1990s, these chefs' frequent visits to Japan and the steady flow of Japanese stagiaires to French restaurants in Europe and the United States encouraged a series of changes that I am calling the “Japanese turn,” which found chefs at fine-dining establishments in Los Angeles, New York City, and later the San Francisco Bay Area using an ever-widening array of Japanese ingredients, employing Japanese culinary techniques, and adding Japanese dishes to their menus. By the second decade of the twenty-first century, the wide acceptance of not only Japanese ingredients and techniques but also concepts like umami (savory tastiness) and shun (seasonality) suggest that Japanese cuisine is now well known to many American chefs.

Objective: Once daily dosing (ODD) aminoglycoside is gaining wide acceptance as an alternative way of dosing. In our setting it is the regimen of choice whenever gentamicin is indicated. The objective of this study was to evaluate the practice of gentamicin ODD in a public hospital in Malaysia. Methods: We conducted a retrospective review of medical records of patients on gentamicin ODD who were admitted to Hospital Melaka during January 2002 until March 2010. All adult patients who were on ODD gentamicin with various level of renal function were included in the study. Patients on gentamicin less than 72 hours and pregnant women were excluded. Results: From 110 patients, 75 (68.2%) were male and 35 (31.8%) were female. Indications for ODD gentamicin included pneumonia, 34 (31.0%) neutropenic sepsis, 27 (24.5%) and sepsis, 11 (10.0%). The mean dose and duration of gentamicin was 3.2 mg/kg/day and 7 days, respectively. Almost all patients were on gentamicin combined with other antibiotics. Clinical cure based on fever resolution was found in 89.1% of patients treated with ODD. Resolution of fever took an average of 48 hours after initiation of therapy. The evaluation for bacteriologic cure could not be performed because of insufficient data on culture and sensitivity. Out of 38 patients with analyzable serum creatinine data, four patients might have developed nephrotoxicity. Conclusion: In our setting, lower dosages of ODD gentamicin when used in combination with other antibiotics seemed to be effective and safe in treating most gram negative infections.

Anaerobic wastewater treatment (AnWT) is considered as the most cost-effective solution for organically polluted industrial waste streams. Particularly the development of high-rate systems, in which hydraulic retention times are uncoupled from solids retention times, has led to a world-wide acceptance of AnWT. In the last decade up to the present, the application potentials of AnWT are further explored. Research shows the feasibility of anaerobic reactors under extreme conditions, such as low and high temperatures. Also toxic and/or recalcitrant wastewaters, that were previously believed not to be suitable for anaerobic processes, are now effectively treated. The recent advances are made possible by adapting the conventional anaerobic high-rate concept to the more extreme conditions. Staged anaerobic reactor concepts show advantages under non-optimal temperature conditions as well as during the treatment of chemical wastewater. In other situations, a staged anaerobic - aerobic approach is required for biodegradation of specific pollutants, e.g. the removal of dyes from textile processing wastewaters. The current paper illustrates the benefits of reactor staging and the yet un-exploited potentials of high-rate AnWT.

Quantitative protein measurements by mass spectrometry have gained wide acceptance in research settings. However, clinical uptake of mass spectrometric protein assays has not followed suit. In part, this is due to the long-standing acceptance by regulatory agencies of immunological assays such as ELISA assays. In most cases, ELISAs provide highly accurate, sensitive, relatively inexpensive, and simple assays for many analytes. The barrier to acceptance of mass spectrometry in these situations will remain high. However, mass spectrometry provides solutions to certain protein measurements that are difficult, if not impossible, to accomplish by immunological methods. Cases where mass spectrometry can provide solutions to difficult assay development include distinguishing between very closely related protein species and monitoring biological and analytical variability due to sample handling and very high multiplexing capacity. This paper will highlight several examples where mass spectrometry has made certain protein measurements possible where immunological techniques have had a great difficulty.

The interest within this study of the school meal is based upon the wide acceptance of the value of its contribution to the nutritional progress of this country in the last few decades and the probably disputable assertion that such a contribution has come to an end with the advent of ‘fast’ and ‘junk’ foods, the indiscriminate consumer and the diminishing motivation of local education authorities to provide a meal within certain nutritional and cost parameters. The main statement of the analysis is that Educational Catering has missed a valuable opportunity to play a significant part in improving the health of the school pupil by providing the appropriate food. Almost equal in importance as a statement is that Educational Catering must become fully involved in the school food education programme. Lost opportunities are of less importance for the future than determining ways in which the school meals staff can be integrated into the team of people in the school interested in improving the dietary choices of its pupils and, as a consequence, improving their health.

Guar gum and its derivatives are highly important industrial hydrocolloids as they find applications in various industrial sectors. Guar is a polymer of high molecular weight and its aqueous solutions exhibit unique rheological properties, which has led to its wide acceptance by the industry. In certain industrial applications low molecular weight guar and its derivatives are needed, and conventionally chemical depolymerisation of guar is carried out for this purpose. Radiation processing is a novel and green technology for carrying out depolymerization and can be an ideal substitute for chemical depolymerisation technique. In order to study the effect of radiation on guar derivatives, three types of derivatives have been taken in the present study: carboxymethyl, hydroxyethyl, and methyl guar. The effect of 1–50 KGy radiation dose on the rheological behavior of these derivatives has been studied, and the results have been described in the present paper. The effect on storage and loss modulus with respect to frequency and effect on viscosity with respect to shear rate have been discussed in detail.

Intracranial endoscopy in the treatment of hydrocephalus, arachnoid cysts, or brain tumors has gained wide acceptance, but the use of endoscopy for intradural navigation in the pediatric spine has received much less attention. The aim of the authors' present study was to analyze their experience in using spinal endoscopy to treat various pathologies of the spinal canal. The authors performed a retrospective review of intradural spinal endoscopic cases at their institution. They describe 4 representative cases, including an arachnoid cyst, intrinsic spinal cord tumor, holocord syrinx, and split cord malformation. Intradural spinal endoscopy was useful in treating the aforementioned lesions. It resulted in a more limited laminectomy and myelotomy, and it assisted in identifying a residual spinal cord tumor. It was also useful in the fenestration of a multilevel arachnoid cyst and in confirming communication of fluid spaces in the setting of a complex holocord syrinx. Endoscopy aided in the visualization of the spinal cord to ensure the absence of tethering in the case of a long-length Type II split spinal cord malformation. Conclusions Based on their experience, the authors found intradural endoscopy to be a useful surgical adjunct and one that helped to decrease morbidity through reduced laminectomy and myelotomy. With advances in technology, the authors believe that intradural endoscopy will begin to be used by more neurosurgeons for treating diseases of this anatomical region.

Now that the microcomputer has gained wide acceptance as a teaching tool, it seems appropriate to review the extent to which phoneticians can make use of the new technology. Educational applications will be our primary interest in this survey. Many of these applications involve phonetics word-processing, and of course this has direct relevance for the current debate in this Journal concerning the possibility of a standard for computerization of the IPA.

The Greek cities of the western coast of the Black Sea knew both foundation myths and the phenomenon of the second foundation, associated with the rebuilding of civic life after the invasion of Burebista, the king of the Getae and Dacian tribes from 82 bce to 44 bce. In most foundation stories the ktistes is either a god (in the case of the city of Dionysopolis) or a hero (in the cases of the cities of Kallatis, Tomis and Anchialos), and the stories date mostly to the Antonine age. The story of Tomos of Tomis stands out owing to its wide acceptance among the local elite, while that of Melsas of Mesambria may have never gained official acceptance: it was created in the late Hellenistic age, probably reviving a Thracian tale of Melsas, perhaps a hero, known from early-third century bce coins. The Melsas story is a prime example of cultural transfers from the native population to Greek-majority Mesambria in the Hellenistic and early Roman ages.
