Incidence of thyroid dysfunction increases in HIV/AIDS patients. It can be a hypothyroid or hyperthyroid, with subclinical hypothyroid predomination. Thyroid dysfunction is associated with the progression of HIV and low CD4 counts. There has been controversy over how HIV affects thyroid function. Opportunistic infections and HBV/HCV co-infections might increase the probability of thyroid dysfunction. Medication of HIV is also an important factor of thyroid dysfunction. Research shows that the use of ART increases the probability of thyroid dysfunction. This study aimed to analyze the association between CD4 counts, CD8, CD4/CD8 ratio, and HIV RNA viral load with thyroid dysfunction in HIV/AIDS patients. An observational study with a cross-sectional design was conducted from August to September 2020 in Dr. Moewardi Hospital, Surakarta on 60 HIV/AIDS patients. All subjects were 18-60 years old. The research data were analyzed with a 2x2 test table to determine the Prevalence Ratio (PR) of each variable, then multivariate analysis with logistic regression was continued. The study showed 6.7% thyroid dysfunction in HIV/AIDS patients, 5% subclinical hyperthyroidism, and 1.7% subclinical hypothyroidism. The CD4 counts [PR 13.36 (95% CI: 1.53-116,65; p=0.017) and CD8 counts [PR 0.91 (95% CI: 3 0.02-0.51; p=0.032)] significantly associated with thyroid dysfunction in HIV/AIDS patients. CD4 counts < 200 cells/mm and 3 CD8 counts ≥500 cells/mm were associated with the incidence of thyroid dysfunction in HIV/AIDS patients, while CD4/CD8 ratio and HIV RNA viral load were not associated. CD4 count was not an independent predictor of thyroid dysfunction in HIV/AIDS patients.

Approach to PLWHA by providing interpersonal counseling is the right thing to do to overcome psychological problems including social, emotional and spiritual aspects of PLWHA. This study aims to look at the effect of interpersonal counseling on social, emotional and spiritual responses in HIV/ AIDS patients. This research is a type of non-experimental research with a cross-sectional approach. The sample is post-test people and tested positive for HIV in the Kediri region, which is 32 with purpossive sampling technique. Data on social, emotional and spiritual responses were obtained based on the results of data recapitulation from the instrument in the form of a questionnaire. The results of the study with the non-parametric Wilcoxon test showed there were differences between social responses of HIV/AIDS patients before and after given interpersonal counseling with p value = 0,000, there were differences between the emotional responses of HIV/AIDS patients before being given interpersonal counseling and after being given counseling with p value = 0,000, and there is a difference between spiritual responses in HIV/AIDS patients before being given interpersonal counseling and after being given interpersonal counseling with p value = 0,000. It can be concluded that interpersonal counseling influences social, emotional and spiritual responses of HIV/AIDS patients. It is recommended that the mentoring of HIV/AIDS patients through interpersonal counseling can continue so as to enhance physiological responses that will have an impact on disease prognosis, prevention of opportunistic infections and reduce mortality rates for HIV patients/ AIDS.

Abstract Background Opportunistic infections are late complications of HIV infection is the depletion of the immune system. It is a major public health problem and high morbidity AIDS patients die of AIDS-related infections in developing countries like Ethiopia. Identification of opportunistic infections (OIs) is important to develop a specific intervention. Therefore, this study aimed to assess the burden and associated factors of opportunistic infections.Method A facility-based cross-sectional study was conducted on 420 randomly selected HIV/AIDS patients taking anti-retroviral therapy. Data was collected from selected hospitals in Sidama Zone based on population proportion to size. Data was collected by a pre-tested questionnaire and a pre-tested checklist from the medical records of patients. Data entry and analyzed for descriptive and logistic regression models by SPSS v.23. The result declared as statistically significant at p < 0.05.Result The magnitude of opportunistic infections was 39.6%. Major identified OIs was oral candidacies 23.2%, recurrent bacterial pneumonia 21.5%, Herpes zoster 6.3%, and Pulmonary Tuberculosis 6.0%.The magnitude of opportunistic associated with; older age [AOR=2.69, 95% CI: 1.33-5.43], No formal education [AOR=3.12, 95% CI: 1.06-9.25], Low monthly income [AOR=2.27, 95%CI:1.35-3.83], initial CD4 count less than 200 cells/mm3 [AOR=1.91, 95% CI:1.05-3.46), WHO clinical stage II [AOR=2.43, 95% CI:1.34-4.42] and stage III and IV [AOR=4.27, 95% CI: 2.12-8.59], had no extra medicine additional to ART (prophylaxis) had [AOR= 5.91, 95% CI: 3.31-10.56], who interrupt ART medicines [AOR=2.03, 95% CI: 1.09- 3.80] and Khat chewing [AOR=5.48, 95% CI: 2.32-12.96] when compared to their counterparts.Conclusions The overall magnitude of opportunistic infections was high when compared with other studies. Health officials and clinicians need to give attention on the strengthening of the provision if ART with prophylaxis on early-stage and adhrerence, implementation of the TB/HIV collaboration activity, and early initiation of ART to reduce opportunistic infections.

The Acquired Immune Deficiency Syndrome (AIDS) is the presence of symptoms caused by Human Immunodeficiency Virus (HIV)which belongs to human retroviruses (retroviridae). Thrombocytopenia is a common finding in patients with HIV infection. HIV infectionmay induce thrombocytopenia through immune and non-immune mechanisms, autoimmune combination and inhibition of plateletproduction. The aim of this study is to analyze the correlation between thrombocyte and CD4 count in HIV/AIDS patients. This studyuses a cross sectional design with a total of 17 patients. The subject of this study is HIV/AIDS patients who came to and examined atVCT clinic, dr. Moewardi Hospital Surakarta. To analyze this result the researchers used Spearman (r) correlation with p<0.05, andconfidence interval 95%. Patients’ median age was 30 (21–49) years, 11 (64.7%) men and 6 (35.3%) women. The subjects with AIDSwere 11 (64.7%), and HIV were 6 (35.3%) patients. The duration of antiretroviral (ARV) was 7.5 (4–20) months in 10 subjects.The median of thrombocyte count was 203 (143–327)×103/μL, CD4 absolute 207 (5.0–734)/μL, and CD4 (% lymphocytes) 13.0(2.0–29.0)%. The thrombocyte count was not correlated with CD4 absolute (r=0.456; p=0.066) and CD4% (r=0.218; p=0.400). InHIV patients, low platelet counts will be the result of a host of problems and complications that are associated with the progressive HIVinfection or its management.

Abstract Objectives The tear, as an important bodily secretion, plays a crucial role in preventing infection and maintaining homeostasis of ocular surfaces. Although accumulating studies have reported on the HIV-1 viral load profile among varying bodily fluids and secretions, little was known concerning HIV-1 dynamics in tears. Therefore, the objectives of this study were to investigate the HIV-1 viral load in tears of HIV/AIDS patients and study factors influencing their tear viral load. Methods A cross-sectional study was conducted. 67 patients with a confirmed HIV-1 infection or AIDS were recruited from the Beijing You’an Hospital, China between April 2018 and September 2018. Socio-demographic information and laboratory test results were collected. At the same time, ophthalmic examinations were carried out and tear samples were tested. Results Of 30 highly active antiretroviral therapy (HAART)-naïve patients, 53.3% had detectable HIV-1 RNA in tears. Of 37 patients on HAART, HIV-1 RNA was undetectable in their tears, regardless of treatment duration and blood viral load. Tear viral load ranged from TND (target not detected) to 13,096 copies/mL. Viral load was lower in tears than in blood plasma (p < 0.001), and was significantly correlated with plasma viral load (Rho = 0.566, p < 0.001) and AIDS stage (Rho = 0.312, p = 0.01), but negatively correlated with CD4 + T cell count, CD4 +/CD8 + T cell count, and duration of HIV infection (Rho = -0.450, Rho = − 0.464, Rho = − 0.565; p < 0.001). Conclusions HIV-1 RNA is present in tears of more than half of the HAART-naïve patients, whereas absent in tears of patients on HAART. Tear viral load is positively associated with plasma viral load while it is negatively correlated with CD4 cell count. This study provides novel insights into the area with limited understanding–HIV-1 viral load in tears.

HIV causes a decrease in immunity which makes the patient susceptible to opportunistic infections such as tuberculosis. This research aims to find out the prevalence, patient descriptions’, treatment pattern and therapeutic goals for tuberculosis in HIV/AIDS patients who were taking the medication in polyclinic of DOTS and polyclinic of VCT in Regional General Hospital Of Bandung City. This was nonexperimental research. Data was collected retrospectively with cross-sectional approach using secondary data which were TB-01 form and HIV Care Summary Form and Antiretroviral Therapy from 1 January 2016 to 31 December 2018.  From 668 Tuberculosis occurrence, there were 25 patients suffered HIV/AIDS. Result obtained the most risk factors was homosexual (60%), with the most ART treatment combination were Tenofovir, Lamivudine, and Evarirenz, 4% has a drug allergic, the most TB treatment history was new case (92%), with the most kind was pulmonary TB (56%), the most Anti TB treatment was category I (92%) and 60% patients done with full therapy

Pengobatan antiretroviral (ARV) bertujuan untuk menurunkan jumlah virus (viral load) serta menghambat perburukan infeksi oportunistik. Ketidakrasionalan terapi ARV akan menyebabkan peningkatan resiko morbiditas dan mortalitas, serta resiko resistensi obat.  Tujuan dari penelitian ini untuk mengevaluasi rasionalitas antiretroviral pada pasien HIV/AIDS di rumah sakit umum X Surakarta yang meliputi tepat indikasi, tepat pasien, tepat obat, dan tepat dosis. Penelitian ini merupakan penelitian observasional. Sampel penelitian berjumlah 99 pasien dengan metode purposive sampling. Data diambil melalui rekam medis pasien HIV/AIDS rawat inap di rumah sakit X Surakarta. Data dianalisis secara deskriptif berdasarkan Peraturan Menteri Kesehatan Republik Indonesia Nomor 87 Tahun 2014 tentang Pedoman Pengobatan Antiretroviral (2014), Pedoman Penerapan Terapi HIV pada Anak (2014), Pedoman Nasional Tatalaksana Klinis Infeksi HIV dan Terapi Antiretroviral pada Orang Dewasa (2011). Evaluasi dari 99 pasien yang memenuhi inklusi didapatkan hasil 100% tepat indikasi, 88,89% tepat pasien, 96,97% tepat obat dan 96,97% tepat dosis. Data were analyzed descriptively based on Peraturan Menteri Kesehatan Republik Indonesia Nomor 87 Tahun 2014 tentang Pedoman Pengobatan Antiretroviral (2014), Pedoman Penerapan Terapi HIV pada Anak (2014), Pedoman Nasional Tatalaksana Klinis Infeksi HIV dan Terapi Antiretroviral pada Orang Dewasa (2011). An evaluation of 99 patients showed 100% correct indications, 88,89% right patients, 96,97% right drugs and 96,97% right doses. Introduction: Human immunodeficiency virus (HIV) is associated with inflammation. An association between vitamin D deficiency and inflammation also exists. Our study attempts to examine whether there may be a relationship between vitamin D and HIV viral load (HIV RNA) by: 1) characterizing the distribution of 25-hydroxyvitamin D (25-OHD), and 2) determining if 25-OHD is independently associated with HIV RNA. Methodology: A cross-sectional study among HIV-infected adults was conducted. Demographics, clinical / social / HIV characteristics and data on antiretroviral therapy were collected by questionnaire, medical records and laboratory testing. All patients provided blood samples. Bivariate and multivariate analyses were conducted to quantify the relationship between vitamin D and HIV RNA. Results: Among the 564 patients, the median (interquartile range, IQR) 25-OHD value was 24.42 (16.22 – 34.10) ng/mL. The mean (standard deviation, SD) log-HIV RNA was 3.51 (1.11) copies/mL. There were 304 patients (53.9%) with an undetectable HIV RNA (< 500 copies/mL). In the bivariate analyses, no differences were observed between patients with and without an undetectable HIV RNA in mean (SD) 25-OHD, 25-OHD insufficiency (< 30 ng/mL), or 25-OHD deciles. In the log-binomial regression analyses, there was no association between 25-OHD and an undetectable HIV RNA (prevalence ratio: 1.00, 95% confidence interval: 0.99 – 1.01, p = 0.67). Conclusions: No relationship was observed between 25-OHD and HIV RNA among HIV-infected patients in Kazakhstan.

In a bid mitigate the spread of HIV and AIDS since it was diagnosed, several interventions ranging from palliative care, increased access to HIV treatment to manage opportunistic infections and the current drive of improving clinical outcomes through suppression of viral load are all aimed at prolonging survival of HIV and AIDS patients. The authors endeavour to corroborate the effect of viral load (VL) suppression on ART HIV patients. The authors applied a comprehensive search by merging terms for survival of HIV patients to clinical, demographic and socio-economic factors in NCBI and MEDLINE databases. About 15 articles were considered by outlining research questions, generating key words and search terminologies. Only articles published since 2013 during scale up of VL interventions were considered. It is worth noting that unsuppressed VL clinically decisively causes death in HIV/AIDS patients, propelled by a number of associated causes including opportunistic infections (OIs) Tuberculosis (TB), Non communicable diseases (NCDs), age, sex, income, geographic location among others. Whereas there is marked decline in risk of dying among HIV/AIDS patients resulting from VL suppression, other factors including; early detection and timely treatment, managing OIs, NCDs and TB treatment, tackling socio-economic barriers and multi-pronged community focused interventions like Community Client Led ART Delivery CCLAD, Community Drug Distribution Point (CDDP) interventions enhance survival of HIV/AIDS patients on lifelong ART.

<p class="Englishversionofabstract">HIV-TB co-infection still becomes a health problem in Indonesia, including in Nabire district, Papua province, which has the highest number of cases. HIV and TB infections are closely related and affect the epidemiology of one another. This study aims to determine the profile description of HIV-TB co-infected patients in Nabire. This research is a descriptive study with a cross-sectional design on 90 people with HIV/AIDS selected consecutively. Data were analyzed univariately and presented in the form of a frequency distribution table. The results showed that of 55 HIV/AIDS co-infected TB patients, almost all (90.9%) were Papuan ethnic, 70.9% were female and aged 30 to 49 years (50.9%) with a mean of 31.309.36 years. The majority of HIV-TB co-infected patients were married (63.6%), holding secondary education (58.2%), working (67.3%), engaging in sexual activity after 17 years of age, only having one sexual partner, and did not use drugs or obtain blood transfusions. The clinical profile of HIV-TB patients showed that 67.3% of the patients were with baseline CD4 count ≤350 cells/mm3, 64.8% had CD4 count at the time of study &gt;350 cells/mm3, and viral load values fewer than 5000 copies/ml (87.3%). The therapy most widely received for patients with HIV-TB coinfection was the combination of ARV 3TC+EFV+TDF (76.4%). The most common clinical symptoms of HIV/AIDS patients were weight loss (56.4%), cough (40%), recurrent oral thrush (36.4%), chickenpox (32.7%), and tuberculosis lymph nodes (18.2%).</p>

Human Immunodeficiency Virus (HIV) is a retrovirus originating from the retroviridae family of the genus lentivirus that infects and damages cells that have a molecule Cluster of Differentiation 4 (CD4), especially T lymphocytes that have receptors with high affinity for HIV. Total lymphocyte count / TLC has been proposed as an alternative guide to CD4 in limited health facilities. This study aims to determine the correlation between Total Lymphocyte Count (TLC) and CD4 in HIV/AIDS patients in the W.Z. Johannes Kupang hospital. The type of this research was observational analytic with a cross-sectional design. The study was conducted on 121 samples of patients who performed CD4 examination and Total Lymphocytic Count (TLC) in the laboratory of W.Z.Johannes Kupang Hospital. The Spearman correlation test shows a significance value of 0,000 with a Spearman correlation value of 0.799. Based on the results of this study it can be concluded that there is a significant correlation between Total Lymphocyte Count and CD4 and shows the direction of positive correlation with a very strong relationship, where the increase in the number of Total Lymphocyte Count is in line with the increase in CD4 counts.
