Background:Coronary artery calcification (CAC) is frequently encountered as incidental findings during CT evaluation of thorax; however, little is known about its magnitude and association with atherosclerotic risk factors in Nepalese population. The purpose of this study was to evaluate the frequency of incidental CAC in patients undergoing standard thoracic CT examination for non-cardiac pathology and to correlate it with risk factors for coronary atherosclerosis. Methods: A hospital based prospective cross-sectional study was conducted on 216 patients over 8 month duration. The coronary arteries were evaluated for calcification on 16-slice MDCT and the frequency of CAC was correlated with atherosclerotic risk factors viz. age, sex, diabetes mellitus, hypertension, smoking and obesity. Results: Incidental CAC was seen in 72(33.3%) of total 216 patients. The rates of CAC below 40, 40-60 and above 60 years age group were 0%, 31.6% and 43.9% respectively. The frequency of CAC in male and female were 35% and 31.2% for all ages, 43.6% and 20% for 40-60 years group, and 35.2% and 61.1% for above 60 years age group respectively. The frequency of CAC was higher in patients with diabetes mellitus (47.5%), hypertension (42.3%), smoking (43%) and obesity (38.9%).Conclusion: Incidental CAC was seen in 33.3% of the patients and it had significant association with advancing age, diabetes mellitus, hypertension and smoking. Rate of CAC was significantly higher in male for 40-60 years group but the frequency drastically increased in female for above 60 years group. Background: Dyslipidemia, one of the classical Framingham risk factors of cardiovascular diseases, accounts significantly to the rise of non communicable diseases in India. Urbanization, along with greater consumption of dietary fats and decreased physical activity, has led to an increase in this problem manifold. Objectives: To determine the prevalence of dyslipidemia among urban adults of Siliguri city, West Bengal and to identify the cardio-vascular risk factors associated with it. Methods: This descriptive, cross-sectional study was carried out among the 226 adults aged ≥20yrs in an urban area of Siliguri city, West Bengal, India. Data was collected by detailed history and physical examination; biochemical measurements were done using standards procedures. Dyslipidemia was defined by the presence of one or more than one abnormal serum lipid concentration. Statistical analysis was done using chi-square test, t test and logistic regression analysis using SPSS 20 software (Chicago, IL, USA). Results: Dyslipidemia was prevalent among 78.4 % of the total study subjects. Dyslipidemia was more in males than in females & in both males and females it was more prevalent in the age group 40 -59 years than in any other age group. It was associated with higher odds of major cardiovascular risk factors like hypertension, hyperglycemias, and tobacco use. Conclusion: Dyslipidemia remains a significant and growing problem in this part of the country. The reduction in the disease burden will require changes in life style as well as in national policies. Background: Stillbirth (SB) incidents are one of the most common adverse outcomes to occur during pregnancy. Studies indicate that approximately 3.3 million stillbirths are reported annually across the developing world. Institutional registration and under reporting of still birth to an authorized centre is a common problem. Objective: The objective of this study is to find out the incidence of SB after 28 weeks gestation and to find out the associated risk factors at Western Regional Hospital of Nepal. Materials and Methods: This is a cross sectional study carried out at Western Regional Hospital. Stillbirth babies born after 28 weeks of gestation or birth weight ≥ 1 kilogram and their mothers were recruited for the study. Intrapartum stillbirths were included despite of their ages, races or socio-economic status. Descriptive analysis was done on the data. Results: There were 3380 deliveries during the period of study. Among them, 50 cases were stillborn babies with a rate of 14 per thousand deliveries. The major risk factors associated were oligohydramnios, decreased fetal movement, growth restriction, meconium stained liquor and cord prolapse. Conclusion: Oligohydramnios (amniotic fluid index < 5cm) was the most common risk factor for stillbirth. Early identification and appropriate perinatal management could help to promote perinatal health.

Background: Diabetes and higher HbA1c level have increased the incidence of stroke. Hemoglobin levels both high and low are associated with poor outcomes. Leukocytes play an important role in the initiation of the atherosclerosis process. Methods: This was a cross-sectional study with a sample size of 62 people selected by non-random sampling method on a consecutive basis, patients with acute stroke were taken blood to measure blood glucose levels, HbA1c, Hb and Leucocytes when entering the hospital room. Outcomes were assessed using NIHSS and MRS. The calculation of NIHSS and MRS scores was performed on the first day of admission and the fourteenth day. To analyze the correlation of blood sugar levels during HbA1c, Hb and leukocytes in acute stroke, this study used Spearman's correlation test. The p value <0.05 was considered statistically significant. Results: The study subjects of 62 acute stroke patients (acute ischemic stroke 31 people and 31 people hemorrhagic stroke). Acute stroke patients were consisted of 38 men (61.3%) and 24 women (38.7%). Of 31 people with hemorrhagic stroke, there were 16 men (51.6%) and 15 women (48.4%), ischemic stroke patients were consisted of 22 people (71, 0%) and women were 9 people. Spearrman repair test showed no symptoms between blood glucose levels, HbA1c, Hemoglobin and leukocytes with outcomes in acute stroke. Conclusions: There was no significant association between blood glucose levels, HbA1c, hemoglobin and leukocytes with outcomes in acute stroke.

Objectives: The aim of this study is to identify the relationship between diabetes status including characteristics of diabetes and cognition among the middle-aged and elderly population (≥45 years) in China. Methods: A sample of 8535 people who participated in the China Health and Retirement Longitudinal Study (CHARLS) from June 2011 to March 2012 was analyzed. Two cognitive domains including episodic memory and executive function were measured through questionnaires. People were classified into four groups: no diabetes, controlled diabetes, untreated diabetes, treated but uncontrolled diabetes. Weighted multiple regression model was conducted to explore the association between diabetes and cognition in full sample as well as three different age groups (45–59, 60–74, ≥75). Adjustments were made for demographics and cardiovascular risk factors. Results: After adjusting several covariates, untreated diabetes (β = −0.192, p < 0.05) was significantly associated with episodic memory. In the age group of 45–69 years, untreated diabetes (β = −0.471, p < 0.05) and HbA1c level (β = −0.074, p < 0.05) were significantly associated with episodic memory. When adjusting for cardiovascular risk factors, all correlations were non-significant. Conclusion: The cross-sectional study suggests that untreated diabetes and HbA1c are the potential risk factor for cognitive impairment, and these associations are more significant in the age group of 45–59 years old. Cardiovascular factors are important mediating factors in the pathway between diabetes and cognitive impairment. More longitudinal studies are needed to confirm these associations.

Auditory hallucinations are uncommon phenomena which can be directly caused by acute stroke, mostly described after lesions of the brain stem, very rarely reported after cortical strokes. The purpose of this study is to determine the frequency of this phenomenon. In a cross sectional study, 641 stroke patients were followed in the period between 1996–2000. Each patient underwent comprehensive investigation and follow-up. Four patients were found to have post cortical stroke auditory hallucinations. All of them occurred after an ischemic lesion of the right temporal lobe. After no more than four months, all patients were symptom-free and without therapy. The fact the auditory hallucinations may be of cortical origin must be taken into consideration in the treatment of stroke patients. The phenomenon may be completely reversible after a couple of months.
