This research is an analysis of the situation of people with disabilities in Costa Rica, in the past and present, based on the data available from 1821 to this day, referring to the institutional framework that has been created for the attention of this particular sector of the population. The sources that will be used for this purpose are going to be census data, past and current regulations and available information of the institutions and organism present about institutional structures and agencies concerning the situation of people with disabilities. The main discovery refers to the importance of the previous institutional path in the field of disability within the country, which today allows it to sustain the achievements, despite the adjustments period in the country

Overconsumption of plastic goods and improper handling of petroleum-derived plastic waste have brought a plethora of negative impacts to the environment, ecosystem and human health due to its recalcitrance to degradation. These drawbacks become the main driving force behind finding biopolymers with the degradable properties. With the advancement in biopolymer research, polyhydroxyalkanoate (PHA) and poly(lacyic acid) (PLA) and its composites have been alluded to as a potential alternative to replace the petrochemical counterpart. This review highlights the current synthesis process and application of PHAs and PLA and its composites for food packaging materials and coatings. These biopolymers can be further ameliorated to enhance their applicability and are discussed by including the current commercially available packaging products. Factors influencing biodegradation are outlined in the latter part of this review. The main aim of this review article is to organize the scattered available information on various aspects of PHAs and PLA, and its composites for packaging application purposes. It is evident from a literature survey of about 140 recently published papers from the past 15 years that PLA and PHA show excellent physical properties as potential food packaging materials.

Abstract Background France has one of the lowest rates in the world regarding breastfeeding initiation and duration. Few studies have explored breastfeeding practices in France since the middle of the twentieth century, or following from initiation to cessation. The purpose of our study was to determine trends in breastfeeding over the past decades regarding public health recommendations, and to examine mothers’ perceptions about factors known to have an impact on breastfeeding support and cessation. Methods From the NutriNet-Santé cohort, 29,953 parous women (launched in 2009 to study relation between nutrition and health), were included in the present study. Using web-questionnaires, they were asked retrospectively if they had breastfed their youngest child or not, and if so, the duration of exclusive and total breastfeeding. For those who had breastfed, we investigated their perceptions about support at initiation and during the entire breastfeeding period and reasons for breastfeeding cessation. We also asked those who did not breastfeed about their perceptions and reasons for infant formula feeding their youngest child. Analyses were weighted according to the French census data. Results In the NutriNet-Santé cohort, 67.3% of mothers breastfed their youngest child. The proportion of breastfed children increased over the past few decades, from 55.0% (95% CI 54.3, 55.6) in the 1970s to 82.9% (82.4, 83.4) in the 2010s. Total and exclusive breastfeeding duration went from 3.3 months and 2.4 months respectively in the 1970s to 5.9 months and 3.2 months respectively in the 2010s. Most mothers felt supported at initiation and during the breastfeeding period. A reported desire to have breastfed longer than two months was 59.5%. Mothers who did not breastfeed did it by choice (64.3%). They did not feel guilty (78.2%) and did not perceive a problem not to breastfeed (58.8%), but almost half of them would have liked to have breastfed (45.9%). Conclusion Breastfeeding duration has increased in the past decades but did not reach the public health recommendations threshold. Targets other than mothers have to be considered for breastfeeding education, like the partner and her environment, to increase breastfeeding practices. Trial registration The study was registered at ClinicalTrials.gov (NCT03335644).

The dichotomy between hormone recognition by the ectodomain and activation of the G protein by the rhodopsin-like serpentine portion is a well established property of glycoprotein hormone receptors. The specificity barrier avoiding promiscuous activation of the FSH receptor by the high concentration of human chorionic gonadotropin (hCG) prevailing during human pregnancy was thus believed to lie in the ectodomain. In the past two years, mutations responsible for rare spontaneous cases of ovarian hyperstimulation syndromes have partially modified this simple view. Five naturally occurring mutations have been identified which cause an increase in the sensitivity of the FSH receptor to hCG. Surprisingly, these mutations are all located in the serpentine portion of the receptor. In addition to their effect on sensitivity to hCG, they increase sensitivity of the FSH receptor to TSH, and are responsible for activating the receptor constitutively. Together, the available information indicates that the ectodomain and the serpentine domain of the FSH receptor each contribute to the specificity barrier preventing its spurious activation by hCG. While the former is responsible for establishment of binding specificity, the latter introduces a novel notion of functional specificity.Recent data demonstrate that LH and FSH receptors can constitute functional homo- and heterodimers. This suggests the possibility that in cells co-expressing the two receptors, such as granulosa cells, the heterodimers might be endowed with functional characteristics different from those of each homodimer.

Abstract A malpractice lawsuit is in the legal category of an action in tort, which is a demand for compensation for the damages that have occurred. For a physician to be found liable to a patient for malpractice, four essential elements must be proved to sustain an assertion of malpractice: duty, negligence, harm, and causation. The incidence of malpractice litigation in the field of psychiatry is increasing. The most common malpractice claim related to psychiatric practice is the failure to provide reasonable protection to patients from killing themselves. A psychiatrist should be able to evaluate suicide risk on the basis of all available information, including patient responses to direct and indirect questions, known risk factors, information on how the patient behaved under similar circumstances in the past, and collateral information. Reasonable care necessitates that a patient who is either thought of being or established to be suicidal must be the subject of some precautions. A failure either to soundly assess a patient’s suicide risk or to employ an appropriate safety plan after the suicide potential becomes foreseeable is likely to make a physician liable if the patient is harmed because of a suicide event. It is imperative for a psychiatric office or facility to have a good documentation. Careful documentation of evaluations and treatment interventions with a description of changes related to the patient’s clinical condition indicates clinically and legally appropriate psychiatric care.

Abstract Background People with disabilities often face a range of social and economic adversities. Evidence suggests that these disadvantages result in poorer mental health. Some research also indicates that people with disabilities are more likely experience thoughts about suicide than people without disability, although most of this research is based on small cross-sectional samples. Methods We explored the relationship between self-reported disability (measured at baseline) and likelihood of reporting thoughts of suicide (measured at follow up) using a large longitudinal cohort of Australian males. A logistic regression model was conducted with thoughts of suicide within the past 12 months (yes or no) as the outcome and disability as the exposure. The models adjusted for relevant confounders, including mental health using the SF-12 MCS, and excluded males who reported thoughts of suicide at baseline. Results After adjustment, there was a 1.48 (95% CI: 0.98–2.23, P = 0.063) increase in the odds of thoughts of suicide among men who also reported a disability. The size of association was similar to that of being unemployed. Conclusions Males reporting disability may also suffer from thoughts of suicide. We speculate that discrimination may be one explanation for the observed association. More research on this topic is needed.

In light of recent Oceans Policy developments this paper proposes a legal and institutional framework for the management of New Zealand's oceans. It focuses on one key existing problem: the lack of a comprehensive, coherent framework for the sustainable management of the marine environment. The proposed framework addresses this issue by suggesting an overarching statute, based on the approach taken by the Resource Management Act, under which several layers of policy refinement and implementation create national consistency coexisting with localised solutions based on the purpose of sustainability and broad guiding principles. The proposal is consistent with New Zealand's international obligations, other domestic legislation and existing institutional structures. The paper concludes with a statement on the importance to our future well-being of developing a comprehensive and enduring Oceans Policy.

Este artículo trata sobre la capacitación del personal docente de la Universidad Estatal a Distancia de Costa Rica en la atención de personas con discapacidad. Esta investigación se basa en una encuesta de carácter descriptivo y su objetivo es diagnosticar la actitud y la experiencia de los profesores y las profesoras de la UNED con relación a la población estudiantil que presenta alguna discapacidad. Se concluye que para lograr una atención de calidad se debe incrementar la especialización de los profesores y las profesoras en la atención a estudiantes con discapacidad.AbstractThe present article deals with training to professors at Distance State University of Costa Rica in the topic of adequate attention to people with special needs. A descriptive research was carried out. The objective of the research was to diagnose UNED professors’ attitude as well as experience related to students with any kind of impairment. It was concluded that in order to give quality attention, it is necessary to increase the expertise of professors when it comes to deal with students with disabilities.Keywords: Management, Quality of education, people with disabilities, training, distance education


As the usage of the Android smart phones has been considerably increasing, a lot of applications have been developed for the benefits of mobile users. In the past, many applications have been designed aiming to help physically disabled persons. This paper presents an android application which providers several options for controlling the movement of wheelchairs effectively. The proposed application enables People with Disabilities (PWDs) to operate the wheel chair with minimum effort. Apart from voice commands, the proposed application detects and measures the tilt change, and moves the wheelchair based on the degree of the tilt. It also provides a soft joystick as in mobile games to ease the operation of the wheelchairs. Furthermore, sensors that are fixed in the wheelchair can detect and avoid obstacles when the chair is on the move. Hence, it ensures the safety while using the wheelchairs. The proposed application will help both physically challenged persons and elders to operate the wheelchairs more comfortably.

The evaluation of the impacts of land-use change on the water resources has been, many times, limited by the knowledge of past land use conditions. Most publications on this field present only a vague description of the past land use, which is usually insufficient for more comprehensive studies. This study presents the first reconstruction of the historical land use patterns in Amazonia, that includes both croplands and pasturelands, for the period 1940-1995. During this period, Amazonia experienced the fastest rates of land use change in the world, growing 4-fold from 193,269 km2 in 1940 to 724,899 km2 in 1995. This reconstruction is based on a merging of satellite imagery and census data, and provides a 5'x5' yearly dataset of land use in three different categories (cropland, natural pastureland and planted pastureland) for Amazonia. This dataset will be an important step towards understanding the impacts of changes in land use on the water resources in Amazonia.

According to the second economic census data of Jiangsu Province, this paper firstly uses PCA-HCA model based on provincial cities data to classify economic regions. And then, it uses BLR-HCA model to reclassify the economic regions based on counties data. Finally, it comes to the conclusion that the past regional classification ways of Jiangsu Province need to be updated. The research on regional economy is dynamic and timely, while deepening the division of labor and finance is an effective way to develop Jiangsu’s regional economy.
