Objective. To investigate the endotoxicosis indices dynamics in patients, suffering extended purulent peritonitis (EPP) during the treatment process. Маterials and methods. In the investigation 39 patients, suffering EPP in terminal stage, were included. Quantitative contents of endotoxin in the blood serum (the amoebacytes lysate Limulus) was determined in dynamics, using LAL-test, and the patient’s state severity was estimated, basing on clinical and laboratory indices. Hospital lethality have characterized the disease course. Results. In patients with positive clinical dynamics (Group I) the average quantitative indices of endotoxicosis have lowered trustworthily (р=0.0001). In the patients, who died (Group II), the endotoxicosis quantitative indices dynamics were trustworthily higher, comparing with average indices in patients of Group I (р < 0.05). Conclusion. Еndotoxicosis, measuring more 5 еndotoxic units in 1 ml of the blood serum, during 3 postoperative days constitutes one of indications for reoperation. LAL-test in patients, suffering EPP, has statistically significant prognostic and diagnostic value, what substantiates recommendation for its application as indicator of the pathological process course.

Objective Search of pathogenetic mechanisms and risk factors of atherosclerosis in the employees of the cleaning service in Tbilisi. Materials and Methods As a result of a preliminary survey and examination of 200 employes of Tbilisi cleaning service aged 25-45 years (2014-2016), 22 patients with angina, hypercholesterolemia, intimae-media thickness > 0.65 mm, were selected into I group, and 23 individuals without these disorders into II group. In the blood plasma of the selected patients the intensity of oxidative metabolism parameters, TAA and MDA were determined. The variance and correlation analysis (АNOVA) was used for conducting the comparative analysis of the levels of studied parameters. Results In the combined group (I+II) there are several reliable correlations between the Age -TCol, Age-MDA, BMI-Tg, BMI-MDA, LDLChol-HDLChol, LDLChol–TChol, HDLChol-TChol, LDLChol-MDA, LDLChol-TAA. no correlation between these parameters in individual groups (I and II) was found. That indicates that we have an imaginary correlation related to the large intergroup difference between the average values of the group indicators, that is the values of various indicators change during the development of the pathological process, but there is no causal relationship between these alterations. The reliable TAA-MDA correlation in the combined group (I+II) is related to the high anticorrelation between these parameters and the significantly higher average value of TAA in the low-risk group (II) in comparison to the high-risk group (I). Conclusion The results analysis indicates both the diagnostic value of redox status indicators and their leading role in the atherogenesis processes. In populations with a high risk of atherosclerosis, monitoring of serum TAA is recommended.

Abstract The aim of the presented study was the estimation of optimal Ca and P levels applied before calving together with anionic salt addition, as an element of hypocalcaemia and hypophosphataemia prevention. The experiment was carried out during the dry period on 48 cows with similar milk yield in the previous lactation. Cows were divided into four groups. In group I (control) the amount of minerals was in accordance to NRC standards. In experimental groups (groups II-IV), two weeks before calving, cows received 140 g/day/head of hydrated magnesium sulphate to achieve dietary cation- anion difference at the level of about 50 mEq/kg DM. In groups II and III cows received calcium carbonate (100 g/day) 10 days a.p. (antepartum) (group II), or 5 days a.p. (group III), while cows in IV group received dicalcium phosphate (100 g/day) for 5 days a.p. Application of MgSO4 × 7H2O significantly affected the urine pH of cows from group III and IV 4-5 d. before calving - 6.45 and 6.81, respectively. The acidification of urine was observed after calving in group IV (7.13). In cows from group II (100 CaCO3 10 days a.p.) urine pH decline was not found (7.97-7.75). In that group the incidences of hypophosphatemia were noted (blood serum inorganic P level 1.41-1.46 mmol/l). Addition of magnesium sulphate prevented hypocalcaemia occurrence - 4-5 d. before calving the concentration of ionized Ca in blood serum was 1.11, 1.13 and 1.16 mmol/l (respectively for group II, III and IV). Reproductive functions were significantly improved after the application of CaCO3 and CaHPO4 for 5 days a.p. in comparison with control and group II - progesterone concentration in the blood serum on the 45th day of lactation was 1.396 - 1.409 vs 0.799 - 0.401. The correlation between progesterone and inorganic P level in serum was almost significant. Based on the obtained results a treatment optimal in prevention of hypocalcaemia and hypophosphataemia is the application of 50 g CaCO3 and 50 g of CaHPO4 for the last 5 days of the dry period together with MgSO4 × 7H2O given for 14 days a.p.

The study was conducted on sows (hybrids of wbp × pbz breeds) and their offspring (until day 75 of life) kept on a farm. The aim of the experiment was to compare the effects of different sources of selenium (Se) on the production performance of the animals, Se content in their blood, the level of Se in sow’s colostrum, as well as Gpx, haptoglobin and immunoglobulin levels in the serum of sows and their offspring. Experimental feed mixtures for pregnant sows (LP), lactating sows (LK) and piglets (prestarter and starter) in each treatment had an identical basic composition, differing only in the type of selenium forms. Group I received a mineral form of Se in an amount of 0.2 mg/kg; group II received a mixture of a Se amino acid chelate and the mineral form of Se (0.1 mg/kg of each); group III received a Se amino acid chelate (0.2 mg/kg), and group IV received Se-enriched yeasts (0.2 mg/kg). Beneficial effects of the organic forms of Se were evident already in the lactation period. Sows, especially those from group II receiving 0.2 mg/kg of organic Se, had a higher feed intake, which was related to a higher milk production during lactation. As a result, on the weaning day, piglets from this group were significantly heavier than the other piglets. After weaning, as well, the piglets in this group were significantly heavier. These results were confirmed by parameters of blood serum and whey colostrum. Selenium as a chelate was more available than the mineral and enriched yeast forms. For this reason, the animals receiving the chelate were healthier (fewer inflammations were noted). The animals in this group also showed a better feed conversion compared with the others. The Gpx level in sows’ serum varied depending on the treatment. The highest level of this parameter was determined in sows from group III (receiving 0.2 mg/kg of organic Se), and it differed significantly from its value in the control group. The results showed that the Gpx level was related to the Se concentration in blood serum, which was also confirmed by a higher production of selenocysteine (a part of Gpx). Cells of the animals from this group were better protected against free radicals. Administration of 0.1 mg/kg of organic Se positively affects the performance of animals, but the recommended level in feed is 0.2 mg/kg of a selenium-containing amino acid....

This study aimed to assess potential feeding effect of camel milk curd mass and its mixes to experimental rat’s blood serum biochemical parameters, enzymatic activity and the peptide toxicity. Fifty healthy male Sprague-Dawley rats were divided into five groups (n = 10 each). Each group was fed with camel milk pure curd mass and its mixes for 16 days. At the end of the experiment, rats were sacrificed to collect the samples from the blood serum. Blood serum biochemical parameters total protein, cholesterol, glucose, albumin, triglycerides; the enzymatic activities of alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase were determined on the A25 automatic analyser, and peptide toxicity analysed by the reference method. The statistical data have shown no significant differences in body weight gain in all groups. Total protein decreased in group II, IV, and V; however, it increased in group III compared to the control group. Cholesterol grew up in group II and it slightly increased in group V, dropped in groups III and IV compared to group I result. Glucose increased in groups II, III, IV compared to group I; still, group V results show a slight decrease. Albumin decreased in group IV, yet in group V it increased than the group I result. Simultaneously, groups II and III results were changed with less percentage. Triglyceride grew up in groups II, V, and it dropped significantly in groups III, IV compared to the control group. De Ritis ratio of enzymes in groups II, III, and IV fluctuated between 1.31 and 0.98 IU/L; however, group V demonstrated significant data versus group I. Diets peptide toxicity in all groups was lower than control group data. The experimental results indicated that curd mass from camel milk could be used as a pure or with additives and it did not discover the observed side effects.

Objective: The aim of this work is to investigate the diagnostic value of platelet count (PC), mean platelet volume (MPV), the PC to MPV ratio and platelet distribution width (PDW) for prediction of pre-eclampsia (PE). Subjects and Methods: This prospective cohort study included 100 pregnant women, in the first trimester of pregnancy attending to University Hospital, Obstetric Outpatient Clinic, for routine obstetric care from January 2019 to December 2019. Routine obstetric follow-up consists of monthly visits until 32nd gestational week, bimonthly visits between 32nd and 36th gestational week, and weekly thereafter. Patients were classified into two groups: group I: 9 pre-eclamptic patients and group II: non pre-eclamptic 91 patients. CBC indices were measured at each planned visit Results: PC, PC/MPV were significantly decreased, MPV and PDW were significantly increased in group I than group II at the 2nd part of pregnancy. To predict pre-eclampsia, PC at cut-off ≤214, sensitivity was 77.78, specificity was 76.92. MPV at cut-off >9.7, sensitivity was 77.78, specificity was 100.00, PC-MPV at cut-off ≤26.89, sensitivity was 88.89, specificity was 78.02. PDW at cut-off >10.4, sensitivity was 88.89, specificity was 54.95. Conclusion: The increase in the MPV and PDW and the decrease in PC and PC/MPV were observed in preeclampsia. Thus, the platelet indices which are easily available, as well as economical, can also be used in the prediction and early diagnosis of preeclampsia.

The peculiarities of the formation of colostrum in the udder of cows and the immune status in the body of calves in the first hours and days after birth, when included in the diet of cows, haylage from eastern goat’s rue prepared with the use of bio-preservatives “Silostan” (group II) and “GreenGrass 3 × 3”(Group III). The study of colostrum of the first milk yield showed that feeding cows with silage with the bio-preservative “Silostan” contributed to an increase in the content of immunoglobulins by 4.0% (Р<0,001), with bio-preservative “GreenGras 3 × 3” - by 5.3% (Р<0,001). After drinking the first portion of colostrum, immunoglobulins appear in the blood serum of calves after 2 hours. After 6 hours, the content of immunoglobulins in the blood serum of calves of group II was 21.9% higher than in group I (haylage without preservative) (P < 0.05), group III - by 27.1% (P <0.001). The proportion of calves with an immunoglobulin content 6 hours after the first feeding with colostrum not more than 6.0 mg / ml, decreased in group II by 12.0%, in group III - by 20.0%.

Objectives: To evaluate the diagnostic value of the rtRT-PCR test and CT in patients presenting with typical clinical symptoms of COVID-19. Methods: The study with the participation of four center in Turkey was performed retrospectively from 20 March-15 April 2020 in 203 patients confirmed for COVID-19. The initial rtRT-PCR test was positive in 142 (70.0%) of the patients (Group-I) and negative in 61 patients (Group-II). Results: The mean age of the patients in Group-I was 49.7±18.0 years and the time between the onset of symptoms and admission to the hospital was 3.6±2.0 days; whereas the same values for the patients in Group-II were 58.1±19.9 and 5.3±4.2, respectively (p=0.004; p=0.026). Initial rtRT-PCR was found positive with 83.5% sensitivity and 74.1% PPV in patients with symptom duration of less than five days. It was found that rtRT-PCR positivity correlated negatively with the presence of CT findings, age, comorbidity, shortness of breath, and symptom duration, while rtRT-PCR positivity correlated positively with headache. Presence of CT findings was positively correlated with age, comorbidity, shortness of breath, fever, and the symptom duration. Conclusions: It should be noted that a negative result in the rtRT-PCR test does not rule out the possibility of COVID-19 diagnosis in patients whose symptom duration is longer than five days, who are elderly with comorbidities and in particular who present with fever and shortness of breath. In these patients, typical CT findings are diagnostic for COVID-19. Objective — improvement of methods of treatment of chronic generalized periodontitis based on the analysis of the cytokine profile of patients. Methodology. A study of 120 patients with chronic periodontitis of varying severity aged 30 to 50 years was conducted. All patients were randomized into 5 groups according to the severity of the disease and therapy. all patients received traditional anti-inflammatory therapy. In group 4, in addition to the main treatment, patients received laser therapy sessions. In group 5, patients with severe chronic periodontitis were administered remaxol along with laser therapy. Evaluation of the condition of periodontal tissues was carried out according to clinical indices. PMA, SBI, API, OHI-s, and periodontal indices were used. The serum levels of IL-1β, IL-2, IL-4, IL-6, IL-8, IL-10, IFN-γ, and TNF-α were studied by enzyme immunoassay. Results: In chronic generalized periodontitis of moderate and severe severity, the content of all studied proinflammatory cytokines in the blood serum increases, and more significantly in the group of patients with severe disease. At the same time, the content of anti-inflammatory mediators undergoes ambiguous changes: in group I, the content of IL-4 increases significantly, and in group II it does not change significantly. In turn, IL-10 in group I does not decrease significantly, but in group II it becomes significantly less than normal. The inclusion of laser therapy and remaxol in the complex of standard measures showed high efficiency in reducing the severity of the pathological process, which was expressed in a faster subsiding of inflammatory phenomena in periodontal tissues. Conclusions: An increase in the content of proinflammatory cytokines in the blood serum in chronic generalized periodontitis was revealed, and more significantly in the group of patients with a severe course of the disease. At the same time, an imbalance of the system of pro-inflammatory and anti-inflammatory cytokines is registered. The use of laser therapy and remaxol in the complex treatment of CPT was the most effective.

Objective. To investigate serum levels of free β-HCG, progesterone, and ischemia-modified albumin (IMA) and their combined use in the prediction of first trimester abortions. Methods. A total of 156 pregnant women between 5 and 13 weeks of gestational age were included in this study. At admission, serum levels of free β-HCG, progesterone, and IMA were noted and all cases were divided into two groups; Group I (n=77) resulted in abortion including missed abortion, incomplete/complete abortion, and inevitable abortion whereas Group II (n=79) included normal pregnancies. Results. Compared to Group II, the significantly decreased value of free β-HCG progesterone and significantly increased value of IMA were found in Group I (P<0.01, P<0.01, P<0.01, resp.). When combining all three parameters, sensitivity 75%, specificity 99%, PPV 98%, and NPV 76% were obtained. The multivariate logistic regression analysis revealed the free β-HCG, progesterone, and IMA independent factors in the prediction of abortions. Conclusions. The combined use of free β-HCG, progesterone, and IMA levels can be useful in the prediction of first trimester spontaneous abortions.


Abstract. The disturbed placentation in a certain way plays an important role in the pathogenesis of great obstetric syndromes. Therefore, the possibility of using biochemical indicators of the level of placental growth factor (PlGF) and fms-like tyrosine kinase (sFlt-1) for predicting preterm labor is of great interest. The aim of the study was developing criteria for predicting preterm labor based on the combined use of ultrasound cervicometry and the concentration of PlGF and sFlt-1. Materials and methods. A total of 227 pregnant women were examined, 190 of whom had preterm birth. Group I included 48 women whose pregnancy completed at 23 to 27 weeks. In group II, 142 women with prematurity in terms of 28 to 36 weeks were observed. Group III included 37 women with a healthy pregnancy, which completed at 38–41 weeks. All patients involved in the study underwent ultrasound cervicometry on a Voluson 730 (GE Healthcare, USA) at 16 weeks, and also the concentration of PlGF and sFlt-1 in blood serum by electrochemiluminescence immunoassay using a Cobas e411 analyzer was studied (Roche diagnostics, Switzerland). Results. The study revealed a significant (p < 0.001) decrease in the average concentration of PlGF in the blood serum by 65 % associated with an increased level of sFlt-1 by 93% in women with early termination of pregnancy – 23–27 weeks (group I) in relation to women with a healthy pregnancy. In women with preterm birth at 28–36 weeks of gestation (group II), a similar trend of changes was observed: the PlGF level significantly (p = 0.014) decreased on average by 68 % in relation to women with a healthy pregnancy. In women of group I, the value of the sFlt-1 / PlGF ratio exceeded (p < 0.001) the values of patients in group III by 14 times. But an even more pronounced imbalance of angiogenic factors was observed in women of group II – on average 16 times. Two main markers were used to predict premature birth: shortening of the cervix according to ultrasound cervicometry data of less than 30 mm, as well as the sFlt-1/PlGF ratio of more than 50 conv. units. The odds ratio (OR) of preterm birth in the case of using only the length of the cervix was 34,133 (95 % confidence interval – 12.308–94.660). OR in the case of combined use of cervicometry data and angiogenesis variables was 148.750 (95 % confidence interval – 33.243–665.593). Conclusion. Thus, the use of an additional biochemical marker significantly improved the accuracy of preterm labor prediction.
