POSSIBILITIES OF THE USE OF INTRAUTERINE CONTRACEPTION IN WOMEN OF DIFFERENT AGE GROUPS

Currently, the problem of family planning is not only medical, but also social. The reason for this is the widespread use of artificial termination of pregnancy in our country. Moreover, the majority of women in Russia resort to the extremely dangerous method of abortion - acute curettage, which is associated with a high risk of further infertility and the development of long-term adverse consequences. It is necessary to inform all patients of reproductive age about the neg-ative consequences of artificial termination of preg-nancy and the possibilities of modern methods of con-traception. One of the most popular in our country (in second place after combined oral contraceptives) and highly effective (Pearl index is 0.1-0.5) methods of contraception is the intrauterine device. Intrauterine systems (IUDs) are reversible long-term contraception, which makes them easy to use and affordable. The IUD is installed for 5 years and does not require constant monitoring by the patient. If necessary, the device can be removed at any time. Another positive aspect is that fertility is restored after 12 months in 80-96% of cas-es. Also, intrauterine contraceptives have the thera-peutic effect, since the local action allows you to nor-malize the menstrual cycle, which is especially im-portant for women suffering from hyperpolymenor-rhea and chronic anemia. It has been proven that IUDs do not increase the risk of developing endome-trial and cervical malignant tumors and, in addition, have antiproliferative and antimitotic effects, which makes it possible to use them in patients with complex atypical hyperplasia and highly differentiated stage IA endometrial adenocarcinoma.

Background: Ovarian malignancy is the second most common cancer of the female reproductive system and the leading cause of death from gynecologic malignancy. With increase in longevity, the incidence of epithelial ovarian cancer is increasing and its etiopathology remains unknown. A female’s risk at birth of having ovarian tumour sometime in her life is 6-7%. Two third of ovarian tumours occur in women of reproductive age group. The aim was to study the distribution of morphological pattern of benign, malignant, and nonneoplastic lesions of the ovary in different age groups and to determine the likelihood of bilateral involvement in different morphologic subtypes.Methods: A retrospective study from January 2018 to December 2020 was undertaken. A total of 210 surgical specimens were obtained. Detailed clinical information and radiological findings were recorded from the case sheets. Grossing of the surgical specimens was done in the pathology department, followed by histological examination.Results: Of 210 cases, benign cases were 140 (66.7%), malignant cases were 70 (33.3%). Surface epithelial tumors were most common (116/55.2%) followed by germ cell tumors (76, 36.1%) followed by others. Serous cystadenoma was commonest benign tumor (58, 41.4%). Serous adenocarinoma was commonest malignant tumor (19, 27.1%). Benign tumours were more common in the younger age group i.e. <40 years of age whereas malignant tumours were supervenes with increase in age. Most benign ovarian tumors (54, 38.6%) were seen between 31-40 years whereas most malignant tumors (24, 34.3%) were seen above 40 years. In 1st two decades, germ cell tumors were more common than other tumours.Conclusions: The prognosis and varying therapeutic strategies of ovarian tumours necessitate an accurate pathological evaluation. Histopathological study is still the gold standard in diagnosing most of these tumours.

The essence of the chapter is use of communication mix used for addressing the dissonance of the women among reproductive age regarding long-term contraceptives. The use of Intrauterine Device (IUD) has remained static at around two percent for the last decade and a half, despite IUDs being an effective reversible contraceptive. There are many myths and misconceptions prevalent in the communities, which have resulted in this low use of IUD. The different communication channels, interpersonal counseling, mass media, medical detailing and helpline, have the same objective of raising awareness and dispelling negative words regarding IUD. An outbound call through helpline is based on the principle of AIDAS (Awareness Interest Desire Action Satisfaction). A helpline call through counselor help in assuring women of the method adopted and creates brand ambassadors for IUD, who in turn motivate other women in the community.

The importance of solving this problem is due to the significant prevalence of this pathology, its long-term recurrent course and the presence of a medico-social task of preserving the reproductive function of patients. The age of women who develop dyshormonal diseases is already 30–35 years old, which leads to premature loss of reproductive function, an increase in the frequency of surgical interventions. The systemic nature of the lesion in dyshormonal pathology is due to the common links of pathogenesis. The leading factor in the pathogenesis of hormone-dependent pathology is absolute or relative hyperestrogenism. Menstrual disorders (MD) is one of the most overlooked problems in modern gynecology. Over the past decade, an increase in the frequency of MD in women of reproductive age has been noted in the world by almost 11 times. In the structure of gynecological morbidity, according to most authors, MD s account for about 60% of all dyshormonal disorders, which often lead to an underestimation of future not only reproductive, but also general somatic problems: from miscarriage and infertility to the formation of both benign and malignant tumors in the female organism. Abstract Background Ovarian torsion is a gynecological surgical emergency whose diagnosis remains a challenge. Torsion occurs most frequently in women of reproductive age. It is usually associated with the presence of benign masses in the ovary, as malignant tumors are less frequent and less prone to undergo torsion. Case presentation We report the case of a 17-year-old Caucasian patient who presented to the emergency department with lower abdominal pain. Ultrasonography evaluation revealed a unilateral ovarian lesion, 11.2 cm, with features suspicious for malignancy and torsion. The patient was referred for surgical torsion treatment and underwent unilateral salpingo-oophorectomy. The pathology report confirmed a serous borderline ovarian tumor with torsion. Conclusions Malignant ovarian torsion in pediatric age groups is rare. Ultrasound examination should be recognized as a powerful tool for diagnosis and management, especially when performed by an experienced ultrasonographer.

Contraceptives are the methods, medicines, or devices which are used to prevent pregnancy. The women can select any kind from the available contraceptives. The objectives are to investigate the types of contraceptive methods used by Iraqi women of reproductive age and to throw lights on their complications.  A cross-sectional study had been done in the outpatient department of gynecology and obstetrics of Maternity and Children Hospital in Al-Diwaniya city on 400 women of reproductive age between (16-45) years during the period from 20th of September 2016 to 30th of March 2017. A questionnaire distributed among participating women. All the involved women gave their informed consent after describing the aim of the study and ensuring the confidentiality of the data.  The data were gathered and analyzed statistically. The study revealed that the most commonly used contraceptives were oral pills, the intrauterine device (IUD),  injections, and the condom  (53.7%, 35,7%, 7.5%, 3%), respectively. Oral pills are the most commonly used method in all age groups. The complications were present in (55.0%) of women of reproductive age, and they were present in (69.2 %) of users of the Intra-Uterine Device followed by pills users (45.5%). The most common complications for the oral contraceptives were pain, malaise, and psychiatric disturbances (21.8%, 10.6%, 8.3%), respectively, while in the case of Intra-Uterine Device, the most common complications were bleeding and infection (31.4%, 21.6%) respectively. Injection contraceptive users had suffered from the infection (14.3%). The women’s age, residence, level of education, occupation, and economic status show a significant association with the use of contraceptives methods. The oral contraceptive pills and Intra-Uterine Device are the most frequently used contraceptives. The most common complaints were bleeding, infection, and pain. There is a significant association between the use of contraceptives methods and the women’s age, residence, level of education, occupation, and economic status.

Relevance. Endometriosis is a common gynecological disease that affects up to 10% of women of reproductive age worldwide and is the main cause of pain and infertility. Endometriosis is a disease, although it has been known for a long time, nevertheless, in many ways it represents terra incognita for modern medicine. Aim. Analysis of literature on the feasibility of long-term and the use of the drug dienogest 2 mg (Vizanne), which has a positive effect on the quality of life of patients with endometriosis. Materials and methods. To write this review, a search was made for domestic and foreign publications in Russian and international search engines (PubMed, eLibrary, etc.) over the past 13 years. The review included articles from peer-reviewed literature. Results. The review presents data on the difficulties of verifying the diagnosis of endometriosis due to a combination of this pathology with pain, infertility, abnormal uterine bleeding. The pathogenesis of the origin of endometriosis-associated pain is presented. The efficacy of the use of the drug dienogest (Vizanne), which has a powerful antiproliferative effect that reduces the main symptoms of endometriosis (pain, bleeding), is substantiated. The expediency of long-term and safe use of the drug dienogest (Vizanne), which has a positive effect on the quality of life of patients with endometriosis, has been proved. Conclusions. Numerous scientific publications confirm the feasibility of prolonged use of the drug dienogest (Vizanne), to achieve remission during endometriosis.

In order to determine possible trends in the dynamics and characterological structure of personality in the General population caused by the COVID-19 pandemic, which is a long-term strong stressful effect and clinically and psychopathologically comparable to chronic personality changes after experiencing a disaster, the conditions predisposing to personal transformation, including clinical and prognostic patterns, are analyzed. The age-dependent nature of these changes is shown, and a number of features identified for different age groups are discussed.

Animal toxins and venoms have recently been developed as cancer treatments possessing tumor cell growth-inhibitory, antiangiogenesis, and proapoptotic effects. Endometriosis is a common benign gynecological disorder in reproductive-age women, and no definite treatment for this disorder is without severe side effects. As endometriosis and malignant tumors share similar characteristics (progressive, invasive, estrogen-dependent growth, and recurrence), animal toxins and venoms are thought to be effective against endometriosis. The objective of this study was to outline studies using toxic animal-based medicinal materials (TMM) as endometriosis treatment and to explore its clinical applicability. Preclinical and clinical studies using TMM were searched for in four databases from inception to October 2020. A total of 20 studies of TMM on endometriosis were included. In eight clinical studies, herbal medicines containing TMM were effective in relieving symptoms of endometriosis, with no side effects. In twelve experimental studies, the main therapeutic mechanisms of TMM against endometriosis were proapoptotic, antiangiogenesis, estrogen level-reducing, and possible anti-inflammatory effects. TMM are thus considered promising sources for the development of an effective treatment method for endometriosis. Further studies are needed to clarify the therapeutic mechanism of TMM against endometriosis and to provide sufficient grounds for clinical application.
