During last years, ultrasound duplex angioscanning (USD) gives opportunity to get the most complete information respecting the venous pathology and is used as a method of navigation during conducting of endovascular manipulations. Purpose of research — to study the ultrasound changes in the thrombosed vein before and after the influence of endovascular high-frequency electric welding and to evaluate the effectiveness of the method in the complex surgical service of acute ascending thrombophlebitis of the GSV. During the period from 2015 to 2017 in the surgical departments of the city clinical hospital №8 in Kyiv there were 38 patients with acute ascending thrombophlebitis of the great saphenous vein (AATGSV), who were treated with endovascular electric welding (EVEW). The age ranged from 19 to 78 years (average age 51.28±2.63 years). According to the international classification of CEAR (Clinical Etiological Anatomical Pathophysiological), C2 was noted in 6 (15.8%) patients, C3 — in 13(34.2%), C4 — in 7(18.4%), C5 — in 9(23.7%), C6 — in 3 (7.9%). All patients was treated with ultrasound duplex angiography (USD) using TOSHIBA Nemio XG (Japan), equipped with a convex sensor with an operating frequency of 3.5–5 MHz and a linear sensor with the range of 7.5–12 MHz. Domestically produced electric welder “Swarmed” EK-300М1 were used as a feeding device “Swarmed” EK-300М1. In manual mode, 15 sec. Parametric methods were used for statistical processing of the received results. Average arithmetic meanings (M), standard error of arithmetic mean (m) were calculated. The authenticity of the differences were evaluated using the Student`s criterion. Differences were recognized as statistically significant in p <0,05. Electric welding of the thrombosed vein segment was performed using an endovascular electric welding catheter (EC). With ultrasound examination of the thrombosed vein segment with acute ascending thrombophlebitis of the GSV the average diameter of the vein to effect by EVEW was 9.6±0.4 mm. Multiple hyperechogenicity thrombotic masses were visualized in the vein lumen. On the first day after the EVEW, the diameter of the GSV was increased by 1.3 times and averaged was 12.2±0.4 mm (p<005). The diameter of the vein increased due to the edema of the venous wall after the EVEW. After 1 month, the vein decreased in diameter significantly compared to the previous indicator and its average diameter was 7.4±0.5 mm (p<005), the vein lumen had a hypoechogenic character. After the 3 months period, compared to the 1 month after the EVEW, the vein diameter continued to decrease to 4.7±0.5 mm (p<005). After 6 months from the moment of the EVEW, the diameter of the vein was even more reduced compared to the period of 3 months — 2.4±0.4 mm (p<005). After the period of 1 year or more duplex ultrasound picture showed the completion of the process of obliteration with total vein ablation. In the observation period of 3-6 months, the occurrence of GSV repatency was observed for 3 (7,89%) patients. The ultrasound picture in the thrombosed vein after the endovascular high-frequency electric welding undergoes changes from anechogenic, hypoechogenic to echogenic character, reflects the occlusion process, obliteration and total ablation of the vein.

Relevance: the reason for the development of complications in endodontic treatment is poor-quality instrumental treatment root canals.Aims: a study of the animicrobial action and clinical efficacy of high-frequency monopolar diathermocoagulation in the treatment of chronic forms of pulpitis.Materials and methods: 102 patients with various chronic forms of pulpitis were divided into three groups of 34 patients each. In the first two groups, high-frequency monopolar diathermocoagulation was used in endodontic treatment in different modes. In the third group, endodontic treatment was carried out without the use of diathermocoagulation (comparison group). The root canal microflora in chronic pulpitis in vivo was studied twice-before and after diathermocoagulation.Results: it was established that high-frequency monopolar diathermocoagulation in the effect mode is 3, power is 4 (4.1 W) and effect is 4, power is 4 (5.4 W) with an exposure time of 3 seconds, it has a pronounced antibacterial effect on all presented pathogenic microflora obtained from the root canals of the teeth.

Objectives: To assess the effect of electronic acupuncture combined with “Tam ty thang” remedy in the treatment of rheumatoid arthritis by wind-cold-damp. Materials and methods: prospective study, comparing before and after treatment, including 43 patients who were diagnosed with rheumatoid arthritis by wind-cold-damp according to traditional medicine. Results: good level occupied 69,8%, and fair good level occupied 30,2%.The improved motor function accounted for 95.3%, to relieve pain up to 95,3%, reducing inflammation of VSS in 1 hour at the moment No 31,65, N28 17,88. The improvement in the level of activity of the hand reached 48,8% for good level. Conclusions: The method of electronic acupuncture combined with “Tam ty thang” remedy have a good effect in the treatment of rheumatoid arthritis by wind-cold-damp and it didn’t cause significantly side effects in clinic. Postthrombotic syndrome is the most common complication after deep venous thrombosis. Postthrombotic syndrome is a debilitating disease and associated with decreased quality of life and high healthcare costs. Postthrombotic syndrome is a chronic disease, and causative treatment options are limited. Prevention of postthrombotic syndrome is therefore very important. Not all patients develop postthrombotic syndrome. Risk factors have been identified to try to predict the risk of developing postthrombotic syndrome. Age, gender, and recurrent deep venous thrombosis are factors that cannot be changed. Deep venous thrombosis location and extent seem to predict severity of postthrombotic syndrome and are potentially suitable as patient selection criteria. Residual thrombosis and reflux are known to increase the incidence of postthrombotic syndrome, but are of limited use. More recently developed treatment options for deep venous thrombosis, such as new oral factor X inhibitors and catheter-directed thrombolysis, are available at the moment. Catheter-directed thrombolysis shows promising results in reducing the incidence of postthrombotic syndrome after deep venous thrombosis. The role of new oral factor X inhibitors in preventing postthrombotic syndrome is still to be determined.

Objective. To assess volume guarantee (VG) ventilation combined with high-frequency oscillatory ventilation (HFOV) strategy on PaCO2regulation in an experimental model of neonatal distress syndrome.Methods. Six 2-day-old piglets weighing  kg were used for this interventional experimental study. Animals were ventilated during physiologic lung conditions and after depletion of lung surfactant by bronchoalveolar lavage (BAL). The effect of HFOV combined with VG on PaCO2was evaluated at different high-frequency expired tidal volume (VThf) at constant frequency () and mean airway pressure (mPaw). Fluctuations of the pressure (ΔPhf) around the mPaw and PaCO2were analyzed before and after lung surfactant depletion.Results. PaCO2levels were inversely proportional to VThf. In the physiological lung condition, an increase in VThf caused a significant decrease in PaCO2and an increase in ΔPhf. After BAL, PaCO2did not change as compared with pre-BAL situation as the VThf remained constant by the ventilator.Conclusions. In this animal model, using HFOV combined with VG, changes in the VThf settings induced significant modifications in PaCO2. After changing the lung condition by depletion of surfactant, PaCO2remained unchanged, as the VThf setting was maintained constant by modifications in the ΔPhf done by the ventilator.

Background: The aim of the study is to investigate whether the circadian IOP rhythm can be influenced by combined cataract surgery with high frequency deep sclerotomy (HFDS) and whether intraocular pressure (IOP) can be significantly reduced by HFDS. Methods: In our study 10 patients were included, in whom 24 h IOP monitoring was installed before and after HFDS/cataract surgery using a Triggerfish. HFDS is a minimally invasive glaucoma surgery (MIGS). Results: After performed HFDS combined with cataract surgery, the IOP was reduced from 27.7 ± 2.11 mmHg to 14.4 ± 2.59 mmHg, which is highly significant (p < 0.001). The contact lens sensor (CLS) cosinor analysis pre- and postoperatively showed that the circadian rhythm is not influenced by the surgery, i.e., the circadian IOP rhythm did not show significant differences before and after surgery. Conclusions: HFDS combined with cataract surgery is a potent surgical method that can significantly reduce the IOP. However, the circadian rhythm cannot be changed by the surgery. The acrophase remained during the night in all patients.
