Rhinology rapid development, the emergence of new and improvement of already known methods of diagnosis and treatment of paranasal sinuses (PNS) inflammatory diseases almost did not affect the prevalence growth rate of this pathology. Surgical interventions did not contribute to reducing the frequency of recurrence and chronicity of paranasal sinusitis, while the need for drug therapy is steadily increasing. The availability of the mucous membrane for drug exposure, its continuity in both the upper and lower respiratory tract, high reflexogenic response allow topically stimulating secretory activity, causing dehydration of all glandular components of the mucous membrane. The article presents the experience of using the drug, an extract of Cyclamen europaeum, in the treatment of acute rhinosinusitis. Clinical examples show the efficacy of its use both as monotherapy and in combination with systemic antibiotics. The significant drainage effect of the drug allows avoiding invasive methods of PNS lavage (puncture, intubation, etc.) in the complex therapy of this pathology and reducing the treatment duration by 3–4 days compared to the control group. Therefore, it can be recommended for wider use in the treatment of catarrhal processes and purulent inflammation of the nasal mucosa and PNS. The aim of the study was to identify possible changes in the maxillary sinuses during new coronavirus infection in comparison with the changes in the sinuses occurring during acute respiratory infections and to correlate the gender structure of the study groups. Material and methods. The study included 40 patients (18 women and 22 men) with a confirmed new coronavirus infection, hospitalized at the Covid Hospital Clinics of Samara State Medical University in 2020. All patients underwent multispiral computed tomography of the paranasal sinuses on a Ge revolution Evo 128 tomograph on the 10th day after the appearance of the first clinical symptoms. We also examined a control group of patients with acute respiratory infections with similar complaints of impaired sense of smell, rhinorrhea, and upper maxillary sinus discomfort. This group also included 40 people (24 women and 16 men). Statistical processing of the study results was performed using Microsoft Excel software package, the Mann – Whitney method of statistical significance calculation was used. Results and discussions. In 18 (45%) patients with a confirmed new coronavirus infection, a slight mucosal thickening along the lower wall of the maxillary sinuses was found symmetrically on both sides. Six (15%) of them were women and 12 (30%) were men. The age of the examined patients ranged from 31 to 83 years. Chronic sinusitis was noted in the history of 8 (20%) of the examined patients. In 6 (15%) patients with chronic sinusitis no abnormalities were detected in the maxillary sinuses at the time of examination. Two of them had parietal thickening of the mucous membrane along the lower wall of the maxillary sinuses. In the control group, there was significant symmetric parietal thickening of the mucous membrane of the paranasal sinuses in 24 (60%) examined patients (14 women and 10 men). There were no statistically significant differences in the prevalence of changes in the studied and control groups. Conclusion. According to the results of this study, there were no data on the pattern of occurrence of changes in the mucosa of the maxillary sinuses in new coronavirus infection with gender background or the presence of chronic inflammation of the maxillary sinuses in the history compared with the control group of examined patients after acute respiratory infections.

Acute rhinosinusitis is one of the most common diseases in the world, characterized by acute inflammatory processes in the nasal cavity and paranasal sinuses, lasting less than 4 weeks. According to statistics, in Russia, this disease is recorded in 6–15% of the population annually, in European countries in every seventh person, while there is a tendency to an increase in the incidence. Acute viral rhinosinusitis has the highest incidence among all rhinosinusitis and occurs in most patients in a mild form, however, untimely treatment of inflammation of the nasal mucosa can lead to obstruction of the natural anastomoses, impaired physiological ventilation and drainage of the paranasal sinuses, stagnation of secretions with further development acute bacterial rhinosinusitis. The overuse of local vasoconstrictor drugs (intranasal decongestants) leads to the development of drug-induced rhinitis, long-term imbalance of autonomic innervation of the nasal mucosa and edema, called "rebound syndrome". In the article we consider the issues of etiology, pathogenesis, features of the course of rhinosinusitis. The principles of differential diagnosis of viral and bacterial rhinosinusitis are stated, taking into account the latest recommendations and research in this area. The features of the use of the most frequently used topical preparations in acute rhinosinusitis in the practice of an ENT doctor have been analyzed.

The issue of treating paranasal sinusitis, improving the methods of their treatment and prevention remains relevant to the present. Particular attention shall be paid to patients with concomitant somatic pathology, which contributes to the chronicity of inflammatory processes in the paranasal sinuses and increases the frequency of their recurrence. Among the pathology of ENT organs, acute sinusitis is most common (5–10%). Inflammatory diseases of the nose and paranasal sinuses are fraught with various complications. Thus, the percentage of orbital rhinogenic complications ranges from 6.6 to 12.4%. Patients with a history of diabetes mellitus are more exposed to various diseases, including ENT organs, with their rapid development and subsequent complications. The clinic of purulent sinusitis in them is characterized by a prolonged course with periodic exacerbations and the involvement of nearby anatomical structures in the pathological process with atypical data of radiation studies and frequent complications. The disease proceeds against the background of pronounced changes in the immune status, which affect all links of immunity, including a significant decrease in phagocytosis indicators and an increase in the content of circulating immune complexes of small size. Prolonged exposure of the body to products of free radical oxidation, especially in excess volume, leads to metabolic disorders at all levels and also disrupts the immune status, which leads to a complicated course of diseases. The presence of a pronounced imbalance towards the production of free radicals and a weakening of antioxidant protection leads to the development of oxidative stress and destructive changes at different levels. Treatment of this category of patients is a difficult task, in the solution of which the state of immunity and antioxidant system plays an important role.

Relevance: Modern endonasal surgery allows to remove polyps from all affected paranasal sinuses, following the principles of minimal invasiveness. Minimal traumatic of surgical intervention gives the best results, accompanied by less progression of the disease. Minimal invasiveness of surgical intervention and its obligatory combination with postoperative medical treatment are common practice in many countries. One of the drugs that actively effect the restoration of the mucous membrane of the nasal cavity and paranasal sinuses in the early postoperative period is Nazomer, which includes sodium hyaluronate and dexpanthenol in saline solution. The purpose of the study is to investigate the effectiveness of the drug Nazomer in patients with polyposis rhinosinusitis after endoscopic polyposynosotomy. Results and discussion: The main group consisted of 30 patients with polyposis rhinosinusitis, who were prescribed Nasomer in addition to standard treatment in the postoperative period. The control group included 30 patients who underwent standard treatment in the postoperative period. As criteria for clinical efficacy, data from endoscopic examination of the nasal cavity and indicators of anterior rhinomatometry, measured by the «Optimus» device, were selected. In the main group, the index of nasal breathing according to rhinomatometry was better than the results of patients in the control group on the 3rd and 5th day of the postoperative period by 26% and 24%, respectively. Conclusion: The use of the drug Nazomer in the postoperative period in patients with polyposis rhinosinusitis contributes to more active restoration of respiratory function of the nasal cavity compared with the control group, according to rhinomatometry, up to 26%. Based on the results obtained, the drug Nazomer is an effective anti-inflammatory and regenerative agent in the postoperative period in patients with polyposis rhinosinusitis.

The pediatric nasal cavity and paranasal sinuses, when compared to those in adults, differ not only in size but also in proportion. Knowledge of the unique anatomy and pneumatization of children's sinuses is an important prerequisite to understanding the pathogenesis of sinusitis and its complications. It is also important in evaluation of radiographs and in planning surgical interventions. In order to study the development of the paranasal sinuses in children and relate clinical anatomy to sinus surgery, the sinuses in 102 pediatric skulls and cadaver heads were measured. The results were classified by stage of development into 4 different age groups: newborn and 1 to 4, 4 to 8, and 8 to 12 years. The characteristics of each group and their clinical importance for paranasal sinus surgery are described.

Currently, there is a rapid development of commercial salmon farming in Russia, which has grown almost 3 times in terms of production since 2013 and exceeded 116 thousand tons in 2020. In turn, this requires the appropriate development of the feed base and the development of methods for the effective use of fairly expensive fi sh feed. From these positions the involvement of the feed additive “Vinivet”, which is a waste of beekeeping in the feed production for fi sh as a protein additive is relevant. The feed additive “Vinivet” contains bee products such as bee bread and slumgum, a significant complex of vitamins, amino acids, macro- and microelements. The purpose of the work was to assess the influence of the feed additive “Vinivet” on the livability of juveniles, growth rate and feed consumption of rainbow trout of Steelhead salmon breed when growing in a recirculating aquaculture system. For the experiment juveniles with an average juvenile weight of 2,37±0,72 g have been selected, from which the control and 3 experimental groups have been formed. The control group has received a standard starting compound feed as the main diet, the 1st experimental group in addition to the main diet has received the additive “Vinivet” in the amount of 2,5 % of the mass of the main diet, the 2nd experimental group has received 5,0 % of the additive, and the 3rd experimental group in the first week received 2,5 % of the additive, and from the second week of studies the dose of the additive was increased to 5 %. The conducted studies have shown that the inclusion of the feed additive “Vinivet” in the feed when growing juveniles of Steelhead salmon is most advisable by introducing it at the dosage of 2,5 % of the mass of the main diet in the first week of cultivation, followed by the increase in the dose to 5,0 %, which allows achieving the highest fi sh productivity of pools in the recirculating aquaculture system compared to all variants of the experiment and by 24 % compared to the control group.

Irrigation therapy is widely used in clinical practice for the treatment of acute rhinosinusitis. One of the methods of irrigation therapy is the Proetz method. This method has many contraindications and is associated with the development of a number of complications, which are not always taken into account by otorhinolaryngologists. We present a number of observations of complications associated with an uncontrolled pressure decrease in the nasal cavity and paranasal sinuses.

Background: Failure of rotator cuff healing is a common complication despite the rapid development of surgical repair techniques for the torn rotator cuff. Purpose: To verify the effect of atelocollagen on tendon-to-bone healing in the rabbit supraspinatus tendon compared with conventional cuff repair. Study Design: Controlled laboratory study. Methods: A tear of the supraspinatus tendon was created and repaired in 46 New Zealand White rabbits. They were then randomly allocated into 2 groups (23 rabbits per group; 15 for histological and 8 for biomechanical test). In the experimental group, patch-type atelocollagen was implanted between bone and tendon during repair; in the control group, the torn tendon was repaired without atelocollagen. Each opposite shoulder served as a sham (tendon was exposed only). Histological evaluation was performed at 4, 8, and 12 weeks. Biomechanical tensile strength was tested 12 weeks after surgery. Results: Histological evaluation scores of the experimental group (4.0 ± 1.0) were significantly superior to those of the control group (7.7 ± 2.7) at 12 weeks ( P = .005). The load to failure was significantly higher in the experimental group (51.4 ± 3.9 N) than in the control group (36.4 ± 5.9 N) ( P = .001). Conclusion: Histological and biomechanical studies demonstrated better results in the experimental group using atelocollagen in a rabbit model of the supraspinatus tendon tear. Clinical Relevance: Atelocollagen patch could be used in the cuff repair site to enhance healing.

Aim. Study of microbiocenosis of nose mucous membrane during allergic rhinosinusitis. Materials and methods. Patients with polypous (PRS) and atopic (ARS) rhinosinusitis were examined, as well as a control group. Standard general clinical methods taking differential diagnostics of atopic diseases and rhinitis into consideration were used for the PRS and ARS diagnosis. Results. Microbial content during different forms of rhinosinusitis has varying directionality that is determined by different pathogenetic mechanisms. ARS microflora has a significantly extended range and was characterized by an increase of concentration of opportunistic microorganisms not characteristic for normoflora. Microbial composition for PRS was significantly depleted by a lack of certain permanent members of microflora, whereas the quantity of opportunistic bacteria was significantly above normal. Conclusion. Disturbance of microbiocenosis in patients with allergic rhinosinusitis was detected, more pronounced in the PRS group. Staphylococcus strains isolated from patients with ARS and PRS possess pathogenic properties in equal ratios, wherein the percentage of strains in ARS group that have persistence properties is higher than in other studied groups. This could give evidence regarding their role in development of inflammatory process on the nose mucous membrane.

Results of randomized prospective study with participation of 94 patients aged from 20 to 70 years with monosegmental lumbar spine lesions are presented. Minimum invasive surgical interventions were performed in 55 patients from the main group. Control group consisted of 39 patients in whom decompressive-stabilizing operations via conventional posteromedian approach with skeletization of posterior segments of vertebral column were performed. Average size of operative wound in open interventions more than 10 times exceeded that size in minimum invasive interventions and made up 484 ± 56 and 36 ± 12 sq.cm, respectively. Mean blood loss was 326.6 ± 278.0 ml in the main group and 855.1 ± 512.0 ml in the comparative one. In the main group no one patient required substitution hemotransfusion, while in 13 patients from the comparative group donor erythrocytic mass and/or fresh-frozen plasma were used to eliminate the deficit of blood components. Intensity of pain syndrome in the zone of surgical intervention by visual analog scale in the main group was lower than in comparative group. In the main and comparative groups the duration of hospitalization made up 6.1 ± 2.7 and 9.7 ± 3.7 bed days, respectively. In no one patient from the main group complications in the zone of operative wound were noted. Three patients from the comparative group required secondary debridement and in 1 patient early deep operative wound suppuration was observed. Application of low invasive surgical techniques for the treatment of patients with degenerative lumbar spine lesions enabled to perform radical surgical treatment with minimal iatrogenic injury. The method possessed indubitable advantages over the conventional open operations especially intraoperatively and in early postoperative period.
