Objective: We identified the main occupational hazards in the hospital environment and determined their role in the formation of the safety of medical personnel at healthcare institutions. Materials and methods: We applied the bibliosemantic, questionnaire, hygienic and statistical research methods. Results: In the course of the labour activity at the health care institutions (HCI), the medical workers are exposed to the risk of negative effects of biological, psychophysiological, chemical, physical and social harmful and dangerous factors. The biological and psychophysiological factors are the most harmful factors. The adverse occupational factors of the hospital environment do not have an isolated effect, but have a complex, combined and joint effect, and their priority varies depending on the specificity of medical speciality. Under conditions of COVID-19 pandemic, the risk of the negative impact of production environment factors on the medical personnel at the HCI has been increased. In particular, the risk of infection with pathogens of infectious diseases increases, the work load, psycho-emotional tension, the duration of the use of personal protective equipment and the frequency of contact with the disinfectant solutions increase, the probability of violence, harassment, stigmatization, and discrimination related to the professional affiliation of health care personnel increases. The lack of a safety culture is one of the most important mechanisms for launching and realizing the dangers of a hospital environment, and just a creation of positive practice of a safety culture at work can significantly reduce the risks of realizing potential hazards that exist at health care institutions. Conclusion: The identification of the dangers of the hospital environment with the formation of a comprehensive understanding of their importance for the ensurance of the safety for medical personnel is an urgent task for modern medical science and will make it possible to develop reasonably the measures aimed at the ensurance of the safety of the hospital environment in terms of the safety of medical personnel.

The term viral (interstitial) pneumonia, widely used in the clinical presentation, describes its development. This article describes the development of the disease symptoms, the safety of medical personnel in the conditions of coronavirus spread, principles of health care delivery to patients in a hospital environment.

The characteristics of the safety culture of patients and personnel in health care facilities in Ukraine as a whole and separately among doctors-pathologists are analyzed with correlation of the data obtained with similar indicators of the culture of patient safety in medical facilities of other countries and comparison with the safety culture of workers of domestic nuclear power plants. It was confirmed that the weaknesses of the safety culture of the personnel of domestic hospitals is characterized by "Reaction to mistakes", which indicates the prevalence of the culture of blame (unfair culture) in domestic hospitals and, as a result, the absence of real data on medical errors and other incidents of patient safety. The high percentage of positive responses to the safety culture characteristic “Response to mistakes” among the workers of Ukrainian nuclear power plants is an example of the possibility of forming an appropriate safety culture in a separate domestic industry, and the high percentage of positive answers by this characteristic in domestic pathologists is a significant potential for the development of a safe hospital environment for patients. in Ukraine.

This article is devoted to V.V. Trofimov, Minister of Health of the RSFSR, an outstanding health care manager, who tried to introduce the modern methods of self-financing, automated information systems, etc., into the management of medical science during the period 1962-1983. At the initiative of V.V. Trofimov, for the first time in the country, the Main Department of research institutes and coordination of scientific research was established, the purpose of which was to ensure a scientific breakthrough in medicine through the development and implementation of innovative methods. The experience of joint work of health authorities, medical universities, and research institutions began to accumulate. It allowed solving problems of improving the quality of medical care according to united comprehensive plans. The material and technical base for public health protection was developed. During the tenth five-year plan alone, 20 large multi-specialty hospitals were built for 600-1200 beds, 88 polyclinics for 750-1200 visits per shift, 80 maternity hospitals and departments for 8849 beds were put into operation, 24 women’s consultations for 1000 visits per shift, and 13 multi-specialty children’s hospitals. The availability of medical personnel increased from 34.8 in 1975 to 40.0 in 1980 (per 10,000 population). The reform of the financing of medical institutions included the transfer of hospitals and polyclinics to economic accounting. The reform was introduced to enhance medical workers’ financial incentives to improve medical care, as savings increased doctors’ and nurses’ salaries. In essence, it was an anti-pod to the extensive, expensive way of conducting the national economy adopted in the country. As the first Editor-in-Chief of the journal “Health Care of the Russian Federation” and he prioritized innovative research in editorial policy for many years to come.

Today requires an important focus on providing psychological comfort both for health care professionals in preventing emotional burnout syndrome and inpatient comfort for patients in these settings. The purpose of the work is to carry out a comparative analysis of European experience on the principles of functional and aesthetic organization of interiors and decoration materials of premises of psychiatric health care institutions, to determine the characteristics of the influence of natural and artificial lighting on the psychophysiological state of the body of patients and medical professionals. The material of the study was Form № 18 of the Ministry of Health of Ukraine "Report on work on control of environmental factors affecting the state of health of the population" of the State Institution "Vinnytsia Regional Laboratory Center of the Ministry of Health of Ukraine" for the period 2016-2019. Content analysis of domestic and foreign scientific sources, as well as bibliosemantic, theoretical and analytical methods of research were used in the work. The statistical processing of the study results was performed in a licensed standardized package "Statistica 6.1 for Windows". As a result of studying the experience of European countries on the creation of in- hospital comfort for patients and medical staff of psychiatric health care institutions, the following has been established: taking into account the sanitary and hygienic requirements in the design and arrangement of facilities for mentally ill persons should correspond to the therapeutic environment and promote the establishment of security regimen, improving treatment and prevention work, preventing hospital-acquired infections and therefore being one and the same priorities reform of mental health in Ukraine. According to the results of studying foreign experience on the basis of a systematic and ergo design approach to the re-profiling and reconstruction of existing psychoneurological hospitals in Ukraine, it is determined that its main purpose is to provide comfortable conditions for patients stay and create conditions for conducting psychosocial therapy and rehabilitation of persons with mental disorders. Therefore, the adaptation of hygiene requirements during the design and arrangement of the premises of psychiatric health care facilities should be aimed at ensuring the optimal individual and psychological status of patients and the appropriate parameters of the hospital environment and its sanitary and hygienic characteristics, which must correspond to the latest technology in providing high quality medical care using a biopsychosocial approach that will facilitate the transition to European standards.


The special attention to the problems of economic security of health care institutions is paid in the paper. The essence of the category «health care institution» and its classification are considered. Such problems of the economic security of health care institutions were identified as insufficient level of budget financing, loss of scientific and qualified personnel, decline of use of innovative products, increasing dependence on imported medicines, increasing circulation of counterfeit medicines. The direct threat to the security of health care institutions under the influence of an open reorganization of state and municipal health care institutions in state and communal non-profit enterprises is determined. The main statistical trends of budget financing of health care institutions in Ukraine are studied due to the volume of expenditures on the branch. The reasons for the low level of financing are determined; the mathematical model of the dependence of the nominal gross domestic product and expenditures on health protection is formed. The extremely low level of health care expenditures in Ukraine is noted in comparison with other countries. The negative dynamics of availability of doctors and medical personnel in the country was revealed. The using of outdated fixed assets and equipment in health care institutions and the Ministry of Health’s inability to provide them with new equipment are determined in the paper. The pharmaceutical market in the state is analyzed and the growing dependence of health care institutions on imported medicines is established. The content of falsification of medicinal products due to the possible absence of active substance in preparations, unknown technologies of obtaining medicinal products and substituting medicinal substances for cheaper analogues are disclosed. The sources of receipt of counterfeit medicines to health care institutions have been established. The authors use such research methods as analysis and synthesis, abstraction and generalization, comparison, statistical method, method of mathematical modeling.

Introduction. The main goal of the state policy in the field of healthcare is to form a system that ensures the availability of medical care and increases the efficiency of medical services. The volume, type and quality of medical care must correspond to the level of morbidity, the needs of the population and the advanced achievements of medical science. Primary health care is the backbone of the health care delivery system. The problem of it improvement, including in rural areas, is a priority and has national significance. The development of measures to increase the availability of primary health care according to the territorial principle is an urgent scientific and practical problem.The purpose of the study — based on the analysis, methodologically substantiate and develop measures to increase the availability and improve the system of organizing the provision of primary health re to the rural population on a territorial basis.Materials and methods. Mathematical-statistical and analytical (quantitative, structural, comparative) research methods were used.Results and Discussion. In the Chelyabinsk Region, there are disproportions in the provision of the population with medical personnel: the provision of the urban population with doctors and l personnel is significantly higher than that of the rural population. The indicator of staffing with paramedical personnel in full-time positions of feldsher-obstetric stations is 20.1% lower than in the Russian Federation. The index of "combination rate" among doctors working in outpatient departments is 1.2 times higher than the target indicator of the National Health Project, and in rural areas it is 1.1 times. In order to unify the organizational and methodological approaches, the authors proposed a classification of medical stations and their organizational and functional structure.Conclusions. The organization of the provision of primary health care to the rural population according to the territorial-district principle with the formation of an organizational model of the rural medical district will increase the availability of primary medical health care to the rural population.

The need for improved communication about health-related topics is evident in statistics about the health literacy of adults living in the United States. The negative impact of poor health communication is huge, resulting in poor health outcomes, health disparities, and high health care costs. The importance of good health communication is relevant to all patient populations, including those from culturally and linguistically diverse backgrounds. Efforts are underway at all levels, from individual professionals to the federal government, to improve the information patients receive so that they can make appropriate health care decisions. This article describes these efforts and discusses how speech-language pathologists and audiologists may be impacted.
