Aim: to analyse the thyroid profile, hs-CRP and lipid profile in newly detected hypothyroid adults in comparison to controls and also to compare the above parameters in subclinical and clinical hypothyroid cases. Material and methods: The study was a cross sectional study which was carried in Nalanda Medical College and Hospital, Patna, Bihar, India from October 2019 to march 2020. Total 240 patients were divided into 2 groups. Group-1 for newly detected hypothyroid adults and Group 2 as Controls. Blood samples were collected with full aseptic precautions after obtaining informed consent. Clot activator that contains vacuum evacuated tubes for analysis of serum TSH, FT3, FT4, TC, HDL-c, LDL-c, TG, hs-CRP. Then after collection, serum samples were stored at -200 until analyzed. Anthropometric measurements for BMI, height (cm) and body weight (kg) were measured. Results: The mean age of cases and controls in our study was found to be 36.12±12.21years and 35.87±11.06years respectively (p = 0.81). BMI values in the study were higher in cases (27.24 ± 4.65kg/m2) compared to controls (25.17 ± 4.37kg/m2) and was statistically significant (P = 0.03). In the study, the mean TSH levels (15.27 ± 9.2µIU/ml) of cases were high compared to controls (3.1 ± 0.88µIU/ml) and were statistically significant (p<0.001). The mean serum hs-CRP levels in both the study groups was within the reference range, but it was high and statistically significant in cases than in control (p = 0.004). The total cholesterol level in cases (182.29 ± 39.75mg/dl) and control (184.27±28.37mg/dl) were within the reference range and there was no statistical significance (p = 0.82). Further it was found that HDL-c in cases (45.89±9.47mg/dl) and control (52.87±6.7mg/dl) were found to be lower in cases compared to controls and the difference was statistically significant (p < 0.001). The mean LDLc value in cases (145.14±34.12mg/dl) and control (132.05±32.14mg/dl) was high in cases and the difference was statistically significant (p= 0.01). The triglyceride levels of cases (159.26±49.87mg/dl) were significantly higher than that of control (146.23±29.27mg/dl) and was statistically significant (p=0.03). hs -CRP levels were in within reference range for 92 (76.67%) of cases and 109(90.83%)controls whereas above the normal range was seen in 28 (23.33%) cases and only 11(9.17%) controls. Out of 120, 64.17% (n=77) were subclinical hypothyroid (SCH) and 35.83% (n=43) were clinical hypothyroid (CH) cases. There was a significant increase in serum TSH in CH (24.11 ± 9.1µIU/ml) as compared to SCH (10.2 ± 2.2 µIU/ml). The difference was statistically significant (p<0.001). Conclusion: we concluded that the hypertriglyceridemia and at risk hs-CRP levels though seen in hypothyroid cases were more prominent in CH cases than SCH. Dyslipidemia and inflammatory markers were found to be increased in the cases that helped in prediction and evaluation of patients at risk of cardiovascular disease.

BACKGROUND:The aim of this study was to evaluate the information quality of YouTube videos on hallux valgus. METHODS:A YouTube search was performed using the keyword 'hallux valgus' to determine the first 300 videos related to hallux valgus. A total of 54 videos met our inclusion criteria and evaluated for information quality by using DISCERN, Journal of the American Medical Association (JAMA) and hallux valgus information assessment (HAVIA) scores. Number of views, time since the upload date, view rate, number of comments, number of likes, number of dislikes, video power index (VPI) values were calculated to determine video popularity. Video length (sec), video source and video content were also noted. The relation between information quality and these factors were statistically evaluated. RESULTS:The mean DISCERN score was 30.35{plus minus}11.56 (poor quality) (14-64), the mean JAMA score was 2.28{plus minus}0.96 (1-4), and the mean HAVIA score was 3.63{plus minus}2.42 (moderate quality) (0.5-8.5). Although videos uploaded by physicians had higher mean DISCERN, JAMA, and HAVIA scores than videos uploaded by non-physicians, the difference was not statistically significant. Additionally, view rates and VPI values were higher for videos uploaded by health channels, but the difference did not reach statistical significance. A statistically significant positive correlation was found between video length and DISCERN (r= 0.294, p= 0.028), and HAVIA scores (r= 0.326, p= 0.015). CONCLUSIONS:This present study demonstrated that the quality of information available on YouTube videos about hallux valgus was low and insufficient. Videos containing accurate information from reliable sources are needed to educate patients on hallux valgus, especially in less frequently mentioned topics such as postoperative complications and healing period.

Deep digital flexor (DDF) tenotomy is a technique employed for years to treat selected disorders of the musculoskeletal system in horses. Although two different surgical approaches (i.e. mid-metacarpal and pastern) have been described for performing the procedure, in vitro quantitative data regarding the modifications induced by either technique on the distal articular angles is lacking. Therefore, the purpose of the study reported here was to investigate the viability of a proposed biomechanical system of induced-traction used to compare the two DDF tenotomy techniques by measuring the distal articular angles of equine cadaver forelimbs. Ten pairs of forelimbs were collected and mounted to a biomechanical system developed to apply traction at the toe level. Dorsal articular angles of the metacarpophalangeal (MP), proximal interphalangeal (PIP) and distal interphalangeal (DIP) joints were determined by geometric lines on radiographs taken before and after performing each technique. Comparisons between each tenotomy group and its own control, for each joint, and between the two tenotomy groups using as variable the difference between the tenotomy and control groups were tested. Despite the lack of statistical significance, the DDF tenotomy technique at the pastern level produced extension, to a lesser and greater extent, of the PIP and DIP joints, respectively when compared to the mid-metacarpal level. No remarkable differences could be observed for the MP joint. The developed traction-induced biomechanical construct seemed to be effective in producing valuable quantitative estimations of the distal articular angles of equine cadaver forelimbs subjected to different DDF tenotomy techniques.

The present study is aimed to study and analyze the true and pseudo Cholinesterase levels in the subjects exposed to pesticides during short term by intentional or accidental intake and long term exposure due to their occupation were studied at Department of Biochemistry, SVS medical college and hospital mahbubnagar district. Whole blood cholinesterase levels and Pseudo cholinesterase levels were estimated. 150 people were taken as controls who had no medical illness and a total number of 300 cases of pesticide poisoning were selected, aged between 20 to 55 years, which consist of 150 acute poisoning and 150 chronic poisoning subjects in Mahbubnagar district, were taken as case study, the detailed case history and the type of organophosphorus pesticide taken were recorded. Mean and standard deviation (S.D) of all variables were calculated and compared with those of controls. Statistical significance was assessed and P-value <0.05 were considered significant.           During acute poisoning the mean value of Whole blood cholinesterase/True cholinesterase (U / L) in acute poisoning cases on first day was 1.267± 0.612 on 3th day was 1.651±0.647, on 7th day was 2.221±0.684 and at the end of 6 months was 3.970±0.404.The difference between the study group and control group (4.0 ± 0.39) was found to be statistically significant in1st, 3rd, 7th day but not significant at the end of 6 months. The mean value of serum cholinesterase/pseudo cholinesterase (U/L) in acute poisoning cases on first day was 2213.05 ± 1749.81, on 3th day was 2862.3 ± 2025.6, on 7th day was 4008.4 ± 2355.9 and at the end of 6 months was 7708.34 ± 880.72.The difference between the study group and control group (7991.97 ± 1276.5) was found to be statistically significant in1st, 3rd, 7th day but not significant at the end of 6 months. During chronic poisoning (exposure) the mean value of Whole blood cholinesterase (U/L) in controls is 4.0 ± 0.39 as compared to 3.019 ± 0.848 in cases of chronic poisoning. The difference between the study group and control group was found to be statistically significant. The mean value of serum cholinesterase/pseudo cholinesterase (U/L) in controls was 7991.97 ± 1276.5 as compared to 6214 ± 1189 in cases of chronic poisoning. The difference between the study group and control group was found to be statistically significant.

This research attempted to explore the effects of divergent and convergent tasks on the successful reading of EFL students at the preparatory stage. To verify the analysis, the null hypothesis was established that states "There are no differences of statistical significance at the level of (0,05) among the mean of scores of the experimental group who study according to divergent and convergent task technique and the mean of the scores of the control group who study according to the regular method".  The experimental approach is used by designing two equivalent experimental groups of 32 students studying the technique proposed, and an 8-week (2019-2020) control group of 32 students, three classes each week using the Google Classroom Platform and Telegram. The present study utilized the platform Google Classroom (GC) and Telegram as an educational platform to assist students during their course learning process. The writing skills test was administered after checking with the experts. The results show that there are statistical differences at level (0,05) between the average of the experimental and control groups' reading skills and those of the experimental group. This difference is because the experimental group uses divergent and convergent tasks.

Abstract AimTo investigate the effect of orthokeratology (OK) lens on axial length (AL) elongation in myopia with anisometropia children.MethodsThirty-seven unilateral myopia (group 1) and fifty-nine bilateral myopia with anisometropia children were involved in this 1-year retrospective study. And bilateral myopia with anisometropia children were divided into group 2A (diopter of the lower SER eye under − 2.00D) and group 2B(diopter of the lower SER eye is equal or greater than − 2.00D). The change in AL were observed.The datas were analysed using SPSS 21.0.Results(1) In group 1, the mean baseline AL of the H eyes and L eye were 24.70 ± 0.89 mm and 23.55 ± 0.69 mm, respectively. In group 2A, the mean baseline AL of the H eyes and L eyes were 24.61 ± 0.84 mm and 24.00 ± 0.70 mm respectively. In group 2B, the mean baseline AL of the H eyes and L eyes were 25.28 ± 0.72 mm and 24.70 ± 0.74 mm. After 1 year, the change in AL of the L eyes was faster than the H eyes in group 1 and group 2A (all P<0.001).While the AL of the H eyes and L eyes had the same increased rate in group 2B. (2) The effect of controlling AL elongation of H eyes is consistent in three groups (P = 0.559).The effect of controlling AL elongation of L eyes in group 2B was better than that in group 1 and group 2A (P < 0.001). And the difference between group 1 and group 2A has no statistical significance. (3) The AL difference in H eyes and L eyes decreased from baseline 1.16 ± 0.55mm to 0.88 ± 0.68mm after 1 year in group 1.And in group 2A, the AL difference in H eyes and L eyes decreased from baseline 0.61 ± 0.34mm to 0.48 ± 0.28mm. There was statistically significant difference (all P<0.001). In group 2B, the baseline AL difference in H eyes and L eyes has no significant difference from that after 1 year (P = 0.069).ConclusionsMonocular OK lens is effective on suppression AL growth of the myopic eyes and reduce anisometropia value in unilateral myopic children. Binocular OK lenses only reduce anisometropia with the diopter of the low eye under − 2.00D. Binocular OK lenses cannot reduce anisometropia with the diopter of the low eye equal or greater than − 2.00D. Whether OK lens can reduce refractive anisometropia value is related to the spherical equivalent refractive of low refractive eye in bilateral myopia with anisometropia children after 1-year follow-up.

Objective To determine the ototoxic potential of ciprofloxacin/dexamethasone eardrops. Methods Ventilation tubes were inserted in the ears of 15 chinchillas. One ear was randomized to receive four drops of ciprofloxacin/dexamethasone. A control solution (0.45% NaCl) was applied in the contralateral ear. The eardrops were delivered twice daily for 7 consecutive days. Auditory brain stem response (ABR) measurements were recorded at baseline (post-tympanostomy tube insertion), and on days 4, 8, 15, 22, and 60. Results An initial transient increase of 8.11 dB nHL was noted in the mean thresholds of the experimental ears at day 4. On the last ABR evaluation (day 60), the mean thresholds were 16.33 ± 6.93 dB nHL in the experimental ears and 13.66 ± 6.65 dB nHL in the control ears. The difference in the mean thresholds between the experimental and control ears at day 60 was not significant (95% confidence interval, 3.12, –8.02). Scanning electron microscopy demonstrated normal morphology in all experimental ears that were assessed. Conclusion Ciprofloxacin/dexamethasone eardrops did not appear ototoxic when used in chinchillas post-ventilation tube insertion.

Objectives: The aim of this study was to explore the cytomorphometric and morphological effects of Trichomonas vaginalis in exfoliated epithelial cells. Study Design: Ninety-six Pap-stained cervical smears were divided into a study group and two control groups as follows: T. vaginalis cases, a first control group with inflammation, and a second control group without inflammation. Micronucleated, binucleated, karyorrhectic, karyolytic, and karyopyknotic cells and cells with perinuclear halos per 1,000 epithelial cells were counted. Nuclear and cellular areas were evaluated in 70 clearly defined cells in each smear using image analysis. Results: The frequencies of morphological parameters in the T. vaginalis cases were higher than the values of the two control groups, and the difference among groups was found to be significant (p < 0.05). The nuclear and cytoplasmic areas of epithelial cells were diminished in patients with trichomoniasis. The mean nucleus/cytoplasm ratio in T. vaginalis patients was higher than the value in the control groups, and the difference between the study group and control group 1 was significant. However, there was no statistically significant increase between the study group and control group 2. Conclusions:T. vaginalis exhibited significant changes in the cellular size and nuclear structure of the cells. The rising frequency of micronuclei, nuclear abnormalities, and changing nucleus/cytoplasm ratio may reflect genotoxic damage in trichomoniasis.

ABSTRACT The respective mean 24 h urinary excretions of sulphoconjugated 16α-hydroxydehydroepiandrosterone, 16-oxo-androstenediol, and dehydroepiandrosterone were found to be 1040, 230, and 668 μg/24 h for normal males, and 621, 209, and 304 μg/24 h for normal non-pregnant females. None of the differences in mean steroid excretions between males and non-pregnant females was statistically significant according to the usual criterion for significance (P < 0.05). The mean excretions of sulphoconjugated 16α-hydroxydehydroepiandrosterone, 16-oxo-androstenediol and dehydroepiandrosterone between the 10th week of normal pregnancy and term were 1300, 505 and 226 μg/24 h respectively. No significant trend in the excretions of these neutral steroid sulphates between the 10th week of pregnancy and term could be detected. The increased excretion of both C-16 oxygenated steroid sulphates in pregnancy was highly significant (P < 0.001). In contrast, the statistical significance of the difference in excretion of dehydroepiandrosterone sulphate between pregnant and non-pregnant females could not be established. There did not appear to be any relationship between the excretion of 16α-hydroxydehydroepiandrosterone sulphate and oestriol in normal pregnancy. The results are considered to be consistent with a limited transplacental passage of neutral steroids from the foetoplacental to maternal circulation.

BACKGROUND & OBJECTIVE: Kisspeptin (KP) and spexin (SPX) are neuropeptides that play a substantial role in metabolism and sexual function. Our objective is to explore the association of spexin and kisspeptin with sperm parameters and lipid profile in the infertile male population. METHODOLOGY: The cross-sectional study was conducted from March 2017 till July 2018 in Aga Khan University Hospital, Karachi, Pakistan, after acquiring ethical approval (4813-BBS-ERC-17). Convenient sampling was applied to recruit 44 normozoospermic subjects as controls and 44 patients with abnormal sperm parameters”. Serum levels of kisspeptin and spexin were measured by enzyme linked immunosorbent assay and Cobas c111 analyzer was used for estimation of high and low density-lipoprotein, triglyceride and serum cholesterol levels. Mann-Whitney U tests and spearman’s rank correlation test were used to analyze quantitative variables, and data was presented by means, standard error (SE) and correlations coefficient, with a statistical significance set at p≤0.05. RESULTS: The mean kisspeptin values of ‘altered sperm parameters’ males 13.23 ± 3.34 were lower than normal sperm parameters 31.89 ± 3.25 (p<0.001). The mean spexin values of altered sperm parameters 383.57 ± 21.09 were less than compared to normal sperm parameters 502.76 ± 19.77 (p<0.001). Correlation of KP and SPX levels was (r-value = 0.332, p= 0.002). There was no significant correlation was found between KP and SPX with lipid profile parameters. CONCLUSION: Spexin and kisspeptin levels observed were higher in male subjects with normal sperm parameters and had a significant positive correlation with each other. There was no impact of spexin on lipid profile and obesity.

The current study determines the effectiveness of using task-based language instructions in teaching writing at a high school in Thai Nguyen province. The participants consist of 92 grade 10 students from Bing Yen high school, Dinh Hoa district. The study employs a mix method research design (MMR) as the plan for the research. The results of the study reveals that the results of writing performance were reported at a low level (M=3.72). The results also confirmed that the intervention worked well for the experimental group. The mean score of the pre-test (the baseline for both groups) was M=3.72. The intervention focused on TBLT instructions. After first five weeks, the means of post-test1 were M=4.96 and M=3.83 for experimental group and control group respectively. There was a significant difference between the mean of pre-test and post-test1 for the experimental group (M=3.72 compared to M=4.96), while the control group showed a little improvement (M=3.72 compared to M=3.83) the difference was not significant. After another five weeks, the difference in the mean scores of the experimental group were far more than the mean scores of the control group, M=5.56 and M=4.45 respectively. By the end of the fifteenth week of the intervention, the mean of post-test 3 of the experimental group reached M=6.41, the mean for control group was M=5.54. The difference of the mean scores in the pre-test and post-test revealed a significant improvement in both groups. However, the experimental group showed a greater improvement, M=6.41 and M=5.54 respectively. It can infer that the uses of TBLT writing instruction improved writing achievement greatly.
