Background: Hypertension is an important public-health challenge worldwide. It is important because of its high frequency and its effect on cerebrovascular, cardiovascular and kidney disease.Methods: This cross-sectional descriptive study was conducted to assess the treatment compliance of hypertensive patients and medical history of patients with hypertension in the outpatient department of Cardiology, National Institute of Cardiovascular Diseases (NICVD), Dhaka. Considering inclusion and exclusion criteria, 350 hypertensive outdoor patients were included in this study. Data was analyzed by SPSS version 20.Results: The study revealed that the mean age of the respondents was 50.49 years (SD ± 8.449). Approximately 67.2% of the respondents were suffering from hypertension from <5 years and 15.6% respondents were hospitalized three times and 12.0% respondents were hospitalized more than three times. The mean duration of taking antihypertensive medicine was 4.91 years. The study also found that 48.8% respondents were taking antihypertensive medication irregularly. The mean systolic blood pressure was 129.96 mm of Hg (SD±23.60) and diastolic blood pressure was 86.08 mm of Hg (SD ±12.76).Conclusion: The study results revealed that about half of the patients do not take drugs regularly, which is an alarming sign. Therefore, the instructions and counseling should be given to the patient to fulfill the treatment for prevention of complications related to hypertension.Cardiovasc. j. 2016; 8(2): 110-114

Background: Risk factors can lead to clinical conditions, like metabolic syndrome, that predisposes the development of cardiovascular diseases. Objective: The goal of this population-based, prospective and non-randomised cohort study was to study the association between patients with metabolic syndrome and other various factors defining metabolic syndrome. Methods: All the patients referred to the department of Medicine, Al-Ameen Medical college hospital and District Hospital, Vijayapur, Karnataka, India over a period of twenty-two months extending from December 2013 to September 2015 were considered in this study. Results: In the current study, out of 100 patients, 62.9% patients had metabolic syndrome with positive family history of hypertension, diabetes mellitus. 70.8% patients had metabolic syndrome with positive history of smoking. 64.3% patients had metabolic syndrome with positive history of alcohol .73.9% patients had metabolic syndrome with positive history of IHD. 87% of the patients with metabolic syndrome had SBP> 130 mmHg, and 78.85% patients had DBP>85 mmHg. And metabolic syndrome was observed in 71.8% patients on anti hypertensive drugs. The mean level of total cholesterol, LDL cholesterol, triglyceride is increased whereas the mean level of anti-atherogenic HDL cholesterol is low in subjects with MS. At least one lipid abnormality was present in > 95 % of cases. Around 81% subjects with BMI <25 (out of 38) had metabolic syndrome and 58% subjects with BMI>25(out of 62) had metabolic syndrome. Conclusion: All the components defining the metabolic syndrome correlated positively with the abdominal obesity. Systolic blood pressure values were significantly higher than diastolic blood pressure in subjects with abdominal obesity. Metabolic syndrome has multiple risk factors determined by various aspects like the race, the life style, geographical factors larger study is needed to understand the correlation between various components defining it. A healthy lifestyle, that includes avoiding tobacco exposure and proper weight control, must be encouraged in this high-risk population. Background: Tramadol is a widely prescribed analgesic and due to its opioid-like effects, the potential for abuse of tramadol is noticeable. Besides, the complications of tramadol abuse have become a public health concern. This study aimed to investigate the affecting factors on the seizure, as one of the most common complications of tramadol consumption. Methods: A total number of 64 patients from 315 patients who were referred to Sina Hospital, Tabriz, Iran because of tramadol toxicity were included in this 9 months cross-sectional retrospective study. Results: There were 52 males and 12 females in the study. The seizure happened in 53.1% of the subjects and the Mean±SD time between tramadol consumption and seizure was 5.9±7.36 hours. There was no significant association between seizure and sex, age, the dose of tramadol, and previous tramadol consumption history. A significant association was seen between the dose of tramadol and the time of seizure. Conclusion: Seizure that happens due to tramadol overdose is not dependent on sex, age, and previous history of tramadol consumption. As the dose of tramadol is higher, the seizure happens later. More research is needed to understand why the seizure occurs later in higher doses.

Objective: The aim of the present study was to investigate the epidemiological pattern of factors associated with ischemic stroke among patients under 50 years old. Methods: The present cross-sectional study was performed on 197 Stroke patients. Individuals with confirmed ischemic stroke based on of CT scan were included in the study. Demographic information included age, sex, history of smoking, place of residence, season of disease incidence, history of contraceptive use in women, history of hypertension, having high blood pressure at the onset of diabetes, hyperlipidemia, heart disease and patient weight. Finally, the collected data was analyzed using SPSS ver. 22. Results: The mean age of participants was 40.18 years. The mean weight, height, body mass index (BMI) were 79.83, 168.63, and 28.12, respectively. A total of 60.4% of the participants were male and 39.6% were female. Moreover, 66.9% were urban residents, 38.6% had complications in winter, 67% were smokers, and 50% used contraceptives. The history of hypertension, diabetes, hyperlipidemia, and heart disease was seen in 69%, 49.7, 66%, and 73.6% of cases, respectively. Also, 53.8% of patients had high blood pressure upon admission. Conclusion: The present study showed that a history of heart disease and a history of hypertension in the past and nicotine use are very common in people with stroke. Considering the above mentioned epidemiological factors, it is recommended to prioritize the mentioned factors when identifying new cases

Objective: The study aimed to compare state and trait anxiety, blood pressure, and heart rate of patients with and without coronary angiography experience who undergo coronary angiography intervention. Methodology: This study is cross-sectional comparative research. A total of 160 patients, including 80 patients undergo for the first time coronary angiography and 80 patients with at least one experience of coronary angiography were included in the sample of the study. In the study, data were collected using 'Patient Information Form' and 'State-Trait Anxiety Inventory', blood pressure measurement device, and pulse oximeter. Results: The mean of state anxiety scores of the patients with and without CA experience were 39.35±5.31 and 39.98±4.04, respectively; (p=0.395). The mean of trait anxiety scores of the patients with and without CA experience were 44.73±6.84 and 44.51±6.05, respectively (p=0.826).  There was no statistically significant difference observed in state and trait anxiety between two groups. Before the CA procedure, the systolic and diastolic blood pressure average of the patients with CA experience were statistically higher than those without CA experience (p<0.05). Conclusion: The anxiety levels of the patients before the CA were observed to be similar and moderate, regardless of their CA experience. It was observed that patients with CA experience before CA procedure had higher systolic and diastolic blood pressures compared to the group without experience.

Despite Hypertension is a global public health challenge and a leading modifiable risk factor for cardiovascular disease and death attention was not given in developing countries. Therefore measuring the prevalence and identifying predictors of Hypertension is very important. Institution based cross sectional study design was employed from March–April, 2016 by taking 319 randomly selected civil servants working in in Arba Minch town. Data was collected using structured questionnaire and standardized instruments for physical examination by 5 trained nurses. SPSS version 20 was used for data analysis. Bi-variable and Multivariate logistic regression was employed for analysis of risk factors. The mean SBP and DBP of study participants were 120.87 + 14.15 mmHg and 80.28 + 8.8 mmHg, respectively. The prevalence of hypertension was found to be 27.8% (95% CI = 22.9-32.7%). Civil servants of age 50 years and above [AOR = 13.3], age 40-49 years [AOR = 5], age 30-39 years [AOR = 3.5], abdominal obesity [AOR=12.2], general obesity [AOR = 4.2], stress status [AOR = 12.3], current alcohol drink [AOR = 3.3], ex-drinker [AOR = 8.9] and family history of hypertension [AOR = 5.6] were found to be significantly associated with hypertension. The prevalence indicates that it is hidden epidemic in this population; therefore for screening and risk reduction program are needed.

Background: Hypertension is estimated to affect about 40% of adults above 25 years of age globally. While many of the risk factors of hypertension are well known, such is not the case with smoking. Pertinent published literature is heterogeneous, with a dearth of relevant local data. Objectives was to study the relationship between smoking and blood pressure levels in hypertensive patients and to assess the effects of gender, age and hypertension duration on such a relationship.Methods: A cross-sectional study was carried out on a total of 298 conveniently sampled patients, aged 18 or above, from the medical outpatient department of a secondary care hospital of Karachi. The relevant data were collected by means of a structured questionnaire whereas the blood pressure level was checked using sphygmomanometer with stethoscope. Mann Whitney U test was used for inferential analysis whereas the significance level was set at 0.05.Results: Overall, both the systolic and diastolic blood pressure levels were significantly associated with smoking history (p<0.05 for both) where they were found to be higher in smokers than in non-smokers. Furthermore, after gender, age and hypertension duration based stratifications; both the systolic and diastolic blood pressure levels were still significantly associated with smoking history in patients who were male, were ≥35 years old and had <5 years hypertension duration (p<0.05 for both) where they were again found to be higher in smokers than in non-smokers.Conclusions: The study results showed a significant association of positive smoking history with higher mean systolic and diastolic blood pressure levels, though after stratifying for gender, age and hypertension duration, this relationship persisted only in patients who were male, were ≥35 years old and had <5 years hypertension duration.

Objective: Intermolar width is a key measurement which assists in treatment planning of orthodontic patients requiring expansion as an alternate to premolar extraction. The present research was aimed at determining the mean value of intermolar arch width (IMW) of untreated normal arch Pakistani patients visiting tertiary care dental hospital Material & Methods: This cross sectional study was carried out using IMW measurements on plaster model of 120 untreated normal occlusion patients, at Department of Orthodontics, Faisalabad Medical University and de’Montmorency College of dentistry, from 15-12-2016 to 15-10-2017. The non probability consecutive sampling technique was used in this study. Data analysis was done using SPSS software 21.0.0. Results: The mean age of the subjects was 18.23±3.75 years. The mean value of IMW in selected subjects was 45.33±3.42 mm. Conclusion: Study results concluded that in Pakistanis, ideally align maxillary arch and occlusion can be achieved with upper intermolar distances of 45.33±3.42 mm

Abstrak Puskesmas merupakan Fasilitas Kesehatan Tingkat Pertama yang berfungsi menjaga kesinambungan pelayanan kesehatan secara paripurna. Hingga saat ini penyakit tidak menular yang masih merupakan tantangan besar di Puskesmas dalam menurunkan angka morbiditas hipertensi adalah adalah tatalaksana hipertensi salah satunya adalah tingkat kepatuhan mengkonsumsi obat antihipertensi bagi penderita masih sangat rendah. Penelitian ini bertujuan untuk menganalisis determinan kepatuhan pasien menjalani pengobatan hipertensi dan pengaruhnya terhadap kendali tekanan darah pasien hipertensi. Metode penelitian observasional analitik dengan pendekatan cross sectional. Penelitian dilakukan di kota Palembang pada bulan Agustus sampai Oktober 2017. Sampel adalah pasien yang berobat ke Puskesmas Karyajaya, Kenten, Plaju, Sei Baung, dan Kampus. Sampel yang memenuhi kriteria inklusi sebanyak 90 pasien. Analisis data dengan analisis univariat dan bivariat dengan Chi Square Test. Determinan yang berpengaruh terhadap kepatuhan pasien hipertensi dalam mengkonsumsi obat adalah lama menderita hipertensi: POR: 0,11, nilai p =0,04 (CI 95% 0,02-0,52).Determinan jenis kelamin, usia, tingkat pendidikan, jenis pekerjaan, Indeks Massa Tubuh, keikutansertaan BPJS, dan riwayat hipertensi dalam keluarga tidak berpengaruh secara signifikan terhadap kepatuhan berobat pasien hipertensi. Kepatuhan pasien dalam menjalani pengobatan hipertensi juga merupakan determinan yang berpengaruh terhadap kendali tekanan darah responden. Keberhasilan terapi hipertensi di tingkat pelayanan pertama bisa meningkat, maka disarankan bagi tenaga kesehatan memperhatikan lama atau riwayat menderita hipertensi pasien dalam memberikan pelayanan. Abstract Puskesmas is a First Level Health Facility Until now, the management of non-communicable diseases which is still a big challenge in Puskesmas is hypertension. One effort that can be done to reduce the morbidity rate of hypertension is the consumption of antihypertensive, but still few patients who adhere to this treatment.This study aims to analyze the determinants of adherence in patients undergoing treatment of hypertension and how it affects blood pressure control of hypertensive patients. This research is an observational analytic research with cross sectional approach. This research was conducted in Palembang city from August to October 2017. The samples were patients who went to Puskesmas Karyajaya, Kenten, Plaju, Sei Baung, and Campus which fulfilled the inclusion criteria with a total sample of 90 patients. Data analysis in this research was univariate and bivariate analysis with Chi Square Test. Determinant which influence to adherence in this research was duration of hypertension with POR 0,11 with CI 95% 0,02-0,52 (p value = 0,04). While other determinants of gender, age, education level, occupation, Body Mass Index, BPJS participation, and family history of hypertension have no significant effect on hypertension patient’s treatment compliance. Compliance of patients in undergoing treatment of hypertension is also a determinant that affects the control of blood pressure of respondents. The determinants that affect the adherence in this study are duration of hypertension, thus health workers at first-level facility is very necessary attention to a history of hypertension patients in order to improve therapeutic efficacy.
