Calcium imbalance in comorbid gastroduodenal ulcers

Purpose of the study. To study the level of blood calcium, reflecting the functional state of the calcium-regulating system (CRS), in the comorbid course of gastroduodenal ulcers (GDU) with chronic erosive gastritis / chronic erosive duodenitis (CEG/CED), arterial hypertension (AH), their symptomatic nature when taking non-steroidal drugs (NSAIDs) and find out its effect on the activity of the ulcerous process, the state of regional microcirculation and the secretory function of the stomach.Materials and methods. 132 patients with GDU were examined. All patients were divided into groups: the first (39 people) – patients with recurrent peptic ulcer (PU) and CEG/CED, the second – 23 people with recurrent peptic ulcer and hypertension, the third – 20 patients with symptomatic gastroduodenal ulcers (SGDU) when taking NSAIDs. The fourth (control) group included 56 patients with PU without associated pathology.Results and discussion. Recurrence of PUr, comorbid to its course with CEG/CED, AH, SGDU, when taking NSAIDs, occurs with an increase in the level of calcium in the blood, which contributes to the activation of the acid-peptic factor, impaired microcirculation and repair processes in the mucous of the gastroduodenal zone, the development and maintaining the ulcerous process.Conclusion. Gastroduodenal ulcers are accompanied by dysfunction of the calcium regulatory system, which is characterized by an increase in blood calcium, which supports the formation of the main ulcerous mechanisms. In the treatment of comorbid and symptomatic gastroduodenal ulcers, it is necessary to include drugs for correcting the calcium-regulating system, which will increase the activity of sanogenic mechanisms.

Improvement of methods of prevention and treatment led to an increase in the number of patients with comorbid occurring chronic disease that requires effective integrated therapy to reduce the polypharmacy. Aim. Find out the calcium content of the blood reflecting its balance and functional status the calcium regulatory system when it is comorbid for gastroduodenal ulcers (GDU), developed on the background of chronic erosive gastritis (CEG), chronic erosive duodenitis (CED), arterial hypertension (AH) and osteo-articular pathology with the use of nonsteroidal anti-inflammatory drugs (NSAIDs), and the impact of his changes on the activity ulzerogennogo process, the state of regional microcirculation and the functions of the stomach. To determine the pathogenetic justification for and effectiveness of blockers of slow calcium channels (BSCaC) in complex treatment. Materials and methods. Examined 132 patients with GDU. All patients were divided into groups: the 1st (n=49) - patients with recurrent of peptic ulcer disease (PUD) and CEG/CED; the 2nd (n=23) - with recurrence of PUD and AH, the 3rd (n=14) - with GDU and osteoarticular pathology, taking NSAIDs. Patients of these three groups for the treatment of erosive ulcerous lesions of gastroduodenal zone (GDZ) has been appointed complex therapy with inclusion of nifedipine. The 4th (control) group consisted of 56 patients with recurrent BU without concomitant pathology, applying integrated therapy with nifedipine. The results and discussion. The PU relapse, comorbid her over with erosive gastroduodenitis, hypertension, GDU with of osteoarticular pathology and taking NSAIDs is accompanied by a calcium imbalance with increased levels of calcium in the blood, contributing to increase of acid-peptic factor in the formation of hypermotor dyskinesia stomach, disruption of regional microcirculation and repair processes, activation of ulcerogenesis in GDZ. Inclusion in the complex therapy of GDU of nifedipine leads to the recovery of calcium balance, functions of the stomach and regional microcirculation, accelerates the timing and increases the percentage of scarring ulcers. Conclusion. GDU accompanied by dysfunction the calcium regulatory system with increasing levels of blood calcium, contributing to the formation of the major pathogenetic mechanisms of ulcerogenesis. BSCaC application in complex therapy of GDU is pathogenetically justified and clinically effective, reduces the excessive drug treatment in the treatment.

PURPOSE: Evaluation of colonic healing in spontaneously hypertensive rats. METHODS: Fifty male, young and inbred rats were used. Twenty-five Wistar Kyoto rats (WKY) as control and twenty-five spontaneously hypertensive rats (SHR) as an experimental group. Colotomy and bowel suture at 2.5 cm from the peritoneal reflection were performed. All animals were allocated randomly into sub-groups for review at the third, seventh and fourteenth days after surgery. We evaluated the concentration of angiotensin II, the burst pressure, epithelialization, the organization of the tunics of the bowel wall, inflammatory response and collagen deposition. RESULTS: The burst pressure, epithelialization, organization of the tunics and collagen deposition was not significant between groups. The inflammatory reaction was more intense in the control group on the third postoperative day (p=0.023) as the experimental group on the remaining time. CONCLUSION: Systemic arterial hypertension in rats did not influence significantly the healing process of colonic anastomoses.

Relevance. Myocardial infarction and its complications in young and middle-aged men with arterial hypertension remains an important problem of modern cardiology. Aim. To evaluate the cardiovascular risk factors structure features in men under 50 years old with arterial hypertension to improve prevention and outcomes. Material and methods. The study included 209 men aged 19-50 years old with type I myocardial infarction, who underwent a standard diagnostic algorithm in the first 48 hours and at the end of the third week of the disease. The patients were divided into two age-matched groups: with arterial hypertension (121 patients, 88 patients without arterial hypertension). A comparative analysis of the frequency of identifying the main and additional cardiovascular risk factors and their parameters were performed in the selected groups. Results. The study group showed more pronounced glycemia (5.6±1.4 mmol/l) than in the control group (5.2±1.3 mmol/l; p=0.04) at the end of the third week of myocardial infarction. In this group, there was a high incidence of obesity (42.1 and 25.0%, respectively; p=0.01), changes in peripheral arteries (86.0 and 3.5%; p<0.0001) and target organ damage , meteorological dependence (31.4 and 9.1%; p=0.0001), impaired peripheral hemodynamics, as well as hereditary burden of vascular pathology (100 and 56.8%; p=0.03). Conclusions. The data obtained suggest a worse long-term prognosis and a greater degree of resistance to treatment in men with arterial hypertension, which must be considered when implementing preventive programs.

Aim. To clarify blood calcium concentrations (BCCs) as an indicator of the functional state of the calcium-regulating system in the concomitant course of recurrent peptic ulcer disease (PUD) and hypertension, by comparing with the severity of a ulcerous process, with changes in regional microcirculation, and with the functions of the stomach. To elucidate the pathogenetic justification for and clinical efficacy of slow calcium channel blockers (SCCBs) in the treatment of this comorbidity. Subjects and methods. In the case-control study, each patient with recurrent PUD and grade 1, Stage I hypertension (Group 1; n=23) corresponded to a recurrent PUD patient matched for sex, age, and ulcer site (Group 2, n=23). The complex of treatment for these patients included the SCCB nifedipine. A control group consisted of 56 recurrent PUD patients who received combination therapy without nifedipine. All the patients over time underwent clinical and endoscopic examinations and determinations of BCCs, indicators of gastric secretory and motor functions, and regional microcirculation in the gastroduodenal mucosal biopsy specimens. Results. Recurrent PUD was present with a reliable BCC increase that was more substantial when it was associated with hypertension. Calcium imbalance was accompanied by changes in regional microcirculation and gastric secretory and motor functional indicators forming an acid peptic factor, as well as by hypermotor dyskinesia, which were more pronounced in patients with comorbidity. Incorporation of a SCCB into a complex of therapy for recurrent PUD to eliminate the pathogenic effect of blood calcium contributed to more rapid arrest of the clinical symptoms of a recurrence, to elimination of acute-phase microcirculatory disorders in the gastroduodenal zone, and to the recovery of gastric functional indicators. Elevated blood pressure was ruled out during the therapy of concomitant diseases. Conclusion. Incorporation of a SCCB into the combination therapy of recurrent PUD associated with hypertension is pathogenetically sound and clinically effective.

The comorbidity of various diseases requires the use of drugs that affect common etiological and pathogenetic aspects, which reduces the risk of side-effect.The Aim. Find out the state of calcium regulating system in the comorbidity of peptic ulcer disease (PUD) with osteoporosis (OP), and the effect of their shifts on the acid secretion and regional microcirculation.Materials and methods. We examined 10 patients with comorbid relapse PUD with OP and a group of patients corresponding to them by gender, age and location of the ulcer, only with a relapse of PUD. In all patients, except for clinical and endoscopic studies, the levels of parathyrin, calcitonin, calcium and phosphorus in the blood were studied, the secretory function of the stomach, densitometry was performed.Results. Relapse of ulcer was characterized by a increase in parathyroid hormone and calcium, a decrease in blood phosphorus. These changes were more pronounced in patients with a comorbidity of PUD with OP and were accompanied by an increase in acid secretion and regional microcirculation.Conclusion. The comorbidity of PUD with OP is accompanied by functional changes in calcium regulating system, which is one of the common pathogenetic mechanisms of these conditions and requires correction in their treatment.

Turkish dermatologist, H. Beheet described the disease as recurrent triad of iritis, oral aphthous lesions and genital ulceration. Auto immune disease is the recent focus on the unknown etiology which is still being discussed. Among the other immunosupressive drugs, CyA included in it's treatment newly. One of the important side effects of this drug is gingival hyperplasia which has a direct relation with the presence of teeth and periodontal tissue. We are interested in the ultrastructure of immunocompetent target cells that were affected by CyA in BD.Three groups arranged in each having 5 patients with BD. Control group was the first and didn’t have CyA treatment. Patients who had CyA, but didn’t show gingival hyperplasia assembled the second group. The ones displaying gingival hyperplasia following CyA therapy formed the third group. GMC of control group and their granules are shown in FIG. 1,2,3. GMC of the second group presented initiation of supplementary cellular activity and possible maturing functional changes with the signs of increased number of mitochondria and accumulation of numerous dense cored granules next to few normal ones, FIG. 4,5,6.

This study was conducted as an attempt to discover the effect of applying Demonstration Method on students’ achievement in speaking skill. It was an experimental research. The subject was students of Grade XII, Vocational High School (Sekolah Menengah Kejuruan: SMK), which consisted of 79 students. The research was divided into two groups: experimental and control groups. The instrument used to collect the data was speaking test. To obtain the reliability of the test, the writer applied Kuder Richardson 21 formula. The result of the reliability was 0.7, and it was found that the test was reliable. The data were analyzed by using t-test formula. The analysis showed that the scores of the students in the experimental group were significantly higher than the scores of the students in the control group at the level of significant m = 0.05 with the degree of freedom (df) 77, t-observed value 8.9 > t-table value 1.99. The findings indicate that using Demonstration Method significantly affected the students’ achievement in speaking skill. So, English teachers are suggested to use Demonstration Method in order to improve students’ achievement in speaking skill.

<p>Penelitian ini bertujuan untuk melihat pengaruh pemberian ekstrak etanol kulit petai (Parkia speciosa Hassk) terhadap penurunan kadar glukosa darah mencit jantan yang diinduksi aloksan. Hewan percobaan dibagi atas 5 kelompok diantaranya kelompok kontrol negatif, kelompok kontrol positif,dosis I (280 mg/kgBB mencit), dosis II (560 mg/kg BB mencit), dosis III (840 mg/kg BB mencit). Penelitian dilakukan selama 21 hari. Persentase penurunan kadar glukosa darah mencit jantan setelah diberikan ekstrak etanol kulit petai pada hari ke-21 adalah dosis I (77,52 %) lebih besar dibandingkan dengan dosis II (69,5 %) dan dosis III (73,37 %). Data yang diperoleh dianalisis dengan uji Two Way Anova dengan program SPSS 17. Hasil penelitian ini menunjukkan bahwa pemberian ekstrak etanol kulit petai untuk tiga variasi dosis menyatakan perbedaan yang bermakna secara statistik terhadap penurunan kadar glukosa darah mencit jantan.</p><p><em>Petai (Parkia speciosa Hassk) has a compound β-sitosterol and stigmasterol that have efficacy to decreased blood glucose levels. This study aimed to determine the effect of ethanol extract of petai peel for decrease blood glucose levels of male mice induced by alloxan. Experimental animals were divided into 5 groups including negative control group, positive control group, the first dose (280 mg/kg in mice), the second dose (560 mg/kg in mice), the third dose (840 mg/kg in mice). The study was conducted for 21 days. After 21 days, the result found that the percentage of blood glucose levels after the male mice given the ethanol extract of petai peel was, the first dose (77.52%) biger than the second dose (69.5%) and the third dose (73.37%). The data obtained were analyzed by Two Way ANOVA using SPSS 17. The results showed that have signicantly difference between three dose variation of ethanol extract of petai peel in blood glucose levels.</em></p>

AimsTo determine myopia progression in children who continued to wear the defocus incorporated multiple segments (DIMS) lenses or switched from single vision (SV) to DIMS lenses for a 1-year period following a 2-year myopia control trial.Methods128 children participated in this study. The children who had worn DIMS lenses continued to wear DIMS lenses (DIMS group), and children who had worn SV lenses switched to wear DIMS lenses (Control-to-DIMS group). Cycloplegic spherical equivalent refraction (SER) and axial length (AL) were measured at 6-month interval. Historical controls were age matched to the DIMS group at 24 months and used for comparing the third-year changes.ResultsOver 3 years, SER and AL changes in the DIMS group (n=65) were −0.52±0.69D and 0.31±0.26 mm; these changes were not statistically significant over time (repeated measures analysis of variance, p>0.05).SER (−0.04±0. 38D) and AL (0.08±0.12 mm) changes in the Control-to-DIMS group (n=55) in the third year were less compared with the first (mean difference=0.45 ± 0.30D, 0.21±0.11 mm, p<0.001) and second (0.34±0.30D, 0.12±0.10 mm, p<0.001) years.Changes in SER and AL in both groups over that period were significantly less than in the historical control group (DIMS vs historical control: mean difference=−0.18±0.42D, p=0.012; 0.08±0.15 mm, p=0.001; Control-to-DIMS versus historical control: adjusted mean differences=−0.30±0.42D, p<0.001; 0.12±0.16 mm, p<0.001).ConclusionsMyopia control effect was sustained in the third year in children who had used the DIMS spectacles in the previous 2 years and was also shown in the children switching from SV to DIMS lenses.

Abstract Background This study was done to observe the incidence of Osteo-line on the femur neck and to explore the clinical application of Osteo-line in osteotomy. Methods Eighty-nine adult femur specimens were selected to observe the incidence of Osteo-line on the femur neck. From August 2015 to January 2019, a total of 278 patients who completed unilateral hip arthroplasty at the Third Hospital of Hebei Medical University were retrospectively included. Patients who accepted osteotomy via Osteo-line on the femur neck were defined as the experimental group (n = 139), and patients who accepted osteotomy via traditional method (The femoral distance 1.5 cm above the trochanter was retained for osteotomy by visual inspection.) were defined as the control group (n = 139). According to the postoperative pelvic X-ray, Photoshop was used to evaluate the leg length discrepancy (LLD) by the CFR-T-LT method. Results Among the 89 specimens, the incidence of anterior Osteo-line was 75.28%, and the incidence of posterior Osteo-line was 100%. According to the clinical application results, the incidence of anterior Osteo-line on the femur neck was 80%, and the incidence of posterior Osteo-line was 100%. The Osteo-line was clearer than those on the femoral specimens. Twenty-six cases had LLD greater than 1 cm (9.29%), including 2 cases in the experimental group and 24 cases in the control group. The average postoperative LLD in the experimental group (0.19 ± 0.38 mm) was significantly shorter than in the control group (0.54 ± 0.51 mm)(P = 0.005). Conclusion The incidence of Osteo-line on the femur neck was high, and patients who accepted osteotomy via Osteo-line on the femur neck can achieve shorter postoperative LLD than the control group.
