The bronchodilator test in patients with airway obstruction: the responsiveness of lung function parameters

Interpretation of bronchodilator (BD) test based on reaction of forced expiratory in one second (FEV 1). For assessing bronchodilator responsiveness of lung volumes, airway resistance remains largely unexplored. Therefore, we assessed the response of pulmonary function parameters to BD to reveal the most responsive parameter. 90 patients with chronic airway obstruction (61 male and 29 female; aged 55±11; post-  BD FEV 1 was 63.1+18.3 % predicted) performed spirometry and static lung volume measurements before and after inhalation of BD. We calculated effect size (ES) for each parameter from the difference between two means divided by the standard deviation of baseline score. There was a significant increase both FVC and FEV 1by 8.2 and 12.3 % from baseline (p<0.001). ES were 0.34 for FEV1 and 0.26 for FVC. The ES for lung volumes were from -0.07 (total lung capacity) to -0.31 (residual volume). The ES for sRtot (specific airway resistance) was -0.5 and ES for sGeff (specific effective airway conductance) was 0.95. The parameters of airway resistance and conductance were more responsive for the assessment of pulmonary function changes than spirometry and lung volumes parameters in patients with chronic airway obstruction.

AbstractDuring winter 1999 three large avalanche events were triggered by explosives at SLF’s avalanche test site, Vallée de la Sionne, canton Valais, Switzerland. One important goal of these large-scale field experiments was to measure the release and deposition volumes of avalanches by photogrammetric methods. In this paper, the photogrammetric measurements of all three avalanches are summarized. For one avalanche event it was possible to realize the whole measuring procedure as planned, and to obtain volume measurements before and after the avalanche triggering In the other two avalanche events, the photographs before the triggering of the avalanche failed. Nevertheless the photographs taken after the avalanche provide valuable information on the fracture depth at the fracture line. The mean fracture depth of the largest avalanche was about 2.10 m, varying between 1 and 3.5 m over a width of > 1000 m. The total volume of the deposition of all three avalanche events was about 1300 000 m3. The deposits are distributed over a length of > 1000 m with depths up to 30 m. The difference between the released and deposited volumes proved that avalanches entrain a large amount of snow along the avalanche track. Furthermore, the snow distribution in the deposition zone provides important information about the behaviour of a dense flowing avalanche in the runout zone.

We examined the effects of lung volume change and volume history on lung resistance (RL) and its components before and during induced constriction. Eleven subjects, including three current and four former asthmatics, were studied. RL, airway resistance (Raw), and, by subtraction, tissue viscance (Vtis) were measured at different lung volumes before and after a deep inhalation and were repeated after methacholine (MCh) aerosols up to maximal levels of constriction. Vtis, which average 9% of RL at base line, was unchanged by MCh and was not changed after deep inhalation but increased directly with lung volume. MCh aerosols induced constriction by increasing Raw, which was reversed by deep inhalation in inverse proportion to responsiveness. such that the more responsive subjects reversed less after a deep breath. Responsiveness correlated directly with the degree of maximal constriction, as more responsive subjects constricted to a greater degree. These results indicate that in humans Vtis comprises a small fraction of overall RL, which is clearly volume-dependent but unchanged by MCh-induced constriction and unrelated to the degree of responsiveness of the subject.

Evidence has been presented that some patients with cystic fibrosis have significant amounts of obstruction to the ventilation of portions of lung, as indicated by tests of the distribution of ventilation, but they have little or no evidence of obstruction by conventional measurements of airway resistance.1-5 This discrepancy may be attributed to the fact that various tests of pulmonary function differ in sensitivity with respect to small and large airway obstruction. It is the purpose of the present Commentary to summarize the evidence for this hypothesis and to suggest that differences in the extent to which measurements of airway resistance and distribution of ventilation are altered in patients with cystic fibrosis may be used to localize the site of obstruction and to follow the different stages in the natural history of the pulmonary disease.

The effect of alveolar surface tension on lung dynamic behavior was investigated by measuring total lung and tissue impedances in excised rabbit lungs at breathing frequencies of 0.2–0.8 Hz and tidal volumes of 10, 20, and 30 ml before and after lavage with 3-dimethyl siloxane, which provided a constant surface tension of 16 dyn/cm. The lungs were oscillated around the mean deflation pressures of 5 (control) and 8 cmH2O (lavaged), i.e., lung volume of 60% of total lung capacity. The total lung impedance was calculated from measurements of pressure and airflow at the trachea, and tissue impedance was measured by the alveolar capsule technique. The airway contribution was obtained as the difference between total lung and tissue impedances. In the lavaged lungs, dynamic elastance (Edyn) decreased and tissue resistance (Rti) increased relative to the control values over the entire frequency range. Airway resistance increased at the higher flow rates only. The decrease in Edyn could be attributed to the absence of surface film elastance in the lavaged lungs. The increase in airway resistance could be attributed to accentuated flow dependence due to changes in airway geometry and residual lavage liquid. However, the most intriguing result was the increase in Rti in the lavaged lungs. It could be attributed to altered mechanics at the alveolar duct level after lavage. It is concluded that dissipative properties of lung tissue are major determinants of Rti, whereas elastic properties of both tissue and surface film are important determinants of Edyn.

This study was conducted to determine whether horses with naturally occurring, severe chronic recurrent airway obstruction (RAO) 1) have a greater resting energy expenditure (REE) than control horses, 2) suffer body mass depletion, and 3) have significantly decreased REE after bronchodilation and, therefore, also 4) whether increased work of breathing contributes to the cachexia seen in some horses with RAO. Six RAO horses and six control horses underwent indirect calorimetric measures of REE and pulmonary function testing using the esophageal balloon-pneumotachograph method before and after treatment with ipratropium bromide, a parasympatholytic bronchodilator agent, at 4-h intervals for a 24-h period. Body condition scoring was performed, and an estimate of fat mass was determined via B-mode ultrasonography. O2 and CO2 fractions, respiratory airflow, respiratory rate, and pleural pressure changes were recorded, and O2 consumption, CO2 production, REE, pulmonary resistance, dynamic elastance, and tidal volume were calculated. In addition, we performed lung function testing and calorimetry both before and after sedation in two control horses. RAO horses had significantly lower body condition scores (2.8 ± 1.0 vs. 6.4 ± 1.2) and significantly greater O2 consumption than controls (4.93 ± 1.30 vs. 2.93 ± 0.70 ml·kg−1·min−1). After bronchodilation, there was no significant difference in O2 consumption between RAO horses and controls, although there remained evidence of residual airway obstruction. There was a strong correlation between O2 consumption and indexes of airway obstruction. Xylazine sedation was not associated with changes in pulmonary function but did result in markedly decreased REE in controls.

Rapid thermal annealing (RTA) of ion-implanted Si is important for device fabrication. The defect structures of 2.5, 4.0, and 6.0 MeV As-implanted silicon irradiated to fluences of 2E14, 4E14, and 6E14, respectively, have been analyzed by electron diffraction both before and after RTA at 1100°C for 10 seconds. At such high fluences and energies the implanted As ions change the Si from crystalline to amorphous. Three distinct amorphous regions emerge due to the three implantation energies used (Fig. 1). The amorphous regions are separated from each other by crystalline Si (marked L1, L2, and L3 in Fig. 1) which contains a high concentration of small defect clusters. The small defect clusters were similar to what had been determined earlier as being amorphous zones since their contrast was principally of the structure-factor type that arises due to the difference in extinction distance between the matrix and damage regions.

SummaryChanges in platelets in 48 patients with uterine myoma before and after hysterectomy with and without ovariectomy were examined. Bilateral ovariectomy in 25 cases (ovariec-tomized group) and unilateral or non-ovariectomy in 23 cases (control group) were performed at the hysterectomy. Platelet count and an appearance rate of secondary aggregation decreased at one day after and increased at one week after the operation, similarly in both the ovariectomized and the control group. The appearance rate of secondary aggregation was reflected in an intensity of aggregation at 5 min after the addition of reagent to PRP. At one month after the operation, the appearance rate of secondary aggregation induced by 3 μM ADP showed a statistically significant decrease in comparison with the preoperation value (P <0.05) and the enhancement of 5-min aggregation was still observed in the control group, while ceased in the ovariectomized group. The difference between the two groups was significant (P < 0.05). There was almost no change in the speed and intensity of primary and secondary aggregation during the observation period. No significant differences in collagen-induced aggregation were noted between the two groups. The results suggest that ovarian hormones, mainly estrogen, facilitate platelet activation which is mediated by the so-called secondary aggregation.

The sample in this study were 55 elderly. Data analysis wasused to determine the difference in blood pressure before and after laughing therapy with a ratio datascale that was using Pairs T-TestResult: There is an effect of laughing therapy on blood pressure in the elderly at UPT Panti WredhaBudhi Dharma Yogyakarta marked with a significant value of 0.000 (P <0.05)

In Uruguay, Lacanian ideas arrived in the 1960s, into a context of Kleinian hegemony. Adopting a discursive approach, this study researched the initial reception of these ideas and its effects on clinical practices. We gathered a corpus of discursive data from clinical cases and theoretical-doctrinal articles (from the 1960s, 1970s and 1980s). In order to examine the effects of Lacanian ideas, we analysed the difference in the way of interpreting the clinical material before and after Lacan's reception. The results of this research illuminate some epistemological problems of psychoanalysis, especially the relationship between theory and clinical practice.

ABSTRACT The fractional determination of 17-ketosteroids in the daily urine was performed in nine cases of hyperemesis gravidarum and in four control cases, in the first trimester of pregnancy both before and after corticotrophin administration. The excretion of total 17-KS is similar in the two groups. Only in the hyperemesis group does the excretion of total 17-KS increase significantly after corticotrophin administration. The fractional determination reveals no difference between the two groups of patients with regard to the values of the fractions U (unidentified 17-KS), A (androsterone) and Rest (11-oxygenated 17-KS). The excretion of dehydroepiandrosterone is significantly higher in the hyperemesis group than in the control group. The excretion of androstanolone seems to be lower in the hyperemesis group than in the control group, but the difference is not statistically significant. The differences in the correlation between dehydroepiandrosterone and androstanolone in the two groups is significant. The high excretion of dehydroepiandrosterone and low excretion of androstanolone in cases of hyperemesis gravidarum is a sign of adrenal dysfunction.
