TPKU PP.Tebuireng's business is engaged in school uniforms. The results of previous research, it was found that there were seven Critical to Quality (CTQ). The seven CTQs are button attachment, accessories installation, untidy stitching, loose overlock, uniform stain, slanted pocket, and sloping belt. Based on these findings, the objective of the activity is transforming knowledge to develop the quality of school uniform using the six sigma method. The six sigma method is improving quality, aiming 3-4 defects per million products or services. The results of the service activities have been delivery of material and the application of the six sigma method. The six sigma method stages include define, measure, analyze, improve, control. The results of the define application by check sheet, identified seven CTQs. The application of measure is 2.95 sigma level, is equivalent to 74,830 defective in a million productions. The analyze application with pareto and causal diagram shows two types of CTQ. That have dominant causes of defects, namely untidy seams and uniform stains. The implementation of improve is based on the analyze stage results. Implementation of control by means of documentation, disseminate all relevant sections. As a step to improve the performance of the next process.

The rapid rate of growth of technology and information in the modern era today, encourages companies to further develop their thoughts and views to maximize the goals and objectives set. The Place to Practice Business Skills (TPKU), is a business engaged in the field of convection managed by Tebuireng Boarding School. One of the most widely used convection products is the school uniform. In producing school uniforms, convection also strives to continuously improve quality by pressing the number of defects in the product. Some things that need to be done are identifying the factors that cause disability using the Six Sigma method, calculate the quality of sigma, and propose appropriate actions to control the quality of school uniforms. Six sigma is a tool of quality improvement toward the 3,4 target of failure per a million opportunities for each production. So six sigma is one of the new methods in the field of quality management aimed at quality control and improvement. By using the six sigma method, it can be seen that there are seven types of defects (CTQ) that occur in school uniform products. The two most common types of defects are: seams that are not tidy with a percentage of 51.1% and stain on uniform of 28.4%. For the quality of school uniforms produced by Tebuireng Convection is still low ie 2.95 sigma with a product defect rate as much 74830 products in one million products (DPMO). From the application of these six sigma, it can be concluded that the proposed action to be done convection to reduce the level of product defects is to focus its supervision on improving employee performance, engine performance, applicable work standards, existing facilities and infrastructure, provide motivation to work, and improve time management well.

Tatung Electric Indonesia is a peripatetic company in manufacturing industry assembling of electro motor. So that product yielded by PT. Tatung Electric Indonesia can continue to fulfill requirement of customer, company try to improve the quality of its product by minimizing defect which during the time still happened. This defect arising caused by the existence of variation at yielded product and also that happened in the process. Six Sigma represent control technique or method to reach storey or level quality of 3.4 DPMO ( Defects Per Million Opportunities), which applied by Motorola company since year 1986. Applying strategy to be used is Define-Measure-Analyze-Improve-Control (DMAIC). This Method represent recuring step and form a make-up of quality cycle. Calculations show the company is 3.641 sigma level with DPMO 15,830. The sigma levels are on the average level of sigma industries in Indonesia (3-4 sigma). Increased levels of sigma would happen if the company immediately continuous improvement over time. Conclusion of the application of Six Sigma is the DMAIC method in the assembly process of electro motors can analyze and reduce disability that occurs that will increase the quality of products.

Introduction: The problem faced by the company is the increase in the number of damaged products during the production process, which affects the quality of the product resulting decreased sales levels. The production process in food, beverage, and other processing industry is the most important activity to produce products that later to be marketed. Methods: The research method used in the study is Six Sigma method. Six Sigma is a system which has purpose to make a business become successful in a comprehensive and flexible manner. The product taken and examined is the sweet fermented Carica fruit. This fruit will be produced by CV. Gemilang Kencana, wheresoever the fruit is ready for the packing process, both cups and bottles.Results: The results of the calculation of Six Sigma value from CV. Gemilang Kencana has a sigma level 3.8 with the possibility of damage/ defects to the product up to 11,760 pcs in a million chance of production (DPMO). To reduce the number of damaged/ defective products that occur in the production process by sorting the highest percentage of causes damage, namely packaging leakage by 37%, untidy products by 50%, and less full products by 13%.Conclusion and suggestion: After knowing the results in this study, if the cause of the damage is not handled properly, more products will fail during the production process, which will cause an increase in production costs and have an impact on company losses.

The research was conducted at PT. XYZ where there were defect problems in bed series products so that additional time is needed to repair a product. One way to make improvements and improve quality in a production process is the Six Sigma method with the stages of DMAIC. The results of the calculated average of DPMO values and Sigma Level have the result 58558.56 and 3.07 which are still not good because still far from 6 sigma. After that analyzing the root causes of defect product problems, then the priority problem is carried out using (RPN) where the highest value is at the assembly work station because the material that has been used up in the warehouse has an RPN of 405 so that it becomes a top priority for repairs. With the standardization and documentation, proposed improvements that have been given then the possible defects of Electric Lovina Bed 3 Motor will be reduced.

Quality is a key to be able to compete in the industrial world [16. Quality control is needed to reduce the number of defective products produced by the company, thus reducing losses experienced by the company. The Bima Mandiri Rembang Pasuruan cigarette company is a company that produces cigarettes, one of which is INNO cigarettes. The number of defective products produced by the company so that the company must make an increase in quality by using a method to reduce the number of defects that occur.  The number of defective products causes the company to suffer losses. For that, we need a method that can reduce cigarette defective products which in turn can improve the quality of the company's production[12]. Six Sigma with the DMAIC stage (Define, Measure, Analyze, Improve, Control) is a method used in this research. Based on these steps, defects that often occur are less dense cigarettes as many as 289 sticks with a percentage of 25%, tearing on the cigarettes as many as 227 cigarettes with a percentage of 20% and peeling cigarettes as many as 208 sticks with a percentage of 18%. Factors that cause defects include humans, machines, methods and materials. After calculating using the Six Sigma method, before the improvement, the DPMO value is 113988.1 and the Sigma Level value is 2.722. After the improvements were made, the DPMO value decreased to 76488.1 and the Sigma Level was 2.94715. To achieve the six sigma target, the company is expected to be able to carry out improvements with a focus on the factors that cause product defects and always carry out regular control to reduce product defects. The corrective steps taken in the Bima Mandiri Rembang Pasuruan Regency cigarette company are human: regular training for machine operators and employees; methods: inspection of raw materials, blending machines and glue residue in the teat; engine: inspection of engine components; material: reprocessing less refined raw materials and using better quality glue.

A city as a system that constitutes one of the most important areas of human activities. The significant role to fulfill their expectations pay the goods transport and deliveries. These issues are the subject of urban logistics. In broad terms, urban logistics may be construed as a number of processes focused on freight flows, which are completed in cities, including deliveries, supply, goods transfer, services, etc. Due to the different urban logistics stakeholders’ expectations, these systems generate many challenges for managers, especially in the context of city users’ needs and their quality of life. Today, there is a lack of broadened approach and methodology to support them from the processes’ efficiency perspective. To fulfill this gap, the purpose of this paper is to apply the Six Sigma method as a support in last mile delivery management. Six Sigma method plays important role in production systems processes management. However, it could be useful in much wider perspective, including transport and logistics processes. The Authors emphasize that the Six Sigma method could be efficient approach in the last mile delivery processes’ analysis in the context of their efficiency. It helps positioning the customer satisfaction level and quantify the delivery processes defects, related to the undelivered goods. Following that it could improve significantly the last mile delivery processes efficiency. The concept is illustrated by a sample evaluation of one of the urban logistics processes: completion of deliveries. To this end, urban logistics processes were defined. In particular, the delivery completion process was mapped, and the process client was defined along with their expectations. The defects that occurred in the process were identified and analyzed. A DPMO (Defect per Million Opportunities) indicator was established for three randomly selected months. In addition, a root cause analysis of errors was performed. The sigma level for the studied process ranged from 2.61 to 2.89. The factor that had the greatest impact on the number of defects defined as failed deliveries was customer’s absence. The obtained results indicate that the delivery process should be examined in more detail.

<em>CV Jaya Reksa Manggala (JRM) is one of clothing company. With increasing competition in this clothing company, the companies are required to keep improving the quality of clothing products. One of the important things in quality is reduction of product defects. One of the proven methods of  quality improvement is the Six Sigma DMAIC method, which is a continous improvement method that aims to achieve the best quality. One of the products of CV JRM which is still have a lot of defects is 514 products. From the initial data obtained, DPMO value for process I and II for 514 products is 10,768.52 and 27,341.42 with sigma level 3.79 and 3.43. The steps undertaken in this Six Sigma DMAIC method include define, measure, analyze, implementation and control. Several proposed improvements were then made to reduce the problem of these defects and from the data at the control stage, the DPMO values of processes I and II became 2,941.76 and 1,812.69 with sigma levels of 4.25 and 4.41. From this result it can be said that with Six Sigma DMAIC method, there is improvement of quality for product 514 at CV JRM. </em>

XYZ is a company that produces Medical Devices. One of the products produced is 16x16 hydrophilic sterile gauze. One of the factors that affect the quality of a product is the presence of defects in the product and waste which results in a longer lead time for a product. The investigative stages used in this study are using tools: seven tools, Big Picture Mapping, Value Stream Analysis (VALSAT) and then analyzing the causes of defects using the Ishikawa diagram (fishbone chart) where the output of this study is a proposed improvement using the FMEA method ( Failure Mode Effect and Analysis) to eliminate defects and waste that occur. Based on the research results, it is known that the type of waste identified by the calculation of Value Stream Activity and obtained Value Added with a percentage of 84.4%, Non Value Added by 1.0% and Necessary Non Value Added by 14.5%. From the results of production output in December 2019 to November 2020 amounting to 2,650,200 boxes, the number of defects is 1,316 boxes with an average sigma level of 5.16, while in FMEA it is known that the largest defects are spots with an RPN value of 504 and a suggestion for improvement is to place Experienced operators who have gone through the recruitment and training stages on superior qualifications on one machine according to their skills who understand SOPs and operators are given good comfort in the production process.                                                                                                   Keywords: Lean Six Sigma, Seven Waste, DMAIC, FMEA, kasa steril hidrofil 16x16.

The main purpose of this research is to use &ldquo;DMAIC&rdquo; and &ldquo;DMADV&rdquo; framework of six sigma to reduce cost of projects, increase yields, improve performance and reduce defects. This study conclude that the sigma level of cement bag production in four production lines is &ldquo;4.7 DPMO&rdquo; values of 710 and the possibility of defects per unit of 11 possibilities this situation was handled by using six sigma. Any business or industry run only to satisfy customer and increase their profits, this can be attained as development of quality product..For the ranking of newly established universities the certification of those institutes is very important and also a critical process. For this process we used Lean six sigma approach that can identify the wastes that affect this process.Six sigma can be applied to any work field such as education, power optimization and other types of industries Six sigma &ldquo;DMAIC&rdquo; approach is also used for the testing of EDA tools that occurs due to the complex coding or configurations, flows and platforms they support. To improve the process quality of SDLC it&rsquo;s necessary to remove the defects of system in advance along with thorough valuation of size and it also makes project accordant with real time environment

<p>PT. Bakrie Pipe Industries is a company engaged in the production of steel pipe. Pipe -<br />steel pipe produced by PT. Bakrie Pipe Industries is used for a variety of needs such as: pipes for<br />oil and gas , for the installation of water pipes, pipes for construction purposes, and for other<br />purposes. PT. Bakrie Pipe Industries always maintain the quality of the resulting production and<br />efforts to minimize the failure existing production of the company's. Therefore, Do research using<br />the six sigma method with methodology DMAIC (Define, Measure, Analyze, Improve, and<br />Contorol) expected by using this method can minimize the defects and also improve the speed of<br />production.In the define stage, the selection of products to research the specs API steel pipe with<br />14-inch size that has defects percentage of 5.25% and therefore this product the focus of<br />research.Furthermore the Measure stage is measurement sigma level company, the results of these<br />measurements show the value of 26,285 DPMO with sigma level was 3.44 sigma that indicates that<br />value still far from the target of 6 sigma.The next stage is a Analayze stage to analyze the causes<br />of production failure that occurred in production, using Ishikawa diagram and FMEA tables.Then<br />the Improve stage is giving suggestions for improvement and suggestions given for companay with:<br />the application of methods of DOE (Design Of Experiments) with factorial 21 to find the optimal<br />distance in screw machine, giving proposed a tool is Roll Waterpass and application form<br />monitoring for the surveillance for removing the beads process. After do some implementations<br />gained increasing value 21,662 DPMO and sigma value of 3.52 sigma.</p>
