Healthcare workers were the first group scheduled to receive COVID-19 vaccines when they became available in South Africa. Therefore, estimating vaccine confidence levels and intention to receive COVID-19 vaccines among healthcare workers ahead of the national vaccination roll-out was imperative. We conducted an online survey from 4 February to 7 March 2021, to assess vaccine sentiments and COVID-19 vaccine intentions among healthcare staff and students at a tertiary institution in South Africa. We enrolled 1015 participants (74.7% female). Among the participants, 89.5% (confidence interval (CI) 87.2–91.4) were willing to accept a COVID-19 vaccine, 95.4% (CI 93.9–96.6) agreed that vaccines are important for them, 95.4% (CI 93.8–96.6) that vaccines are safe, 97.4% (CI 96.2–98.3) that vaccines are effective, and 96.1% (CI 94.6–97.2) that vaccines are compatible with religion. Log binomial regression revealed statistically significant positive associations between COVID-19 vaccine acceptance and the belief that vaccines are safe (relative risk (RR) 32.2, CI 4.67–221.89), effective (RR 21.4, CI 3.16–145.82), important for children (RR 3.5, CI 1.78–6.99), important for self (RR 18.5, CI 4.78–71.12), or compatible with religion (RR 2.2, CI 1.46–3.78). The vaccine confidence levels of the study respondents were highly positive. Nevertheless, this could be further enhanced by targeted interventions.

ABSTRACT Introduction In South Africa, healthcare workers were the first group scheduled to receive a COVID-19 vaccine when it became available. Therefore, estimating their vaccine confidence levels and gauging their intention to vaccinate ahead of the COVID-19 vaccination roll-out was imperative. Methods An online survey was conducted among current staff and students of the Faculty of Medicine and Health Sciences of Stellenbosch University in South Africa using a succinct questionnaire. Sentiments about vaccines were estimated using five validated statements. The intention to receive a COVID-19 vaccine was also investigated. Results The response rate was 21.8%, giving a sample size of 1015. Females were 70.0% in the source population and 74.7% in the study sample. The proportion of participants who agreed that vaccines are important for children and for self was 97.4% (95% confidence interval [CI] 96.1% to 98.3%) and 95.4% (95% CI 93.9 to 96.6) respectively. In addition, 95.4% (95% CI 93.8 to 96.6) agreed that vaccines are safe, 97.4% (95% CI 96.15 to 98.28) that vaccines are effective, and 96.1% (95% CI 94.6 to 97.2) that vaccines are compatible with their religious beliefs. The proportion of participants who were willing to receive a COVID-19 vaccine was 89.5% (95% CI 87.2 to 91.4). ABSTRACTIntroductionThe outbreak of novel coronavirus disease 2019 (COVID-19) caught the world off guard in the first quarter of the year 2020. To stem the tide of this pandemic, the development, testing, and pre-licensure approval for emergency use of some COVID 19 vaccine candidates were accelerated. This led to raised public concern about their safety and efficacy, compounding the challenges of vaccine hesitancy which was already declared one of the top ten threats to global health in the year 2019. The onus of managing and administering these vaccines to a skeptical populace when they do become available rests mostly on the shoulders of healthcare workers (HCWs). Therefore, the vaccine confidence levels of HCWs becomes critical to the success of vaccination endeavors, especially COVID 19 vaccination. This proposed study aims to estimate the level of vaccine confidence and the intention to receive a COVID 19 vaccine among future HCWs and their trainers at a specific university in Cape Town, South Africa, and to identify any vaccination concerns early for targeted intervention.Methods and analysisAn online survey will be distributed to current staff and students of an academic institution for HCWs. The survey questionnaire will consist of a demographic questions section consisting of six items and a vaccine confidence section comprising six items in Likert scale format.A multinomial logistic regression model will be employed to identify factors associated with vaccine confidence and intention. The strength of association will be assessed using odds ratio and its 95% confidence interval. Statistical significance will be defined at a p-value <0.05.Ethics and disseminationEthics approval has been obtained for the study from Stellenbosch University (HREC Reference # S19/01/014 (PhD)). The results will be shared with relevant health authorities, presented at conferences, and published in a peer-reviewed journal.ARTICLE SUMMARYStrengths and limitations of this study▸The proposed study will generate baseline knowledge of the vaccine confidence among future healthcare workers and their trainers in its specific context.▸It will contribute to addressing the knowledge gap about the intention to receive a COVID 19 vaccine among health care workers in Africa.▸It will enable the early identification of vaccine concerns of healthcare workers while they are still in training and assist in informing tailored measures to address them.▸A limitation of the study is the possibility of a low response rate which is an inherent challenge of online surveys.

To control the spread of the novel coronavirus disease 2019 (COVID-19), COVID-19 vaccination has been quickly developed. However, the COVID-19 pandemic will not be controlled if the COVID-19 vaccination uptake willingness is low. Therefore, the study aim was to explore the COVID-19 vaccination uptake willingness among the outpatient population and healthcare workers in Taiwan during the worldwide pandemic period without community outbreaks. A cross-sectional survey was conducted among healthcare workers (HCWs; n = 500; mean age = 32.96 years) of National Cheng Kung University Hospital (NCKUH) and outpatients (n = 238; mean age = 34.43 years) arriving at NCKUH. We used an online survey conducted between September 24 and 21 November 2020, for healthcare workers, and between 27 October and 31 December 2020, for the outpatient sample. Information regarding willingness to receive vaccination, willingness to rapid test, fear of COVID-19, risk perception, and preventive behaviors was collected in both samples; information regarding willingness to care for patients was collected in healthcare workers. Willingness to receive vaccination was the main variable in the present study; willingness to rapid test, willingness to care for patients, fear of COVID-19, risk perception, and preventive behaviors were the secondary variables in the study. The factors associated with vaccination willingness were identified through logistic regression analysis. The participants’ willingness to receive vaccination was low for both healthcare workers (23.4%) and the outpatient sample (30.7%). Similarly, their willingness to take rapid tests was low (23.6% for healthcare workers and 28.6% for outpatient sample). Risk perception (crude odds ratio (COR) = 1.29; 95% confidence interval (CI) = 1.03, 1.63), willingness to take rapid test (COR = 9.24; 95% CI = 5.76, 14.83), and preventive COVID-19 infection behaviors (COR = 2.32; 95% CI = 1.52, 3.56) were significant factors explaining the healthcare workers’ willingness to receive vaccination. Willingness to take a rapid test (COR = 8.91; 95% CI = 4.71, 16.87) and preventive COVID-19 infection behaviors (COR = 1.69; 95% CI = 1.09, 2.60) were significant factors explaining the outpatient sample’s willingness to receive vaccination. Willingness to vaccinate against COVID-19 among HCWs and outpatients is low due to the relatively safe status of COVID-19 infection in Taiwan. These findings can help policymakers advocate for the effectiveness of and provide transparent information on COVID-19 vaccination uptake in a country/region with a relatively safe COVID-19 outbreak status.

ObjectivesThe aim of this study was to compare the perception, confidence, hesitancy and acceptance rate of various COVID-19 vaccine types among healthcare workers (HCWs) in Saudi Arabia, a nation with Middle East respiratory syndrome coronavirus experience.DesignNational cross-sectional, pilot-validated questionnaire.SettingOnline, self-administered questionnaire among HCWs.ParticipantsA total of 2007 HCWs working in the Kingdom of Saudi Arabia participated; 1512 (75.3%) participants completed the survey and were included in the analysis.InterventionData were collected through an online survey sent to HCWs during 1–15 November 2020. The main outcome measure was HCW acceptance of COVID-19 candidate vaccines. The associated factors of vaccination acceptance were identified through a logistic regression analysis and via measurement of the level of anxiety, using the Generalised Anxiety Disorder 7 scale.ResultsAmong the 1512 HCWs who were included, 62.4% were women, 70.3% were between 21 and 40 years of age, and the majority (62.2%) were from tertiary hospitals. In addition, 59.5% reported knowing about at least one vaccine; 24.4% of the participants were sure about their willingness to receive the ChAdOx1 nCoV-19 vaccine, and 20.9% were willing to receive the RNA BNT162b2 vaccine. However, 18.3% reported that they would refuse to receive the Ad5-vectored vaccine, and 17.9% would refuse the Gam-COVID-Vac vaccine. Factors that influenced the differential readiness of HCWs included their perceptions of the vaccine’s efficiency in preventing the infection (33%), their personal preferences (29%) and the vaccine’s manufacturing country (28.6%).ConclusionsAwareness by HCWs of the several COVID-19 candidate vaccines could improve their perceptions and acceptance of vaccination. Reliable sources on vaccine efficiency could improve vaccine uptake, so healthcare authorities should use reliable information to decrease vaccine hesitancy among frontline healthcare providers.

Background: Healthcare workers intention to be vaccinated is an important factor to be consider for successful COVID-19 vaccination programme. Our study aimed to understand the intention of health workers to receive COVID-19 vaccine and associated concerns across 10 countries in Eastern Mediterranean Region (EMRO). Method: A cross-sectional study was conducted in January 2021 among healthcare workers using an online survey. A total of 2806 health workers (physicians, nurses and pharmacists) completed and returned the informed consent along with the questionnaire electronically. Data were analyzed using IBM SPSS software package version 20.0. Results: More than half of the respondents (58.0%) intend to receive COVID-19 vaccine, even if the vaccination is not mandatory for them. On the other hand, 25.7% of respondents were not intending to undertake COVID-19 vaccination while 16.3 % answered undecided. The top three reasons for not intending to be vaccinated were unreliability of COVID-19 vaccine clinical trials (62.0%), fear of the side effects of the vaccine (45.3%), and that COVID-19 vaccine will not give immunity for a long period of time (23.1%). Conclusion: Overall, our study revealed suboptimal acceptance of COVID-19 vaccine among healthcare workers in the 10 countries in EMRO.

Background All healthcare workers (HCWs) in Yongin Severance Hospital were allocated to receive the ChAdOx1 nCov-19 vaccine according to national policy. A report of thrombosis and thrombocytopenia syndrome (TTS) associated with ChAdOx1 nCoV-19 led to hesitancy about receiving the second dose among HCWs who had received the first dose. Methods From 7 to 14 May, 2021, we performed a survey to identify the factors associated with hesitancy about receiving the second vaccine dose among HCWs at the hospital who had received the first dose of the vaccine. Based on survey results, a hospital-wide campaign was implemented on 18 May 2021 to improve vaccine coverage. HCWs who completed the second dose completed a self-administered questionnaire to evaluate the effect of the campaign. Findings Of 1,171 HCWs who had received the first dose of the vaccine, 71.5% completed the online survey, of whom 3.7% refused to take the second dose and 22.3% showed hesitancy. Hesitancy to receive a second dose was significantly associated with age under 30 years and concerns about TTS, and was less common among those who trusted effectiveness and safety of the vaccine. Among HCWs who received the first dose, 96.2% completed vaccination with the second dose between 27 May and 4 June, 2021. Of those who answered the questionnaire asked about the timing of their decision to receive the second dose, 57.1% reported that they were motivated by the hospital-wide campaign. Conclusion A tailored intervention strategy based on a survey can improve COVID-19 vaccination uptake among HCWs.

Background: Healthcare workers are among the first group to receive coronavirus 2019 disease (COVID-19) vaccine, and their attitude towards the vaccine is likely to affect vaccine acceptance among general population. We aimed to study the attitude of HCWs towards COVID-19 vaccine and determine the factors associated with it.Methods: A cross-sectional study was conducted through a written questionnaire prior to first dose of COVID-19 vaccine in the month of January, 2021, at a district hospital in Ludhiana, Punjab. HCWs were categorized as having negative, positive or mixed attitude based on their response to a written questionnaire. Data was analysed using SPSS version 24.0.Results: More than half of the participants had negative attitude towards COVID-19 vaccine (N=274, 54.5%), while less than a third had positive attitude (N=141, 28.0%) and rest had mixed attitude (N=88, 17.5%). Younger HCWs (<50 years), nurses, females and those living with chronic medical or psychiatric illness showed higher chance of negative attitude towards COVID-19 vaccine, while doctors, paramedical staff, HCWs with higher qualification and urban dwellers had mixed response. On logistic regression analysis, being a female HCW was significantly associated with negative attitude while being a doctor or nurse was significantly associated with mixed attitude towards COVID-19 vaccine. Doubt regarding safety was the commonest response observed among all variables.Conclusions: In the presence of a high proportion of HCWs having negative attitude, targeted interventions should be planned to address specific concerns of HCWs, so as to increase the overall vaccine acceptance for COVID-19 vaccine.

Background: Sudden changes in clinical practice and the altered ability to care for patients due to the COVID-19 pandemic have been associated with moral distress and mental health concerns in healthcare workers internationally. This study aimed to investigate the severity, prevalence, and predictors of moral distress experienced by Australian healthcare workers during the COVID-19 pandemic. Methods: A nationwide, voluntary, anonymous, single time-point, online survey of self-identified frontline healthcare workers was conducted between 27th August and 23rd October 2020. Participants were recruited through health organisations, professional associations, or colleges, universities, government contacts, and national media. Results: 7846 complete responses were received from nurses (39.4%), doctors (31.1%), allied health staff (16.7%), or other roles (6.7%). Many participants reported moral distress related to resource scarcity (58.3%), wearing PPE (31.7%) limiting their ability to care for patients, exclusion of family going against their values (60.2%), and fear of letting co-workers down if they were infected (55.0%). Many personal and workplace predictors of moral distress were identified, with those working in certain frontline areas, metropolitan locations, and with prior mental health diagnoses at particular risk of distress. Moral distress was associated with increased risk of anxiety, depression, post-traumatic stress disorder, and burnout. Conversely, feeling appreciated by the community protected against these risks in healthcare workers. Conclusions: Safeguarding healthcare workforces during crises is important for both patient safety and workforce longevity. Targeted interventions are required to prevent or minimise moral distress and associated mental health concerns in healthcare workers during COVID-19 and other crises.

Healthcare disturbance is a form of workplace violence against healthcare workers perpetrated by patients, their relatives, and gangs hired by them. It is a prevalent phenomenon in China, where evidence suggests that it impacts on the job satisfaction of healthcare workers. This study aims to examine the relationship between healthcare disturbance, surface acting as a response to emotional labour, and depressive symptoms in Chinese healthcare workers. The study adopted a cross-sectional design and used an online survey methodology. Data were collected from 418 doctors and nurses from one hospital in China. The results showed that frequency of healthcare disturbance was positively related to surface acting and depressive symptoms, respectively; surface acting was also positively related to depression, while deep acting showed no effect on symptoms of depression. Furthermore, surface acting in response to emotional labour mediated the relationship between healthcare disturbance and depressive symptoms. The results highlight the importance of preventing healthcare disturbance and of training healthcare staff in strategies for managing emotional demands in reducing depressive symptoms in Chinese healthcare staff.

Abstract Background Coronavirus disease 2019 (COVID-19) vaccination has raised concerns about vaccine hesitancy in general and COVID-19 vaccine hesitancy in particular. Understanding the factors driving the uncertainty regarding vaccination against COVID-19 is crucial. Methods This cross-sectional study was designed to identify the perceptions and attitudes of healthcare workers (HCWs) towards COVID-19 vaccines and determine the predictive factors that affect their willingness to receive the COVID-19 vaccine. An online survey was distributed among HCWs to collect data assessing demographic and general characteristics of the participants and vaccine-related characteristics, including source of information about the vaccine. In addition to items assessing the perception of COVID-19, there were items on COVID-19 vaccines and attitude towards vaccination in general and towards COVID-19 vaccines in particular. Results The participants were classified according to their willingness to take the COVID-19 vaccine as follows: hesitant (41.9%), refusing (32.1%), and willing (26%). Statistically significant differences were observed among the three groups for the perception of COVID-19 vaccines, attitude towards vaccination in general, and COVID-19 vaccines in particular (p < 0.01). Conclusions Although the participants adequately perceived COVID-19 severity, prevention, and COVID-19 vaccine safety, they were widely hesitant or refused to be vaccinated. A multidimensional approach is required to increase the vaccine acceptability rate. Higher income and increased years of work experience are positive predictors of willingness to receive a vaccine. Thus, further studies addressing the scope of COVID-19 vaccine hesitancy are warranted as an initial step to build trust in COVID-19 vaccination efforts with continuous monitoring of attitudes and practices of HCWs towards COVID-19 vaccines in the future.
