EFFECT OF DIABETES MELLITUS ON THE LEVEL OF GROWTH FACTORS IN GUERIN CARCINOMA IN RATS OF BOTH SEXES

Diabetes mellitus is an independent risk factor for a number of malignant diseases. It is associated with increased mortality from cancer. The role of growth factors in the pathophysiology of diabetes mellitus and malignant tumors is dual and not fully defined. The aim of the study was to examine the level of some growth factors and their receptors in the tissue of Guerin carcinoma and its perifocal zone growing in rats of both sexes with diabetes mellitus. Materials and Methods. The authors determined the content of VEGF-A, VEGF-C their receptors sVEGF-R1 and sVEGF-R3, EGF and sEGF-R, TGFβ1 and sTGFβ1-R2, TNFα and sTNFα-R1, FGF21 in the samples of the tumor and the perifocal zone of Guerin carcinoma in nonlinear white rats of both sexes with diabetes mellitus (main group). ELISA test was used for this purpose. The control group consisted of animals with Guerin carcinoma without comorbid pathology. Results. Tumors in female rats (main group) had a higher content of VEGF-C, EGF, TNFα and FGF21, VEGF-C/VEGF-R3, but a lower TGFβ1 level. In the perifocal zone these indicators, except TNFα, were lower than in the control group. In males (main group), TNFα and VEGF-A/VEGF-R1 levels in the tumor were higher. The content of VEGF-C, EGF, EGF-R and TGFβ1 was lower than in the control. At the same time, all the studied parameters were lower in the perifocal zone, except FGF21 and EGF-R. Conclusion. Diabetes mellitus as a concomitant disease decreased the life expectancy of animals of both sexes with Guerin carcinoma. At the same time, high ratios of VEGF-A/VEGF-R1 in males, and VEGF-C/VEGF-R3, EGF/sEGF-R, and FGF21 in females in tumor samples were accompanied by the tumor growth in males and activation of metastatic processes in females. Сахарный диабет является самостоятельным фактором риска возникновения ряда злокачественных заболеваний и связан с повышенной смертностью от рака. Роль факторов роста в патофизиологии сахарного диабета и злокачественных опухолей двойственна и до конца не определена. Целью настоящего исследования явилось изучение уровня некоторых факторов роста и их рецепторов в ткани карциномы Герена, растущей на фоне сахарного диабета, и ее перифокальной зоне у крыс обоего пола. Материалы и методы. У нелинейных белых крыс обоего пола в образцах опухоли и перифокальной зоны карциномы Герена, растущей на фоне сахарного диабета (основная группа), методом ИФА определяли содержание VEGF-A, VEGF-C и их рецепторов sVEGF-R1 и sVEGF-R3, EGF и sEGF-R, TGFβ1 и sTGFβ1-R2, TNFα и sTNFα-R1, FGF21. В качестве контрольной группы выступали животные с карциномой Герена без коморбидной патологии. Результаты. У самок крыс основной группы в опухоли было выше содержание VEGF-C, EGF, TNFα и FGF21, VEGF-С/VEGF-R3, но ниже уровень TGFβ1, в перифокальной зоне эти показатели, за исключением TNFα, были ниже, чем в группе контроля. У самцов основной группы в опухоли был выше уровень TNFα и VEGF-A/VEGF-R1, содержание VEGF-C, EGF, EGF-R и TGFβ1 оказалось ниже, чем в контроле. При этом в перифокальной зоне все исследованные показатели были ниже, за исключением FGF21 и EGF-R. Выводы. Сахарный диабет как сопутствующее злокачественному росту заболевание способствовал снижению продолжительности жизни животных обоего пола с карциномой Герена. При этом высокие в образцах опухоли соотношения VEGF-A/VEGF-R1 у самцов, VEGF-С/VEGF-R3, EGF/sEGF-R и FGF21 у самок сопровождались ростом объемов опухоли у самцов и активацией метастатических процессов у самок. Acute generalized peritonitis (AGP) and diabetes mellitus (DB) remains an urgent problem in modern medicine, as rapid disability and high mortality have been leading to cardiovascular disease and oncology pathology in recent years. The purpose is to study and evaluate the morphological changes in the lungs in animals with experimental AGP against the background of DM. Іn the experiment, 32 white rats were used. The main group consisted of 24 animals with simulated AGP against the background of CD; control group – 8 intact animals. The experimental CD was reproduced by intraperitoneal administration of streptozotocin at a dose of 60 mg/kg. On the 14th day after administration of streptozotocin, animals of the main group were injected with 10 % filtered stool suspension into the abdominal cavity of the study rats at a dose of 0.5 ml per 100 g of body weight. Removal of material for histological examination was carried out on days 1, 3, and 7. For the morphological study of the lungs of rats under experimental peritonitis, the method of light microscopy was used. Micropreparations were stained with hematoxylin and eosin, according to Hart, by the Van Gizon method. Histological examination of the lung tissue of animals with simulated AGP against the background of CD compared with intact rats on day 1 of the experiment showed a tendency for vasodilation. For 3 days, the lungs of the organs were observed in the lungs of animals; the wall was thickened in the vessels of large and medium caliber, the phenomena of mucoid edema and perivascular edema with small cell infiltration were noted. At day 7, significant changes in blood vessels and bronchi in all components were detected in the lungs of animals; Perivascular edema and inflammatory infiltration along the interval septa were observed in animals with simulated AGP against the background of CD in comparison with intact rats for 1 day of the experiment in lung tissue. The wall of small bronchi was thickened by leukocyte infiltration. For 3 days, the lungs of the animals were observed full blood vessels of the pulmonary components, the phenomena of mucoid edema were noted. For 7 days in the lungs of animals found the expansion of the lumen of the arteries and veins from their full blood, thickening of the outer shell of the vessel wall.

Aim: to evaluate the efficacy and safety of the L-lysine escinate venotropic drug in the treatment of cephalgic, atactic and asthenic syndromes in patients with chronic cerebral ischemia, mainly caused by venous disorders. Patients and Methods: the study involved 60 patients with a diagnosis of chronic cerebral ischemia with signs of chronic venous insufficiency. All patients were randomly divided into two groups consisting of 30 people. Patients in the main group received L-lysine escinate, and the control group received Vinpocetine. The drugs were administered by intravenous drip for 10 days. A dynamic assessment was performed using a modified subjective scale of asthenia evaluation (MFI-20), 100 mm long visual analogue scale (VAS) for headache, subjective neurological impairment scale (SNIS), and Tinetti test. An ophthalmic examination with an assessment of the fundus vessels and an ultrasound duplex scanning of head vessels (veins) were also conducted. Results: it was shown that L-lysine escinate therapy showed a statistically more significant reduction in the severity of headache (according to VAS) and an improved stability measured by Tinetti test versus during Vinpocetine treatment. There were no significant differences in the level of asthenia (MFI-20), the severity of emotional disorders, and the SNIS score. Also, the number of patients with dilated fundus veins significantly decreased during treatment in the main group (p<0.05), which was not the case in the control group. According to ultrasound duplex scanning data, the venous flow velocity through the veins of Rosenthal and angular veins during L-lysine escinate therapy was significantly higher (p<0.05) than in patients receiving Vinpocetine. Conclusion: the inclusion of vasotropic drugs (in particular, the L-lysine escinate venotonic drug) in the treatment regimen of patients with chronic brain ischemia seems appropriate, especially if there are signs of venous insufficiency. The aim — to study the role of myokines in the development of insulin resistance in children with type 1 diabetes mellitus.Materials and methods. Observations involved 68 children with type 1 diabetes mellitus (DM 1), with the mean age 11 to 17 years. Depending on the glycemic controllevel, patients were divided into 3 research groups. The control group consisted of 20 relatively healthy children. Muscle mass, the skeletal muscles index, fat mass and the percentage of fat in the bodywere determined in all patients. The Lovett’s test was used to assess the loss of muscle strength; evaluation of insulin resistance was made based onthe triglyceride­glucose index (TYG). Levels of myostatin, irisin, interleukins ­6 and ­13were measured in blood serum.Results and discussion. It has been established that with deterioration in the level of glycemic controlin DM 1 children, the component redistribution of body composition took place with an increased fat mass proportionand decreased muscle mass. This resulted in the reduced insulin-mediatedabsorption of glucose, that was confirmed by the significant increase in TYG level compared to control group. The analysis of cytokines in the blood serum showed a significant increase in the level of myostatin and interleukin­6 compared with the control group and the tendency to increased levels of the interleukins ­13 and the level of irisin in the blood serum in pediatric patients with DM 1. The increased levels of myostatin in DM 1childrenassociated with an increase in the triglycerides content (r = 0.44, p < 0.05) and raised TYG index (r = 0.33, p < 0.05), testifying theclose correlation between the high myostatin levels and the development of insulin resistance.Conclusions. In children with diabetes mellitus, the reduction of muscle strength and muscle mass take place with a deterioration in the state of glycemic control, accompanying by the development of insulin resistance. The violation of myokines synthesis,along with the chronic hyperglycemia and diabetic myopathy, plays the leading role in the formation of insulin resistance in pediatric patients with DM 1. It is manifested by the increased production of myostatin and interleukin­6 in the absence of activation of irisin and interleukin­13synthesis.

Inadequate breastfeeding or its total neglect has been mentioned in several studies as a contributing factor to the globally rising incidence of Type 2 diabetes mellitus (T2DM). However, the anti-diabetic role of breast-milk has not been given much attention. As such, this study was initiated to review and bring to update on the role of breastfeeding in the risk reduction of T2DM. Relevant information on the topic was retrieved from the reliable science databases, including PubMed, MedLine, Google Scholar, Researchgate, etc. The results showed that breast-milk is not energy dense and contains several health-enhancing bioactive molecules, including adipokines, antimicrobial and growth factors, cytokines, nutrients, and immune cells. Adipokines interact with the central nervous system to modulate certain physiological processes involved in energy balance, thereby programming an infant to be at a reduced risk for overweight, obesity and T2DM later in life. The antimicrobial and growth factors, as well as immune cells and bioactive nutrients may stimulate the growth of beneficial bacteria and/or inhibit the growth of pathogens. Thus, strengthen neonate defense mechanisms to effectively prevent infections as well as short and long-term disorders such as obesity and T2DM. In conclusion, nursing mothers are advised to breastfeed babies adequately before introducing them to complementary foods. To cater to the need of babies who may not have access to breastfeeding, healthcare providers should formulate infant formula using breast-milk components as basic constituents.

Gestational diabetes mellitus (GDM) is considered to be one of the most frequent medical complication observed among pregnant women. The role of adipokines in the pathogenesis of GDM remains strictly unknown. Different adipokines have been studied throughout gestation, and they have been proposed as biomarkers of GDM and other pregnancy-related complications; however, there is no biomarker reported for GDM screening at present. The aim of this study was to evaluate serum nesfatin-1 and vaspin levels in GDM and non-GDM women, to characterize the correlation between these adipokines, and to assess the potential role of circulating adipokines in the prediction of risk of gestational diabetes mellitus. Serum concentrations of nesfatin-1 and vaspin were measured in 153 women with GDM, and in 84 patients with uncomplicated pregnancy by enzyme-linked immunosorbent assay (ELISA) kits, according to the manufacturer’s instructions. Circulating levels of nesfatin-1 and vaspin were significantly lower in the GDM group than in the control group. Nesfatin-1 levels were negatively correlated with vaspin levels. The results of this study point out the possible role of nesfatin-1 and vaspin as potential novel biomarkers for the prediction and early diagnosis of GDM. Further studies are necessary to evaluate the influence of nesfatin-1 and vaspin on glucose metabolism in the early stages of GDM.

Objective. To evaluatethe role of allelic and genotypic variants of gene IL1β polymorphism rs1143627 in susceptibility to typhoid fever (TF). Materials and methods. 41 patients with TF and 84 chronic carriers of S. typhi were endrolled in the study. Control group included 91 healthy individuals, All of the individuals under study were Uzbeks. DNA samples were isolated from peripheric blood with the kit Ribo-sorb (AmpliSens®, Russia). Genotyping of polymorphism rs1143627 of gene IL1β was carried out by the standard PCR using kits “SNP-Express” (LLC NPF “LITECH”, Russia) according to the instruction of manufacturer. Results. The domination of allele 31T was found in the main group (patients and carriers). Its frequency was significantly lower in comparison with the control group (51.2% and 67.0%, respectively; χ2 = 10.8; p = 0.001). Minor allele -31С cytokine IL1β, on the contrary was detected more frequently in patients with TF (48.8%), than in the control group (33.0%) in χ2 = 10.8 and p = 0.001. Calculated relative chance of this allele detection in the main group in comparison with control group was OR = 1.9 in 95% CI 1.304-2.88. Conclusion. Genotypic variant C/C of polymorphism rs1143627 of the gene IL1β made a certain contribution into forming of generic structure of susceptibility to S. typhi. Risk of susceptibility of macro organism to pathogen in the presence of this genotype is higher more than 2.5 times (χ2 = 4.3; p = 0.04; CI 95% 1.037–7.359). Twelve white rats learned to press a bar or lever when this act was intermittently followed by pellets of food. Once a stable rate of pressing had been established, the animals were subjected to electric shock as a punishment for each response during alternate five minute periods within the experimental session. A difference in rates during the safe and punished phases was manifested both by the experimental group (8 rats), who were provided with a light as a signal when it was safe to respond, and also, contrary to expectation, by the control group (4 rats), who continued in darkness throughout the session. The differential responding by the control group was greatly reduced, however, when the duration of each phase was reduced from five minutes to one. The investigation was then continued to determine the role of some of the other factors in the situation. Increasing the severity of the shock increased the difference between the light and the dark rates. Withholding the pellets of food reduced the overall rates but did not affect the light-dark discrimination. Withholding the shock, on the other hand, permitted an increase in rate and caused the animals to lose their discrimination; when food had previously been withheld, however, so that the rate of response was relatively low, this deterioration in the discrimination was not as rapid as before. Restoration of the discrimination training under a lower hunger drive confirmed the finding that the formation of the discrimination was quite rapid and showed that the level of drive did not affect the proportionality between the rates in the light and in the darkness. These findings were interpreted by comparing the current procedure with those previously used in studies of avoidance reactions and conflict.

BACKGROUND: Diabetes mellitus is associated with an increased production of reactive oxygen species (ROS) and a reduction in antioxidant defense. Glutathione S-transferases (GSTs) is group of multifunction antioxidant enzyme can be used as important biomarkers for DM..  GSTM1, T1 genes variant polymorphism result in decreased or loss of enzyme activity. AIM: The study aimed to evaluate the role of GSTM1 and GSTT1 gene polymorphism in the risk of developing T2DM. METHODS: GSTM1 and GSTT1 polymorphisms were genotyped in 87 T2DM patients and 87 healthy control group to analyze their association with T2DM susceptibility by using multiplex Polymerase Chain Reaction (PCR). PCR products were electrophoresed using agarose 2%. Odds ratio (OR) with 95% confidence interval (CI) and P value were calculated using SPSS software (version 21.0). RESULTS: The genotype distribution of GSTM1 and GSTT1 were not different between T2DM patients and healthy control group (p = 0.542, OR= 0.780, CI 95%=0.350-1.737 and p=0.879, OR=1.047, CI 95%=0.577-1.903). The genotype distribution of combination of GSTM1 and GSTT1 were also not not different between T2DM patients and healthy control group (p = 0.640, OR= 0.640, CI 95%=0.224-1.83 and p=0.551, OR=0.721, CI 95%=0.245-2.120. CONCLUSION: In summary, this study showed that GSTT1 null, GSTM1 null, the combination of GSTM1 null and GSTT1 null genotype or combination of GSTM1 null and GSTT1 positive (or contrary) did not have any risk of developing T2DM at Universitas Sumatera Utara Hospital, Medan.

Relevance. Some non-pharmacological methods, such as low-intensity pulsed ultrasound (LIPUS) proved to be effective in ED treatment due to stimulation of penile tissue regeneration and penile hemodynamics increase. The biological methods that utilize cellular growth factors such as platelet-rich plasma (PRP) therapy proved to be effective in stimulation of neovascularization processes and correction of endothelial dysfunction while ED treatment.Local negative pressure (LNP) therapy shall be considered as an auxiliary method to PRP and LIPUS due to its ability to slow down PRP outflow from injection zones. Sildenafil («Strondex») is perspective pharmacological addition to ED treatment protocol. Aim. Evaluation of the efficacy and safety of therapeutic model of combined application of PRP intracavernous injections, LNP, LIPUS and Sildenafil («Strondex») for vasculogenic (arterial and corporal veno-occlusive) ED treatment.Materials and methods. Prospective study was conducted in a group of 64 outpatients with diagnosis of erectile dysfunction of vascular genesis (ICD-10: N48.4) in Men’s Health Clinic (Kyiv, Ukraine). “International Index of Erectile Function-5” (IIEF-5), “Erectile Hardness Score” (EHS) were used as the tools for clinical assessment. Ultrasonic cavernous bodies scanning and pharmacodopplersonography were used for the data validation.Study results. Main group patients underwent treatment consisting of 6 sessions of PRP intracavernous injections and 12 sessions of local penile LIPUS combined with LNP and Sildenafil («Strondex») pharmacotherapy. Control group underwent 12 sessions of local penile LIPUS combined with LNP and the same pharmacotherapy. According to the IIEF-5 data obtained 12 weeks after the study the erectile function improvement was found in 27 patients of the main group and 20 patients of the control group. EHS improved by 1 point or more in 29 and 20 patients correspondingly.Conclusions. Treatment protocol specifying combination of PRP, LIPUS, LNP and Sildenafil («Strondex») is promising due to PRP-therapy augmentation by intracavernous ultrasonic activation of platelet growth factors, but the data obtained require further physiological validation and clinical placebo-controlled studies with involvement of larger groups of population.

Introduction: Modern studies demonstrate an epidemiological and pathogenetic role of systemic hormonal and metabolic disorders in men with prostate adenoma (PA), so it is obvious that a pharmacotherapeutic correction of these disorders can increase the efficacy of the traditional therapy of the disease. Aim of study: To investigate the frequency, relationships among themselves and with PA local parameters of key systemic hormonal and metabolic disorders (obesity, insulin resistance, testosterone deficiency, vitamin D deficiency) and to develop practical algorithms for optimizing diagnosis and management based on an integrative approach. Material and Methods: The results of a comprehensive examination of 160 patients with PA (main group; average age 62.3 ± 4.2 years) and 30 healthy men without PA of the same age (control group), including: collection of anamnesis and complaints; questionnaires; physical, hormonal and sonographic studies, – are presented. Pharmacotherapeutic correction methods were tested in some patients of the main group. The data was processed using descriptive and comparative statistics. Results and Discussion: In the patients with PA, a significantly higher frequency of concomitant systemic hormonal and metabolic disorders formed at a younger age was established, compared to the men without PA, and significantly worse local characteristics of PA compared to the patients with PA without such (p &lt; 0.05). Reliable connections of some studied systemic hormonal and metabolic disorders with one another and with local parameters of PA (p &lt; 0.05) were revealed. A more severe vitamin D deficiency in the patients with PA compared to the control group was revealed, and the safety and a significant positive effect of its drug compensation on the parameters of hormonal and metabolic status and PA in D-deficient men with PA were shown (p &lt; 0.05). Conclusion: The results of the study confirm an important role of the studied systemic hormonal and metabolic disorders in the pathogenesis of PA and the need for their diagnosis and pharmacotherapeutic correction in all patients with PA on the basis of an integrative approach, according to the proposed algorithms.
