AGE CHARACTERISTICS OF PRIMARY DISABILITY OF ADULT POPULATION DUE TO DISEASES OF DIGESTIVE ORGANS IN THE BELGOROD REGION DURING 2010-2018

Проведен сравнительный анализ показателей первичной инвалидности вследствие болезней органов пищеварения у населения старше трудоспособного и трудоспособного возраста в Белгородской обл. в сравнительном аспекте с общероссийскими показателями за 8 лет. Определены ранговые места болезней органов пищеварения в структуре первичной инвалидности. Выявленные основные особенности и тенденции инвалидности представлены применительно к дальнейшей оптимизации оказания медико-социальной помощи и поддержки данного контингента инвалидов. The analysis of indicators of primary disability due to diseases of digestive organs among pension age and working-age population of the Belgorod region is carried out in comparative aspect with the national indicators during 8 years. Rank places of diseases of digestive organs in structure of primary disability were defined. The revealed main features and trends of disability, were investigated to further optimization of medico-social service and improved support to this contingent of disabled people.

The relevance of the development of developing experience and methodological support of the constituent entities of the Russian Federation. To organize activities to provide activities aimed at the development of the direction, the experience of St. Petersburg, as a subject of the leader in the Russian Federation in the development of technologies for accompanied accommodation, was studied and analyzed. The study was conducted on the basis of the results of the annual monitoring study of materials submitted by the executive authorities of 85 constituent entities of the Russian Federation on the organization of technologies for accompanied accommodation.As a result of the study, the main activities in the development of this direction in St. Petersburg were identified: expanding the regional list of social services provided by social service providers; assigning disabled people of working age with intellectual disabilities to a separate social group; developing recommended individual social service programs, including: for disabled people of working age with intellectual disabilities when living in social apartments, development of industry-specific technological regulations for the provision of measures to support disabled people, creation of equal opportunities to enter the social services market for both state and non-state social service organizations, implementation of infrastructure measures, identification of financial sources, etc. The use by the constituent entities of the Russian Federation of recommendations on reforming the regional regulatory legal framework and the implementation of key events will allow introducing the best practices of St. Petersburg in organizing various forms of accompanied residence.

Introduction. The quality of labour potential is one of the most important factors of economic growth, which largely depends on the health status of the working-age population. Today, incidence and prevalence rates in the Russian working-age population are not monitored at the national level. Materials and methods. We conducted a descriptive epidemiological study based on retrospective data on disease incidence in the adult population of the Russian Federation. We also assessed disease incidence and prevalence in the Russian working-age population (men aged 18-59 and women aged 18-54) in 2015-2019 using the method of continuous observation. We estimated and ranked incidence and prevalence rates per 100,000 working-age population of some constituent entities and federal districts of the Russian Federation to establish the territories with the highest and lowest rates. Results. We applied and tested methodological approaches to assessing the morbidity of the working-age population living in different regions of the Russian Federation. Our findings demonstrate a high degree of variability in both incidence and prevalence rates in the working-age population in general and by ICD-10 disease categories. Conclusion. Results of our assessments and ranking of the constituent entities of the Russian Federation by disease-specific incidence and prevalence rates in the working-age population provide an opportunity to areas at risk with specific health status enabling the most efficient management of risks of labour and economic losses. The proven method approaches may be used to address the challenges of public health monitoring.

В статье представлены сведения о значительной численности лиц старше трудоспособного возраста, инвалидов и получателей социальных услуг в стационарной форме и о неблагоприятном прогнозе численности лиц старше 65 лет на 2024 и 2030 гг. в РФ. Это определяет необходимость правового регулирования вопросов их жизнеустройства с соблюдением прав человека и инвалидов на ведение самостоятельного образа жизни. Авторами представлен анализ федеральной и региональной правовой базы в части определения обстоятельств, при которых гражданин признается нуждающимся в социальном обслуживании, участия органов местного самоуправления и подведомственных организаций в оказании социальных услуг, осуществления социального сопровождения, внедрения стационарзамещающих технологий социального обслуживания. В статье рассматриваются тенденции и пути совершенствования правового регулирования социального обслуживания граждан пожилого возраста и инвалидов на дому в ракурсе внедрения стационарзамещающих элементов. The article presents information about a significant number of people older than working age, disabled people and recipients of social services in stationary form and about the unfavorable forecast of the number of people older than 65 years for 2024 and 2030 in the Russian Federation, which determines the need for legal regulation of their lifestyle issues with respect for human rights to lead an independent lifestyle. The authors present an analysis of the federal and regional legal framework in terms of determining the circumstances under which a citizen is recognized as needing social services, the participation of local self-government bodies and subordinate organizations in the provision of social services, the implementation of social support, the introduction of hospital-substituting social service technologies. The article discusses the trends and ways to improve the legal regulation of social services for elderly and disabled citizens at home from the perspective of the introduction of hospital-substituting elements.

The article presents the medical and social characteristics ofprimary disability in the period from 2007 to 2009 and from 2013 to 2015. In the article there are presented medical-social characteristics of the primary disability over the periods from 2007 to 2009 and from 2013 till 2015. There were analyzed extensive and intensive indices of the primary disability which manifest as dynamics characteristics of the public health ofthe adult population of St. Petersburg. There were revealed clear cut downward trends in the number ofpersons for the first time recognized as disabled and the level of the primary disability in a major metropolis. There is established a consistent trend of the increase of specific weight of working-age persons for the first time recognized as disabled in the total number of the disabled adult population. In the compared periods there were revealed trends of increasing of the specific gravity of cases for the first time recognized as disabled at a young age. The use of indices obtained in the study can serve as the basis for the analysis and evaluation of the activities of the Bureau of the Socio-Medical Expertise in the modern conditions.

This article examines the state policy of Japan in the labor market for older and disabled people in the changing demographic situation associated with a decrease in the birth rate, an aging population and a reduction in the country's working-age population. Against the background of increasing social spending, the government is trying to solve the problem of labor shortages by increasing the employment of older people, of course on a voluntary basis. The government's measures are shown to increase the retirement age and to reduce the distance between the retirement age and the maximum permissible age of employment of employees (teinen). The data on changes in the structure of workers' employment after reaching teinen are presented. The policy of attracting disabled people to the labor market in order to improve their social status, improve living conditions and positively influence the economic situation is revealed. Both achievements and shortcomings in solving this issue are noted.

Diabetic retinopathy is a frequent manifestation of long standing diabetes mellitus. According to the World Health Organization diabetes affects nine percent of the adult population worldwide (Diabetes Fact Sheet, 2015). Of all of the common diseases of the eye, diabetic retinopathy has a great impact on the working age population and therefore presents a huge socioeconomic burden in all areas of the world.

The article presents the results of analysis of morbidity of diseases of digestive organs шт the Republic of Armenia. It is established that in the Republic of Armenia during 1990-2015 primary morbidity of diseases of digestive organs in children population reliably increased up to 2.4 times and in adult population - up to 1.9 times. The leading diseases in the structure of primary morbidity among children population is gastroduodenitis (39.7%) and among adult population - gastritis (54.6%). In the Republic, the indices of total morbidity of diseases of digestive organs during the same period of time increased reliably from 57,5‰ to 100,3‰ in children population and from 50,9‰ to 60,2‰ adult population. At the same time, during last 15 years at stabilization of level of morbidity with temporary disability average duration of one case of disease with temporary disability up to 11.6% and primary disability up to 2.9%.

In the Irkutsk region there are recorded high indices of rates of morbidity, disability, mortality rate of the working-age population and low levels of life expectancy of the population, that is confirmed by ranking position levels among the all subjects of the Russian Federation. According to all mentioned indices of health the region is inside the top ten unfavorable regions of Russia. In relation to the problem in the state of health of the adult population the estimation of the causal relationships between environmental factors and certain health indices is actual. The list of studiedfactors included health indices that characterize the harmful working conditions of the working population and basic socioeconomic indices in the region. Estimation of causal-relationship relationships was performed with the use of methods of multivariate analysis - correlation and multiple linear regression. In the selection offactors for the construction of mathematical models of multiple regression there were used methods of the analysis of variables variability, pair correlation coefficients matrix and sequential switching covariates to eliminate the problems of multicollinearity, pre-standardization of indices for the elevation of the numerical stability of regression analysis algorithm. As a result of the execution of the analysis there were constructed statistical models for the dependence in the system variables “environment - public health”, which allowed to identify the most informative regression models for the adult population health according to indices of primary disability of the population, the mortality rate and life expectancy of the working age population. According to results of the analysis there were identified priority factors affecting on the health of the adult population of the Irkutsk region. To these factors there are referred the proportion of workplaces failing to meet sanitary standards for vibration and 8 socio-economic indices of living standards of the population.

The analysis of primary disability as a result of diseases of the urinary system with the outcome of chronic kidney disease among the adult population of Moscow, the Central Federal District and the Russian Federation for the period 2015-2019 has revealed that the extensive indicators for the number of certified as disabled for the first time in Moscow is comparable in terms of primary disability in the Central Federal District and are lower than in the Russian Federation. Intensive indicators in Moscow are lower than in the Central Federal District and the Russian Federation. Extensive and intensive indicators of primary disability in Moscow, the Central Federal District, and the Russian Federation tended to increase. The age structure of the certified as disabled for the first time contingent as a result of diseases of the urinary system in Moscow, the Central Federal District and the Russian Federation do not differ significantly in values and dynamics among young and middle-aged population. The predominance of persons of retirement age was revealed with a tendency to increase their level. The severity of disability was dominated by I group of disabled people with a tendency to increase their specific weight. The second place was taken by disabled people of III group with a tendency to increase their share and a stable level of disability.
