The Faculty of Health Sciences (FIK) is one of the faculty under the Situbondo Ibrahimy University that develops education in health sector, which in its implementation of the learning process do a lot of practicum both in the laboratory or in the classroom. One of the laboratories is midwifery laboratory. Midwifery laboratory of FIK serves the lending of tools, rooms and packages to students and lecturers, both to be used for the learning process, or to the needs of research and community service. The lending transaction process, with many packages containing a variety of tools, requires an effective and efficient recording data system. The purpose of this research is to help the staffs of midwifery laboratory to serve all the processes of lending, returning, procuring and repairing equipment. This information system provides information about student activity in practicing and using tools in the midwifery laboratory, as well as the availability of tools with student ratios so that they can become decision support for faculty leaders in the procurement and monitoring of tool use in the laboratory.

Student daily activity records are kept as documentation tools to monitor the development of students' academic and non-academic abilities in capturing the learning process. The student's daily activity record is carried out by Purba Adhi Suta Elementary School. Purba Adhi Suta Elementary School is located in Purbalingga Regency which is located at Jalan Letnan Jenderal S.Parman Number 19 B Purbalingga. The obstacles faced by Purba Adhi Suta Elementary School in the form of the process of recording student activities are still done manually so that there is often a delay in managing the data of students' daily activity records and delays in the collection of student activity record recap books. The software development method used is a prototype. In this method the steps taken are the identification of user needs, making prototypes, adjusting prototypes, creating new systems, testing the system and using the system. From the results of the benefit test it can be concluded that functionally the system has produced the output as expected. Thus it can be concluded that the Android-Based Student Daily Activity Report Information System at Adhi Suta Elementary School purbalingga regency can help improve time efficiency in the creation of recap of student activity reports.

The Bali Institute of Health Sciences (STIKES) is a higher education institution in the health sector. Education in an institution needs to be monitored especially the lecture process. At present, the process of monitoring lectures is carried out by recording into paper documents. Several problems arise, namely at the end of each semester staff who perform lecture attendance recapitulation must open a sheet per lecturer attendance form sheet. Then from the lecturer attendance form, the staff in charge must type back into the Microsoft Excel application. This resulted in 2 problems, namely the occurrence of delays in reporting and the difficulty of monitoring lectures by Puket I. From these problems a system is needed that is able to provide information on the presence of lecturers and is able to present information on lecture activities in realtime. The design of this system is translated through Data Flow Diagrams and built using PHP language, for display using CSS, and database using MySQL. Testing this system uses blackbox testing. The lecture monitoring information system can display information about the implementation of lectures for Chairperson I Assistant in real time and can facilitate STIKES Bali staff in conducting lecturers' attendance.

This has increased the importance of information systems in the decision-making process. Information systems consist of computer and communication technology, data base management and model management and include activity processing system, management information system, decision support systems, senior management information system, expert systems and office automation systems. Information systems in the health sector aim at the management and provision of preventive and curative health services. The use of information systems in healthcare has the benefits of increasing service quality, shortening treatment processes, maximizing efficiency of the time, labour and medical devices. The use of information systems for clinical decision making and reducing medical errors in the healthcare industry dates back to the 1960s. Clinical information systems involve processing, storing and re-accessing information that supports patient care in a hospital. Clinical information systems are systems that are directly or indirectly related to patient care. These systems include electronic health/patient records, clinical decision support systems, nurse information systems, patient tracking systems, tele-medicine, case mix and smart card applications. Diagnosis-treatment systems are information-based systems used in the diagnosis and treatment of diseases. It consists of laboratory information systems, picture archiving and communication system, pharmacy information system, radiology information system, nuclear medicine information system. This study aims to evaluate the effectiveness of health information system applications in Turkey. The first part of the study focuses on the concept of information systems and the types of information systems in organization structures. In the second part, clinical information systems and applications for diagnosis-treatment systems in Turkey are examined. The study considers the problem of decision-making in the actvites of agricultural enterprises in conditons of environmental uncertainty. The problem is formalized as a multcriteria conditonal optmizaton problem. The novelty of the work is that the factors are considered as dynamic fuzzy quanttes. The choice of the best alternatve is made between the elements of a predetermined fnite set of alternatves. Based on this formalizaton of the decision-making task, six requirements to the informaton system of decision support in the conditons of ecological uncertainty are formulated.
