TÓM TẮT Đặt vấn đề: Bệnh COVID-19 đa dạng từ không có triệu chứng đến có các triệu chứng nhẹ cho đến viêm phổi nặng, hội chứng suy hô hấp cấp tiến triển (ARDS), nhiễm khuẩn huyết suy đa tạng và tử vong. Người cao tuổi, người có bệnh mạn tính sẽ có nguy cơ diễn biến nặng nhiều hơn. Nghiên cứu này nhằm xác định nồng độ lactate và PCT ở những bệnh nhân Covid-19 và xét mối liên quan giữa lactate và PCT trên bệnh nhân Covid-19. Đối tượng và phương pháp: Nghiên cứu mô tả cắt ngang trên 126 bệnh nhân được chẩn đoán nhiễm Sars-Cov-2 bằng xét nghiệm RT PCR. Kết quả: Tuổi trung bình 55,98 ± 17,1 tuổi (4 - 98 tuổi). Bệnh nhân > 60 tuổi chiếm tỉ lệ cao nhất (42,8%). Trung vị PCT: 3,6 (95%CI:3,21 - 3,75) ng/ml; trung vị lactate 1,5 (95%CI:1,21 - 1,91) mmol/L; lactate có tương quan thuận và yếu với procalcitonin với r = 0,241; p < 0,001. Nồng độ procalcitonin > 0,1 ng/ml; lactate > 2 mmol/l ở bệnh nhân Covid-19 chiếm tỷ lệ cao với 89,7% và 39,7%. Kết luận: Chỉ điểm procalcitonin, lactate tăng cao ở bệnh nhân Covid-19. ABSTRACT ASSESSMENT OF SERUM LEVEL OF LACTATE AND PROCALCITONIN IN COVID-19 PATIENTS Background: Sars-CoV-2 has been identified as the cause of acute respiratory infections in Wuhan city, Hubei province, China, and has since spread worldwide. Sars-CoV-2 is capable of aerosol transmission in enclosed, crowded, and poorly ventilated spaces. COVID-19 illness ranges from asymptomatic to mild symptoms to severe pneumonia, acute respiratory distress syndrome (ARDS), sepsis, multiple organ failure, and death. This study aims to determine lactate and PCT levels in Covid-19 patients and examine the relationship between lactate and PCT in Covid-19 patients. Methods: A cross-sectional study was performed on 126 patients diagnosed with Sars-Cov-2 infection by RT-PCR. Results: Mean age was 55.98 ± 17.1 years (range: 4-98 years). Patients more than 60 years old were accounted for the highest rate (42.8%). Median PCT: 3.6 (95%CI:3.21 - 3.75) ng/ml; median lactate 1.5 (95%CI:1.21 - 1,91) mmol/L; lactate has a positive and weak correlation with procalcitonin with r = 0.241; p < 0.001. Procalcitonin concentration > 0.1 ng/ml; lactate > 2 mmol/l in patients with Covid-19 accounted for a high rate with 89.7% and 39.7%. Conclusion: Serum level of procalcitonin and lactate raise highly in Covid-19 patients. Keywords: Covid-19, procalcitonin, lactate.

Background: Perinatal asphyxia is a condition associated with multiple organ dysfunctions inclusive of cardiovascular dys- function. Neurologic predictors of survival have been studied, but little has been reported regarding cardiovascular signs and their role in determining outcome in asphyxia. Objective: The study aimed to determine the relationship between cardiovascular signs and outcomes in asphyxiated new- borns with hypoxic ischaemic encephalopathy. Methods: This was a cross sectional study involving asphyxiated new-born babies recruited within the first 24 hours of life. Hypoxic ischaemic encephalopathy staging was done using Sarnat and Sarnat staging. All patients had a detailed cardi- ovascular examination on admission, after initial resuscitation (30 – 60 minutes) into admission, and were followed till final outcome: discharge or death. Results: Eighty-five asphyxiated new-borns with HIE were studied over seven months. Abnormal cardiovascular-related signs identified in the patients included respiratory distress (64.7%), small volume pulse (57.6%), hypotension (52.9%), hy- poxemia (48.2%) and shock (32.9%). Five babies died. None of the clinical signs had a significant relationship with mortality. Conclusion: Abnormal cardiovascular signs increased with the progression of HIE staging but had no relationship with mortality. Background: The World Health Organization (WHO) recommends the use of tachypnea as a proxy to the diagnosis of pneumonia. Objective: The purpose of this study was to examine the relationship between body temperature alterations and respiratory rate (RR) difference (RRD) in children with acute respiratory infections(ARI). Methods:This cross-sectional study included 297 children with age 2-60 months who presented with cough and fever at the pediatric emergency and outpatient clinics in the Department of Pediatrics, Baskent University Hospital, from January 2016 through June 2018. Each parent completed a structured questionnaire to collect background data. Weight and height were taken. Body temperature, respiratory rate, presence of the chest indrawing, rales, wheezing and laryngeal stridor were also recorded. RRD was defined as the differences in RR at admission and after 3 days of treatment. Results: Both respiratory rate and RRD were moderately correlated with body temperature (r=0.71, p<0.001 and r=0.65, p<0.001; respectively). For every 1°C increase in temperature, RRD increased by 5.7/minutes in overall, 7.2/minute in the patients under 12 months of age, 6.4/minute in the female. The relationship between body temperature and RRD wasn’t statistically significant in patients with rhonchi, chest indrawing, and low oxygen saturation. Conclusion: Respiratory rate should be evaluated according to the degree of body temperature in children with ARI. How- ever, the interaction between body temperature and respiratory rate could not be observed in cases with rhonchi and severe pneumonia. Introduction: Natriuretic peptides such as brain natriuretic peptide (BNP), atrial natriuretic peptide (ANP) and pro-BNP are secreted in response to atrial and/or ventricular stretch. Left to right shunts such as ventricular septal defect (VSD), atrial septal defect (ASD), and patent ductus arteriosus (PDA), are treated medically or surgically. We aimed to evaluate whether the serum level of pro-BNP would be useful to measure the amount of the shunt. Methods: In this cross sectional study, 60 infants and children, in whom physical examinations approved heart murmur, and had undergone echocardiography by which VSD, ASD, or PDA had been proven, were included in the study. The relationship between serum BNP levels and severity of shunt (Qp/Qs) based on echocardiographic and hemodynamic evaluations, was studied. Results: There was a significant relationship between serum level of pro-BNP and the amount of the shunt in the patients with VSD, ASD, and PDA (P=0.01). A positive correlation was seen between pro-BNP serum level and Qp/Qs ratio. The mean ± SE serum level of pro-BNP in patients with Qp/Qs ratio of less than 1.5, equal to 1.5-2, and more than 2 was 30.83±2.4, 217.88±44.6, and 217.13±51.8, respectively showing a significant relationship (P=0.0001). The cut-off point of pro-BNP demonstrating a Qp/Qs ratio more than 1.5 was measured at the level of 40.36 pg/mL, with a sensitivity and specificity of 92% and 79%, respectively. Conclusion: Based on our study, the cut-off point of 40.36 pg/mL or more for pro-BNP, showing a Qp/Qs ratio more than 1.5, can be considered as an indication for interventional procedures.

Acute Respiratory Infections (ARI) are infectious diseases that cause one and better parts of the channel (alveolus) including the adnexal tissue, such as sinuses, ear cavities, and pleura. Environmental saturation is an activity aimed at improving and maintaining standard environmental condition fundamentals that affect humans. Improper home sanitation is associated with high health problems, such as acute respiratory infections. The purpose of this study is to determine the relationship between the home environment with the incidence of ARI disease in Tuapukan Village, Kupang Timur Sub-district, Kupang District. This study uses an observational method with cross-sectional study design. The population of this study was 519 and the sample was 81. The bivariate analysis used Chi-Square statistical test with 95% significance level. The results shows that variables related to ARI occurrence were house wall (p=0,029), house ventilation (p=0,011), house lighting (p=0,003), occupancy density (p=0,021) and smoking habit (p=0.002). It is recommended that the societies should keep the cleanliness of the house so it will not be a place for the growth of germs; thus the societies are able to prevent ARI disease.

Many people following different religions undergo fasting as their religious courtesy but the effects of being in a Lenten diet among Russians have not been fully studied yet. The research evaluates the ef-fect of fasting on nutritional status during Great Lent. Methodology. The study was performed among 44 people aged 23 to 65 years, including 9 men and 35 women. A physical examination was carried out and their complaints about well-being were noted carefully. The nutrient composition of the diet was determined by the method of frequency analysis of food consumption during the period of the Great Lent. A general blood test was done to assess anaemia. Results. High rate of vitamins A, D, H, and B12, calcium, zinc, chromium, iodine, and molybdenum deficiencies in diet were observed. Many patients complained of dyspepsia, skin dryness, skin peeling, high affinity towards respiratory infections, fatigue, and irritation. The given symptoms were correlated with decreased intake of iodine, zinc, molybdenum, chromium, vitamins H and B12. Conclusion. People who follow Great Lent are recommended to consume a high amount of various fruits and vegetables, nuts, sea algae, green leafy vegetables, thermally processed legumes and grains. For the prevention of vitamins D and B12 deficiencies, it is suggested to consume fortified foods and supplements.

AbstractBACKGROUNDCOVID-19, the disease caused by the new coronavirus (SARS-CoV-2), became a pandemic in 2020 with mortality rate of 2% and high transmissibility, which makes studies with an epidemiological profile essential.OBJECTIVESTo characterize the population that performed the SARS-CoV-2 molecular and serological tests in Carlos Chagas-Sabin laboratories in Cuiabá.METHODSA retrospective cross-sectional study was carried out with all the samples collected from nasal swab tested by RT-PCR and serological for SARM-CoV-2 IgM / IgG from the population served between April and July 2020.FINDINGSIn the analyzed period, 11,113 PCR-Covid-19 exams were registered. Of this total of cases, 3,912 (35.20%) tested positive, while 6,889 (61.90%) did not detect viral RNA and 312 (2.80%) of the visits resulted as undetermined. The peak of positive RT-PCR performed in July (n = 5878), with 35.65% (n = 2096). A total of 6,392 tests performed on SOROVID-19, with a peak of 1161 (18.16%) of the positive tests for SARS-CoV-2 in July.MAIN CONCLUSIONSMolecular positivity and serological tests, both peaked in July 2020, were mostly present in women aged 20-39, characterizing Cuiabá as the epicenter of the Midwest region in this period due to the high rate of transmissibility of SARS-CoV-2.

Community-acquired pneumonia (CAP) remains a common and serious illness, in spite of the availability of potent new antimicrobials and effective vaccines. Despite Nepal being one of the four developing countries accounting for 40.0% of global acute respiratory infections, studies on CAP are limited and the status of adult pneumonia in our community is unknown. This cross-sectional study reviewed the clinical, bacteriological, radiological profile of 100 cases of adult CAP and followed them during the hospital stay for the outcome. The age group with the highest incidence was 60-79 years with females (55.0%) being more affected than males (45.0%). Risk factors were present in 86.0% of cases, chronic obstructive pulmonary disease (COPD), and smoking was the most common, each present in 43.0% of cases. The most common presenting feature was cough (89.80%) followed by sputum production (78.60%), fever (67.30%), shortness of breath (63.30%), chest pain (38.80%), gastrointestinal symptoms (26.50%), altered sensorium (13.30%), and hemoptysis (13.30%). Only 48.0% of patients had leukocytosis. Klebsiella pneumoniae was the most frequent organism isolated (n=4) followed by Pseudomonas aeruginosa (n=3). Fungi were isolated in 3 cases. Lobar pneumonia was seen in 99.0% of cases with the right lower zone being the most commonly involved zone on chest x-ray. Severe pneumonia with CURB-65 (confusion, blood urea nitrogen, respiratory rate, blood pressure, age>65) Score ≥3 was seen in 15.0% of cases. The mean hospital stay was 7.55 days with 28 cases requiring ICU admission and 5 cases of mortality.

The new coronavirus has been affecting health worldwide and essential service workers are continually exposed to this infectious agent, increasing the chance of infection and the development of the disease. Thus, this study aimed to estimate the frequency of infection and seroprevalence for SARS-CoV-2 in military firefighters in a city in Northeastern Brazil in January 2021. An observational cross-sectional study was carried out with 123 firefighters who answered a brief questionnaire to collect socio-epidemiological data and underwent RT-PCR and immunofluorescence test (IgM and IgG). The results found reveal a positive seroprevalence, with a high rate of infection in this class of workers, since they are essential service professionals who are exposed to risk due to their working hours, in addition to sharing some spaces and work materials. Besides, there were significant associations between positivity for IgG and IgM, as well as for positive RT-PCR prior to the study and the presence of IgG, with odd ratios of 3.04 and 4.9, respectively. These findings reinforce the need for immunization in this category, whose line of service hinders the adoption of distancing measures, since in many situations physical contact is inevitable.

Purpose To assess the clinical characteristics of subjects with gender dysphoria (GD). Method A cross-sectional study of adults with GD. Symptoms of anxiety and depression were measured using the Hospital Anxiety and Depression Scale (HADS). Sociodemographic data, clinical data and life habits were recorded. Results Total of 44 subjects participated in the study: 36 (82%) trans women and 8 (18%) trans men. Forty-three (98%) of the GD patients had anxiety (36 [100%] trans women and 7 [87.5%] trans men), and 36 (82%) had depression (29 [80.5%] trans women and 7 [87.5%] trans men). Suicide had been attempted by 32 (73%) subjects. The rates of depression were lower among the subjects living with partners, parents, or other people than among those living alone (p = 0.03), and it was also lower among the subjects who were married compared to those who were dating or single (p = 0.03). Conclusion Improving the relationship status may reduce the prevalence of depressive symptoms in GD patients. There was a high rate of attempted suicide in this sample.

Abstract Background Limited and inconsistent epidemiological evidence for the relationship between vitamin D level and risk of osteosarcopenic obesity. We aimed to find out the relationship between serum level of 25-(OH)-D3 and osteosarcopenic obesity.Methods This study was a cross-sectional study. Residents from nine communities of the Tiexi District of Shenyang City were enrolled from May to October 2017. We included 4,506 eligible participants (1,601 men) for analyses. Participants were asked for an overnight fast. Thereafter, Blood samples were collected, and serum level of 25-(OH)-D3 was estimated using liquid chromatography–tandem mass spectrometry. We undertook logistic regression models adjusted for most known osteosarcopenic obesity risk factors. Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated.Results Our study included a total of 4,506 participants (2,905 women). In multivariable-adjusted analyses, compared to the lowest tertile, a higher 25-(OH)-D3 level (OR was 0.77, 95% CI was 0.61, 0.98; and P for trend was 0.026) was inversely correlated with osteosarcopenic obesity. A positive association between vitamin D deficiency and osteosarcopenic obesity was found (OR was 1.33, 95% CI was 1.10, 1.62). Inverse relationships between serum level of 25-(OH)-D3 and low bone mass, as well as low muscle mass were found (P values for trend were 0.035 and 0.014, respectively). However, vitamin D deficiency only showed a positive correlation with low muscle mass (OR=1.26, 95 %CI: 1.07, 1.44).Conclusion An independent inverse dose-response association of serum 25-(OH)-D3 level with osteosarcopenic obesity, as well as its compositions was found. Our findings implied that serum 25-(OH)-D3 could be a good predictor of osteosarcopenic obesity in older northeastern Chinese.
