Downsizing as a Public Policy and Downsizing Practices in the Health Sector

Health services provided by foundations in the pre-Republic period became more systematic and organized after the foundation of the Turkish Republic. Healthcare services, offered by the state in the period between 1920 and 1980 and accepted as one of the main duties of the state, became one of the areas in which the state wanted to withdraw after 1980. As a result of the neoliberal wave in the globalized world, policies aimed at downsizing healthcare services in Turkey have been developed and implemented since the 1980s. Accelerated downsizing practices by the Health Transformation Project (2003) continues today. In this study, literature review and descriptive analysis method were used.The aim of this study is to discuss the downsizing practices in the health sector of Turkey. This debate will take place over two periods (before 2002 and after 2002). Especially, the post-2002 period and works on behalf of health sector downsizing in this period will be discussed as to whether there is downsizing or not.

<p>Until 2016, very few works had investigated the use of the VSM. With this research, we will discover if the situation has changed in the last 3 years. In the lean manufacturing context, different techniques that help the continuous improvement process can be used (Marin-Garcia &amp; Bonavia, 2011; Marin-Garcia &amp; Carneiro, 2010; Marin-Garcia et al., 2012; Scott, 2001). One is the Value Stream Map (VSM) (Coetzee et al., 2016; Marin-Garcia &amp; Mateo Martínez, 2013; Vidal-Carreras et al., 2015). This publication is a protocol (Marin-Garcia, 2015; Marin-Garcia, 2019) that aims to promote research transparency and replication. The concepts investigated in it are defined (VSM and health services sector), a research niche is justified, and the search and codification procedure of the systematic literature review is established. Although there are different versions of the VSM (Dinis-Carvalho et al., 2018; Hines &amp; Rich, 1997; Shou et al., 2017), we will focus on that proposed by Rother and Shook (1998). The main peculiarity of the version by Rother and Shook (1998) is that it graphically shows the flow of information and the flow of materials in the same diagram (Lucherini &amp; Rapaccini, 2017; Shou et al., 2017; Vidal-Carreras et al., 2015), which is necessary to complete a project, a product or a service (Bevilacqua et al., 2014; Lucherini &amp; Rapaccini, 2017). It is also very intuitive and easy to understand, even by non-technical people (Lucherini &amp; Rapaccini, 2017).</p><p>The VSM version that we have chosen is usually applied via standardized symbols (Lucherini &amp; Rapaccini, 2017; Vidal-Carreras et al., 2015) following a 4-stage procedure (Rother &amp; Shook, 1998; Shou et al., 2017): 1) select a product family (each VSM represents a family of sufficiently homogeneous products to represent the process); 2) draw the current VSM; 3) model the improved process by drawing the desirable future VSM; 4) implement actions to obtain a similar process to the future VSM. These four stages can be split into eight in other implementation versions (Tapping, 2007; Tapping et al., 2002; Tapping &amp; Shuker, 2003). The above procedure allows value-added (VA) and nonvalue-added (NVA) activities and initiating actions to be identified to improve the proportion of VA versus NVA (Bevilacqua et al., 2014; Shou et al., 2017; Vidal-Carreras et al., 2015). Our goal is to include any healthcare level (primary care, secondary care - medical specialists, hospitals, referral centers for rare diseases, and geriatric or disability care). We wish to explore the use in organizations of any country worldwide whose ownership is public, private or a nonprofit foundation. We will focus on patient health services. We will not include the pharmaceutical industry or the operation of governmental or nongovernmental public health structures (e.g. ministries, the Red Cross or similar). Different literature reviews on the VSM have been published. Some focus on analyzing several sectors, predominantly manufacturing. Previous research seems to indicate that the VSM allows the transparency of the process to improve by making it much more understandable for the agents involved in it (Shou et al., 2017; Vidal-Carreras et al., 2015); reduce process times (lead times) (Shou et al., 2017) and inventories (Shou et al., 2017). However, these results come mostly from repetitive manufacturing contexts (linked to the automotive or consumer electronics sectors, or their auxiliary industries), and normally from Anglo-Saxon countries. There do not seem to be enough publications in order to generalize these results to all kinds of contexts. Some publications reveal that the barriers from using such tools can overcome facilitators in public service contexts (Marin-Garcia et al., 2018b).</p><p>Very few reviews have focused specifically on the VSM and the health services sector (Nowak et al., 2017; Vidal-Carreras et al., 2015). Both conclude that there is not enough material to provide evidence for and a conclusive answer to our research questions. The systematic review that we propose in this protocol intends to answer (in a future publication) the following questions: 1) what is the VSM research gap that applies to the health services sector that currently exists?; 2) is the VSM being used in hospitals or other health centers?; 3) what VSM version is common in health sector publications?; 4) collect examples of the VSM in hospitals/heath centers; 5) how was the VSM used in the hospitals/health centers that have applied it?; 6) what problems and/or difficulties have arisen while drawing the VSM or after drawing it? Different programs will be used for the bibliometric analysis (see details in Marin-Garcia and Alfalla-Luque (2019)). First, the R Bibliometrix package (Aria &amp; Cuccurullo, 2017; Garfield, 2004; Wulff Barreiro, 2007) and also the suitability of SciMAT (Cobo et al., 2012; Santana &amp; Lopez-Cabrales, 2019) to visualize thematic maps and strategic maps will be tested.</p>

PurposeIndustry 4.0, artificial intelligence and digitalization have got a momentum in health care. However, scholars and practitioners do not agree on their implications on health services' quality and effectiveness. The article aims at shedding light on the applications, aftermaths and drawbacks of industry 4.0 in health care, summarizing the state of the art.Design/methodology/approachA systematic literature review was undertaken. We arranged an ad hoc research design, which was tailored to the study purposes. Three citation databases were queried. We collected 1,194 scientific papers which were carefully considered for inclusion in this systematic literature review. After three rounds of analysis, 40 papers were taken into consideration.FindingsIndustry 4.0, artificial intelligence and digitalization are revolutionizing the design and the delivery of care. They are expected to enhance health services' quality and effectiveness, paving the way for more direct patient–provider relationships. In addition, they have been argued to allow a more appropriate use of available resources. There is a dark side of health care 4.0 involving both management and ethical issues.Research limitations/implicationsIndustry 4.0 in health care should not be conceived as a self-nourishing innovation; rather, it needs to be carefully steered at both the policy and management levels. On the one hand, comprehensive governance models are required to realize the full potential of health 4.0. On the other hand, the drawbacks of industry 4.0 should be timely recognized and thoroughly addressed.Originality/valueThe article contextualizes the state of the art of industry 4.0 in the health care context, providing some insights for further conceptual and empirical developments.

This study aims to describe and reveal the presentation form of Galombang dance at a wedding party in Jorong Koto Kociak Tujuah Koto Talago village, Guguak sub-district, Lima Puluh Kota district. This is a qualitative research using a descriptive analysis method. The object of the research was Galombang dance at a wedding party in Jorong Koto Kociak, Tujuah Koto Talago village, Guguak sub-district, Lima Puluh Kota district and was focused on the form of its presentation. The data were collected through literature review, observation, interview, and documentation. The data analysis was done by using triangulation technique which compared the data from observation to those from interview and documentation data. The results of the study generally show that the Galombang dance is performed in groups by11 dancers. It is performed for eight minutes in front of the bride's house, and it is performed at noon after the procession. Interestingly, the Galombang dance in Jorong Koto Kociak, Tujuah Koto Talag village, is performed by 50 year-old dancers and over (elderly). The presentation form of Galombang Dance at a Wedding Party in Jorong Koto Kociak, Tujuah Koto Talago village, Guguak sub-district, Lima Puluh Kota district is a representational form.The elements of the presentation form of this dance are as follows: (1) The motion of respect, the opening motion, the maagiah kaba motion, the maimbau motion, the basuluah motion, the bagalanggang motion, and silang jentik motion. (2) the Galombang Dance uses a floor pattern of two rows backwards and three carano bearers.(3) The music that accompanies the Galombang dance consists of: bansi, gandang, talempong, and tambourine. (4) The property of Galombang Dance isa carano. (5) The make-up used is pretty makeup. The dancers wear baju kuruang basiba, skirts, scarves, tikuluak kompong and accessories such as necklaces. (6) In addition, the Galombang dance is performed in front of the bride's house.Keywords: Presentation, Galombang Dance, Wedding Party

Health conditions in Pakistan are very bad. Government and non-governmental organizations are trying to meet international standard of health services. Some healthcare providers are educated, trained and certified from the government but some are unqualified and not certified. In this research two national newspapers are analyzed the daily Jang Urdu and the daily Dawn English. These newspapers cover healthcare services such as indoor patient, outdoor patient maternity and other health services in news stories. These services are also seen in the Literature Review. The researcher analyzed on coverage of healthcare in newspapers the daily Jang and the daily Dawn. This research has been performed by using content analysis technique under quantitative methodology. Media agenda setting theory was adopted in the theoretical framework as in the literature review. The statistical analyses of data analysis have been done in the form of frequency distribution and correlation analysis. SPSS is used to analyze the content of data. The results show that daily Jang is giving more coverage to health-related news stories than daily Dawn.

The new challenges of globalisation have made it essential to achieve and sustain a high level of efficiency in the provision of healthcare. Even though the Government of Maharashtra clearly views assuring efficiency and providing public health services to the people as an important policy objective, data reveals that the amount of resources spent on public healthcare especially public hospitals is very meagre as compared to the needs of the population. This study attempts to understand the shift from public to the private health sector by the users of health services in the city of Mumbai. This changing trend towards increasing use of private hospitals for healthcare services was analysed to understand the specific reasons behind the privatisation of health services in this metropolitan city. Using primary survey, the analysis revealed that majority of the users preferred private hospital services due to various reasons like cleanliness, better infrastructure, efficient and competent doctors and other staff. The results also indicated significant inefficiencies in the functioning of the public hospitals as noted from the responses of the users.

The employee professionalism and behavior development are important issues related to their job performance, job satisfaction, and future career. The future of human resource’ needs are determined by the organization’s mission, goal, and strategiy. This research used literature review and other study sources related to the topic using descriptive analysis method. The view study shows that demand for employees are result of demand for the organization’s products or services, more productivities, to reach high performances of revenue and profit. Based on that, management can attempt to establish the number of mix employees needed attempt to reach it. Company’s compensation and benefit program didn’t allow for giving rises to employee who stayed in the same position. The only way for management to reward the top performers was to give them a bonus or promote them to another position. It is more important to adjust the individual professionalism and behavior with the organizational culture and behavior through employee training and course program. The best opportunity for an employee is to catch on job at or to be a manager in another country to get the different experience than their host country as a global attitude. This is a global-oriented view that focuses on using the best approaches.

Tulisan ini bertujuan mengungkap konflik batin tokoh-tokoh perempuan dalam novel Tempurung. Sumber data penelitian ini adalah novel Tempurung karya Oka Rusmini yang diterbitkan tahun 2010. Pengumpulan data dilakukan dengan teknik kepustakaan. Penelitian ini menggunakan teori psikoanalisis sosial Karen Horney. Metode yang digunakan dalam tulisan ini adalah metode deskriptif analisis dengan pendekatan psikoanalisis. Pembahasan ini menghasilkan hal-hal berikut. Pertama, adanya hubungan buruk orang tua-anak, dan keadaan budaya dan tradisi telah menciptakan berbagai peristiwa yang mendorong munculnya konflik batin tokoh-tokoh perempuan. Kedua, tindakan-tindakan yang dilakukan tokoh-tokoh perempuan untuk menanggulangi atau me- ngurangi konflik batinnya diekspresikan dalam tindakan balas dendam. Abstract: This paper aims to reveal the inner conflicts of women characters in the novel of Tempurung. The source data of this study is Tempurung, a novel of Oka Rusmini which was issued in 2010. The data was collected by librarian techniques. This study uses the theory of Karen Horney’s social psychoanalysis. The method used in this paper is descriptive analysis method with the approach of psychoanalysis. The discussion results in the following. First, the bad relationship of parent-child and the state of culture and tradition have created a variety of events that encourage the emergence of inner conflicts of the women characters. Second, the actions taken by the women characters to overcome or reduce their inner conflicts expressed in acts of revenge. In running the government, a State must run the economy well. This affects the country for the prosperity of society development in order to achieve the objectives set by the State Act. One aspect in the implementation of the necessary development funding comes from tax revenue. The purpose of this study was to determine the mechanism of calculation, recording and reporting of Income Tax Article 23 on the procurement of goods in this respect rental services heavy loads PT.William Perkasa Makmur. The analysis method used in this research is descriptive analysis method. The results showed that as a company engaged in the field of contracting and rental of heavy equipment, PT. William Makmur Perkasa continue to provide tools that have the best quality and the weight to be able to sustain the work. The company realizes the development in this area both by the Government and private parties, seen also with the advent of the projects undertaken by the Government to prepare the facilities and infrastructure to support the development of the area, of course, which is a prerequisite for the entry of investors in investing. Laws and regulations on taxation applicable always changes the company should remain always pay attention to it. This is important so that it can determine the amount of income tax 23 in accordance with the existing rules.

Data collection techniques were carried out through interviews and literature review. This study foundthat the community’s local wisdom based on its adaptability to use agricultural technology at the Community ofKasepuhan Sinar Resmi is related to environmental preservation and the effort to make the environment useful for life
