During the COVID-19 pandemic, the rush to scientific and political judgements on the merits of hydroxychloroquine was fuelled by dubious papers which may have been published because the authors were not independent from the practices of the journals in which they appeared. This example leads us to consider a new type of illegitimate publishing entity, ‘self-promotion journals’ which could be deployed to serve the instrumentalisation of productivity-based metrics, with a ripple effect on decisions about promotion, tenure and grant funding, but also on the quality of manuscripts that are disseminated to the medical community and form the foundation of evidence-based medicine.

Purpose: Peer review of a proposed treatment plan is increasingly recognized as an important quality activity in radiation medicine. Although peer review has been emphasized in the curative setting, applying peer review for treatment plans that have palliative intent is receiving increased attention. This study reports peer review outcomes for a regional cancer center that applied routine interprofessional peer review as a standard practice for palliative radiotherapy. Methods and Materials: Peer review outcomes for palliative radiotherapy plans were recorded prospectively for patients who began radiotherapy between October 1, 2015, and September 30, 2017. Recommended and implemented changes were recorded. The content of detailed discussions was recorded to gain insight into the complexities of palliative treatment plans considered during peer review. Results: Peer review outcomes were reviewed for 1,413 treatment plans with palliative intent. The proportions of detailed discussions and changes recommended were found to be 139 (9.8%) and 29 (2.1%), respectively. The content of detailed discussions and changes recommended was categorized. Major changes represented 75.9% of recommended changes, of which 84.2% were implemented clinically. Conclusion: Many complexities exist that are specific to palliative radiotherapy. Interprofessional peer review provides a forum for these complexities to be openly discussed and is an important activity to optimize the quality of care for patients with treatment plans that have palliative intent.

2%. The strength and physico-mechanical properties of the shell of rapeseed in terms of its destruction and removal are investigated. It is established that the most rational is a two-stage method of seed hulling: the 1st stage – on roller mills, the 2nd – on a centrifugal hulling machine. The parameters of the centrifugal hulling machine to provide the minimum required linear speed of 10,81 m/s are defined: the angle of inclination of the turnback plate (90 ± 2)°, the rotational speed of drum (2400 ± 150) RPM. It is established that the correctly selected conditions and the angle at which the impact occurs, can significantly reduce the amount is not completely hulling seed and the substance of the split nucleus. At the same time, with a free impact on the turnback plate at an angle different from 88–91°, or the supply of an insufficiently uniform flow of seeds, the content is not completely hulling seed increases significantly. On hulling of 1 kg of seeds it is necessary to spend 58,45 J. Thus, according to the calculated data, for rape seeds with a diameter of 1,5 mm, the rotational speed of drum should be in the range from 2221 to 2565 RPM when the seed is struck at an angle of 90°. The diameter of rapeseed grown in the Siberian region ranges from 0,8 to 1,8 mm. It was found that drying of rapeseed for 30–40 s at a temperature of 100°C, can increase the efficiency of destruction of the shell to its complete separation from the core of rapeseed. The presented results make it possible to improve the quality of cleaning of rape seeds from the shell and improve the quality characteristics of the obtained oil, facilitate the subsequent technological processes of obtaining edible oil, reduce the cost of refining and deodorization by 2%.

Background. Much has been written about the care of the hopelessly ill adult, but there is little guidance for pediatric health care professionals in the management of children who are critically or terminally ill. Methods. Through a 3-day meeting in Tarrytown, NY, attended by a group of pediatricians and others directly involved in these issues, a principled approach was developed for the treatment of, and health care decision-making for, children who are gravely ill. Results. The group agreed that the needs and interests of the child must be the central focus of any treatment plan and that the child should be involved to as great extent possible, consistent with developmental maturity, in the decision-making process. Quality of future life should be viewed as being relevant in all decisions. Parents are believed to be the natural guardians of children and ought to have great latitude in making decisions for them. However, parental discretion is not absolute and professionals must maintain an independent obligation to protect the child's interests. Conclusions. Decision-making should be collaborative among patient, parents, and professionals. When conflict arises, consultation and ethics committees may assist in resolution. When cure or restoration of function is no longer possible, or reasonable, promotion of comfort becomes the primary goal of management. Optimal use of pain medication and compassionate concern for the physical, psychological, and spiritual well-being of the child and family should be the primary focus of the professionals caring for the dying child.

There are over 300 high schools in the Republic of Moldova, most of which do not have 10-12th parallel classes, many of which also lead to the lack of qualified teachers in some subjects, which has a negative impact on the quality of students education. The optimization of the high school network can be achieved by keeping only about 100 of their existing number, planning them in new type institutions: district high-schools, boarding schools and mixed high schools of both these categories.
