Setting Time of Calcium Hydroxide from Indonesian Limestone Paste with Various Solvent Vehicle for Intracanal Medicament

Calcium hydroxide (Ca(OH)2 has been recently synthesized from natural Indonesian limestone to be used as an intracanal medicament for root canal infection. Ca(OH)2 is applied into the infected root canal in a non-setting paste form to release calcium and hydroxyl ions which elevates the pH and provide an antimicrobial effect to pathogenic bacteria. To form an injectable paste, Ca(OH)2 powder has to be mixed with a proper solvent to produce optimal consistency, ion dissociation, and maintain its property as a non-set material. Solvent is an important factor affecting ion dissociation and preserving its non-setting paste condition. The aim of this study is to synthesize Ca(OH)2 powder from Indonesian limestone, and evaluate the setting time of Ca(OH)2 paste from mixture of Ca(OH)2 powder synthesized from Indonesian limestone (limestone Ca(OH)2) with various solvent, to evaluate which solvent serve best to prevent the Ca(OH)2 paste from setting, to form an ideal paste be used as an intracanal medicament. This study consists of 5 groups (n=5); commercially Ca(OH)2 paste (Calcipex II) as positive control, Ca(OH)2 powder (Merck) + distilled water as negative control, limestone Ca(OH)2 powder + natrium carboxy methylcellulose (Na CMC) as group 1, limestone Ca(OH)2 powder + propylene glycol (PG) as group 2, and limestone Ca(OH)2 powder + polyethylene glycol (PEG) as group 3. Setting time evaluation was measured according to ISO 9917 by vicat needle in 37°C to mimic the physiological body condition. Results were analyzed by One Way Anova test and Post Hoc Tukey test. The result of this study showed that the setting time of Ca(OH)2 paste mixed with Na CMC solvent was 1:04 hours, PG 72:15 hours, and PEG did not harden until 7 days of observation. PEG is a hygroscopic high viscosity solvent, resulting in low and steady molecule interaction, thus prolonged its setting time. From this study it can be concluded that PEG inhibit Ca(OH)2 setting reaction up to 7 days and might be used as solvent for Ca(OH)2 paste as intracanal medicament.

Background and aim: Comparison of different irrigation and agitation methods for the removal of two types of calcium hydroxide medicaments from the root canal walls.Methods: Fifty extracted single rooted teeth were selected for this study. After decoronation, the root canals of these teeth were prepared to the size F3 (30 no.) using rotary ProTaper file system. These samples were randomly divided into four groups. Group 1 (n=20) were filled completely with water based calcium hydroxide (CH), Group 2 (n=20) were filled with oil based CH using lentulo spiral, Group 3 (n=5) - the positive control group received the CH as intracanal medication, but no subsequent removal, Group 4 (n=5) - the negative control did not receive CH placement. Further on, Group 1 and Group 2 were divided into four sub-groups (n=5). In sub-group A we performed conventional syringe irrigation with side-vented needle sub-group B) manual dynamic agitation, sub-group C sonic agitation using endoactivator, sub-group D passive ultrasonic irrigation (PUI). Roots were split longitudinally into mesial and distal halves. Digital images of the root canal walls were acquired by a Dental Operating Microscope (DOM) and assessed by using a scoring criteria at different thirds (coronal, middle and apical) of the root canal as follows: score 1, score 2, score 3, and score 4. Data were analyzed applying one-way analysis of variance (ANOVA) and Tukey’s multiple comparison tests at a 95% confidence interval (P < 0.05).Results: Statistically significant differences were not found between the experimental groups and the negative group in any one third of the root canal (P>0.05). However, a difference did exist between the experimental groups and the positive control group (P<0.05). None of the experimental groups totally removed CH substances from root canal walls.Conclusion: Among all experimental groups, removal of CH was best achieved by sonic agitation using endoactivator followed by passive ultrasonic irrigation (PUI), manual dynamic agitation and conventional syringe irrigation with side-vented needle.

Recurrent endodontic infections of treated teeth are mainly linked to microbial persistency in the root canal system. Enterococcus faecalis is a bacterium that is often identified in these recurrent infections. Calcium hydroxide (Ca(OH)2) is a widely used root canal medicament and is now being developed to be produced from natural materials. Ca(OH)2, which is synthesized from natural limestone from Palimanan, Indonesia, has a structure similar to Ca(OH)2, which is often used as a root canal medicament. The purpose of this study was to determine the antimicrobial property of Ca(OH)2 paste synthesized from natural limestone compared to conventional Ca(OH)2 paste. This study is an in vitro experimental study consists of 3 groups (n=24, namely, conventional Ca(OH)2 paste, limestone synthesized Ca(OH)2 paste, and saline as the negative control, for 2 intervals of time and evaluated by Total Plate Counting (TPC) method. Samples were mandibular first premolar root canal prepared by root canal preparation and contaminated with E. faecalis (ATCC 29212). The infected root canals of each group were treated by the medicaments, and samples from the root canals were taken on day 7 (n=4) and 14 (n=4). Results showed that both Ca(OH)2 groups had antimicrobial activities against E. faecalis. The limestone Ca(OH)2 paste group had the least amount of bacterial colonies on the 7th day compared to other groups. In contrast, on the 14th day, the conventional Ca(OH)2 paste had the least bacterial colonies. It can be concluded that the Ca(OH)2 paste synthesized from natural limestone has an antimicrobial effect on E. faecalis, the antimicrobial effect was higher on the 7th day but decreases on the 14th day, whereas the conventional Ca(OH)2 had a higher antimicrobial effect on day 14 compared to day 7. Further studies are expected to improve the long-term antimicrobial effectiveness and sustainability of the natural limestone synthesized Ca(OH)2 paste.

Background: Calcium hydroxide is a root canal dressing material that is widely used in dentistry because of its ability to regenerate hard tissue and eliminate bacteria. According to Ferreira et al. (2015), Fusobacterium nucleatum was found in 90% of teeth that had a root canal treatment done using calcium hydroxide as an intracanal medication. Due to this shortcoming of the antibacterial power of calcium hydroxide, additional research on alternative ingredients that can be combined with calcium hydroxide to improve its antibacterial power is necessary. Propolis is a natural material that has high antibacterial power and has long been used in dentistry. The addition of propolis to calcium hydroxide is expected to improve the antibacterial power of calcium hydroxide without eliminating its function in terms of regenerating hard tissue. Purpose: To find out how effective is the antibacterial power of a combination of calcium hydroxide and propolis against Fusobacterium nucleatum. Methods: The research was carried out using 4 treatment groups consisting of 6 samples for each group. Group 1 is given a combination of calcium hydroxide and propolis with a ratio of 1:1, group 2 with a ratio of 1:1.5, group 3 with a ratio of 1:2, and group 4 is a positive control of calcium hydroxide and sterile aquadest suspension. Each sample was put into a test tube containing BHIB and a suspension of Fusobacterium nucleatum, incubated at 37ºC for 24 hours, and vortexed for 1 minute. A total of 0.1 ml of bacterial inoculum was taken from each sample and then put into the MHA and grown for 24 hours. The number of Fusobacterium nucleatum colonies grown on MHA was calculated and expressed using the Colony Forming Unit (CFU). Results: There were fewer colonies of Fusobacterium nucleatum in the treatment group compared to the control group. Conclusion: The combination of calcium hydroxide and propolis has an effective antibacterial power against Fusobacterium nucleatum which the ratio of 1:2 is more effective than ratio of 1:1,5 and 1:1.

The objective of this study was to analyze the time necessary for calcium hydroxide to eliminate microorganisms in infected canals. A total of 168 human anterior teeth were prepared and sterilized. One hundred sixty two teeth were inoculated with suspensions of S. aureus, E. faecalis, P. aeruginosa, B. subtilis, C. albicans. Three teeth were used as negative control and three as positive control. Root canals were irrigated with saline and filled with calcium hydroxide paste (CHP). At intervals of 1 minute, and at 7, 15, 21, 27, 30, 45, 60, and 90 days, CHP was removed, samples were collected and immersed in Letheen Broth (LB). Microbial growth was analyzed by two methods, turbidity of the culture medium and subculture on a Brain heart Infusion. After looking for medium change, an inoculum of 0.1 mL obtained from LB was transferred to 7 mL of Brain Heart Infusion (BHI), and subsequently incubated at 37°C for 48 hours. Microbial growth was checked by turbidity of the culture medium and in some cases by Gram stain. All assays were carried out in triplicate under aseptic technique. The results indicated that the antimicrobial effect on the cultures of S. aureus, E. faecalis, P. aeruginosa, B. subtilis, C. albicans and one mixed culture in infected canals by CHP occurred in 60 days.

The aim of this study was to compare the vertical fracture resistance of roots obturated with different root canal filling materials and sealers. Crowns of 55 extracted mandibular premolar teeth were removed to provide root lengths of 13 mm. Five roots were saved as negative control group (canals unprepared and unfilled). Fifty root canals were instrumented and then five roots were saved as positive control group (canals prepared but unfilled). The remaining 45 roots were randomly divided into three experimental groups (n=15root/group) and obturated with the following procedures: in group 1, glass ionomer-based sealer and cone (ActiV GP obturation system); in group 2, bioceramic sealer and cone (EndoSequence BC obturation system); and in group 3, roots were filled with bioceramic sealer and cone (Smartpaste bio obturation system). All specimens were tested in a universal testing machine for measuring fracture resistance. For each root, the force at the time of fracture was recorded in Newtons. The statistical analysis was performed by using Kruskal-Wallis and post hoc test. There were no significant differences between the three experimental groups. The fracture values of three experimental and negative control groups were significantly higher than the positive control group. Within the limitations of this study, all materials increased the fracture resistance of instrumented roots.

Introduction: Discoloration can be caused by intrinsic or extrinsic factors. One of the discoloration treatments is teeth whitening. Teeth whitening process usually uses chemicals such as hydrogen peroxide or carbamide peroxide which can cause side effects, namely gingival irritation. Previous research has found that malic acid in strawberries can whiten teeth. Watermelons contain greater malic acid than strawberries. Objective: To analyze the  effectiveness of 100% watermelon (Citrullus lanatus) extract on teeth whitening. Methods: The study was a laboratory experimental study with a total of 15 anterior post-extraction teeth which were discolored using black tea, divided into 3 groups. Group 1 was immersed in 100% watermelon extract, group 2 was immersed in 10% carbamide peroxide as positive control and group 3 was immersed in sterile aquades as negative control, for 56 hours, measured using a shade guide and spectrophotometer. Data were analyzed using one way Anova. Results: The 100% watermelon extract was effective for teeth whitening. There was a significant difference between 100% watermelon extract compared to negative control (p < 0.05). However, there was also a significant difference between 100% watermelon extract, and 10% carbamide peroxide gel (p = 0.003). Conclusion: The watermelon extract has the ability as teeth whitening agent. However, further study is still needed to explore this result and determine the proper concentration for teeth whitening.

Streptococcus pyogenes is one of the pathogenic bacteria that causes pharyngitis. Natural treatment to overcome these problems is to use cardamom seeds. The purpose of this study was to test the antibacterial activity, determine the concentration of inhibitory zone and phytochemical compounds from  ethanol 96% extract of Java cardamom seeds (Amomum compactum Soland. Ex Maton) against Streptococcus pyogenes. The method is used   solid dilution and paper disc diffusion method. The solid dilution method is used for the Minimum Inhibitory Concentration (MIC) test with a concentration of 1.25%, 2.5%, 5% and 7.5% while the paper diffusion method is used for the Inhibition zone Diameter (IZD)  using five treatments namely three concentrations of ethanol 96% extract of Java cardamom seeds (7.5%, 10% and 12%), one positive control of amoxicillin 0.01 mg/mL and one negative control of sterile distilled water. IZD data were  analyzed using ANOVA with a confidence level of 95% and α = 0.05 and Duncan's further tests to determine differences between treatments. The results showed that the MIC  was at a concentration of 7.5% while for the IZD test which had the highest activity there was a concentration of 12% with an average inhibition diameter of 12.03 ± 0.14 mm. In addition, ethanol 96% extract of Java cardamom seeds contain alkaloids, flavonoids, terpenoids and tannins which function in antibacterial activity.

This study examined and compared wound healing between Thai propolis product and calcium hydroxide paste as pulp-capping agents after partial pulpotomy in New Zealand white rabbits. Forty incisor teeth from 10 rabbits were treated. Thirty-six teeth received class V cavity preparations with partial pulpotomy and application of either propolis or calcium hydroxide paste. Similar cavity preparations were performed in 2 teeth without any capping material as a positive control, whereas 2 teeth without the cavity preparation served as a negative control. Histological evaluation showed that both groups had dentin bridge formation. Dentinal tubules in the dentin bridge were more orderly arranged in the Thai propolis group than in the calcium hydroxide group. Wound healing and the median number of hyperemic blood vessels were not statistically significant different between the 2 groups. Thai propolis product may be used as a pulp-capping agent.

Abstract Introduction The aim of obturation in the root canal is to completely seal the canal space to eliminate all the portals of entry and exit between root canal and periodontal space. Various techniques have been developed to achieve a hermetic seal. Materials and Methods As many as 150 extracted human maxillary central incisors were taken for the study. Biomechanical preparation was done up to F5 protaper file. According to different obturation techniques, samples were divided into six groups, keeping 30 samples in experimental and 15 samples in control groups. Group I–Lateral Condensation, Group II–Thermafil, Group III–Beefill, Group IV–GuttaFlow, Group V–Positive Control group, Group VI–Negative Control group. After obturation, the samples were immersed in 2% Rhodamine-B dye for 24 hours. Each sample was longitudinally sectioned to examine under confocal laser scanning microscope. Statistical Analysis The results were evaluated with ANOVA and posthoc Tukey honest significant difference (HSD) comparison test. Results The mean values of dye penetration of different groups were Group I (Lateral Condensation) 1.51 ± 0.451, Group II (Thermafil) 0.918 ± 0.399, Group III (Beefill) 1.30 ± 0.559. Group IV (GuttaFlow) 0.655 ± 0.396, Group V (Positive Control group) 1.96 ±0.046, Group VI (Negative Control group) 0 ± 0. The lowest mean value of apical microleakage was found in GuttaFlow amongst all experimental groups. Conclusion It can be concluded that the GuttaFlow obturating material exhibited better apical sealing ability with canal walls.
