Value Analysis of Continuous Comprehensive Health Guidance in the Prevention of Hypertension in the Elderly Community

  La insuficiencia cardiaca (IC) es un trastorno progresivo y letal que constituye la primera causa de ingreso hospitalario en las personas mayores de 65 años. Se ha demostrado la utilidad del NT-proBNP para su diagnóstico; sin embargo, los valores de estos marcadores difieren según las características de los pacientes y la severidad de la enfermedad. El objetivo de este estudio consistió en determinar la utilidad de la medición de NT-proBNP para diagnosticar el riesgo de padecer IC en adultos mayores. Se realizaron análisis de NT-proBNP, creatinina, urea, hemoglobina, glucosa y sodio en 47 muestras sanguíneas de pacientes de un centro gerontológico de Manabí en el periodo octubre a diciembre 2017. Se analizaron los valores y la posible asociación a otras enfermedades. Se compararon los valores de variables demográficas, analíticas y clínicas. Se analizaron las manifestaciones clínicas y antecedentes patológicos personales asociados al riesgo de IC. El valor NT-proBNP se asoció con la presencia de anemia, creatinina mayor de 2 mg/dL e hiponatremia. La edad media de los pacientes fue de 79 años, de predominio masculino, con comorbilidad asociada y cierto grado de dependencia, aunque la edad no fue un factor que influyó en la probabilidad de tener elevación en el valor plasmático de NT-proBNP y por lo tanto del riesgo latente de padecer o no IC. El valor de NT-proBNP mostró ser un analito complementario de gran importancia clínica para la evaluación y el cuidado integral de salud del anciano.   Palabras clave: NT-proBNP, insuficiencia cardiaca, adultos mayores.   Abstract Heart failure (HF) is a progressive and lethal disorder that constitutes the first cause of hospital admission in people over 65 years of age and represents just over 2% of national healthcare expenditure. The usefulness of NT-proBNP for its diagnosis has been demonstrated, however, the values of these markers differ according to the characteristics of the patients and the severity of the disease. The objective of this study is to determine the NT-proBNP usefullness to diagnose risk of HF in older adults. NT-proBNP, creatinine, urea, hemoglobin, glucose and sodium were analyzed in 47 blood samples of patients from the gerontological center from October to December 2017. The values and the possible association to other diseases were analyzed. The values of demographic, analytical and clinical variables were compared. The clinical manifestations and personal pathological background associated with the risk of HF were analyzed. The NT-proBNP value was associated with the presence of anemia, creatinine greater than 2 mg/dL and hyponatremia. The average age of the patients was 79 years, predominantly male, with associated comorbidity and a certain degree of dependence, although age was not a factor that influenced the probability of having an elevated plasma NT-proBNP value and therefore both of the latent risk of suffering or not IC. The value of NT-proBNP was shown to be a complementary analyte of great clinical importance for the evaluation and comprehensive health care of the elderly.   Keywords: NT-proBNP, heart failure, elderly.

Currently, Japan is rapidly aging. Japanese government agencies report that the percentage of elderly people whose ages are at least 65 years will increase by up to about 30 percent in 2025. As one of the measures towards this situation, the community-based integrated healthcare system will be introduced in Japan. The system aims to provide elderly people living at home with appropriate health, medical, and welfare services. We focus on the burden of sharing information on the situation of the elderly at home among health, medical, welfare staffs, and neighbors. We have been developing a supporting system for sharing information on the situation of the elderly at home and conducted a field test around one year. We consider that various stakeholders involved in the community comprehensive health care system could recognize the importance of information sharing and collaboration with them through this kind of social implementation

ABSTRACT The purpose of health education in community is  improve the health statuse, especially in the elderly who increase hypertension through a healthy lifestyle. The  method is through health education about 1) identification of the lifestyle of the elderly in the local area, 2) prevention and management of hypertension in the elderly. The results of health education are well known that participants are very active in participating in this activity. Based on questionnaires given to participants before and after counseling, participants understanding of the knowledge of hypertension increased 80%, understanding of the prevention of hypertension increased 67%, understanding of curing of hypertension increased 68%, understanding of prevention and treatment of hypertension increased 76% . So that, Continuing Health education is very important to increased health live style. Changes in the elderly in developed countries, namely changes in the cardiovascular system which is a major disease that takes its toll because it will affect other diseases such as hypertension, coronary heart disease, pulmonic heart, cardiomyopathy, stroke, kidney failure.Hypertension or high blood pressure is a medical condition where people whose blood pressure rises above normal is 140/90 mmHg and can run the risk of morbidity and even death. Risk factors for hypertension are divided into 2 groups, namely hypertension that cannot be changed and hypertension that can be changed. Modifiable hypertension includes smoking, obesity, a monotonous lifestyle and stress. Hypertension that can not be changed include age, gender, ethnicity, heredity. So this time the researchers tried to predict hypertension sufferers in Malang using the Jenkins ARIMA Box model. Researchers hope that forecasting methods and forecasting results can be used as additional information for the health department in Malang district in determining policies to be taken in the prevention of hypertension patients according to the needs of patients in Malang district.AbstrakHipertensi merupakan sebuah kondisi medis dimana orang yang tekanan darah meningkat diatas normal yaitu 140/90 mmHg dan hipertensi dapat mengalami resiko kesakitan (morbiditas) bahkan kematian (mortalitas). Dalam rangka membuat perencanaan menurunkan jumlah penderita hipertensi di daerah dengan prinsip efektif dan bertanggung jawab diperlukan perkiraan peramalan yang valid. Adanya selisih yang relatif cukup besar antara target dan capaian dalam menanggulangi penderita hiperteni Kabupaten Malang selama tahun 2014 - 2019, dan mengingat pentingnya peramalan sebagai indikator menurunkan penderita hipertensi, maka dipandang perlu melakukan penelitian mengenai penerapan model Box-Jenkins dalam peramalan penderita hipertensi. Penelitian ini bertujuan untuk membuat model estimasi penderita hipertensi kabupaten malang dengan mengunakan data dinkes dari tahun 2014 sampai dengan tahun 2019.Teknik analisis yang diterapkan adalah model Box-Jenkins atau Autoregresive Integreted Moving Average (ARIMA). Hasil penelitian menunjukkan bahwa dengan menggunakan data dinas kesehatan kabupaten malang tahun 2014 sampai dengan tahun 2019, disimpulkan bahwa model peramalan yang terbaik adalah dengan ARIMA(1,0,1). Peneliti berharap metode peramalan dan hasil peramalan tersebut dapat digunakan sebagai tambahan informasi bagi pihak dinas kesehatan di kabupaten malang dalam menentukan kebijakan yang harus diambil dalam pencegahan penderita hipertensi sesuai kebutuhan pasien di kabupaten Malang.

ABSTRACTObjective: to analyze the degree of independence to carry out Activities of Daily Living (ADL) of elderly residents in long-stay institutions-LPI in Natal-RN city. Method: this is about a descriptive-exploratory study performed in charities, with sample of 60 elderly. Data were collected from August to October 2009. The instruments used were a form with demographic and socioeconomic issues and the ADL Katz’s Scale on the activities are fundamental to maintaining independence: the ability to feed themselves is consistent, getting around, bathing, dressing and using the bathroom. The study was approved by the Ethics and Research of the Federal University of Rio Grande do Norte/UFRN, in accordance with Resolution 196/96. Results: we found a higher frequency of older females compared to males respectively mean age and standard deviation of 79.0 ± 16.89 years and 78.8 ± 13.15 years. The ability to perform the ADL was identified that 58.3% of the elderly were dependent for most ADL demonstrated need for assistance and the execution of daily habits. Conclusion: we emphasize the high rate of disability in elderly subjects, suggesting the relevance of interventions on this aspect. It is important to work in an interdisciplinary team as a central strategy in search of comprehensive health care for the elderly. Descriptors: human aging; elderly; functional capacity. RESUMOObjetivo: analisar o grau de independência para a realização de Atividades da Vida Diária (AVDs) dos idosos residentes em instituições de longa permanência-ILPI, da cidade de Natal–RN. Método: estudo descritivo, exploratório, realizado em instituições filantrópicas, com uma população constituída de 60 idosos. Os dados foram coletados no período compreendido entre os meses de agosto a outubro de 2009. Como instrumentos foram utilizados um formulário com questões socioeconômicas e demográficas e a escala de AVDs de Katz sobre as atividades fundamentais à manutenção da independência: capacidade de alimentar-se, ter continência, locomover-se, banhar-se, vestir-se e utilizar o banheiro. A pesquisa foi autorizada pelo Comitê de Ética e Pesquisa da Universidade Federal do Rio Grande do Norte – UFRN, de acordo com resolução 196/96. Resultados: constatou-se maior frequência de idosos do sexo feminino em relação ao masculino respectivamente com idades médias e desvios padrões de 79,0 ±16,89 anos e 78,8 ±13,15 anos. Quanto à capacidade para realizar as AVDs foi identificado que 58,3% dos idosos eram dependentes para a maioria das AVDs demonstrando necessidade de auxilio quanto à execução de hábitos diários. Conclusão: ressalta-se o alto índice de incapacidade funcional dos idosos institucionalizados, o que sugere a relevância de intervenções sobre esse aspecto. Torna-se importante o trabalho em equipe interdisciplinar como estratégia central na busca da integralidade da atenção a saúde da pessoa idosa. La capacidad de realizar la ADL se identificó que el 58,3% de los ancianos dependen de la necesidad demostrada la mayoría de ADL para la asistencia y la ejecución de los hábitos cotidianos. Conclusiones: destacamos la elevada tasa de discapacidad en sujetos de edad avanzada, lo que sugiere la pertinencia de las intervenciones en este aspecto. Es importante trabajar en un equipo interdisciplinario como una estrategia central en la búsqueda de atención integral de salud para los ancianos. Objectives: In the geriatric care provided in health centers, the healthy elderly are neglected. This qualitative study aims to improve the quality of geriatric care for the healthy elderly. Methods & Materials: This community-based participatory study was conducted from 2016 to 2018 with the participation of 11 healthcare providers, 54 older people and 54 family members in Shahid Motahari Comprehensive Health Center in Mashhad, Iran. The mean age of healthcare providers was 32.4±2.7 years with a work experience 8.3 years. The mean age of the elderly was 64.3±3.9 years. Data were collected using the SERVQUAL questionnaire and interviews, focus group discussions, and field note taking. After identifying the problems through interviews with 8 personnel and 19 older people, group discussion sessions were held to design the change programs. After implementing the change programs, 30 interviews were conducted with the participants and comparisons were made before and after the programs. Descriptive statistics was used to analyze the qualitative data, and descriptive statistics and paired t-test were used to analyze the quantitative data. Results: The mean total score of service quality according to the elderly and their families in the pre-intervention stage was 63.02±9.46 and 61.83±9.05, respectively. These values reached 130.09±14.75 and 122.65±13.56 in the post-intervention stage, and this difference was statistically significant (P=0.001). The presented dynamic care model was able to improve geriatric care for the healthy elderly in the comprehensive health center by modifying the attitude, skills and empowerment of staff. Conclusion: The dynamic care model can improve the quality of geriatric services for the healthy elderly with features such as active, participatory, continuous, comprehensive and supportive care. It can be used in other health centers.

Hypertension is one of the main problems in public health in Indonesia and the world. It is estimated that around 80% of cases of hypertension occur mainly in developing countries in the year 2025 from a total of 639 million cases in 2000. Problems faced by partners at this time include people in the village of Prevention rarely conducted health counseling by the Puskesmas especially about Hypertension, none Posyandu for the elderly and the community rarely exercise regularly. This community service activity was carried out in the village of Enforcement which is one of the villages located in Pelawan sub-district, Sarolangun District, Jambi. Time of implementation in February 2018. The target is the elderly.The methods used were survey, lecture, discussion, demonstration / simulation approaches. The results of this program is prevention for parents where they are able to understand of  the management and prevention of hypertension. It is recommended for Health Officers, especially in Puskesmas, to activate posyandu for the elderly so that the elderly can get health services regularly and get more information about health problems.

RESUMOObjetivo: analisar associação gênero, idade, escolaridade, renda familiar, níveis de fragilidade e cinco critérios de fragilidade com ocorrência dos cochilos de idosos. Método: estudo quantitativo, transversal, descritivo. Foram avaliados 3.075 idosos, utilizando questionário sociodemográfico, questão do Minnesota Leisure Activity Questionnaire, fenótipo de fragilidade de Fried e o Mini Exame do Estado Mental. Foram aplicados o teste Qui-quadrado e a regressão logística univariada e multivariada (α=5,0%). Os princípios éticos foram respeitados. Resultados: dos idosos, 61,7% cochilavam durante o dia. Houve associação entre ocorrência de cochilo e idade, gênero e renda familiar. Conclusão: a avaliação dos pacientes deve estudar questões do sono, especialmente considerando idade, sexo e renda, para detectar previamente problemas e desenvolver ações que mantenham a autonomia e a independência dos idosos.
