To study the effect of cluster nursing model combined with psychological intervention on emotional control and life quality of patients with continuous blood purification (CBP) and catheter-related bloodstream infection (CRBSI). 126 patients with CBP catheterization treated in our hospital (November 2018-November 2019) were chosen as the study subjects, and randomly split into group A and group B, 63 cases in each group. Group B adopted routine nursing while group A received cluster nursing model with psychological intervention to compare the emotional control level and life quality between the two groups. No obvious differences in sex ratio, age, BMI, smoking history, drinking history, marital status, disease types, catheter indwelling time and catheter position were found between the two groups(P>0.05). After nursing intervention, MPV and CD62-P in group A were obviously lower while PLT value was obviously higher compared with group B (P<0.001). CRBSI occurred later in group A compared with group B after intervention (P<0.001), and CRBSI duration was shorter compared with group B (P<0.001). The CECS scores in both groups after intervention were obviously higher, and CECS score in group A after nursing intervention was obviously higher compared with group B(P<0.001). SF-36 scores in both groups after nursing intervention were obviously higher, and SF-36 score in group A after intervention was higher compared with group B (P<0.001). The total nursing satisfaction in group A was obviously higher compared with group B (P<0.05). Cluster nursing model combined with psychological intervention can enhance emotional control level of patients with CBP catheterization, shorten the CRBSI duration, improve the life quality and nursing satisfaction, with obvious effect, deserving reference and promotion.

This work verifies the hypothesis, according to which the systems of upbringing and education, historically developed by different nations, are the factor determining the pattern of their socio-economic evolution and the level of life quality of population. In order to verify the hypothesis, two groups of world countries were highlighted with opposite performances of economic and humanitarian development. Comparative analysis of basic socio-economic parameters in these groups of countries as well as their adopted systems of upbringing and education revealed serious distinctions. For countries with a high level of economic and human development, the most important goal of the educational process is the purposeful development of the intellectual, emotional, and volitional characteristics of children; the task of acquiring specific work skills in early childhood is not set. In the courtiers with low level of development, the situation is opposite. The final conclusion is that the level of economic and human development of people is directly related with their national educational principles and traditions. It is possible to develop an educational model, optimum for successful national development, and to create required conditions for its distribution in the world using advanced digital technologies.

Abstract Background: Critically ill patients with COVID-19 disease are an especially susceptible population to develop Post-intensive Care Syndrome (PICS) due to acute respiratory distress syndrome (ARDS). Patients can suffer acute severe pain and may have long-term deterioration in mental, cognitive, and functional health after discharge. However, few controlled trials are evaluating interventions for the prevention and treatment of PICS. The study hypothesis is that a specific care program based on early therapeutic education and a psychological intervention improves the quality of life of patients at risk of developing PICS and chronic pain after COVID-19 disease. The primary objective is to determine if the program is superior to standard-of-care on health-related life quality at six months after hospital discharge. The secondary objectives are to determine if the intervention is superior to standard care, evaluating the health-related life quality, the incidence of chronic pain and the degree of functional limitation, the incidence of anxiety, depression, and post-traumatic stress syndrome at 3 and 6 months after hospital discharge. Methods: The PAINCOVID trial is a unicentric randomized, controlled, patient blinded superiority trial with two parallel groups. The primary endpoint is the health-related quality of life at six months after hospital discharge, and randomization will be performed with a 1:1 allocation. This paper details the methodology and statistical analysis plan of the trial and was submitted before outcome data were available. The sample size calculated is 84 patients, 42 for each arm. Estimating a loss of follow up of 20%, a sample size of 102 patients is necessary (51 each group). Discussion: This is the first randomized clinical trial assessing the effectiveness of an early care therapeutic education, and psychological intervention for the management of PICS and Chronic Pain after COVID-19. The intervention will serve as a sample of the need to implement early care programs on early stages, having an incalculable impact given the current scenario of the pandemic.Trial registration: This study is being in accordance with the tenets of the Helsinki Declaration, and has been approved by the authors’ institutional review board Comité Ético de Investigación Clínica del Hospital Clinic de Barcelona (approval number: HCB/2020/0549) and was registered on May 9, 2020 at clinicaltrials.gov (NCT04394169).
