Pesquisa qualitativa descritiva realizada em 2010 num hospital especializado em psiquiatria do Paraná. Objetivo: Verificar as mudanças ocorridas na prática profissional decorrentes da Reforma Psiquiátrica na visão da equipe de enfermagem. Métodos: Participaram da pesquisa 15 profissionais de enfermagem. Os dados foram obtidos mediante entrevista semiestruturada e os resultados organizados em categorias temáticas. Resultados: Obteve-se que os sujeitos perceberam mudanças na organização da assistência e no modo de assistir a pessoa com transtorno mental, a importância da equipe multiprofissional nesse processo e a transformação do papel da equipe de Enfermagem e do enfermeiro frente às novas formas de tratamento surgidas. Conclusão:Há avanços decorrentes da Reforma e que estes interferem diretamente na formação e atuação das equipes de Enfermagem gerando a necessidade de preparo e capacitação destes profissionais.

ABSTRACTObjectives: to describe the main bases of the therapeutic relationship according to Hildegard Peplau and to discuss on the Brazilian Psychiatric Reform, with emphasis in the Psychiatric Internment and especially the Involuntary Internment and your implications in the care rendered by the nursing team. Methodology: a bibliographical research was accomplished on the principal constructors of these practices medicine from the care of psychiatric nursing referring to the Psychiatric Reform and the involuntary internment. Results: the nursing team sees the patient as the protagonist of your daily practice and he takes care of this person in psychic suffering, although he makes it, most of the team, in an instinctive way. Special manifestation of that team is not observed in relation to the patient of involuntary internment. There is not registration or any action that point there to be a specific glance on that type of patient. Conclusion: the nursing team doesn't get to identify that patient one clearly in the unit of internment, addressing the care for the demand or the patient's solicitation. Like this, it is evidenced that that care is not visible or constant in the daily of the practice of the nursing team. Metodologia: foi realizada uma pesquisa bibliográfica sobre os principais construtos da clinica do cuidado de enfermagem psiquiatrica referentes à Reforma Psiquiátrica e a internação involuntária. Resultados: a equipe de enfermagem vê o paciente como o protagonista da sua prática cotidiana e cuida desta pessoa em sofrimento psíquico, embora o faça, na maioria das vezes, de maneira instintiva. Não é observada especial manifestação dessa equipe em relação ao paciente de internação involuntária. Não há registro, nem qualquer ação que aponte haver um olhar específico sobre esse tipo de paciente. Conclusão: a enfermagem não consegue identificar claramente esse paciente na unidade de internação, direcionando o cuidado pela demanda ou solicitação do paciente. Assim, fica evidenciado que esse cuidado não é visível ou constante no cotidiano da prática da equipe de enfermagem. Metodología: una investigación bibliográfica era cumplida en el construtos principal de la práctica la medicina del cuidado de enfermeria psiquiátrica que se refiere a la Reforma Psiquiátrica y la internación involuntaria. Resultados: el equipo de enfermeria ve al paciente como el protagonista de su práctica diaria y él cuida de esta persona en el sufrimiento psíquico, aunque él lo hace, la mayoría del equipo, de una manera instintiva. La manifestación especial de ese equipo no se observa el paciente de internación involuntaria respecto a. No hay ningún registro, ni cualquier acción que apunta para haber una mirada específica en ese tipo de paciente. Conclusión: el equipo de enfermeria no consigue identificar ese paciente claramente en la unidad de internación, mientras dirigiéndose el cuidado para la demanda o la solicitación del paciente. Así, se evidencia que ese cuidado no es visible o constante en el periódico de la práctica del equipo de enfermeria. ABSTRACT This study aimed at identifying the opinion of professionals about mental health education and professional practice in the psychiatric hospital. It is a descriptive, exploratory study with qualitative approach, developed with 60 professionals of higher education in two psychiatric macro-hospitals in Rio Grande do Norte. Data were collected through a questionnaire, then transcribed, processed according to the Alceste software and subjected to content analysis. The results were structured in two thematic axes and their respective themes: thematic axis 1 - Professional practice in mental health, and thematic axis 2 - Mental health education. The confluence of asymmetries and differences in the performance of the teams in the psychiatric hospital were evident. The resulting scenario reflects the current political and ideological discrepancy of the current Brazilian process of Psychiatric Reform, considering the challenges of implementation and professional qualification in the field of psychosocial care.

Objetivo: conhecer a imagem do enfermeiro frente ao acadêmico de enfermagem. Metodologia: estudo de caso qualitativo fundamentado na Sociologia Compreensiva do Cotidiano, realizado com 26 participantes do 9º período de uma instituição de ensino superior. Os dados foram coletados por meio da entrevista individual e analisados segundo Análise de Conteúdo Temática. Resultados: a imagem se postula em um profissional essencial na área da saúde, que organiza o setor, gerencia ações, lidera a equipe, presta assistência aos pacientes e é referência para outros profissionais. Conclusão: a (des)valorização do profissional foi um dos aspectos na construção dessa imagem e na percepção positiva/negativa do acadêmico frente ao futuro na profissão.Descritores: Educação em Enfermagem, Imagem, Prática Profissional, Pessoal de Saúde, Enfermagem.IMAGE OF NURSE UNDER THE PERSPECTIVE OF THE NURSING ACADEMICObjective: to know the image of the nurse in front of the nursing academic. OBJECTIVE: To investigate trends in the provision of mental health services and financing in Brazil. METHOD: Data from DATASUS (the Brazilian Unified Health Computerized System) with free access in the web were collected regarding the number of beds, the development of new community centers, the number of mental health professionals, and costs involved from 1995 to 2005. RESULTS: In ten years, the number of psychiatric beds decreased 41% (5.4 to 3.2 per 10,000 inhabitants) while community services have increased nine-fold (0.004 to 0.037 per 10,000 inhabitants). Psychologists and social workers have accounted for three and two-fold, respectively, as much hirings as psychiatrists. Psychiatric admissions accounted for 95.5% of the budget in 1995 and 49% in 2005, and the expenses with community services and medication have increased 15% each. As a whole, the expenses in mental health decreased by 26.7% (2.66 to 1.95 US$ per capita). CONCLUSION: There has been a clear switch from hospital to community psychiatric care in Brazil, where the system can now provide a diversity of treatments and free access to psychotropics. However, the coverage of community services is precarious, and the reform was not accompanied by an increased public investment in mental health. The psychiatric reform is not a strategy for reducing costs; it necessarily implies increasing investments if countries decide to have a better care of those more disadvantaged.

There are times when we do not know which words to use. They throw themselves on paper, some or many of them without knowing exactly what will result, as a call for help on what they can show. So simple, so flexible, so hard, so complex they are. The request to write an editorial for the Rene Journal gave me the wheel of time. I’m retired of University teaching and research for a decade and a half. I was editor in chief and also section editor in the mental health area of this journal. So I have a lot to remember.

ABSTRACT Objective: To analyze how the nursing team from a Psychosocial Care Center II relates its insertion in the service with the work process. Methods: Descriptive, analytical research outlined as a case study with a qualitative approach. The data were collected through semi-structured interviews with nursing professionals and went through content analysis subsidized by Bardin’s work. Results: Two thematic categories emerged: a) Ways of intake in the nursing services at the Psychosocial Care Center; b) Setbacks in nursing routine at the Psychosocial Care Center. Final considerations: The fact that the professionals have not planned to work in mental health nor have training impacts the rhythm of psychosocial care development at the Psychosocial Care Center, forcing them to seek training in the field after admission. Unmotivated by the working conditions, the nursing team suffers from the absence of an interdisciplinary partnership and goes to great lengths to approach psychosocial care.

This chapter analyses the influence of Franco Basaglia and the organization ‘Psichiatria Democrática’ on the Spanish critical movements. These movements appeared in 1971 and were organized through a clandestine group known as the ‘Psychiatric Coordinator’. This organization linked professionals (mainly young psychiatrists) who had initiated innovative experiences in various psychiatric hospitals. These developments generated conflict when opposing the norms of the dictatorship. From 1975, and especially after the approval of the 1978 Constitution, the critical movement was a force for change in mental health structures in Spain, through the established organization, the Spanish Association of Neuropsychiatry. This effort made it possible to generate the psychiatric reform in 1985 that advocated community mental health and deinstitutionalization policies. Franco Basaglia began his contacts with the Spanish critical professionals in 1970, and the relationship was maintained periodically until 1980, the year of his death.
