Objetivo: identificar os fatores maternos relacionados ao desmame precoce do aleitamento materno exclusivo. Método: estudo tipo coorte, com dados coletados 24 horas e seis meses após o parto. Na análise utilizada estatística descritiva e inferencial. Resultados: participaram 94 mulheres com idade média de 26 anos, maioria com ensino médio completo, ocupação remunerada, multípara, sem hábitos nocivos ou uso de medicação contínua; iniciou o pré-natal antes de 12 semanas de gestação, realizou sete ou mais consultas, mas não recebeu orientação sobre aleitamento materno, não o executou na sala de parto e nem na primeira hora de vida; apresentou problemas relacionados a amamentação no hospital e no domicilio e mais da metade (57,4%) interrompeu o aleitamento precocemente. Ter realizado aleitamento materno exclusivo em filho anterior constituiu fator de proteção ao desmame precoce. Conclusão: as características da mãe e da assistência ao período gravídico-puerperal não apresentaram associação com desmame precoce.

Objective: To describe the consequences of using artificial nipples to exclusive breastfeeding. Materials and method: An integrative review carried out in the Medline®, Cinahl, Lilacs, Web of Science, and Scopus databases to answer the following question: What are the consequences of using artificial nipples to exclusive breastfeeding? Results: 38 articles were analyzed; the analysis gave rise to two categories: neonatal factors and maternal factors influencing exclusive breastfeeding. Breastfeeding interruption was the neonatal factor most associated with the use of artificial nipples, while the level of maternal education stood out as an intervening factor in exclusive breastfeeding. Pacifiers were the dummy type more commonly mentioned, followed by feeding bottles and nipple shields. Conclusions: The consequences of offering artificial nipples to breastfed infants are mostly negative and associated with the newborn, such as early weaning, refusal to breastfeed, impaired suction technique, incorrect latch-on, interference with orofacial development, and interruption of exclusive breastfeeding. Mothers who offered artificial nipples to their infants are more likely to experience pain, nipple fissures, frustration, and reduced interaction with their infants.

ABSTRACT Objectives: to identify factors that interfere with the practice of exclusive breastfeeding in quilombola communities. Methods: qualitative research based on the Transcultural Theory proposed by Madeleine Leininger, performed with quilombola mothers. For data collection, we used the semi-structured interview, conducted from June 2018 to January 2019. Content analysis was guided by the interactive model of Miles and Huberman. Results: it was revealed that intergenerational cultural myths and customs interfere with breastfeeding practice, and identified the influence of professional nurses on best breastfeeding practices. Final Considerations: cultural and intergenerational factors favor early weaning and expose the child to illness.

Objective Evaluate the clinical use of the Breastfeeding Self-Efficacy Scale as predictive of early weaning and verify if women who had higher self-efficacy scores breastfed for longer periods. Methods Cohort study developed with 100 postpartum mothers. Research instrument used: Breastfeeding Self-Efficacy Scale-Short Form. The feeding was monitored on the 7th, 15th, 30th, 45th and 60th day, by phone. Results The mean duration of exclusive breastfeeding was 53.2 days (SD 14.2). Most mothers (82.3%) had scores compatible with high self-efficacy for breastfeeding, none had low efficacy. There was no statistically significant difference in the comparison of mean duration of exclusive and non-exclusive breastfeeding, with the scores of medium and high efficacy. Conclusion Findings did not enable the confirmation of the use of the scale as a predictor of risk of early weaning. No relation was observed between higher scores of high efficacy and longer periods of exclusive breastfeeding.

ABSTRACT Objective: To verify the association of nursing mothers’ self-efficacy for breastfeeding in the immediate postpartum period and six months after birth and obstetric and sociodemographic variables with the duration of exclusive breastfeeding. Method: Observational, longitudinal, prospective study which followed nursing mothers from the immediate postpartum period to the sixth month postpartum in a municipality in Southern Brazil. For data collection, a questionnaire with sociodemographic and obstetric variables and the scale Breastfeeding Self-Efficacy Scale - short form - were employed in the maternity ward and six months after birth. Inferential and descriptive statistics were employed. Results: A total of 158 nursing mothers have participated. The prevalence of exclusive breastfeeding in the sixth month was 36.70%, out of which 77.34% have presented a high self-efficacy score. Sociodemographic factors had a negative impact on exclusive breastfeeding. Conclusion: High levels of self-efficacy favor exclusive breastfeeding; however, such factor, in isolation, is not decisive for exclusive breastfeeding. Self-efficacy should be identified during the pre-natal period through mothers’ employment and marital status data to promote preventive actions against early weaning.

Breastfeeding is critical for adequate neonatal microbial and immune system development affecting neonate health outcomes in the short and long term. There is a great interest in ascertaining which are the maternal factors contributing to the milk microbiota and the potential relevance for the developing infant. Thus, our study aimed to characterize the effect of mixed and exclusive breastfeeding practices on the milk microbiota and to determine the impact of pre-pregnancy body mass index (BMI) and weight gain over pregnancy on its composition. Breast milk samples from 136 healthy women were collected within the first month post-partum and milk microbiota profiling was analyzed by 16S rRNA gene sequencing. Information on breastfeeding habits and maternal-infant clinical data were recorded. Breastfeeding practices (exclusive vs. mixed), maternal pre-gestational BMI, and weight gain over pregnancy contributed to the milk microbiota variation. Pre-gestational normal-weight women with exclusive breastfeeding habits harbored a significantly higher abundance of Bifidobacterium genus, and also, higher alpha-diversity compared to the rest of the women. Our results confirm the importance of controlling weight during pregnancy and breastfeeding practices in terms of milk microbiota. Further studies to clarify the potential impact of these maternal factors on milk and infant development and health will be necessary.

This study aims to determine the prevalence of and maternal factors that are associated with exclusive breastfeeding (EBF) in Sudair and Al Zulfi, Riyadh, Saudi Arabia. A cross-sectional study was undertaken by means of recruiting 522, mother-infant pairs with infants aged six months. These participants were attending the Well-Baby Clinics in Sudair and Al Zulfi from  January 1st to April 30th, 2016, using a pilot-tested Arabic questionnaire. The EBF prevalence was determined via the “recall since birth” technique and regression analysis. EBF prevalence among mothers of infants (6 months) was 17.1%. Mothers’ insights of insufficient milk were the most recurrently reported reason (42.5%) for not practising exclusive breastfeeding. Saudi mothers (adjusted odds ratio: 10.06; 95% confidence interval: 8.46, 12.53), mothers aged 28–38 years (36.03; 1.96, 62.21), mothers who attended breastfeeding health education (90.52; 5.49, 134.8), multiparous mothers (67.25; 5.83, 122.8), and mothers with housekeeping availability (19.59; 1.36, 281.47) were more likely to practice exclusive breastfeeding in comparison to their counterparts. EBF rates in both areas  are far below the WHO recommended level. The mother’s nationality, age, parity, breastfeeding health education, and housekeeping availability were essential determinants to the practice EBF. Thus, aggressive multisectoral governmental interventions should be made to promote as well as support breastfeeding.

Background: Exclusive Breastfeeding (EBF) for 6 months is the optimal way of feeding infants. NFHS4 data shows 45.5% of children were exclusively breastfed at 6 months in Puducherry. The Global Nutrition Targets 2025 aims at increasing EBF rates in first 6 months up to at least 50%. The main objective is to find areas of needed intervention using LATCH scoring and to find factors responsible for fall in rates of EBF.Methods: A prospective study was conducted between November 2017 and April 2019 (1½years). Mothers were educated about breastfeeding. LATCH score assessed at 8 and 48 hours of life. At 48hours, score >8 indicates there is a high chance for EBF for next 6 months and score <8 indicates, mother is having difficulty in feeding and are intervened at this point. Data about EBF and reason for early weaning at 6 weeks and 6 months collected.Results: LATCH score at 8 hours was >8 in infants delivered via NVD: 50% as against 9.6% in babies delivered via LSCS. At 48 hours LATCH score improved in both groups: 60.8% in babies delivered via NVD, 38.3% in babies delivered via LSCS.  Compared to Primi-mothers, infants born to multigravida mothers had higher scores at 48 hours: 62.2% as against 31.1%. 84% were EBF at 6 weeks. 51% were EBF at 6 months. When mothers with LATCH score <8 at 48 hours were intervened, EBF rate at 6 weeks improved. Most common reason for early weaning at 6 weeks was maternal problems: 6.6% and at 6 months- due to influence of relatives: 27%.Conclusions: LATCH score helps in predicting breastfeeding duration as early as 48 hours of life. Low scores indicate, it requires intervention and identifies the areas of needed intervention. Counselling regarding EBF must include not only mothers but also relatives.

The huge benefits of exclusive breastfeeding have not been able to increase the number of exclusive breastfeeding coverage in Indonesia. This is due to the low knowledge of pregnant women on the importance of exclusive breastfeeding. Low support from families, communities and health workers, especially breastfeeding counselors, is one of the factors causing the low coverage of exclusive breastfeeding. Mothers need support from those around them for success of exclusive breastfeeding behavior. The role of health workers in health promotion is also needed so that the exclusive breastfeeding program runs optimally. This activity aims to increase the knowledge and skills of mothers in exclusive breastfeeding. The community service method used is assistance to mothers in exclusive breastfeeding with the concept of one student one client. It was carried out in the working area of the Surakarta City Health Office with 30 participants. It begins with counseling mothers about exclusive breastfeeding, observations about maternal assistance in exclusive breastfeeding, and an evaluation of the course of mentoring through filling out a questionnaire about exclusive breastfeeding and breastfeeding practices. The role of health workers in supported exclusive breastfeeding includes providing counseling on IMD and breastfeeding since pregnancy with husband, involving husbands in IMD implementation, posyandu with home visits involving cadres and providing counseling for pregnant women, providing lactation corners, counseling involving all puskesmas officers, distributing leaflets and posters, holding pregnancy classes, and involving cadresto hold Kelompok Pendukung Ibu (KP-Ibu). The failure of exclusive breastfeeding was generally because the mother works so that breastfeeding motivation decreased, family support was low, worried about insufficient breastfeeding, and low knowledge of breastfeeding. There was a relationship between knowledge and breastfeeding practice and it was statistically significant (p=0.004, OR: 25.14, CI95%:0.89-5.56). The role of health workers and cadres in supported exclusive breastfeeding through maternal assistance was quite good, however the failure of exclusive breastfeeding generally came from maternal factors. There was a relationship between knowledge and breastfeeding practice and statistically significant.
