Patient safety addresses the risks involved in health care, simplifying or eliminating adverse events, these are defined as incidents that occur during the provision of health care and that result in harm to the patient. Health care is increasingly complex and can increase the potential for incidents, errors or failures to occur. Hemodialysis is a technically complex procedure, with many potential sources of error and which can cause harm to patients. Dialysis is a therapy that in recent years has benefited many patients, but it is a care process that involves important dangers and risks. Hemodialysis is a hospital sector with a great risk potential for the occurrence of adverse events, this occurs for several reasons such as complex procedures, the use of high technology, the characteristic of chronic kidney disease, the high use of medications. Strategies need to be taken to reduce the occurrence of adverse events, thus ensuring the quality of dialysis, consequently the quality of life of patients with chronic kidney disease undergoing dialysis treatment.

PurposeThis study establishes a risk management system for medical and health care integration projects to address the problem of high-risk potential and a strong correlation between risk factors.Design/methodology/approachA new fuzzy WINGS-G1 model for identifying key risk factors in medical and health care integration projects is proposed by introducing the fuzzy theory and the concept of risk incidence into the Weighted Influence Non-linear Gauge System (WINGS) method.FindingsThe authors analyze the fluidity of project risk factors through complex networks to control direct risks and cut off risk transmission paths to provide a reference for risk control and prevention of medical and health care integration projects.Originality/value(1) The integration of fuzzy theory into the WINGS method solves the problem of strong subjectivity of expert scoring in the traditional WINGS method; (2) By the different probabilities of risk factors, the concept of risk incidence is introduced in the WINGS model, which is more conducive to the identification of the critical risk factors and the rational allocation and utilization of organizational resources; (3) The use of the complex network for risk interactivity analysis fully reflects the dynamic nature of risk factors in medical and health care integration projects.

Smoking a cigarette generates over 4000 chemicals that have a deleterious impact on each part of the human body. It produces three main severe effects on the liver organ: oncogenic, immunological, and indirect or direct toxic effects. It results in the production of cytotoxic substances, which raises fibrosis and necro-inflammation. Additionally, it also directs the production of pro-inflammatory cytokines tumour necrosis factor alfa (TNF-α) and interleukins (IL-1β, IL-6) that will be responsible for the chronic liver injury. Furthermore, it gives rise to secondary polycythemia and successively raises the turnover and mass of red cells, which might be a common factor responsible for the development of oxidative stress in the liver due to iron overload. It also produces chemicals that are having oncogenic properties and raises the risk of liver cancer especially in sufferers of chronic hepatitis C. Smoking modulates both humoral and cell-mediated responses by restricting the proliferation of lymphocytes and inducing their apoptosis and ultimately decreasing the surveillance of cancer cells. Moreover, it has been determined that heavy smoking impacts the response of hepatitis C patients to interferon (IFN) therapy through different mechanisms, which can be improved by phlebotomy. Efforts are being made in different nations in decreasing the prevalence of smoking to improve premature death and ill effects of their nation’s individuals.

Operational risks have become increasingly important for banks, especially against the background of growing IT dependency and the increasing complexity of their activities. Further-more, the corona pandemic contributed to the increased risk potential. Therefore, banks have to back these risks with own funds. There are currently three measurement approaches for determining the capital requirements for operational risk. In recent years, and especially during the Great Financial Crisis of 2007/2008, however, some of the weaknesses inherent in these approaches have become apparent. Thus, the Basel Committee on Banking Supervision revised the current capital framework. Therefore, this article examines the various measurement approaches, addresses inherent weaknesses and moreover, presents the future measurement approach developed by the supervisory authorities.

The study investigated the toxicity effects of ‘form specific’ engineered nanomaterials (ENMs) and ions released from nano-enabled products (NEPs), namely sunscreens, sanitisers, body creams and socks on Pseudokirchneriella subcapitata, Spirodela polyrhiza, and Daphnia magna. Additionally, risk estimation emanating from the exposures was undertaken. The ENMs and the ions released from the products both contributed to the effects to varying extents, with neither being a uniform principal toxicity agent across the exposures; however, the effects were either synergistic or antagonistic. D. magna and S. polyrhiza were the most sensitive and least sensitive test organisms, respectively. The most toxic effects were from ENMs and ions released from sanitisers and sunscreens, whereas body creams and sock counterparts caused negligible effects. The internalisation of the ENMs from the sunscreens could not be established; only adsorption on the biota was evident. It was established that ENMs and ions released from products pose no imminent risk to ecosystems; instead, small to significant adverse effects are expected in the worst-case exposure scenario. The study demonstrates that while ENMs from products may not be considered to pose an imminent risk, increasing nanotechnology commercialization may increase their environmental exposure and risk potential; therefore, priority exposure cases need to be examined.

Modern era has an impact on the industry. Relate to technology to turn raw goods into finished goods. Look at competitive in competition between companies that are required to customer demand. PT. XYZ is a publishing and printing company, constructing and concocting books and learning modules. Currently PT. XYZ does not yet have structured risk management to identify and mitigate risks that occur primarily in supply chain functions. Therefore, research is needed that aims to identify possible risks in PT. XYZ supply chain activities. Determine the cause of risk that must be prioritized to be mitigated in the supply chain PT. XYZ and determine mitigation strategies that must be prioritized to address the causes of risks in the supply chain PT. XYZ. The method used in this study is by the House of Risk method. HOR is used to identify risk events, risk agents in the supply chain and design mitigation strategies for risk agents based on ARP (Aggregate Risk Potential) value. The results of this study show that there are 10 risk events and 15 risk agents that become supply chain risks as well as recommendations for tackling priority risk agents.

At present, the environmental risk assessment of major accidents is mainly carried out for stationary risk sources. Only marginal attention is paid to mobile risk sources, while the currently available methodologies require a relevant expertise and time for their application, which is only partially possible in most scenarios. In the present study, an integrated approach to environmental risk assessment in the transport of hazardous substances (iTRANSRISK) was developed. The approach proposed is based on the principle of index-based assessment of leakage scenarios involving toxic and flammable substances during transport, in the context of indexing environmental vulnerability. The key point of the method is the conversion of local-specific data concerning the risk potential of the transported substance, the consequences and the probability of a major accident, and environmental vulnerability assessment into a single entity. The created integral approach is proposed for the needs of carriers of the hazardous substances and the state administration bodies. The proposed approach is determined for the screening risk assessment at the beginning of the process of the planning a suitable transport routes and the results are for information only. An example of the application of the iTRANSRISK integrated approach is demonstrated considering an explosion scenario following a propane tanker leak (18 t) in a forested area, with moderately susceptible soils and no surface water or groundwater affected.
