Stroke is a prevalent disease among geriatric population, which tends to deteriorate cognitive ability and mental health. In such context, cognitive impairment and geriatric depression generate mutually deteriorating impacts on each other. Using the Health and Retirement Study, this study examined depression and cognition before, immediately after, and 2 years after the onset of stroke. Through latent growth mixture modeling, four different trajectories of depression were identified: resilience, recovery, emergent depression, and chronicity. We used demographics including gender, age, race, and ethnicity, activity of daily life, baseline cognition, and trajectories of depression to predict cognitive ability 2 years after the stroke. Both aforementioned demographic factors and pre-stroke cognition were predictive of post-stroke cognition, but the inclusion of depression trajectories further improved the predictive ability. Emergent depression and chronicity were two significant predictors of worse post-stroke cognition. This study showed the importance of considering a more specific trajectotrial interrelationship between depression and cognition in geriatric stroke patients.

IntroductionActivity of daily life is an important key to maintaining and improving the health status. This is especially important for people in physically demanding jobs, such as firefighters, military and police.MethodsIn order to record the actual state of activity of daily life among soldiers, a step count measurement with a pedometer was carried out on 196 soldiers of the German Armed Forces over 7 days and supplemented by a questionnaire on lifestyle factors.ResultsThe subjects exceeded the recommended 7000–8000 steps per day, with the rank and file moving the least and the commissioned officers the most. It was also striking that the soldiers in the study moved an average of 2600 steps less per day on weekends than on weekdays.ConclusionsBased on these findings, targeted physical activity measures can be established for the identified target groups within the framework of corporate health management in order to ensure the physical fitness of soldiers on routine duty and on deployment.

Abstract Purpose High physical fitness and good health are not only essential for the civilian population but also for persons working in physically and psychologically demanding professions, such as armed forces personnel. To date there is no scientific summary of studies dealing with the physical activity of daily life in armed forces personnel, but this is necessary for a targeted intervention. Methods A systematic review was performed in the PubMed database (target date: 24 May 2020), using the search terms “military personnel” and “exercise” or “(physical) activity of daily life” including the assigned MESH words. Results Seven original papers have focused on the daily physical activity of military personal, and they were all conducted in different countries. The activity of daily life was on average 11,540 steps per day. Conclusions It could be shown that soldiers performed more physical activity than the civilian population and that the recommended 10,000 steps per day were slightly exceeded on average. However, the number of steps performed was very low in fields of activity with limited physical activity options, such as on board a ship. Altogether, it thus seems that additional health-promoting physical activity programs are not required for most of the soldiers, but that, in contrast, corresponding offers for physical activity alternatives, e.g., in the form of fitness equipment, are urgently required in specific task areas (e.g., on board ships).

Cleidocranial dysplasia is characterized by hypoplasia of the clavicles, unerupted teeth, narrow pelvis, short stature, and craniofacial malformations. A cause of this skeletal dysplasia is heterozygous mutations of the runt-related transcription factor 2 gene (Runx2), a master regulator for bone and cartilage development. Chondrosarcoma is a primary malignant bone tumor that is usually treated by wide resection surgery. This report shows a case of a 25-year-old female patient with cleidocranial dysplasia who was affected with chondrosarcoma of the left humerus. We performed wide resection of the tumor and reconstruction of the large bone defect of the humerus using free vascularized fibular grafts. The patient preserved the hand function and activity of daily life as the same level as preoperative condition more than five years after the surgery.

Abstract Background Vitamin D deficiency increases muscle weakness and pain leading to reduced power and function, increased bone turnover and increased risk of falls and hip fractures in elderly. We postulated that vitamin D deficiency in sunny Egypt should be lower than western countries so we screen for prevalence of vitamin D deficiency among all elderly patient admitted to orthogeriatric unit by fracture hip. Results Our study showed that vitamin D deficiency is common in elderly Egyptian patients admitted to orthogeriatric unit with hip fractures. Of the study group (n 290 patients) vitamin D levels were sufficient (30 ng/dl or more) in 24 patients (8.3%) and insufficient (between 20 ng and 30 ng) in 125 patients (43.1%) and deficient (less than 20) in 141 patients (48.7%). Among the study group, female patients had lower levels of vitamin D (18.8 ± 6.9) compared to males (20.5 ± 6.7) P value 0.034. Although our study failed to link different levels of vitamin D with activity of daily life: P value was (0.116) in bathing, (0.058) in dressing, (0.115) in toileting and (054) in transfer. A little difference was found in feeding (0.036) and previous fall (0.011) and cognitive impairment; MiniCOG < 3 P value 0.017. Both previous falls and cognitive decline are in turn linked to increased fracture risk Conclusion Hypovitaminosis D is common among older patients with hip fractures even in sunny Egypt remaining slightly lower than its prevalence in western countries. Although no direct significant correlations with activity of daily life, vitamin D deficiency was associated with higher risks of previous falls and cognitive impairment both in turn increases the risk of fractures.

AbstractVery old intensive care unit (ICU) patients, aged ≥ 80 years, are by no mean newcomers, but during the last decades their impact on ICU admissions has grown in parallel with the increase in the number of elderly persons in the community. Hence, from being a “rarity,” they have now become common and constitute one of the largest subgroups within intensive care, and may easily be the largest group in 20 years and make up 30 to 40% of all ICU admissions. Obviously, they are not admitted because they are old but because they are with various diseases and problems like any other ICU patient. However, their age and the presence of common geriatric syndromes such as frailty, cognitive decline, reduced activity of daily life, and several comorbid conditions makes this group particularly challenging, with a high mortality rate. In this review, we will highlight aspects of current and future epidemiology and current knowledge on outcomes, and describe the effects of the aforementioned geriatric syndromes. The major challenge for the coming decades will be the question of whom to treat and the quest for better triage criteria not based on age alone. Challenges with the level of care during the ICU stay will also be discussed. A stronger relationship with geriatricians should be promoted to create a better and more holistic care and aftercare for survivors.

This paper deals with the development of the content of Activity of Daily Life (ADL) based on VR (virtual reality) for cognitive function training for the elderly. The proposed ADL content has been developed focusing on a feedback technique based on performance analysis unlike in the existing VR-based cognitive training content wherein the customized training management based on performance evaluation was difficult. Experiments were conducted with 30 elderly people in the welfare center for 3 months. As a result of the effectiveness evaluation, the proposed ADL content showed higher results in all aspects of immersion, satisfaction, and performance. It is expected that user departure from existing ADL content will be prevented, and various artificial intelligence researches will be carried out to improve the technology accuracy to provide customized difficulty which is an important factor.

Abstract Medulloblastoma is the most common pediatric malignant tumor and about 84.2% of cases are under 15 years old. With a current combined-modality approach that includes surgical approach, chemotherapy and radiation therapy, 5 year-survival rates have become 70–85% and also the so-called long-term survivors have increased in our country. This report reviews the prognosis and delayed complications of medulloblastoma in our institute. Cases were 14 boys and 11 girls from January,2010 to May 2019. Mean age was 6.1 years old (4 months to 14 years old). 18 cases receiver gross total removal and 12 cases were high risk group. Exclude recent 4 cases, 15 cases have indicated complete reaction though 6 cases had relapsed or new tumor. The results are that 5 year-survival rates in our institute is 86.7%, but 4 cases had neuro-psychological complications. 5 cases suffered from endocrinal disorders which were not confirmed after the end of all treatments. 2 secondary cancer had appeared 5 and 8 years after the first treatments. The more the survivor in medulloblastoma cases increase, neurosurgeons have to consider long term follow up more than 5 years and pay more attention to support activity of daily life of these patients.
