Abstract This study is to investigate the natural metatasophalangeal joint (MTP) distance, the appropriate degree of distraction and the associated factors including age, gender, body mass index(BMI). Regarding natural MTP joint space size, the MTP-2 joint had the largest joint size (2.39 ± 0.37 mm). The MTP-5 joint had the smallest joint size (1.59 ± 0.34 mm). In MTP joint arthroscopy, traction power of 10 lbs is sufficient for appropriate distraction for all MTP joints while traction power of 5 lbs is an appropriate power for distraction of the MTP-2 joint. Manual traction is generally not appropriate for distraction because it is not consistent or sufficient. Less distraction power is required for males than for females, especially for MTP-1, MTP-3, and MTP-4 joints.

Introduction.Skeletal traction use generally has decreased over generations and is used most often for temporary fracture stabilization. Proper nursing management of patients in skeletal traction is crucial. A hospital protocol was created and implemented to educate and direct registered nurses (RNs) in the care of patients requiring skeletal traction. Method.A skeletal traction management protocol was drafted and implemented as hospital policy. Twenty-nine RNs from an orthopaedic unit at a level 1 trauma center attended a financially compensated, 45-minute, in-person, off-shift educational session. An anonymous pre-test utilizing a 5-point Likert scale was completed to assess RN knowledge and comfort regarding the following topics of traction care: pin care, manual traction, frame assembly, weight application and removal, skin evaluation, neurovascular checks, and reporting issues. The RNs were provided with a copy of the new hospital policy and key points were highlighted and demonstrated. After the demonstration, the RNs were given a post-test to assess their perceived knowledge and comfort with traction care. Results.Statistically significant improvements in RN knowledge and comfort were seen in six of the seven evaluated topics. The greatest increase was seen in the manual traction topic. No significant change regarding neurovascular checks was observed with this topic having the highest pre-test scores. Conclusion. A hospital protocol was created successfully and implemented that significantly improved the level of RN knowledge and comfort with the management of patients requiring skeletal traction. Future studies should assess the effectiveness of annual education regarding the traction policy.

Osteoarthritis is a chronic degenerative disorder that primarily affects the articular cartilage of the synovial joint, and ultimately bone and remodeling (spurs) at the joint boundary occur. Synovial thickening and capsule progression and joint effusion also occur. Disorders due to Osteoarthritis cause limited activity and limited ability in activities. Because, damage to the cartilage (cartilage) of the joint, so that the thickness of the subcondral bone increases, sclerosis of the bone plate, the growth of osteophytes in the joints, can stretch the joint capsule, resulting in inflammation, and weakening of the muscles connecting the joints. This study aimed to determine the effect of Manual Traction and Isometric Exercise in patients with Knee Osteoarthritis at Grandmed Lubuk Pakam Hospital. Using quasi experiments with one group pretest posttest with a sample of 19 respondents. Measurement of Functional Activity with Jette Scale, using paired sample t-test analysis. There is a significant influence on functional activities before and after. hypothesis test by paired sample t-test obtained with the results of p-value < α (0.001 <0.05). There is the Effect of Manual Traction and Isometric Exercise on Functional Activity Enhancement in Patients with Knee Osteoarthritis at Grandmed Lubuk Pakam Hospital. Researchers suggest that the next researcher add a number of samples and a longer time so that the results obtained are more maximal.

Latar Belakang : Herniated Nucleus Pulposus adalah kondisi penonjolan discus intervertebralis yang dapat menekan akar saraf yang keluar dari foramen intervertebralis sehingga menimbulkan nyeri radikular, dan pada akhirnya menyebabkan disabilitas lumbal. Metode Desain randomized two group pre test – post test dan menggunakan tehnik pulposive sampling bertujuan untuk mengetahui beda pengaruh pada intervensi SNAGs dan Mc kenzie dibandingkan dengan Manual Traction dan Mc.Kenzie terhadap penurunan nyeri dan disabilitas lumbal pada penderita HNP lumbal. Penelitian ini dilaksanakan di RSAD Tk. II Pelamonia Makassar dengan sampel adalah penderita HNP lumbal yang sesuai dengan kriteria inklusi. Jumlah sampel adalah 16 orang yang dibagi secara acak kedalam 2 kelompok yaitu kelompok perlakuan 1 yang diberikan SNAGs dan Mc kenzie sebanyak 8 orang dan kelompok perlakuan 2 yang diberikan Manual Traction dan Mc Kenzie sebanyak 8 orang. Alat ukur yang digunakan adalah Oswetry Disability Index (ODI). Hasil :  Berdasarkan analisis uji paired sample t pada kelompok perlakuan I diperoleh nilai (p = 0.000 < 0,05) untuk nilai Oswetry Disability Index yang berarti bahwa pemberian Sustained Apophysial Glides dan  Mc Kenzie dapat menghasilkan penurunan nyeri dan disabilitas lumbal yang signifikan. Sedangkan kelompok perlakuan II juga diperoleh nilai (p = 0.000 < 0,05) untuk nilai Oswetry Disability Index yang berarti bahwa pemberian Manual Traction dan  Mc Kenzie dapat menghasilkan penurunan nyeri dan disabilitas lumbal yang signifikan. Kemudian berdasarkan uji Indepentent sample t diperoleh nilai (p = 0.000 < 0,05) untuk nilai Oswetry Disability Index yang berarti bahwa pemberian Sustained Apophysial Glides dan  Mc Kenzie lebih efektif secara signifikan dibandingkan dengan pemberian Manual Traction dan  Mc Kenzie terhadap penurunan nyeri dan disabilitas lumbal. Kesimpulan : Dapat disipulkan bahwa  pemberian pemberian SNAGs dan Mc Kenzie lebih efektif dibandingkan dengan Manual Traction dan Mc Kenzie terhadap penurunan nyeri dan disabilitas lumbal pada penderita HNP. Kata kunci : Sustained Natural Apophyseal Glides, Manual traction, Mc Kenzie, ODI, HNP Lumbal

Ankle drawer and varus-valgus stress tests have evolved to examine for excessive joint displacement in the sagittal and coronal planes which may correlate to ligament instability. However, there appear to be no formal manual test(s) assessing for axial plane (longitudinal) instability of the mortise joint. In keeping with Cartesian and Cyriax principles, The TOAST (Traction On Ankle Stress Test) was developed to dynamically assess for 3rd vector axial strain pain generation and long-axis mortise joint gapping. The test is easy to learn as longitudinal manual traction of the mortise joint and applied counterforce hand positions are similar to emergent ankle fracture dislocation reduction technique. The TOAST may also be performed under dynamic ultrasound control.

Category: Arthroscopy; Bunion; Lesser Toes Introduction/Purpose: Appropriate distraction for metatarsophalangeal joint arthroscopy is paramount to increase in visualization and to decrease iatrogenic complications. However, there are few studies on how much adequate distraction is. The purpose of this study was to investigate the appropriate degree of distraction and the associated factors. Methods: 40 patients who underwent toe arthroscopy from April 2015 to June 2018 were enrolled. Under general or spinal anesthesia, the anterio-posterior foot plain radiograph were taken using mini-fluoroscan; no traction, manual traction, 5, 10 and 15 pound traction and were used to measure the metatasophalangeal joint (MTP) distance. Age, gender, body mass index were compared as associated factors. The minimum joint distance of toe arthroscopy defined as 2.8mm which was sheath size of 1.9mm 30° high definition arthroscopy. Appropriate distraction is defined as an over 70% possible rate. Results: 10lb traction was appropriate distraction on first and fourth MTP joint arthroscopy. 5lb traction was appropriate on second, third, fifth MTP. In first MTP with 5lb traction, there was significant difference according to gender; 9 of 23 male, none of 17 female. (p=0.005). With 10lb traction, 16 of 27 patients under 60 and 3 of 13 patients over 60 years old were could arthroscopy (p=0.046). In third MTP with 5lb traction, 18 of 28 patients over 30 were could arthroscopy, but only 4 of 13 under 30 years old were could(p=0.038). In fourth MTP with 5lb traction, there was significant difference according to gender, male (12 of 23 patients) was had arthroscopy, but only 2 of 17 female patients. (p=0.017). Conclusion: In the MTP joint arthroscopy, 10lb was needed in first and fourth MTP joint, and 5lb was needed in second, third, fifth MTP joint. Manual traction is generally not appropriate for distraction. Female and under 60 years old patients needed more distraction in first toe arthroscopy. Under 30 years old patients in third toe and female patients in fourth toe arthroscopy needed more distraction than 5lb. [Table: see text]

The body and chest walls are bent laterally and the thoracic and abdominal viscera are exposed.the pelvis is deformed.liver is cystic and abnormal in shape.The rumen is occasionally distended with fluid.The limb are usually ankylosed and rigid.in rare cases the limbs and head may be enclosed in a complete sac of skin.(Robert J, 1971).A one year old female goat presented with difficulty in birth since eight hours and delivered the foetus by manual traction.

Abstract Introduction the management of patients with infection or malfunction of a cardiac implantable electronic device (CIED) may be challenging. Purpose The aim of the study is to evaluate the safety and efficacy of transvenous lead extraction (TLE) in elderly patients. Methods a retrospective analysis of patients who underwent to TLE in our center was performed. Patients were divided in two groups: 1) patients 80 years of age or older, 2) patients younger than 80 years. All patients were treated with manual traction or mechanical dilatation. Results our analysis included 1316 patients, with a total of 2513 leads extracted. Group 1 (≥80 years) counted 202 patients and group 2 (&lt;80 years) 1114 patients. The group of elderly patients presented more comorbidities, as hypertension, chronic kidney disease, atrial fibrillation and pulmonary disease. Patients 80 years of age or older had more pacemakers than ICDs, whereas the dwelling time of the oldest lead, the number of leads and the presence of abandoned leads was similar despite patients age. In group 1 the rate of radiological success for lead was higher than in group 2 (99.0% vs 95.9%; P &lt; 0.001). The clinical success was obtained in 1273 patients (96.7%), without significative differences between groups (98.0% vs 96.4%; P = 0.36). Major complications occurred in 10 patients (0.7%), without significative differences (1.5% vs 0.6%; P = 0.24) (figure 1). Conclusion TLE in elderly patients is a safe and effective procedure. In patients older than 80 years there are not more major complications than in younger patients, and the efficacy of the procedure seems to be superior. Abstract Figure 1
