Every job has the opportunity of the risk because occurrence of work accidents can be caused by various factors. PT. CA is a large distributor of motorcycle spare parts which has activities including receiving goods from fabrication and then storing them in temporary warehouses before being distributed to dealers in various regions. The company's activities have a large potential for accidents due to several operations. To hazard identification, risk assessment, and determining control in any work that occurs or has the potential to occur, it is necessary to analyze using HIRADC. The method used is a descriptive method described in the flow process and then identified possible hazards that occur in each activity step. After the identification process is complete, a risk assessment is carried out to determine the form of control. The object of research is the receiving section of the work area. The results of this study are that the receiving section has 7 existing activities and there are 20 potential hazards that are possible, from these potentials the current control is to maximize APD to minimize danger. The highest level of risk is substantial in the activity of moving cargo to temporary storage or inspection areas with a potential danger of crashing into workers which has an impact on limb injuries, fractures, and even death, so this activity is a concern to avoid work accidents. In addition, it is determined that hazard control is carried out by means of forklift lanes that are not allowed to be passed by workers and control recommendations are in the form of setting SOPs, ensuring that forklifts complete their activities to carry out inspections. This study aims to determine the potential hazards that have the highest risk value as many as 3 types of work accidents in labs in the Private Vocational Laboratory in the Loceret Nganjuk area in accordance with the Risk Rating Number and provide recommendations for improvement on the types of activities that have the highest risk value as many as 3 types of work accidents in activities practicum at the Private Vocational Laboratory in the Loceret Nganjuk area using the Fishbone Diagram. The research method uses the Hazard Identification And Risk Assessment (HIRA) method and Fishbone Diagram. From the results of the study, the types of activities that have the highest Risk Rating Number are tripping Spare Parts / workshop tools with a Risk Rating Number of 8, tripping handtool / work tools with a Risk Rating Number of 8 and breathing in smoke with a Risk Rating Number of 8. Of the three types These activities are categorized as medium priority risk level / significant risks entered in index III B, namely the occurrence of unwanted potential. Then an analysis is carried out using a Fishbone Diagram by providing recommendations, namely conducting activities in a laboratory carefully and always using PPE as well as complying with the rules and knowing the functions of each - each Spare Part, work equipment and sharp-smelling objects. Keyword : Risk Rating Number, Hazard Identification And Risk Assessment, Fishbone Diagram.Penelitian ini bertujuan untuk mengetahui potensi bahaya yang memiliki nilai risiko tertinggi sebanyak 3 jenis kecelakaan kerja pada praktikum di Laboratorium SMK Swasta didaerah Loceret Nganjuk sesuai dengan Risk Rating Number dan memberikan rekomendasi perbaikan pada jenis kegiatan yang memiliki nilai risiko tertinggi sebanyak 3 jenis kecelakaan kerja di kegiatan praktikum di Laboratorium SMK Swasta didaerah Loceret Nganjuk menggunakan Diagram Fishbone. Pada metode penelitian menggunakan metode Hazard Identification And Risk Assessment (HIRA) dan Fishbone Diagram. Dari hasil penelitian, yaitu jenis kegiatan yang memiliki Risk Rating Number tertinggi adalah tersandung sparepart/ alat bengkel dengan Risk Rating Number sebesar 8, tersandung handtool/ alat kerja dengan Risk Rating Number sebesar 8 dan menghirup asap dengan Risk Rating Number sebesar 8. Dari ketiga jenis kegiatan tersebut dikategorikan tingkat risiko prioritas menengah/ risiko signifikan masuk pada indeks III B yaitu terjadinya potensi tidak diinginkan.Kemudian dilakukan analisis menggunakan Diagram Fishbone dengan memberikan rekomendasi yaitu melakukan kegiatan dilaboratorium secara hati – hati dan selalu menggunakan APD serta mematuhi aturan dan mengetahui fungsi dari masing – masing sparepart, peralatan kerja dan benda – benda berbau tajam. Kata Kunci: Risk Rating Number, Hazard Identification And Risk Assessment, Fishbone Diagram

Abstract This research is about Application of 5R methods ((Ringkas, Rapi, Resik, Rawat, Rajin) and indentification of hazard potential in Chemical Warehouse. And it aims to improve the work environment in the laboratory by using the 5R work method and identify potential hazards contained. Analysis of the two problems was carried out using several methods, namely for the implementation of warehouse layout with the 5R method, and for work safety using hazard identification & risk assessment using Pareto Chart Analysis to discuss the level of work accident risk from the highest to the lowest, and proceed by using a Fishbone Chart Analysis to analyze the types of work accidents from the factors that cause the risk of accidents. The index value before the application of 5R has a value of 20%, while the index value after improvement is 75.6% and the result of hazard identification has 23 hazard risks from 7 activities with an area that has a very high level of hazard risk. The implementation of Occupational Safety and Health (OSH)is one of the efforts to create a workplace that is safe, healthy, and freefrom environmental pollution, to minimize work accidents andoccupational diseases, and to finally improve work efficiency andproductivity. The mortality rate caused by fire cases in the industrial sectorare high. Therefore, fire management is needed as the implementation ofOccupational Safety and Health. This study aims to determine managementpreparedness in dealing with fire emergencies to prevent fire and minimizethe potential hazard risk of PT X. The method in this study uses qualitativemethods with triangulation approach through in-depth interview, observation, checklist form and documentation. The research resultsthrough in-depth interviews show that fire management is still not optimal, i.e. there are no hydrants, automatic alarms and emergency doors. Inaddition, the fire prevention unit has been established but not specifically. From the observation of using the checklist form, it showed that firemanagement has 3 Good (G) categories, includes fire extiguisher (82%), standard operating procedures (100%), and evacuation routes (100%). Theothers are 5 Less (L) categories, includes hydrant (0%), alarm (0%), emergency door (0%), education and emergency response training (33%), fire management unit (25%).

Indonesian Navy ships (KAL) is one element of the Integrated Fleet Weapon System (SSAT), which plays an important role in carrying out tasks in the defense field. So, we need an optimal KAL readiness to support the passage of the operation to be carried out successfully and smoothly. One of the ways used to prepare KAL is to carry out routine maintenance through the docking process. However, in practice there are accidents  suffered by workers.This research was conducted using the HIRARC method. HIRARC is a method of hazard identification, risk assessment and risk control measures that are used for this method is considered moreappropriate and more accurately where the dangers that arise in the process described docking of any work activity. In this method also provides appropriate control measures for each hazard. Based on the results of the HIRARC analysis, each suction work activity of docking based Risk Ranking will be sorted according to the level of Risk Matrix and will be discussed further against high risks and implemented controls. Then the resulting draft recommendation to minimize workplace accidents in the docking process is by way of elimination, Administration Control and Use of PPE (Personal Protective Equipment).Keywords: HIRARC method, Docking process, KAL.

The Berlian Terminal is one of the container terminals located in the Port of Tanjung Perak Surabaya, managed by PT BJTI PORT. Container loading and unloading activities have a risk of work accidents. Work accidents can be prevented by applying Hazard Identification, Risk Assessment, and Determining Control (HIRADC). HIRADC aims to identify potential hazards in the workplace by linking workers, tasks, work equipment, and the work environment. This study aims to determine the application of HIRADC to reduce the risk of accidents. This type of research is descriptive research. The total sample of 52 workers, taken by the total sampling method. Data related to respondent characteristics such as worker productivity and the OHS Program on HIRADC were obtained by direct observation in the field and interviews with all workers. The results showed that workers who had high productivity were (71.1%). Statistical tests using the ordinal regression test showed that there was an influence between the OHS Program on HIRADC and the Productivity of Work for Workers in Berlian Terminal (0,000 <0.05). The implementation of the OHS Program at the Berlian Terminal is good and needs to be maintained, if possible, it can be improved by increasing the competency of OHS Supervisors and OHS management following OHS regulations.

The HIRA (Hazard Identification, Risk Assessment) method is a process of identifying hazards that occur in all company activities, then conducting a risk assessment of the hazards that occur. Introduction: Based on the initial survey conducted at PT. X, it was found that there were work accidents such as pinched hands, cuts, entanglement in processing machines which caused the risk of accidents. This research was conducted at PT.X in February-August 2021. Methods: This study used a descriptive qualitative method. This study analyzes the risk of work accidents using the HIRA (Hazard Identification, Risk Assessment) method which aims to identify hazards and conduct a risk assessment of these hazards. The research informants were 6 people consisting of 2 key informants and 4 main informants. Results: The results showed that there were 44 types of hazards from 8 work stations at PT. X. Among them, 16% have a small risk of harm (trivial), 38.6% have a tolerable risk, 34.1% have a moderate risk of harm, 4.5% have a substantial risk of harm, 6, 8% have an intolerable risk of harm. Conclusion: Risk analysis is a process by which hazards are identified and risks are estimated to assess the damage caused with the likelihood of damage occurring.

Abstract Background: Visitors activities and the environment on the market have potential hazards that pose safety and health risk. Some market visitors lack awareness and often ignored potential hazard and risks due to lack of knowledge. This study aims to analyze the potential hazards and determine the hazard control efforts that have been made by the manager. Met This research is a qualitative study, by an observational approach. Research informants selected by purposive,  they were head of the staffing subdivision, security officers, traders/seller, and porter at the Beringharjo Market. The instruments used were checklist and interview guidance. Risks were analyzed with the AS / NZS 4360 standard. Results: The results of the study contained 40 types of potential hazards and 48 types of potential risks. The risk of market fires due to electricity installation failures and gas usage has an extreme rating. Low Back Pain (LBP) risk to the porter and injured by a coconut grater machine included in the high-risk rating. Control efforts undertaken were replacing the gas stoves to charcoal for cooking, forbidding smoking and the use of matches, training when lifting weights, disposing of garbage regularly, using seat belts and hooks when working at height. Conclusion: Market Managers have made several control efforts but inadequate and uncomfortable for. Advised to provide seat facilities for visitors to rest, safety talk to educate visitors about fires and training traders to use a fire extinguisher, activating the Occupational Health and Safety efforts organized by the community in the informal sector, making Standard Operational Procedure for each trading activity. Kebanyakan pengunjung pasar memiliki kesadaran yang rendah dan sering mengabaikan potensi bahaya karena kurangnya pengetahuan. Penelitian ini bertujuan untuk menganalisis potensi bahaya dan mendapat gambaran upaya pengendalian bahaya yang telah dilakukan oleh manajer Pasar Beringharjo di Yogyakarta. Metode: Penelitian ini adalah penelitian kualitatif, pendekatan observasional. Informan dipilih secara purposive, yaitu kepala bagian Kepegawaian, petugas keamanan, pedagang, dan buruh gendong di Pasar Beringharjo. Instrumen yang digunakan adalah checklist dan panduan wawancara. Risiko dianalisis dengan  standar AS/NZS 4360 Hasil: terdapat 40 jenis potensi bahaya dan 48 jenis potensi risiko. Risiko kebakaran pasar akibat kegagalan instalasi listrik dan penggunaan gas memiliki peringkat risiko yang sangat tinggi (Extreme).  Risiko Low Back Pain (LBP) pada aktivitas buruh gendong dan terluka oleh mesin pemarut kelapa yang termasuk dalam peringkat risiko tinggi (High). Upaya pengendalian yang telah dilakukan adalah mengganti penggunaan kompor gas dengan arang untuk memasak, melarang merokok dan penggunaan korek kecuali untuk memasak, pelatihan tentang beban kerja dan postur tubuh yang benar saat mengangkat beban, membuang sampah secara teratur, menutup makanan yang dijajakan, menggunakan sabuk pengaman dan tali pengait saat bekerja di ketinggian. Kesimpulan: Pengelola Pasar sudah melakukan beberapa upaya pengendalian, namun belum memadai dan belum memberikan kenyamanan pada pengunjung, disarankan menyediakan fasilitas kursi untuk beristirahat pengunjung, edukasi melalui safety talk kepada pengunjung mengenai kebakaran dan pelatihan penggunaan APAR kepada pedagang, mengaktifkan pos Usaha Kesehatan Kerja untuk mengelola kegiatan K3 pasar, pembuatan SOP bagi setiap aktifitas perdagangan. Water Treatment Plant (WTP) is an important infrastructure to ensure human health and the environment. In its development, aspects of environmental safety and health are of concern. This paper case study was conducted at the Water Treatment Plant Company in Semarang, Central Java, Indonesia. Hazard identification and risk assessment is one part of the occupational safety and health program at the risk management stage. The purpose of this study was to identify potential hazards using hazard identification methods and risk assessment methods. Risk assessment is done using criteria of severity and probability of accident. The results obtained from this risk assessment are 22 potential hazards present in the water purification process. Extreme categories that exist in the risk assessment are leakage of chlorine and industrial fires. Chlorine and fire leakage gets the highest value because its impact threatens many things, such as industrial disasters that could endanger human life and the environment. Control measures undertaken to avoid potential hazards are to apply the use of personal protective equipment, but management will also be better managed in accordance with hazard control hazards, occupational safety and health programs such as issuing work permits, emergency response training is required, Very useful in overcoming potential hazards that have been determined.
