AbstractGallstone disease is one of the most common causes of hospitalization for gastrointestinal diseases in the world. Recent studies have examined the presence of bacteria in the formation of stones. Our main goal was to determine the overall composition of gallstone microflora. Gallstones were obtained from 24 patients during laparoscopic cholecystectomy from which DNA were extracted. Composition of bacterial flora was evaluated on 16 s rDNA sequencing technique. In the vast majority of samples, bacteria were present, and four groups could be differentiated regarding the flora. Overall composition shows that 87% of the stones were cholesterol/mixed type of gallstone. Additionally, potentially harmful microorganisms (Streptococcus, Clostridium and Kocuria) that could cause post-surgery complications were identified in several patients. The obtained results indicate that this technique may be useful in analyzing the type of stones and in pinpointing the presence of pathogenic bacteria.

A novel coronavirus outbreak was first documented in Wuhan, Hubei Province, China in December 2019. The World Health Organization (WHO) has declared the coronavirus disease 2019 (COVID-19) a pandemic. A global coordinated effort is needed to stop the further spread of the virus. A novel coronavirus (nCoV) is a new strain that has not been identified in humans previously. Once scientists determine exactly what coronavirus it is, they give it a name (as in the case of COVID-19, the virus causing it is SARS-CoV-2).

Foundries in manufacturing plants are characterized by hazardous working conditions. The paper makes an assessment of industrial health hazards, identifies groups of diseases dominant among the examined individuals. It is clear that gastrointestinal diseases make the largest percentage. The effect of destabilizing industrial factors that lead to the intestinal biome restructuring, help reduce the dominant microorganism’s group and increase opportunistic flora has been determined. The severity of this indicator correlates with professional working experience. Along with the fecal matter bacterial flora of the examined people, the protozoa that belong to the parasitic group (blastocysts, lamblia, amoeba) were identified, and Blastocystis spp. has the largest proportion. The biological properties of blastocysts were studied exemplified by anticarnosine activity (AcrA), which allows them to colonize the intestinal mucous membranes and helps displace obligate symbiotes against the decrease in the overall resistance of the macroorganism. The data obtained make it possible to forecast human health state under the conditions of anthropogenic-disturbed territories and plan measures to improve and protect workers’ health.

The worldwide popularity of Dr. M.S. Reddy’s Multiple Mixed Strain Probiotic Therapy to treat or prevent the hospital acquired infections (nosocomial infections) arose a great interest in the medical community around the world (Reddy and Reddy, 2016; 2017). Reddy’s Multiple Mixed Strain Probiotic Therapy?  To answer these questions, the specific immunomodulation regulatory functions of the individual Probiotic strains (on host) have beenresearched, investigated andoutlined in this article.  A detailed explanation(s) and hypotheses have been proposed outlining the possible cumulativedirect bacteriological and indirect immunomodulatory effects (at the molecular level) of the Multiple Mixed Strain Probiotics used in Dr. M.S. Reddy’s Multiple Mixed Strain Probiotic Therapy to successfully treat C. diff infection.  A detailed scientific and research attempts were made to correlate the Probiotic induced immune activities in relation to the reduction of the symptoms associated with the hospital acquired Clostridium difficile infection during and after the Multiple Mixed Strain Probioitc Therapy.  Results of the clinical trials, microbiological tests on feces, and the clinical blood tests significantly revealed that the reasons for the success of Dr. Reddy’s Multiple Mixed Strain Probiotic Therapy are multifold. Presumably, it is predominantly due to the immunomodulatory effect they have exerted on the host immune system along with the direct inhibition of C. diff bacteria by multiple Probiotics, due to the production of bacteriocins, lactic acid and nutritional competency.In addition, the size of the individual cells of the Probiotic strains in the Multiple Mixed Strain Probiotics and their significant effect on immunomodulation has been thoroughly discussed. AbstractThis study aimed to determine the quality of drinking water supplied in different types of food stalls in Jashore Municipality, Bangladesh. A total of 35 water samples were collected from different tea stalls, street side fast food stalls, normal restaurants and well-furnished restaurants. The water quality was evaluated by determining the distinct physical, chemical and biological parameters. The results revealed that the water used in the food stalls and restaurants for drinking purpose was in desired quality in terms of turbidity, electrical conductivity, pH, total dissolved solids, nitrate (NO3−), sulfate (SO42−), phosphate (PO43−), chloride (Cl−), sodium (Na) and potassium (K) concentrations. The values were within the permissible limit proposed by the Bangladesh Bureau of Statistics and the World Health Organization. Concentrations of calcium (Ca) and magnesium (Mg) found in several samples were higher than the World Health Organization standard. Iron (Fe) concentrations were higher than the permissible limit of the World Health Organization. Only 46% exceeded the permissible limit of Bangladesh Bureau Statistics. The threatening result was that the samples were contaminated by fecal coliform, indicating that the people of Jashore Municipality may have a greater chance of being affected by pathogenic bacteria. The drinking water provided in the street side fast food stalls was biologically contaminated. The findings demonstrate that the drinking water used in food stalls and restaurants of Jashore Municipality did not meet up the potable drinking water quality standards and therefore was detrimental to public health.

Certain post-thoracic surgery complications are monitored in a standard manner using methods that employ ionising radiation. A need to automatise the diagnostic procedure has now arisen following the clinical trial of a novel lung ultrasound examination procedure that can replace X-rays. Deep learning was used as a powerful tool for lung ultrasound analysis. We present a novel deep-learning method, automated M-mode classification, to detect the absence of lung sliding motion in lung ultrasound. Automated M-mode classification leverages semantic segmentation to select 2D slices across the temporal dimension of the video recording. These 2D slices are the input for a convolutional neural network, and the output of the neural network indicates the presence or absence of lung sliding in the given time slot. We aggregate the partial predictions over the entire video recording to determine whether the subject has developed post-surgery complications. With a 64-frame version of this architecture, we detected lung sliding on average with a balanced accuracy of 89%, sensitivity of 82%, and specificity of 92%. Automated M-mode classification is suitable for lung sliding detection from clinical lung ultrasound videos. Furthermore, in lung ultrasound videos, we recommend using time windows between 0.53 and 2.13 s for the classification of lung sliding motion followed by aggregation.

The rapid emergence of antibiotic-resistant pathogenic bacteria has accelerated the search for new antibiotics. Many clinically used antibacterials were discovered through culturing a single microbial species under nutrient-rich conditions, but in the environment, bacteria constantly encounter poor nutrient conditions and interact with neighboring microbial species. In an effort to recapitulate this environment, we generated a nine-strain actinomycete community and used 16S rDNA sequencing to deconvolute the stochastic production of antimicrobial activity that was not observed from any of the axenic cultures. We subsequently simplified the community to just two strains and identifiedAmycolatopsissp. AA4 as the producing strain andStreptomyces coelicolorM145 as an inducing strain. Bioassay-guided isolation identified amycomicin (AMY), a highly modified fatty acid containing an epoxide isonitrile warhead as a potent and specific inhibitor ofStaphylococcus aureus. Amycomicin targets an essential enzyme (FabH) in fatty acid biosynthesis and reducesS. aureusinfection in a mouse skin-infection model. The discovery of AMY demonstrates the utility of screening complex communities against specific targets to discover small-molecule antibiotics.

Colic is the major cause of morbidity and mortality, premature deaths, and is the number one health concern in horses. To increase the quality of life for horses, it is vital to understand the causes, signs, diagnostics, treatment, and prevention of colic. Horses are hindgut fermenters and their cecal microbiome can easily be altered due to environmental and physiological changes. Colic can be induced from starch overloading, which can have detrimental effects on the cecum microbiome, such as decreasing hindgut pH and altering the production of volatile fatty acids. In addition, if the immune system becomes compromised, causing changes in the hindgut microbiome, this will trigger a proliferation of opportunistic and pathogenic bacteria, which can cause numerous gastrointestinal diseases, resulting in colic. However, identifying a variety of microorganisms in the cecum is extremely difficult and understudied because difficulties in sampling and not all bacteria strains can be cultured. Therefore, next generation sequencing has helped in identifying microbes found in the gastrointestinal tract. Using the application of metagenomics, allows scientists to be able to sequence thousands of microorganisms simultaneously. In addition, exploring new techniques to help identify and achieve a clearer picture of the microbiome population and how it may change during bouts of colic could revolutionize the way veterinarians diagnose colic and tailoring a treatment specific to the microoganisms that are proliferating. Therefore, understanding the mechanisms of colic and next technological advances could increase the quality of life for horses and minimize the mortality rates.

Introduction. Norovirus is one of the most common causes of acute gastroenteritis in the world. The aim of this study was to describe characteristics of an outbreak of norovirus infection and present all the steps of an outbreak investigation. Material and Methods. A retrospective cohort study was conducted among all individuals who ate food from the same restaurant. All the exposed persons filled out an adapted questionnaire that is routinely used in patients with food poisoning. Results. This paper presents a norovirus outbreak among film crew members and measures taken to control the epidemic. Gastroenteritis was diagnosed in 20 exposed persons. In most patients, symptoms lasted 1 - 3 days, and every third patient sought medical attention. The predominant clinical manifestations were vomiting and diarrhea. The food that probably caused the infection were sandwiches with ham, cheese and lettuce and microbiological analysis confirmed that the outbreak was caused by norovirus. Conclusion. Although the source of this outbreak remains unclear, the results of environmental investigation of the restaurant showed several irregularities in the kitchen that may have caused norovirus transmission.

Aerobic and anaerobic cultures were performed on cervical swabs from 35 mothers and their newborn infants' gastric aspirates and conjunctival sacs. Four hundred seventy-nine isolates were obtained; 287 were aerobes and 192 were anaerobes. The cervical and gastric cultures overall yielded a similar flora, but this was not always the case when one compared mothers with their own newborn infants. The conjunctival cultures yielded about half of the number of bacteria per specimen that the gastric contents yielded, and a repeat conjunctival culture done 48 hours later showed a marked reduction in the number of potentially pathogenic bacteria isolated. The predominant aerobes isolated were Staphylococcus epidermidis, diphtheroids, viridans streptococci and Haemophilus vaginalis. The predominant anaerohes were the Bacteroides fragilis group, Propionihacterium acnes, Peptococcus, other Bacteroides and Peptostreptococcus organisms. Statistical analysis revealed significant positive correlation between the isolation of certain groups of organisms and increased duration of pregnancy, increased baby's weight, and prolonged duration of labor.

Postcholecystectomy syndrome is a symptom complex that occurs or worsens after cholecystectomy and is a functional and / or organic disorder. It often complicates the post−surgery course of gallstone disease. The presence of symptoms of the disease indicates a deterioration in the quality of life of patients, but the diagnostic examination is not always possible to detect morphological or functional changes. Thus, at present the syndrome is an urgent problem of gastroenterology and biliary surgery. Diagnostic issues with a differentiated approach to the functional or organic nature of postcholecystectomy syndrome are important for the choice of further treatment tactics. In order to improve the diagnostic algorithm taking into account the changes in the area of the major duodenal papilla, a study was conducted in 137 patients. To determine the functional disorders of the sphincter of Oddi there was used the method of ultrasound investigation of hepatobiliary area and Vater's papilla with choleretic loading on Boyden as well as the Grigoriev's methods in the absence of organic obstruction of the terminal choledochus at previous stages of examination. Morphological changes in the major duodenal papilla area were evaluated using the technique of parietal ph−impedancemetry, which was performed on the background of benign mechanical jaundice in the patients after cholecystectomy during endoscopic retrograde cholangiopancreatography prior to endoscopic papillosphincterotomy. The obtained results help to perform a differentiated approach to the patients who underwent cholecystectomy, taking into account morphofunctional changes in the area of the major duodenal papilla and allow the implementation of the selected methods to the research algorithm of patients with postcholecystectomy syndrome.
