OBJECTIVES/GOALS: Septic shock is a lethal condition. Research suggests that overall sepsis mortality varies by race, but less is known about demographic differences in septic shock mortality. Our objectives were to describe the septic shock population using a large, state-wide data repository and identify demographic predictors of septic shock mortality. METHODS/STUDY POPULATION: This was a retrospective review of patients with septic shock in the One Florida Data Trust from 2012-2018. Patients were classified as having septic shock if they received vasopressors and had either 1) an ICD-9 or 10 code for septic shock or 2) an ICD-9 or 10 code for infection and an ICD-9 or 10 code for organ dysfunction. Demographic data and place of residence prior to admission was collected. The primary outcome was 90 day mortality. T-test and chi-square tests were used to test association of individual predictors and mortality. Multiple logistic regression was used to identify predictors of mortality after adjustment for other variables. Level of significance was set at 0.05. SAS v9.4 (Cary, NC) was used for analyses. RESULTS/ANTICIPATED RESULTS: There were 11,790 patients with septic shock. The mean(SD) age was 61(16) years. With regard to race/ethnicity 66% identified as white, 27% as black, 3.7% as Hispanic, and 3.5% as other races (non-white, non-black, non-Hispanic). Most came from home (57%). Overall, 39% died. Mortality varied by race (p<0.01): white 39%, black 39%, Hispanic 31%, other races 51%. In the logistic regression model, age, race, and residence were significant predictors of mortality, after adjustment for other variables. Each additional year of age had a 2.7% increased odds of mortality (OR 1.03; 95% CI 1.02-1.03; p<0.01). Compared to white patients, odds of death were 1.6 times higher for other races (95% CI 1.3-2.0; p <0.01) and non-significantly higher for black patients (OR 1.1; 95% CI 1.0-1.2; p = 0.05). Compared to those from home, odds of death were highest for those from a skilled nursing facility (OR 1.5; p<0.01). DISCUSSION/SIGNIFICANCE OF IMPACT: Patients who identified as other races had increased mortality from septic shock compared to white patients after adjusting for other variables. Septic shock mortality also increased with age and varied by residence. Further analyses are needed to examine racial disparities and control for comorbidities, severity of illness, and aspects of resuscitation. Introduction: Glycemic variability (GV) has been associated with mortality in critically ill patients. Several methods have been used to measure it, but no guideline has so far proposed a gold standard. The aim of this study was to associate different GV measurement methods with mortality in patients in an intensive care unit (ICU) under nutrition therapy (NT). Methods: Retrospective longitudinal study conducted between April and September 2018, in which 82 patients were enrolled in intensive care, with more than 48 hours of hospitalization and using NT. Demographic data, clinical data and all blood glucose values measured during hospitalization were collected. The GV was evaluated through three indicators: standard deviation (SD), coefficient of variation (CV) and glycemic mean (MGL). Statistical analysis was performed using Whitney and Pearson Chi-square tests, univariate and multiple logistic regression, and odds ratio. The analyzes were processed and evaluated using the Statistical Data Analysis software version 12.0. Results: 11,676 blood glucose levels were measured. Most of the sample received enteral diet. Mortality was 42.7%. A total of 36.58% of the patients had a previous diagnosis of diabetes mellitus (DM) and these presented higher values for all evaluated parameters of VG compared to non-diabetics. CV, SD and MGL values were higher among the patients who died. Univariate logistic regression analysis showed that the variables CV, SD and MGL were significantly associated with mortality. However, based on the multivariate logistic regression model, only PD maintained a relationship with mortality. Conclusion: The SD is an indicator of GV is independently associated with increased mortality in a cohort of heterogeneous patients on TN in the ICU.

Background: The study of suicide and identification of mental health problems, social, cultural, and environmental communities are effective for preventive measures and reducing risk factors. Objectives: This study aimed to study the epidemiology of suicide and its associated factors over 2011 - 2015 in Zarand. Methods: In this observational-epidemiological study (cohort study), all people who committed suicide in 2011 - 2015 were recorded on a standardized questionnaire, and demographic data of Zarand were collected by the health center and government. Data were analyzed by SPSS software using descriptive statistics, Chi-square tests, and logistic regression analysis. Results: During 2011 - 2015, 2,401 persons (372 at one hundred thousand) committed suicide, of whom 33 cases (5.1 at one hundred thousand) died. Chi-square and logistic regression information showed suicide attempt rate in people with middle school education was 15.7 times greater than less or more educated ones; similar results showed the rate of suicide of the people in the age group 15-29 was 6.3 times higher than other age groups, retired, and unemployed 3.1 times more than employed and finally, urban living had suicide attempt rate of 1.6 times higher than rural living. Conclusions: In Zarand, the incidence of suicide is much higher than the global, country, and province average (more than three to four times), and leading to death was much less than expected. However, there are significant differences in the cause and manner and demographic characteristics with other studies, which require the creation of Suicide Registry Center (SRC) and qualitative studies with form layer analysis of causes in Zarand.

Background<br />Cysticercosis is an infectious disease caused by the larval form of Taenia solium (cysticercus cellulosae) and has been ranked as the most important food-borne parasite of humans in terms of public health, socioeconomic and trade impact. Cysticercosis is still a health problem in Papua and is inseparable from socio-cultural factors, hygiene and environmental sanitation. The aim of this study was to investigate the seroprevalence of cysticercosis and the risk factors that contribute to cysticercosis.<br /><br />Methods<br />A cross-sectional study was conducted in March-November 2016 involving 800 subjects. Demographic data and risk factors were collected using questionnaires. Cysticercosis serological examination was performed by means of the magnetic microsphere bead immunoassay technique coupled with rT24H recombinant protein to detect serum rT24H cysticercosis specific antibodies. The data obtained were analyzed by bivariate test (chi-square) and logistic regression.<br /><br />Results<br />Cysticercosis seroprevalence in Papua was 3.6% (284/7 874). The logistic regression analysis found that the risk factors playing the role of predictor were cooking pork with hot stones [OR=3.06; 95%CI: 2.19-4.28; p=0.000], nail hygiene [OR=2.05; 95%CI: 1.57-2.67; p=0.000], consumption of raw vegetables or salads [OR=0.52; 95%CI: 0.30-0.91; p=0.022], use of river water for washing foods [OR= 1.92; 95%CI: 1.39-2.64; p=0.000].<br /><br />Conclusions<br />Cooking pork with hot stones was the main risk factor of cysticercosis. Suspected cases of T. solium in pigs should be confirmed by molecular methods. Both taeniasis and human cysticercosis should be notifiable and surveillance in animals should be improved.

ABSTRACT OBJECTIVE To analyze if characteristics of managers, schools, and curriculum are associated with the implementation of programs for drug abuse prevention in elementary and high schools. METHODS Cross-sectional study, with random sample of 263 school managers. Data were collected between 2012 and 2013 by a program that sends forms via internet. A closed self-filling questionnaire was applied online. Statistical analysis included Chi-square tests and logistic regression models. The outcome variable was the presence of program for drug abuse prevention inserted in the daily life and educational program of the school. The explanatory variables were divided into: demographic data of the manager; characteristics of the school and of the curriculum; health education; and drug use in the school. RESULTS We found that 42.5% (95%CI 36.1–49.1) of the evaluated schools had programs for drug abuse prevention. With the multiple logistic regression model, we observed that the more time the manager has worked with education, the chance of the school having a program increased at about 4.0%. Experimenting with innovative teaching techniques also increased at about six times the chance of the school developing a program for drug abuse prevention. The difficulties in the implementation of the programs were more present in state and municipal schools, when compared with private schools, due to, for instance: lack of teaching materials, lack of money, and competing demands for teaching other subjects. CONCLUSIONS The implementation of programs for drug abuse prevention in the city of Sao Paulo is associated with the experience of the manager in education and with the teaching strategies of the school.

Abstract Abstract 1573 Poster Board I-599 Background Aberrant activation of the The Janus kinase(JAK)/signal transducer and activator of transcription (STAT) pathway may predispose to leukemia cells due to deregulation of proliferation, differentiation or apoptosis. One of the members of this family, JAK2, plays a very important role in metabolizing carcinogens and medications. In this study, we aimed to determine whether any association exists between genetic polymorphism in JAK2 A830G and individual susceptibility to acute leukemia. Method We carried out a case-control study using a China sample set with 243 acute leukemia cases and 282 controls matched by age, ethnicity. 68 of the acute leukemia patients were diagnosed with acute lymphoblastic leukemia (ALL) and 175 patients with acute myeloid leukemia (AML). Genomic DNA was isolated from peripheral blood and genomic DNA samples were assayed by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF) in the A830G on JAK2 gene. The data were analyzed statistically employing chi-square and logistic regression analyses. Result The frequencies of G/G genotype (wild type) were 4%, 8% and 31.4% in ALL, AML and control groups, respectively. The frequencies of polymorphic G/A genotype (heterozygous variant) were found to be 69% in ALL patients, 71% in AML patients and 48.6% in controls. The A/A genotype (homozygous variant) were existed to be 69% in ALL patients, 20% in AML patients and 20% in controls. Logistic regression analyses showed a significant correlation between the JAK2 A830G polymorphism (G/G) and acute leukemia patients (OR = 1.309, 95% CI =0.839-2.043, p<0.001). Conclusion Our findings indicate that G/G genotype may be an important genetic determinant for acute leukemias. According to our knowledge, this is the first report of an association between acute leukemia cases and the JAK2 A830G polymorphism. Disclosures No relevant conflicts of interest to declare.

This study aims to describe the effect of sanctions (individual sanctions, collective sanctions, and absence of sanctions) on cooperative behavior of individuals with medium trust in the context of corruption. Both collective sanctions and individual sanctions, are systemic, which means sanctioning behavior is exercised not by each individual but by the system. Cooperative behavior in this context means choosing to obey rules, to reject acts of corruption and to prioritize public interests rather than the personal interests. Conversely, corruption is an uncooperative behavior to the rules, and ignores the public interest and prioritizes personal interests. Research subjects were 62 students. The Chi-Square Analysis was used to see the association between the variables and the logistic regression model was applied to describe the structure of this association. Individual sanction is recommended as punishment to medium trust individuals to promote cooperative behavior in the context of corruption. The results showed that individuals with medium trust had more cooperative behavior.

<p><em>Perawatan pada lansia di panti tresna werdha X dilakukan berdasarkan rutinitas kerja dan petugas belum mendapat pelatihan mengenai kebutuhan dasar lansia yang harus dipenuhi. Berdasarkan hal tersebut, peneliti ingin mengetahui hubungan budaya kerja petugas panti menurut persepsi lansia dengan kejadian pengabaian di PSTW X Sumatera Barat. Desain penelitian deskriptif korelasi, menggunakan pendekatan cross sectional. Pengambilan sampel dengan total sampling, berjumlah 75. Uji statistik digunakan chi square dan regresi logistik. Hasil yang diperoleh seluruh lansia mengalami pengabaian; ada hubungan antara sikap terhadap apa yang dikerjakan, lingkungan pekerjaan; waktu dengan pengabaian lansia; Tidak ada hubungan antara perilaku ketika mengambil keputusan dengan pengabaian lansia. Sikap terhadap apa yang dikerjakan memiliki pengaruh paling besar terhadap kejadian pengabaian lansia. Disimpulkan bahwa petugas harus lebih memahami tentang peran dan fungsinya sebagai pemberi pelayanan pada lansia. Direkomendasikan agar pejabat yang berwenang dapat menempatkan petugas panti sesuai dengan peran dan fungsi serta kewenangannya, dan merekrut perawat sesuai dengan kebutuhan panti.</em></p><p> </p><p><em>The nursing of elderly in </em><em>Tresna Werda X nursing home is mainly based on the workrout in and that the caregiver have not yet received training on the basic needs of the elderly that they should meet. Therefore, the researcher wanted to know the relations of the work culture of caregiver according to the elderly’s perception with the incidence of neglect in PSTW X West Sumatera. This study used Descriptive correlation design with cross sectional approach. The method of sampling used total sampling, which amounts to 75. The statistical test used chi square and logistic regression. The results of the study show that all of the elderly experienced neglect; there is a relation between attitude towards what is conducted, work environment, time with elderly neglect; no relation between behavior in making decisions with elderly neglect. The attitude towards what is conducted has the most impacton the incidence of elderly neglect. It is concluded that the caregiver should have a better understanding on their roles and functions as the caregivers to the elderly. Moreover, it is recommended that the authorities should put nursing personnel in accordance with the roles, functions and competence, and recruit nurses in accordance with the needs of the nursing home.</em></p>

Study Design: Retrospective cohort. Objective: Traumatic facial fractures (FFs) often require specialty consultation with Plastic Surgery (PS) or Otolaryngology (ENT); however, referral patterns are often non-standardized and institution specific. Therefore, we sought to compare management patterns and outcomes between PS and ENT, hypothesizing no difference in operative rates, complications, or mortality. Methods: We performed a retrospective analysis of patients with FFs at a single Level I trauma center from 2014 to 2017. Patients were compared by consulting service: PS vs. ENT. Chi-square and Mann-Whitney-U tests were performed. Results: Of the 755 patients with FFs, 378 were consulted by PS and 377 by ENT. There was no difference in demographic data ( P > 0.05). Patients managed by ENT received a longer mean course of antibiotics (9.4 vs 7.0 days, P = 0.008) and had a lower rate of open reduction internal fixation (ORIF) (9.8% vs. 15.3%, P = 0.017), compared to PS patients. No difference was observed in overall operative rate (15.1% vs. 19.8%), use of computed tomography (CT) imaging (99% vs. 99%), time to surgery (65 vs. 55 hours, P = 0.198), length of stay (LOS) (4 vs. 4 days), 30-day complication rate (10.6% vs. 7.1%), or mortality (4.5% vs. 2.6%) (all P > 0.05). Conclusion: Our study demonstrated similar baseline characteristics, operative rates, complications, and mortality between FFs patients who had consultation by ENT and PS. This supports the practice of allowing both ENT and PS to care for trauma FFs patients, as there appears to be similar standardized care and outcomes. Future studies are needed to evaluate the generalizability of our findings.

Background: Obsessive–compulsive disorder (OCD) is a very heterogeneous condition that frequently includes symptoms of the “symmetry dimension” (i.e., obsessions and/or compulsions of symmetry, ordering, repetition, and counting), along with aggressive, sexual/religious, contamination/cleaning, and hoarding dimensions. Methods: This cross-sectional study aimed to investigate the prevalence, severity, and demographic and clinical correlates of the symmetry dimension among 1001 outpatients from the Brazilian Research Consortium on Obsessive–Compulsive Spectrum Disorders. The main assessment instruments used were the Dimensional Yale–Brown Obsessive–Compulsive Scale, the Yale–Brown Obsessive–Compulsive Scale, the USP-Sensory Phenomena Scale, the Beck Depression and Anxiety Inventories, the Brown Assessment of Beliefs Scale, and the Structured Clinical Interview for DSM-IV Axis I Disorders. Chi-square tests, Fisher’s exact tests, Student’s t-tests, and Mann–Whitney tests were used in the bivariate analyses to compare patients with and without symptoms of the symmetry dimension. Odds ratios (ORs) with confidence intervals and Cohen’s D were also calculated as effect size measures. Finally, a logistic regression was performed to control for confounders. Results: The symmetry dimension was highly prevalent (86.8%) in this large clinical sample and, in the logistic regression, it remained associated with earlier onset of obsessive–compulsive symptoms, insidious onset of compulsions, more severe depressive symptoms, and presence of sensory phenomena. Conclusions: A deeper knowledge about specific OCD dimensions is essential for a better understanding and management of this complex and multifaceted disorder.

Purpose: To retrospectively review a single institution experience of ultrasound guided axillary vein port placement. Methods: In this retrospective study, a patient list was generated after searching our internal database from 1/1/2012 to 10/1/2018. Patients who had undergone axillary vein port placement were included. Chart review was performed to confirm approach, laterality and to gather demographic data, clinical indications, technical outcomes, and complications. Descriptive statistics were used to analyze this cohort. Chi-square statistics were used to compare outcomes by laterality. Results: Three hundred seven patients (51% female) with an average age of 58 years were included. The port was placed via the right axillary vein in 85% (261/307), predominantly for the indication of chemotherapy access (296/307). Technical success was achieved in all 307 cases. Peri procedural complications occurred in 1% (4/307) of cases and included port malpositioning requiring replacement and a case of port pocket hematoma. Post procedural complications including deep vein thrombosis and port malfunction occurred in 17% (52/307) of cases and port removal as a result of complication occurred in 9% (29/307) of cases. Conclusions: Ultrasound guided placement of an axillary port is a safe procedure to perform and demonstrates good clinical outcomes.
